[The concept of futility in medicine].

A 79-year-old woman with a history of breast cancer was incidentally found to have a suspicious lung nodule. Though reluctant to accept treatments with disabling side effects, she was persuaded to undergo biopsy after oncologists stressed the low toxicity and potential benefit of EGFR inhibitors. The biopsy confirmed lung adenocarcinoma but caused complications, including pneumothorax and progressive dyspnea, without improving outcome. Her primary wish was to return home and limit hospital exposure, an expectation ultimately unmet. This case underlines the need to respect patient preferences,

[Neuropsychiatric coaching of an adult with Asperger syndrome].

Asperger syndrome is a lifelong neurodevelopmental condition. The major features of the syndrome include problems in social interaction and communication, narrow interests and stereotyped behaviour. Cognitive abilities are usually within normal. The syndrome potentially leads to a diminished level of life management in adulthood. Neuropsychiatric coaching is a solution-focused and practically oriented process of interventions for clients with neurodevelopmental problems. The methods include forms of evaluation and self reflection, structuring, guidance and visualization aids. Coaching does not

Propylthiouracil treatment reduces long-term potentiation in area CA1 of neonatal rat hippocampus.

Rat pups were made hypothyroid by exposure to propylthiouracil in drinking water beginning at 1 week of age, and the degree of long-term potentiation (LTP) in hippocampal area CA1 determined from brain slices of animals ranging in age from 2 to 6 weeks. Serum T3 levels were less than 20% of that of age matched controls after 3 weeks of treatment, and remained at that level. Relative to the age-matched controls, LTP was reduced significantly after 2 weeks of treatment. These observations are consistent with the conclusion that LTP magnitude is a reflection of cognitive function, which is known

Can grip strength capture what chair standing does? methodological considerations on functional measures and successful aging in three international cohorts.

Population aging has accelerated globally, making successful aging a key public health priority. Zhuang and Gao address the prospective association between cumulative average grip strength and successful aging using three international cohorts. While their multi-cohort design offers valuable cross-population insights, several methodological choices warrant consideration. Grip strength may not fully capture functional determinants of successful aging. Self-reported chronic disease introduces misclassification bias varying across socioeconomic groups. The two-stage temporal design does not fully

Maternal depressive symptomatology and mother-infant bonding difficulties: the mediating role of brooding rumination.

BACKGROUND: While the association between maternal depressive symptomatology and mother-infant bonding (MIB) difficulties is well established, the mechanisms underlying this relationship remain unclear. This study innovatively investigates the mediating role of rumination (considering its two dimensions - brooding and reflection), a key cognitive process in depression that has been rarely explored in the postpartum period. AIMS: To compare rumination and MIB levels between mothers with and without depressive symptoms and assess whether maternal depressive symptomatology is associated with MIB

Neuroetymology: bridging memorisation and meaning in medical education through narrative and critical reflection.

The vast and complex terminology of clinical neuroscience often drives students towards rote memorisation, yielding short-term recall but undermining long-term retention and the humanistic connection between words and meaning. To address this, we introduce neuroetymology as a formal, scalable pedagogical framework that differs from general medical etymology or narrative medicine by focusing specifically on the terminology of clinical neuroscience. It operationalises a set of teaching practices that link terminology to narrative, critical reflection and clinical application, transforming passiv

Neural signal variability relates to maladaptive rumination in depression.

Rumination is a common feature of depression and predicts the onset and maintenance of depressive episodes. Maladaptive and adaptive subtypes of rumination contribute to distinct outcomes, with brooding worsening negative mood and reflection related to fewer depression symptoms in healthy populations. Neuroimaging studies have implicated several cortical midline and lateral prefrontal brain regions in rumination. Recent research indicates that blood oxygen level-dependent (BOLD) signal variability may be a novel predictor of cognitive flexibility. However, no prior studies have investigated wh

Therapy chatbots and emotional complexity: do therapy chatbots really empathise?

Internet-based cognitive behavioural therapy (iCBT) and, more specific, therapy chatbots are at the heart of this article. We review studies about therapy chatbots in terms of risks, benefits, and 'empathetic design'. Research shows that these chatbots can relieve symptoms of stress, depression, and anxiety, by tracking emotions over time, encouraging self-reflection, and helping to gain insight into cognitive biases, among other things. However, several studies focus on the short term, have limited samples, and include self-report. While there are certainly benefits, risks such as privacy, da

Assessing prospective and retrospective metacognitive accuracy following traumatic brain injury remotely across cognitive domains.

The ability to monitor one's behaviour is frequently impaired following TBI, impacting on patients' rehabilitation. Inaccuracies in judgement or self-reflection of one's performance provides a useful marker of metacognition. However, metacognition is rarely measured during routine neuropsychology assessments and how it varies across cognitive domains is unclear. A cohort of participants consisting of 111 TBI patients [mean age = 45.32(14.15), female = 29] and 84 controls [mean age = 31.51(12.27), female = 43] was studied. Participants completed cognitive assessments via a bespoke digital platf

Personality traits and thinking dispositions: Individual differences call for 'smart' messaging.

This paper reveals potentially innate psychological differences to explain differing responses to the same information that cannot be explained by personal circumstances, such as age, financial situation and physical constraints. The study examines three well-known cognitive, psychological constructs as measures of these innate differences: cognitive reflection, need for cognitive closure and psychological function preferences. The data reveal who innately: (1) makes the best decisions without full information; (2) is the most/least receptive to simple, instructive messages; and (3) is the mos

Flexibility in abstract thought: Context-dependent representations of integer magnitude.

