Ocular manifestations of systemic inflammatory diseases.

Inflammation of the eye is often times seen in association with systemic inflammatory diseases. Understanding the various forms of ocular involvement in these conditions is important as untreated ophthalmic involvement can lead to severe vision loss. In addition to providing a basic framework for diagnosis and treatment, this review will highlight the ocular manifestations of the following systemic inflammatory conditions: rheumatoid arthritis, systemic lupus erythematosus, Wegener's granulomatosis, Sjögren's syndrome, polyarteritisnodosa, primary antiphospholipid syndrome, Behçet's syndrome,

[Posner-Schlossman syndrome].

The clinical features of Posner-Schlossman syndrome and the various differential diagnoses are described in a 20-year-old patient. Additional to Posner-Schlossman syndrome this patient showed signs of Axenfeld's anomaly. He also presented symptoms of a status-dysrhaphicus-like state after atrial septal defect and the ability to overstrain the distal finger joints. Posner-Schlossman syndrome, heterochromic cyclitis, anterior-chamber cleavage syndrome and ICE syndrome (iridocorneal-endothelial syndrome: Chandler's, Cogan-Reese syndrome and progressive essential iris atrophy) often produce simila

Immune-mediated conditions affecting the brain, eye and ear (BEE syndromes).

The triad of central nervous system symptoms, visual disturbance and hearing impairment is an oft-encountered clinical scenario. A number of immune-mediated diseases should be considered among the differential diagnoses including: Susac syndrome, Cogan syndrome or Vogt-Koyanagi-Harada disease; demyelinating conditions such as multiple sclerosis or neuromyelitis optica spectrum disorder; systemic diseases such as systemic lupus erythematosus, Sjögren syndrome or Behcet disease and granulomatous diseases such as sarcoidosis. In this article, we coin the term 'BEE syndromes' to draw attention to

Inhibition of the TNF-pathway: use of infliximab and etanercept as remission-inducing agents in cases of therapy-resistant chronic inflammatory disorders.

OBJECTIVE: To examine the potential of the two tumour necrosis factor (TNF) inhibitors infliximab and etanercept as remission-inducing agents in chronic therapy-resistant inflammatory disorders of immune or non-immune pathogenesis. METHODS: 14 patients with adult Still's disease/macrophage activation syndrome (4), Wegener's disease (3), Behçet's disease (3), keratoscleritis (1), lymphomatous tracheo-bronchitis (1) Cogan's syndrome (1), and rapidly destructive crystal arthropathy (1) were treated with infliximab (n = 10) and etanercept (n = 4). All patients showed organ-threatening progression

[Histological findings in dysgenesis mesodermalis iridis et corneae Rieger].

The histological examination of an enucleated eye with dysgenesis Rieger showed microcornea, broad anterior synechia, hypoplasia and atrophia of the stroma of the iris as well as of the ciliary body. Excessive proliferations of the endothel combined with the formation of thick glass membranes allow the assumption that the dysgenesis Rieger, the essential progressive atrophy of the iris, the Chandler and Cogan-Reese syndrome are related in their aetiopathogenesis and are only variations of the same proliferative endotheliopathy.

[Hearing loss in autoimmune diseases].

Several autoimmune disorders are associated with inner ear involvement. The resulting sensorineural loss of hearing may be improved by treatment with immunosuppressive agents. We describe a patient with oral ulcerations, systemic vasculitis, iridocyclitis and bilateral sudden deafness. After a course of systemic steroid treatment the hearing improved. Atypical Cogan's syndrome is the most likely diagnosis.

Benign episodic bilateral juvenile internal ophthalmoplegia.

A case of benign episodic bilateral juvenile internal ophthalmoplegia in a nine-year-old female is presented. Two separate episodes of sudden onset of bilateral mydriasis and accommodative loss accompanied by headache occurred, without any other physical, neuroophthalmic, or third nerve involvement. Complete resolution occurred over a period of months in both episodes. The differential diagnosis of isolated internal ophthalmoplegia is reviewed, and the episodic unilateral mydriasis syndrome of Hallett and Cogan appears to be similar to the bilateral involvement in the case presented.

Mutational analysis of the NPHP4 gene in 250 patients with nephronophthisis.

