Periodic breathing and dysphagia associated with a localized lateral medullary infarction.

A 77-year-old man experienced sudden onset of dysphagia. An MRI revealed a left lateral medullary infarction. Portable polysomnography showed Cheyne-Stokes-like breathing with marked desaturation (lowest SaO(2) 74%) associated with apnoea. Oxygen administration reduced the apnoea-hypopnoea index from 30.1 to 20.0 per hour. Sustained periodic breathing under normoxaemia suggested that the brainstem lesion, rather than peripheral factors such as prolonged lung-chemoreceptor circulatory delay, was responsible for the abnormal breathing. Polysomnography or overnight SaO(2) monitoring should be con

Central Sleep Apnea in Adults: An Interdisciplinary Approach to Diagnosis and Management-A Narrative Review.

Central sleep apnea (CSA) is a heterogeneous group of sleep-related breathing disorders characterized by intermittent absence of respiratory effort during sleep. CSA results from impaired neurological signaling from the respiratory centers to the respiratory muscles, leading to airflow cessation for at least 10 s. Major causes include heart failure, opioid use, central neurological disorders, and altitude exposure. This review outlines the pathophysiology of CSA, emphasizing ventilatory instability and brainstem dysfunction as key mechanisms. It details the classification of CSA subtypes, incl

The importance of sleep-disordered breathing in cardiovascular disease.

Obstructive sleep apnoea and central sleep apnoea/Cheyne-Stokes respiration are collectively referred to as sleep-disordered breathing (SDB). Rapidly accumulating evidence suggests that both forms of SDB, and often a combination of both, are highly prevalent in patients with a wide variety of cardiovascular diseases, including hypertension, heart failure, arrhythmias, coronary artery disease, acute coronary syndrome and stroke. The presence of SDB in these patients is independently associated with worse cardiac function and exercise tolerance, recurrent arrhythmias, infarct expansion, decrease

[Sleep related breathing disorders].

Sleep related breathing disorders are common conditions and the management of patients with sleep apnea is an essential component of routine patient care. Daytime sleepiness is the leading symptom of sleep apnea but not mandatory. Especially patients with cardiac, pulmonary or metabolic comorbidities can benefit from treatment of sleep apnea and those patients should be considered for cardio-respiratory screening even with mild clinical symptoms. Continuous positive airway pressure therapy is still the most efficient treatment for sleep apnea and standard treatment for severe forms. With patie

[Central sleep apnea syndrome].

Central sleep apnea (CSA) is characterized by a lack of drive to inspire for at least 10 sec. In the CSA-syndrome accompanying arousals and desaturations of the arterial blood cause sleep disturbances and sympathetic nerve activations which lead to excessive daytime sleepiness and increase the risk for cardiovascular morbidity. There are six manifestations of CSA: a rare primary or idiopathic form, often in hypocapnic patients with an increased hypercapnic ventilatory drive; Cheyne-Stokes respiration, characterised by periodic CSA and a crescendo/decrescendo breathing pattern, often in patient

Non-linear dynamics of human periodic breathing and implications for sleep apnea therapy.

A mathematical model of non-obstructive human periodic breathing (Cheyne-Stokes respiration) or central sleep apnea (CSA) is described which focused on explaining recently reported non-linear behavior. Evidence was presented that CHF (chronic heart failure)-CSA and ICSA (idiopathic central sleep apnea) both involved limit cycle oscillations. The validity of applying linear control theory for stabilization must then be re-examined. Critical threshold values and ranges of parameters were predicted which caused a change (bifurcation) from limit cycle periodic breathing to stable breathing. Change

Interrelated atrial fibrillation and leaks triggering and maintaining central sleep apnoea and periodic breathing in a CPAP-treated patient.

We report the case of a 71-year-old obese continuous positive airway pressure (CPAP)-treated man who developed an acute cardiac failure (ACF) triggered by atrial fibrillation. CPAP data downloaded from the CPAP software (Rescan®) retrospectively demonstrated the progressive development of a high residual central apnoea-hypopnoea index (AHI) with Cheyne-Stokes respiration (CSR). The AHI decreased after cardioversion allowing normalization of cardiac rhythm and function. Raw data extracted from CPAP software showed a gradual decrease in the periodic breathing cycle length related to a simultaneo

Heart failure and sleep-disordered breathing.

