Antibiotic resistance is one of the world's greatest public health challenges and adjunct probiotic therapies are strategies that could lessen this burden. Clostridioides difficile infection (CDI) is a prime example where adjunct probiotic therapies could decrease disease incidence through prevention. Human-derived Lactobacillus reuteri is a probiotic that produces the antimicrobial compound reuterin known to prevent C. difficile colonization of antibiotic-treated fecal microbial communities. However, the mechanism of inhibition is unclear. We show that reuterin inhibits C. difficile outgrowth

BACKGROUND: Screening for Clostridioides difficile (CD) colonization can be performed using molecular testing to identify the presence of microbial DNA of the toxin gene. Colonization rates for hospitalized patients are as high as 20% and may be considerably higher in solid organ transplant (SOT) recipients. Treatment for CD should be based on clinical disease and not colonization, yet clinicians may misinterpret a positive CD screen resulting in overtreatment.
OBJECTIVES: The objective of this analysis is to determine how often positive CD screens resulted in inappropriate treatment with ora

Background. A 76-year-old male patient who suffered small bowel anastomotic dehiscence believed to be a complication provoked by Clostridioides difficile enteritis. Case Presentation. The patient was a 76-year-old male who underwent small bowel resection with primary anastomosis for a small bowel obstruction. On postoperative day #7, he rapidly decompensated and upon return to the operating room was found to have complete anastomotic dehiscence with copious enteric spillage. The presentation appeared as if the staple line had burst open. Enteric contents confirmed the diagnosis of Clostridioid

IMPORTANCE: Clostridioides difficile infection is the most frequent health care-associated infection in the United States. However, exposure to this organism might occur outside the health care setting.
OBJECTIVE: To examine whether exposure to environmental factors, such as livestock farms, is associated with a higher probability of being colonized with C difficile at hospital admission.
DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study was conducted from May 1, 2017, to June 30, 2018, at a teaching-affiliated hospital in Milwaukee, Wisconsin. All consecutive patients under

BACKGROUND & AIMS: The protease plasmin is an important wound healing factor, but it is not clear how it affects gastrointestinal infection-mediated damage, such as that resulting from Clostridioides difficile. We investigated the role of plasmin in C difficile-associated disease. This bacterium produces a spore form that is required for infection, so we also investigated the effects of plasmin on spores.
METHODS: C57BL/6J mice expressing the precursor to plasmin, the zymogen human plasminogen (hPLG), or infused with hPLG were infected with C difficile, and disease progression was monitored.

We describe 9 patients at a medical center in Detroit, Michigan, USA, with severe acute respiratory syndrome coronavirus 2 and Clostridioides difficile. Both infections can manifest as digestive symptoms and merit screening when assessing patients with diarrhea during the coronavirus disease pandemic. These co-infections also highlight the continued importance of antimicrobial stewardship.

Antibiotic drug-mediated changes in the human gut microbiota affect Clostridioides difficile colonization. We created a compartmental model of C. difficile colonization in nursing home residents to improve understanding of how the gut microbiota mediates the transmission process. When analyzing C. difficile colonization as a function of the daily percentage of the nursing home resident population on antibiotic drugs, the model displayed increases in C. difficile colonization as antibiotic drug use increased. Reducing the daily percentage of the nursing home resident population receiving antibi

Clostridioides difficile infection (CDI) is a major cause of antibiotic-associated diarrhea and colitis, driven by toxin-mediated epithelial injury and inflammation. While antibiotics such as vancomycin remain the primary treatment, they can further disrupt the gut microbiota and promote recurrence. Probiotics, including yeast strains, have emerged as potential adjunctive therapies for mitigating CDI. In this study, several Saccharomyces cerevisiae strains were evaluated for their probiotic potential, and strain 48338 was identified as the most promising candidate based on its gastrointestinal

