Cardiac herniation post left upper lobectomy and thymectomy: a case report.

BACKGROUND: Cardiac herniation occurs when there is a residual pericardial defect post thoracic surgery and is recognised as a rare but fatal complication. It confers a high mortality and requires immediate surgical correction upon recognition. We present a case of cardiac herniation occurring post thymectomy and left upper lobectomy. CASE PRESENTATION: Initial presentation: A 48-year-old male, hypertensive smoker presented with progressive breathlessness and was found to have a left upper zone mass confirmed on CT biopsy as carcinoid of unclear origin. PET-CT revealed avidity in a left anter

Waitlist and transplant outcomes for patients bridged to heart transplantation with Impella 5.0 and 5.5 devices.

BACKGROUND: Impella devices are increasingly utilized as a bridge to heart transplantation (BTT) and are now prioritized as Status 2 under the current heart allocation policy. This study evaluated waitlist and post-transplant outcomes of patients supported with Impella 5.0/5.5 devices. METHODS: The United Network of Organ Sharing registry was used to identify adults waitlisted or transplanted with Impella 5.0 or 5.5 devices from 2010 to 2021. Separate analyses were performed for waitlist and transplantation outcomes for patients supported by Impella 5.0/5.5 devices. Competing outcomes for the

Development and Validation of a Deep Learning Model for Prediction of Adult Physiological Deterioration.

BACKGROUND: Prediction-based strategies for physiologic deterioration offer the potential for earlier clinical interventions that improve patient outcomes. Current strategies are limited because they operate on inconsistent definitions of deterioration, attempt to dichotomize a dynamic and progressive phenomenon, and offer poor performance. OBJECTIVE: Can a deep learning deterioration prediction model (Deep Learning Enhanced Triage and Emergency Response for Inpatient Optimization [DETERIO]) based on a consensus definition of deterioration (the Adult Inpatient Decompensation Event [AIDE] crit

Time to detection of serious adverse events by continuous vital sign monitoring versus clinical practice.

BACKGROUND: Continuous vital sign monitoring detects far more severe vital sign deviations (SVDs) than intermittent clinical rounds, and deviations are to some extent related to subsequent serious adverse events (SAEs). Early detection of SAEs is pivotal to allow for effective interventions but the time relationship between detection of SAEs by continuous vital sign monitoring versus clinical practice is not well-described at the general ward. AIM: To quantify the time difference between detection of SAEs by continuous vital sign monitoring and clinical suspicion of deterioration (CSD) in maj

Massive Ischemic Stroke Due to Pulmonary Barotrauma and Cerebral Artery Air Embolism During Commercial Air Travel.

BACKGROUND Air embolism into the systemic arterial circulation secondary to pulmonary barotrauma has rarely been reported. Herein, we report the clinical course of an extremely rare presentation of cerebral air embolism likely due to ruptured pulmonary bullae during commercial air travel. CASE REPORT A 65-year-old man suddenly became unconscious during an airplane descent. Upon landing, he was immediately transferred to the nearest emergency department where he was intubated for airway protection. His head CT angiogram showed multiple air pockets in the right parietal lobe suspicious for multi

Prognostic Value of the Systemic Immune-Inflammation Index (SII) for short-term clinical outcomes in the emergency department: a retrospective cohort study.

BACKGROUND: The Systemic Immune-Inflammation Index (SII = PLT × NE# / LY#) is a composite haematological biomarker derivable from a routine complete blood count (CBC) that integrates neutrophil, platelet, and lymphocyte counts into a single value reflecting systemic inflammatory status. OBJECTIVE: Studies simultaneously evaluating SII across multiple concurrent outcomes in an unselected ED population remain limited. We aimed to evaluate SII's independent prognostic value for hospitalisation, ICU admission, clinical deterioration, in-hospital mortality, and 72-hour ED return, and its increment

The use of the modified Dutch worry criteria, combined with vital signs and early warning scores, to improve nurses' recognition and escalation of ward deteriorating patients: A prospective cross-sectional observation multi-method study.

AIM: To determine if a modified nurse worry criteria, combined with vital signs and early warning scores: 1) improved nurses' ability to anticipate, recognise, prioritise, and escalate deteriorating patients and organise treatment and care; and 2) reduced the number of medical emergency team escalations. METHODS: A prospective observational multi-method research design collected data using: 1) a questionnaire assessing the impact the modified nurse worry criteria had on nurses' response to patient deterioration, and 2) the critical care outreach data-base to determine a change in the number o

Treatment of early neurological deterioration in lacunar stroke.