PURPOSE OF REVIEW: Sleep-disordered breathing, which includes both obstructive and central sleep apnoea (OSA and CSA, respectively), is highly prevalent in patients with heart failure. In this review, we outline our current understanding of the bidirectional relationship between these disorders and heart failure. We also explore the role of recent advances in therapeutics. RECENT FINDINGS: Although early studies suggest promise of adaptive servoventilation in treating sleep-disordered breathing, particularly CSA with associated Cheyne-Stokes respiration, the recent clinical trial in the heart

Contemporary insights and novel treatment approaches to central sleep apnea syndrome in heart failure.

Central sleep apnea (CSA) is a common and under-diagnosed condition commonly associated with Cheyne-Stokes respiration. It is particularly prevalent in the heart failure population affecting up to 40 % of all patients with heart failure. The pathophysiology associated with CSA is based on the underlying effects of hypoventilation and hyperventilation, with neurologic dysregulation of respiratory control as the primary defect. However, therapeutic options are limited because of the prevailing perception that CSA is a consequence, rather than cause of morbidity and mortality. At present, the mai

Sleep apnoea in patients with heart failure. Part I: diagnosis, definitions, prevalence, pathophysiology and haemodynamic consequences.

Sleep disorder specialists manage 90% of patients suffering from obstructive sleep apnoea, which affects 10% of the general population. From another perspective, cardiovascular disease specialists are particularly challenged by sleep apnoea, since it affects a large proportion of their patients and its complications are largely cardiovascular. At least 50% of patients with chronic heart failure (HF) suffer from sleep apnoea, predominantly central and/or Cheyne-Stokes respiration as opposed to obstructive sleep apnoea. While its effect on survival remains uncertain, sleep apnoea promotes the pr

Common slow modulation of respiration, arterial blood pressure and cortical activity during sleep onset while napping.

Using healthy subjects, concomitant 30- to 60-s modulations of respiration, arterial blood pressure and EEG activity were found in 21 experiments about napping. Although mean arterial pressure (MAP) modulations above and below 1/30 Hz increased, in respiratory amplitude (RA) only the lower frequency components augmented significantly. This slow modulation of RA was found to be asymmetrical in time, the duration of RA decreasing parts in the modulation waves being 42% longer than the duration of RA increasing parts. The concomitance of the slow modulations in the different organ systems is acco

Effects of mechanical unloading/loading on respiratory loop gain and periodic breathing in man.

We investigated the effect of mechanical unloading and loading on Cheyne-Stokes respiration (CSR) in seven intubated patients with preexisting CSR. For mechanical loading patients had to breathe against the resistance of the endotracheal tube. For mechanical unloading patients were supported with a volume-proportional pressure support in the proportional assist ventilation (PAV) mode whilst the flow-dependent (nonlinear) endotracheal tube resistance was continuously compensated for by means of the automatic tube compensation (ATC) mode. Mechanical unloading aggravated CSR as revealed by a prol

Periodic breathing in siblings of sudden infant death syndrome victims.

To determine the incidence of periodic breathing (PB) episodes during sleep, 48 subsequent sudden infant death syndrome siblings and 48 control infants, matched by sex, gestational age, and chronologic age, were studied by home pneumogram technique. The sudden infant death syndrome siblings had a statistically significant increase in the percent of sleep time spent in PB, in the number of episodes/100 minutes of PB, in the longest episode PB, and in the average duration of all episodes of PB. We conclude that PB is present in a higher amount in sudden infant death syndrome siblings as compared

[Central sleep apnea: cessation of central respiration control while we sleep : Suggestions for daily practice in diagnosis and therapy].

Central sleep apnea (CSA) is a sleep-related breathing disorder characterized by repeated cessation of respiratory drive during sleep. Unlike obstructive sleep apnea, CSA is not caused by upper airway obstruction but by a dysfunction in central respiratory regulation. The prevalence of CSA is low in the general population (0.9-1.5%), but significantly higher in risk groups such as patients with heart failure (up to 50%). Pathophysiologically, CSA is marked by instability of respiratory brainstem drive, prolonged circulation time, and increased CO2 sensitivity. Cheyne-Stokes respiration, a comm

Sleep-related breathing disorders and pulmonary hypertension.

Sleep-related breathing disorders (SBDs) include obstructive apnoea, central apnoea and sleep-related hypoventilation. These nocturnal events have the potential to increase pulmonary arterial pressure (PAP) during sleep but also in the waking state. "Pure" obstructive sleep apnoea syndrome (OSAS) is responsible for a small increase in PAP whose clinical impact has not been demonstrated. By contrast, in obesity hypoventilation syndrome (OHS) or overlap syndrome (the association of chronic obstructive pulmonary disease (COPD) with obstructive sleep apnoea (OSA)), nocturnal respiratory events con