Clostridioides difficile infection (CDI) is a major global health concern driven by the virulence factors toxins A (TcdA) and B (TcdB). Vaccination targeting these toxins, particularly via the oral mucosal route, represents a promising preventive strategy. This study investigated the efficacy of genetically engineered probiotics Lactobacillus gasseri NM0323 and Enterococcus faecium NM0423 as oral vaccine carriers expressing the C-terminal domain of the tcdB gene. Mice were orally immunized, boosted, and subsequently challenged with C. difficile. Quantitative PCR analysis revealed a significant

BACKGROUND: Community-acquired Clostridioides difficile infection (CDI) represents an increasing proportion of cases in the United States, yet contemporary data describing epidemiology and management in the emergency department (ED) are limited. We evaluated national trends in CDI incidence, antibiotic treatment, and outpatient medication exposures among ED patients.
METHODS: We conducted a retrospective cohort study using the TriNetX Global Collaborative Network (163 healthcare organizations). ED encounters with CDI from 2014 to 2024 were identified, with community-onset CDI defined as an IC

Clostridioides difficile infection (CDI) typically presents with diarrhea in elderly or antibiotic-exposed patients. We report a 40-year-old immunocompetent man with alcohol use disorder who presented with abdominal pain and jaundice, rapidly progressing to refractory constipation and colonic dilation (14.7 cm) mimicking acute colonic pseudo-obstruction. Conservative management including bowel regimens and neostigmine (generally contraindicated in suspected toxic megacolon) were attempted. Colonoscopic decompression revealed cecal pseudomembranes whereas rest of colon was grossly normal. Stool

BACKGROUND AND AIMS: Recurrent Clostridioides difficile infection (rCDI) results from persistent microbiome dysfunction and impaired colonization resistance. Although fecal microbiota transplantation (FMT) is effective, defined and scalable alternatives are needed. We evaluated whether a single commensal strain could restore key microbiome functions and prevent recurrence.
METHODS: We assessed Faecalibacterium prausnitzii EXL01 in a murine CDI model and a multicenter, open-label single-arm phase I trial including adults with ≥3 CDI episodes. Following vancomycin preconditioning, patients rece

BACKGROUND: Asymptomatic carriers significantly influence the transmission dynamics of C. difficile. This study aimed to assess the prevalence of toxigenic C. difficile asymptomatic colonization (tCDAC) and investigate its heterogeneity across different populations. We searched MEDLINE, Web of Science, and Scopus for articles published between 2000 and 2023 on tCDAC. Studies including asymptomatic adults with laboratory-confirmed tCDAC were eligible. We performed a random-effects meta-analysis to estimate the pooled prevalence by clinical characteristics, settings, and geographic areas. In add

BACKGROUND AND OBJECTIVES: Patients with recurrent Clostridioides difficile infection (rCDI) suffer long after infection. However, knowledge is incomplete regarding their perceptions of in-hospital care, antibiotic treatment and the most common risk factors for rCDI. Valid information processes are crucial for enabling such patients to avoid the suboptimal use of healthcare services and gain health literacy, good health and quality of life. This study aims to explore rCDI patients' perceptions of the causes of rCDI, information processes and strategies for the future.
METHODS: Ten interviews,

Bile acids, the primary components of bile, are cholesterol-derived molecules synthesized in the liver and secreted to the small intestine. Besides their primary digestive roles, bile acids have antimicrobial properties and serve as an environmental cue for intestinal pathogens, modulating the expression of virulence factors, e.g., toxins and effector proteins. Whereas timely recognition and neutralization of pathogenic toxin effectors by the host is critical, our understanding of the effects of bile on their structure and function is limited. In this work, we found that bile effectively prote

The dipeptide D-Ala-D-Ala is an essential component of peptidoglycan and the target of vancomycin. Most Clostridioides difficile strains possess the vanG operon responsible for the synthesis of D-Ala-D-Ser, which can replace D-Ala-D-Ala in peptidoglycan. The C. difficile vanG operon is regulated by a two-component system, VanRS, but is not induced sufficiently by vancomycin to confer resistance to this antibiotic. Surprisingly, in the absence of the VanS histidine kinase (HK), the vanG operon is still induced by vancomycin and also by another antibiotic, ramoplanin, in a VanR-dependent manner.