Negative numbers are challenging because they lack direct perceptual referents and require reasoning about magnitude in a relational, bi-directional manner. This review examines how people represent and use integer magnitude on the mental number line, a cognitive tool that learners initially use for positive numbers, focusing on the flexibility with which this tool is deployed across contexts. Synthesizing evidence from cognitive psychology, developmental science, and mathematics education, we evaluate three broad accounts: componential accounts, which treat sign and magnitude as separable sym

Hemispheric disconnection as a basis for neurodegeneration in apraxia and Alzheimer's disease.

INTRODUCTION: Apraxias, traditionally viewed as executive disorders secondary to focal lesions, are here reformulated as systemic outcomes of progressive interhemispheric disconnection-particularly relevant in the early stages of Alzheimer's disease. OBJECTIVE: To propose an integrated theoretical model that redefines apraxias from a neurosemiotic and embodied perspective, linking them to corpus callosum degeneration and the breakdown of sensorimotor gesture networks. METHODS: This contribution is based on a transdisciplinary conceptual review, integrating data from neuroscience, body semiot

Overcoming Barriers for Older Adults to Maintain Virtual Community and Social Connections during the COVID-19 Pandemic.

OBJECTIVES: We describe the evaluation of remote training, an innovative use of technology to maintain older adults' virtual connection with their community and socialization, which were disrupted by the pandemic. Remote training was conducted via telephone using principles of cognitive rehabilitation and delivered by trained clinicians. METHODS: We thematically analyzed trainer reflection notes and interviews with older adult participants. RESULTS: The main facilitators were technology training with exposure, and the main barrier was fear of technology. CONCLUSIONS: We describe how telepho

Progressive neuropsychiatric and brain abnormalities after smoke inhalation.

A 46-year-old man inhaled combustible smoke of unknown chemical composition for 15-20 min in an automobile body shop. Within 1 month, he noted headache, sadness, anergia, anhedonia, agitation, poor sleep and impairment of concentration, attention and learning skills. Three years later, mental status examination showed major depression and cognitive disorder manifested by apprehension, continuous sadness, agitation, exhaustion, difficulty with word finding, bradyphrenia, short-term and long-term memory impairment, and judgement impaired by impulsive and affect-laden reactions without ref

Lack of awareness of symptoms in people with dementia: the structural and functional basis.

OBJECTIVE: To review studies investigating the brain correlates of unawareness of cognitive and behavioural symptoms in people with dementia. DESIGN: A detailed search of the literature was conducted to include all the peer-reviewed studies published in English aimed at identifying the structural or functional brain correspondents of unawareness in dementia patients. Their results were interpreted in relation to the methodological differences in terms of type of dementia studied, the protocol adopted to measure lack of awareness, the imaging techniques employed, the experimental designs and s

Face to face: visual scanpath evidence for abnormal processing of facial expressions in social phobia.

Cognitive models of social phobia propose that cognitive biases and fears regarding negative evaluation by others result in preferential attention to interpersonal sources of threat. These fears may account for the hypervigilance and avoidance of eye contact commonly reported by clinicians. This study provides the first objective examination of threat-related processing in social phobia. It was predicted that hyperscanning (hypervigilance) and eye avoidance would be most apparent in social phobia for overt expressions of threat. An infrared corneal reflection technique was used to record visua

Paramagnetic Rim Lesions and Choroid Plexus Volume at Diagnosis Are Associated With Cognitive Progression Independent of Relapse and MRI Activity in Early Relapsing-Remitting Multiple Sclerosis.

Paramagnetic rim lesions (PRLs) and choroid plexus (CP) enlargement reflect smoldering inflammation in multiple sclerosis. Their role in cognitive progression remains unexplored. Eighty-seven early relapsing-remitting MS patients were enrolled at diagnosis and followed longitudinally. PRLs and CP volume were evaluated at diagnosis using 3 T-MRI. Cognitive decline over time was classified as cognitive progression independent of relapse/MRI activity (PIRMA). Fifty-five patients (63.2%) showed meaningful cognitive decline: 44 (80.0%) were cognitive PIRMA. PRLs and CP volume at diagnosis were asso

Adverse Events in Older Hospitalized Patients With Cognitive Impairment.

OBJECTIVES: As an increasing number of hospitalized inpatients are older and frail, cognitive impairment is becoming more common. Cognitive impairment may increase susceptibility to adverse events (AEs). This study aimed to identify the prevalence of AEs, potentially preventable AEs, and deaths in patients with and without cognitive impairment and the nature, causes, and prevention strategies. DESIGN: We analyzed data from 1959 records of a nationwide retrospective record review study of hospitalized older deceased patients. Cognitive insight and objective quality of life in people with schizophrenia and auditory hallucinations.

Poor cognitive insight in schizophrenia has been linked to delusions, hallucinations, and negative symptoms as well as to depressive/anxiety symptoms. Its impact on quality of life has been less studied, especially in schizophrenia subjects with ongoing auditory hallucinations. The Beck Cognitive Insight Scale (BCIS) and the Quality of Life Scale (QLS) were administered to subjects who met DSM IV criteria for schizophrenia after due translation and validation. All subjects reported ongoing auditory hallucinations at recruitment. Mean composite cognitive insight scores from participants (N = 60