OBJECTIVE: Early neurological deterioration (END) in lacunar infarcts is a common complication, particularly during the first days of hospitalization, and still lacks a standardized therapeutic approach. This study aimed to describe the clinical presentation, predictive factors, and therapeutic strategies implemented in these cases, as well as to assess their impact on patients' functional outcomes. PATIENTS AND METHODS: We conducted a single-center, retrospective observational study including 269 patients admitted with a diagnosis of lacunar stroke between 2013 and 2023. END was defined as c

Paricalcitol as an Antiproteinuric Agent Can Result in the Deterioration of Renal and Heart Function in a Patient with Fabry Disease.

BACKGROUND Fabry disease is a rare and progressive X-linked inherited disorder of glycosphingolipid metabolism that is due to deficient or absent lysosomal a-galactosidase A activity. Among its other associated signs and symptoms, patients present with renal failure and proteinuria, which are markers of disease progression. Renin-angiotensin-aldosterone system (RAAS) blockers can slow the progression of chronic renal failure and proteinuria. In fact, some studies have shown the beneficial effects of paricalcitol on proteinuria. CASE REPORT We present a case of a female patient with the classic

In-Flight Deterioration Occurs Early in Aeromedical Trauma Patients.

Reliably defining the risk of adverse in-flight events in aeromedical trauma patients could enable more informed pre-departure treatment and guide central asset allocation to achieve better system-level outcomes. Unfortunately, the current literature base specifically examining the in-flight period is sparse. Flight duration is often considered a proxy for the risk of in-flight deterioration; however, there is limited data to support this commonly held assumption. This paper examines the association between flight duration and the risk of in-flight deterioration in aeromedical trauma patients.

Safety of High-Flow Nasal Cannula Outside the ICU for Previously Healthy Children With Bronchiolitis.

BACKGROUND: High-flow nasal cannula (HFNC), a form of noninvasive respiratory support, is effective for the treatment of respiratory distress in ICUs. Although HFNC has been used outside of the ICU, there is little research that examines its safety in this less-monitored setting. METHODS: Children ≤ 24 months old admitted with bronchiolitis to a pediatric floor at a tertiary care center from April 1 2013, to March 31 2015, were identified by using standard diagnostic codes. Exclusion criteria were concomitant pneumonia or complex comorbidities. Demographic and clinical characteristics were ab

Early warning- and track and trigger systems for newborn infants: A review.

Tools for clinical assessment and escalation of observation and treatment are insufficiently established in the newborn population. We aimed to provide an overview over early warning- and track and trigger systems for newborn infants and performed a nonsystematic review based on a search in Medline and Cinahl until November 2015. Search terms included 'infant, newborn', 'early warning score', and 'track and trigger'. Experts in the field were contacted for identification of unpublished systems. Outcome measures included reference values for physiological parameters including respiratory rate a

Accuracy of Intensivist Prognostications of Deterioration within the Intensive Care Unit and Development of Persistent Critical Illness: A Prospective Cohort Study.

Rationale: Persistent critical illness (PerCI) is costly, increasing in incidence, and not reliably predicted with existing risk-prediction tools. Objective: To assess whether attending intensivists can identify patients at heightened risk of developing PerCI. Methods: We conducted a prospective longitudinal assessment from August 2020 to January 2023. Intensivists were assessed on each patient within 24 hours of admission to the medical intensive care unit (ICU) and on ICU Day 3. We measured intensivists' prognostication of within-ICU events (late-onset shock and/or acute hypoxic respiratory

Frequency of medical emergency team activation prior to pediatric cardiopulmonary resuscitation.

BACKGROUND: Medical Emergency Teams (METs) are designed to respond to signs of clinical decline in order to prevent cardiopulmonary arrest and reduce mortality. The frequency of MET activation prior to pediatric cardiopulmonary resuscitation (CPR) is unknown. METHODS: Within the Get With The Guidelines-Resuscitation Registry (GWTG-R), we identified children with bradycardia or cardiac arrest requiring CPR on the general inpatient or telemetry floors from 2007 to 2013. We examined the frequency with which CPR outside the ICU was preceded by a MET evaluation. In cases where MET evaluation did n

Empirical Antimicrobial Therapy of Neonates with Necrotizing Enterocolitis: A Systematic Review.

OBJECTIVE: Necrotizing enterocolitis (NEC) is an inflammatory disease of the gastrointestinal tract characterized by ischemic necrosis of the intestinal mucosa, mostly affecting premature neonates. Management of NEC includes medical care and surgical approaches, with supportive care and empirical antibiotic therapy recommended to avoid any disease progression. However, there is still no clear evidence-based consensus on empiric antibiotic strategies or surgical timing. This study was aimed to review the available evidence on the effectiveness and safety of different antibiotic regimens for NEC
