Relationship between entry into child welfare and mental health service use.

OBJECTIVE: This study examined the relationship between initiation of outpatient mental health service use and level of child welfare involvement. METHODS: Three levels of child welfare involvement were examined: in-home care and no child welfare services beyond an initial investigation, in-home care and additional child welfare services, and placement in out-of-home care (foster care). Longitudinal data were collected for a subsample of children (N=3,592) aged two through 14 years who were enrolled in the National Survey of Child and Adolescent Well-Being, a nationally representative sample

Emotional abuse in a sample of multiply maltreated, urban young adolescents: issues of definition and identification.

OBJECTIVE: The main purpose of this paper is to use the Brassard and Donovan [Brassard, M. R. & Donovan, K. L. (2006). Defining psychological maltreatment. In M. M. Freerick, J. F. Knutson, P. K. Trickett, & S. M. Flanzer (Eds.), Child abuse and neglect: Definitions, classifications, and a framework for research (pp. 151-197). Baltimore, MD: Paul H. Prevalence and Severity of Child Impairment in a US Sample of Child Maltreatment Investigations.

OBJECTIVES: Although there is agreement that childhood disability is both a risk and result of maltreatment, the extent of disability in the child welfare system remains unclear. Our objective is to determine the prevalence and severity of child impairment in a national sample of child abuse and neglect investigations in the United States. METHODS: We used data from the National Survey of Child and Adolescent Well-Being II, a study of 2644 children older than 3 years who were subjects of child abuse and neglect investigations. Groups of children were identified by latent profile analysis usin

Reactive attachment disorder in maltreated toddlers.

OBJECTIVE: To determine if Reactive Attachment Disorder (RAD) can be reliably identified in maltreated toddlers in foster care, if the two types of RAD are independent, and to estimate the prevalence of RAD in these maltreated toddlers. METHODS: Clinicians treating 94 maltreated toddlers in foster care were interviewed regarding signs of attachment disorder at intake in an intervention program. RESULTS: Using categorical and continuous measures, both types of RAD can be reliably identified in maltreated toddlers. Both continuous scores and categorical diagnoses indicated that a substantial m

Inhibited attachment disordered behavior in institutionalized preschool children: links with early and current relational experiences.

Inhibited attachment disordered behavior (IADB) is characterized by difficulty in establishing an attachment relationship with a primary caregiver; is associated with persistent social and emotional problems; and is observed most frequently in contexts of pathological care such as institutional rearing. Here, we seek to enhance understanding of the conditions that give rise to IADB among institutionalized children by examining prior family experiences of neglect and deprivation and concurrent relational experiences at the institution. The sample is comprised of 146 children, between 36 and 78

Emotional availability in mothers and their children with spinal muscular atrophy type 1 who require augmentative and alternative communication: a mixed-methods pilot study.

Emotional availability in parent-child interactions fosters children's socioemotional development. Little is known about the emotional availability of parents and children with profound motor disabilities and complex communication needs or the contributions of augmentative and alternative communication (AAC) to emotional availability. To begin addressing these gaps, this pilot study focused on three mothers and their children with spinal muscular atrophy Type 1 who could not speak and required AAC. The study used a mixed-methods design. Mother-child interactions were rated using the Emotional

Emotion regulation as a mediator of the relation between sexual abuse and behavior problems in preschoolers.

Maltreated children show poor emotion regulation competencies compared to non-maltreated children. Emotion regulation has been found to mediate the association between maltreatment and behavior problems in children. The aim of the present study was to examine the relationships among child sexual abuse (CSA), emotion regulation (ER), and internalized and externalized behavior problems in preschoolers using conditional process analyses. ER competencies were assessed in 127 children aged 41-79 months (62 abused, 65 non-abused) by their parents (N=124) and early childhood educators (N=88) using th

Stress in Parents of School-Age Children and Adolescents With Cochlear Implants.

Parents of children with cochlear implants (CIs) face unique challenges in caring for their child, potentially fostering parental stress. Most studies of stress in parents of CI users do not examine stress specific to having a deaf and hard-of-hearing (DHH) child. This study compares general and condition-specific stress (via the Family Stress Scale) in 31 parents of CI users (8-16 years) to previously published samples of DHH children, and it examines child- and CI-related factors associated with parental stress. Parents of modern-day CI users reported significantly lower stress than parents

Coparenting, Negative Educational Outcomes, and Familial Instability in Justice-involved Families.

Incarceration of a parent is associated with negative consequences for children, such as behavioral problems, the development of antisocial personality traits, and decreased educational attainment. Data from the Multi-site Family Study on Incarceration, Parenting, and Partnering was used to analyze four measures of the coparenting relationship in families in which the father is incarcerated and the mother is not, to examine if a higher quality, more cohesive coparenting relationship is associated with fewer adverse childhood experiences for the parents' shared child. Findings suggest that chil

Fetal alcohol spectrum disorders-- implications for child neurology, part 1: prenatal exposure and dosimetry.