Nephronophthisis (NPH), a recessive cystic kidney disease, is the most frequent genetic cause for end-stage renal disease in the first two decades of life. Mutations in three genes (NPHP1, 2, and 3) were identified as causative. Extrarenal manifestations are known, such as retinitis pigmentosa (Senior-Loken syndrome, SLS) and ocular motor apraxia type Cogan. Recently, we identified a novel gene (NPHP4) as mutated in NPH. To date, a total of only 13 different NPHP4 mutations have been described. To determine the frequency of NPHP4 mutations, we performed mutational analysis by direct sequencing

[Iridocorneal endothelial syndrome. Series of 7 cases].

The ICE syndrome includes 3 variants of the same disease, previously believed to be independent entities. All 3 share clinical and histopathological features such as endothelial abnormalities, iris damage, and progressive closure of the iridocorneal angle. Seven cases of ICE syndrome are reported, affecting 6 women and 1 man, with a mean age of 40 years. Corectopia was found in 5 eyes, a uveal ectropion in 2 cases, and other iris changes in 3 cases. The endothelial changes of the cornea were confirmed by a speculum microscopy in 5 cases. There were 5 cases of Chandler syndrome, 1 of Cogan-Rees

Approach to Oculomotor Apraxia: A Syndromic Approach to Genetic Causes.

BACKGROUND: Oculomotor apraxia (OMA), the clinical manifestation of impaired voluntary initiation of saccadic eye movements, has long been associated with several disorders and genetic mutations in the literature. OBJECTIVES: The present study aims to review all the disorders and genetic mutations associated with OMA reported in the literature. METHODS: PubMed, MEDLINE, Scopus, EMBASE, and Web of Science databases were systematically searched for related keywords, and related publications from January 2000 to January 2024 were reviewed. RESULTS: All the disorders and genetic mutations prese

[Congenital oculomotor apraxia with corpus callosum agenesis and subtentorial atrophy].

A 7 year-old child with congenital oculomotor apraxia (Cogan's syndrome) presented with mainly voluntary horizontal saccadic movements. Fixation of a visual target could be obtained only by a sudden movement of the head preceded by eyelid blinking. Electro-oculography confirmed the predominant disturbance of saccadic movements (delay in onset, hypometria). CT scan showed agenesis of the corpus callosum and atrophy of the vermis together with enlargement of the 4th ventricle and pre-pontine cisternae. The role of these anomalies in the pathogenesis of this rare congenital syndrome of good progn

Netarsudil-Induced Honeycomb Keratopathy in an Eye With Iridocorneal Endothelial Syndrome.

PURPOSE: To describe a case of netarsudil-associated honeycomb keratopathy in a patient with iridocorneal endothelial (ICE) syndrome. OBSERVATIONS: A 43-year-old male with a history of Cogan-Reese variant ICE syndrome presented with ocular irritation, foreign body sensation, blurred vision, photosensitivity, and discharge in the right eye 2 weeks after initiating topical netarsudil 0.02% for secondary angle-closure glaucoma. Slit lamp exam and anterior segment optical coherence tomography demonstrated diffuse reticular epithelial cysts, consistent with netarsudil-induced honeycomb keratopathy

Otolaryngological manifestations of rheumatic diseases in children.

Rheumatic diseases represent a group of autoimmune conditions which primarily affect the musculo-skeletal system but can also involve other internal organs such as the auditory and the respiratory systems. Among the rheumatic diseases of children those which present an otolaryngological involvement at disease onset or during their course are essentially juvenile idiopathic arthritis (JIA), Cogan syndrome (CS), relapsing polycondritis (RPC) and Wegener granulomatosis (WG). In this section, we will review the main characteristics of these conditions with the attempt to propose a few elements for

[Diagnosis of aortitis].

Aortitis can be a component of a variety of diseases, such as Takayasu arteritis, giant cell arteritis, Behçet's syndrome, Cogan's syndrome, spondylarthropathies, rheumatoid arthritis, systemic lupus erythematosus, sarcoidosis, Erdheim-Chester's disease and a variety of infectious processes like syphilis, Salmonella and others. The presentation is variable: aortic valve regurgitation, aneurysm, dissection, stenosis of the aorta or its initial branches. Sometimes systemic manifestations are at first presentation like fever or inflammatory syndrome. The differential diagnosis may be difficult in
