Development of calcitonin gene-related peptide slow-release tablet implanted in CSF space for prevention of cerebral vasospasm after experimental subarachnoid haemorrhage.

The calcitonin gene-related peptide (CGRP), a known potent intrinsic cerebral vasodilator, is contained in the sensory nerves from trigeminal ganglia that inervate the cerebral arteries. We previously reported that human alpha CGRP (hCGRP) dilates spastic cerebral arteries after experimental subarachnoid haemorrhage (SAH) in rabbits. In the present study, we investigated the prophylactic potential of a sustained higher cerebrospinal fluid level of hCGRP against experimental cerebral vasospasm. An hCGRP slow-release tablet (hCGRP s-r tablet) was developed for cisternal implantation. Experimenta

Cerebral aterial spasm. I. Adrenergic mechanism in experimental cerebral vasospasm.

This study demonstrates that an adrenergic mechanism plays an important role in producing the delayed cerebral vasospasm which follows subarachnoid hemorrhage. Results were as follows: 1. Experimental subarachnoid hemorrhage (SAH) was produced by injection of fresh arterial blood into the cisterna magna in cats. The cerebral vasospasm was shown angiographically to be biphasic in nature: immediate constriction lasting 1 h and marked prolonged spasm occurring between the 3rd and 5th day after SAH. The amount of noradrenaline (NA) and dopamine-beta-hydroxylase (DBH) activity decreased over a peri

[Pharyngeal pain during lateral suboccipital puncture].

Oropharyngeal pain was referred by five fully conscious and collaborative patients during lateral suboccipital puncture (LSP) of the cisterna magna, performed for myelography and/or cerebrospinal fluid collection. The anastomotic connections between the lower cranial nerves, the sympathetic nerves and the upper cervical spinal nerves are reviewed, with emphasis on the relationship between the first cervical nerve (C1), the superior cervical ganglion of the sympathetic trunk, and the spinal accessory nerve (nerve XI), and their central connections. The authors conclude that pharyngeal pain duri

How I do it: microvascular decompression for vago-glossopharyngeal neuralgia.

When a neurovascular conflict is demonstrated on high-resolution MRI in patients with drug-refractory Vago-Glossopharyngeal neuralgia microvascular decompression is regarded as the most effective therapeutic option. The aim of surgery is to relieve the conflict, most often produced by the posterior inferior cerebellar artery, not infrequently in association with a compressive megadolicho-vertebrobasilar artery. This can be safely achieved by exposure of the Vago-Glossopharyngeal nerve complex through a retromastoid-retrosigmoid infrafloccular approach through the cisterna magna, allowing expos

From mechanism to classification: Understanding a novel model of cerebral small vessel disease.

The studies explored cerebral small vessel disease (cSVD), emphasizing the need for precise classification to improve prevention and intervention strategies. Kang et al. introduced an intra-cisterna-magna bevacizumab injection (ICM-BI) model in mice, which induced tight junction loss, microbleeds, and amyloid deposits. However, bevacizumab's low affinity for murine vascular endothelial growth factor raises questions about its mechanism of action, suggesting potential off-target effects. While most cSVD models mimic arteriolosclerosis (type 1) or genetic variants (types 2 and 3), the ICM-BI mod

Updated Imaging Findings in Congenital Zika Syndrome: A Disease Story That is Still Being Written.

In congenital Zika virus syndrome (CZS), the most frequent radiological findings are calcifications in the cortical-white matter junction and malformations of cortical development (pachygyria or polymicrogyria, which occur predominantly in the frontal lobes, or a simplified gyral pattern), ventriculomegaly, enlargement of the cisterna magna and the extra-axial subarachnoid space, corpus callosum abnormalities, and reduced brain volume. This syndrome can also result in a decrease in the brainstem and cerebellum volumes and delayed myelination. Infants with CZS may show venous thrombosis and len

Posterior fossa malformations.

Understanding embryologic development of the cerebellum and the 4th ventricle is essential for understanding posterior fossa malformations. Posterior fossa malformations can be conveniently classified into those that have a large posterior fossa and those with normal or small posterior fossa. Disorders associated with a large posterior fossa include classic Dandy-Walker malformation, Blake's pouch cyst, mega cisterna magna, and posterior fossa arachnoid cyst. Disorders associated with normal or small posterior fossa include Dandy-Walker variant, Joubert syndrome, tecto-cerebellar dysraphia, rh

Dry Mouth Caused by Facial Nerve Ischemia due to Subarachnoid Hemorrhage: An Experimental Study.

OBJECTIVE: Parasympathetic network damage results in facial nerve damage, sublingual ganglion degeneration, sublingual gland dysfunction, and dry mouth. In this study, subarachnoid hemorrhage (SAH) was considered to be the cause of dry mouth. METHODS: We assessed 23 hybrid rabbits, including 5 control (group 1, Control). One milliliter of serum saline was injected into the cisterna magna of 5 animals (group 2). SAH was induced by injecting 1 mL of autologous blood into the cisterna magna of 13 animals (group 3). The animals were killed after 3 weeks of induction. The animals' sublingual gangl

Ukrain, a plant derived semi-synthetic compound, exerts antitumor effects against murine and human breast cancer and induce protective antitumor immunity in mice.