In the United States, approximately 80 000 women consume ethanol through all 3 trimesters of pregnancy each year. In this article, we review prevalence rates of prenatal alcohol exposure in the United States and discuss the mechanisms of prenatal alcohol exposure and placental-umbilical effects. Cigarette smoking and delayed prenatal care are often associated with prenatal alcohol exposure. In addition, increased risk for postnatal adversity is common, including maternal depression, foster care placement, and developmental delay. In part 2, we review prevalence rates and the diagnostic criteri

Gender Differences in the Maltreatment-Youth Offending Relationship: A Scoping Review.

Maltreated and child welfare-involved youth are over-represented in juvenile justice systems. These youth are at a greater risk of serious offending and justice system entrenchment relative to their non-maltreated peers. Understanding gender differences in the pathways to justice involvement and the nature of offending among maltreated children is critical for informing policy and practice. Yet, this body of evidence is fragmented. This scoping review identified and narratively synthesized evidence from studies reporting on gender differences in the individual characteristics, maltreatment exp

Esports: What Do Pediatricians Need to Know?

Esports has seen a massive increase in the number of players, especially adolescents. Given the recent addition of gaming disorder as a diagnostic disorder, it is especially timely to understand the pattern and psychological factors associated with Esports. Esports is a recent phenomenon, and there is very limited awareness about its related consequences among child health professionals. The aim of this article is to familiarize pediatricians with these games/Esports, as well as gaming disorder assessment and management. Because digital life begins at a young age, pediatricians can empower par

Dimensions of Child Maltreatment in Australians With a History of Out-of-Home Care.

Research suggests that the dimensions of childhood maltreatment (type, age of onset, duration, frequency and perpetrator) play an important role in determining health and wellbeing outcomes, though little information is available on these dimensions for any care experienced cohorts. This study aimed to determine if any variation in maltreatment dimensions were experienced between two subsets of the nationally representative Australian Child Maltreatment Study, both of which reported childhood maltreatment histories: care-experienced (n = 358) and non-care-experienced (n = 4922). Using a series

Enhancing Permanency in Children and Families (EPIC): a child welfare intervention for parental substance abuse.

BACKGROUND: Across Ohio, parental substance abuse has contributed to a marked increase in the number of children in foster care. Children exposed to parental substance use have a higher likelihood of physical abuse and neglect, and consequently a variety of physical, psychological and cognitive problems. The Enhancing Permanency in Children and Families (EPIC) program is a collaborative effort between the Ohio State University College of Social Work, two county offices of the Ohio Department of Job and Family Services, two juvenile courts and local behavioral health agencies. The goal of EPIC

The association between mothers' and daughters' positive affect is moderated by child cardiac vagal regulation.

INTRODUCTION: Extensive research has supported the importance of children's positive affect in fostering prosperous psychosocial adjustment. Children's positive affect is believed to be significantly shaped by their environment in general and their caregivers' positive affect in particular. The current study investigates the role of child cardiac vagal regulation, a psychophysiological marker for social engagement, in shaping the association between maternal positive affect and child positive affect. METHODS: Mothers and daughters (ndyads  = 28) participated in two experimental conditions. In

Mother-Child Relationship Quality in the Presence of Maternal Mental Disorders: Do Self-Report and Behavioural Observation Differ?

Mental disorders affect not only mothers themselves but also their children and partners. Borderline personality disorder (BPD) in particular is thought to impact the mother-child relationship, but comparisons with mothers with other mental disorders are scarce. Many studies use questionnaires without examining if self-report corresponds to observable behaviour. We assessed the perceived mother-child relationship using the Parenting Relationship Questionnaire and the Child Relationship Behaviour Inventory in three groups: (1) mothers with BPD, (2) mothers with anxiety and/or depression and (3)

Social Determinants of Health and Child Welfare Outcomes in NCANDS Data.

Social Determinant of Health (SDoH) Indices, such as the Social Vulnerability Index (SVI) or the Area Deprivation Index (ADI), are increasingly used in child maltreatment research either to explore the degree which SDoH is associated with maltreatment or as a control variable to more accurately understand other associations of interest. To date, however, there have been no published explorations of which SDoH measures are most predictive of child maltreatment outcomes. This paper compares a range of publicly available SDoH measures in terms of their ability to explain variability in rates in N

Parental neurotic excoriation injury of children: A case series of hidden physical abuse.

BACKGROUND: Recognizable skin findings of child physical abuse include bruises, abrasions, lacerations, bite marks, burns, and oral injuries. Self-induced skin picking, or neurotic excoriation, can correspond to emotional stress, underlying psychiatric illness, or substance abuse. Parental neurotic excoriation injury of children has not been reported previously as a form of physical abuse. METHODS: We present a case series of five children abused via parental excoriation. All affected children were three years of age or younger and otherwise healthy. Each child presented with wounds determine

Diagnosing attention-deficit hyperactivity disorder (ADHD) in children involved with child protection services: are current diagnostic guidelines acceptable for vulnerable populations?

Children involved with child protection services (CPS) are diagnosed and treated for attention-deficit hyperactivity disorder (ADHD) at higher rates than the general population. Children with maltreatment histories are much more likely to have other factors contributing to behavioural and attentional regulation difficulties that may overlap with or mimic ADHD-like symptoms, including language and learning problems, post-traumatic stress disorder, attachment difficulties, mood disorders and anxiety disorders. A higher number of children in the child welfare system are diagnosed with ADHD and pr