OBJECTIVE: To characterize and establish a reproducible model that demonstrates delayed cerebral vasospasm after aneurysmal subarachnoid hemorrhage (SAH) in rats, in order to identify the initiating events, pathophysiological changes and potential targets for treatment. METHODS: Twenty-eight male Sprague-Dawley rats (250 - 300 g) were arbitrarily assigned to one of two groups - SAH or saline control. Rat subarachnoid hemorrhage in the SAH group (n=15) was induced by double injection of autologous blood, 48 hr apart, into the cisterna magna. Similarly, normal saline (n=13) was injected into th

Effects of oxyhemoglobin in vitro in cerebral arteries from normal animals and animals subject to subarachnoid hemorrhage or indomethacin treatment.

Experiments were performed to test the hypothesis that subarachnoid hemorrhage (SAH) causes functionally relevant perturbations of cyclooxygenase activity in cerebral arteries. Four groups of rabbits were formed: (I) controls; (II) sham injected animals (2 ml physiological solution in the cisterna magna); (III) SAH group (2 ml blood in cisterna magna); (IV) indomethacin group (4 mg/kg i.v. 30 min before sacrifice). Animals of groups II and III were used 3 days after injection. The basilar arteries (BAs) were removed and perfused at a constant flow rate (after electrocoagulation of all branches

Release of beta-endorphin immunoreactivity into ventriculo-cisternal perfusate by lumbar intrathecal capsaicin in the rat.

A model employing perfusion of artificial cerebrospinal fluid from the lateral ventricle to the cisterna magna in the halothane anesthetized rat was used to study beta-endorphin release in the brain. Injection of 75 micrograms capsaicin into the lumbar intrathecal space released beta-endorphin immunoreactivity into perfusate. The release was blocked by intrathecal pretreatment with 1.25 mg lidocaine and the capsaicin receptor antagonist capsazepine (92 micrograms), showing that the release is caused by binding of capsaicin to a spinal receptor. The release was also blocked by intrathecal pretr

[Megacisterna magna as incidental finding in a boy with ADHD].

In this case report we describe how a 13-year-old boy with a complex development profile was diagnosed with adhd and who was also found to have a megacisterna magna, a posterior fossa anomaly in the Dandy-Walker continuum. We searched the literature for reports of other patients who had this (mild) brain anomaly along with psychiatric problems in general and attention problems in particular. Our search of the literature suggested a possible link between the two diagnostic entities.

Microsurgical and endoscopic anatomy of the supratentorial arachnoidal membranes and cisterns.

OBJECTIVE: A limitation of previous studies of the arachnoid cisterns and membranes is that the act of opening the sylvian and interhemispheric fissures and basal arachnoid often led to destruction of the cisternal compartments and their membranous walls. The goal of this study was to overcome this limitation by combining the surgical microscope and endoscope for the examination of the cisternal compartments and their membranous walls. METHODS: The supratentorial cisterns were examined in 22 cadaveric brains using both the operating microscope and the endoscope. RESULTS: There are 2 types of

[Less invasive cistemal approach and removal of subarachnoid hematoma for the treatment of ruptured cerebral aneurysms].

The risk of pial or brain injury while dissecting the cerebral cistern to treat acute subarachnoid hemorrhage might be higher than that of unruptured aneurysms, because visualizing the arachnoid trabeculae between the pia mater and the vessels can be complicated by thick subarachnoid clots filling the cerebral cistern. The author describes technical points of dissecting the cerebral cistern and the removal of subarachnoid hematomas during acute surgical treatment for subarachnoid hemorrhage. Tough arachnoid trabeculae form a "perivascular cistern" around the relatively major vessels in both th

High-resolution MR cisternography of the cerebellopontine angle, obtained with a three-dimensional fast asymmetric spin-echo sequence in a 0.35-T open MR imaging unit.

High-resolution MR cisternography performed with 3D fast asymmetric spin-echo imaging (3D fast spin-echo with an ultra-long echo train length and asymmetric Fourier imaging) was optimized in a 0.35-T open MR imaging unit. The 0.35- and 1.5-T images of the two volunteers and three patients with acoustic schwannomas were then compared. The optimal parameters for images obtained by 3D fast asymmetric spin-echo imaging at 0.35 T were as follows: field of view, 15 cm; matrix, 256 x 256 x 40; section thickness, 1 mm; echo train length, 76; and imaging time, 10 minutes 44 seconds. Ventriculocisternal cerebrospinal perfusion in unanesthetized fetal lambs.

Methods have been developed for the placement of cannulas in the lateral cerebral ventricle and cisternum magnum of the fetal lamb. Three to seven days after surgery, perfusion of the cerebral ventricular system was carried out in unanesthetized lambs in utero. Artificial cerebrospinal fluid (CSF) was perfused at 123 microliter/min for 4-5 h with no change in fetal arterial O2 pressure, CO2 pressure, pH, mean arterial blood pressure, or heart rate. The preparation is suitable for measurements of net formation of fetal central CSF and also for producing changes in the ionic composition of the f

Metrizamide and radionuclide cisternography in communicating hydrocephalus.

Simultaneous 111In-DPTA and metrizamide CT cisternography correlated closely in the qualitative imaging of cerebrospinal fluid (CSF) dynamics in 9 normal patients and in 11 patients thought to have communicating hydrocephalus. CSF clearance of both tracers was similar; significant absorption occurred in the spinal dural sac. Although delayed elevated serum iodine levels correlated highly with abnormal persistent ventricular penetration (detection at 48 hours), these levels cannot be used to predict abnormal ventricular stasis. The pattern of ventricular and cortical sulcus enlargement on the p
