BACKGROUND: Cancer-related cognitive impairment (CRCI) is increasingly recognized in patients with non-central nervous system malignancies, including non-small cell lung cancer (NSCLC), although evidence on pre-treatment cognitive functioning and associated psychosocial factors remains limited. This exploratory study investigated baseline cognitive functioning in NSCLC patients before oncological treatment and examined the role of cognitive reserve, emotional distress, sleep quality, and subjective cognitive complaints.
METHODS: Fifty NSCLC patients underwent comprehensive neuropsychological

Levels of educational and occupational attainment, as components of cognitive reserve, may modify the relationship between the pathological hallmarks and cognition in Alzheimer's disease (AD). We examined whether exposure of a Tg2576 transgenic mouse model of AD to environmental enrichment (EE) at a specific period during the amyloidogenic process favored the establishment of a cognitive reserve. We found that exposure to EE during early adulthood of Tg2576 mice--before amyloidogenesis has started--reduced the severity of AD-related cognitive deficits more efficiently than exposure later in li

In the course of neurodegenerative disorders there are several mechanisms that may counteract the pathological process, mitigating the clinical manifestations of the disease. Usually referred as cognitive reserve hypothesis, this theoretical framework posits that individuals with enriched cognitive status (i.e. with higher educational and occupational levels and higher individual social achievement) may cope better with the occurrence of cognitive decline by a more efficient recruitment of neural networks sustaining higher-level functions. Cognitive reserve was initially studied in Alzheimer's

Electroconvulsive therapy (ECT) is the most effective treatment for severe depression but is associated with cognitive side effects. Side effects on executive function and memory may persist for months. Patients with a low cognitive reserve and/or poor cognition before ECT seem to be at greater risk of cognitive side effects than patients with a high cognitive reserve. However, better understanding of the nature and predictors of these side effects is needed. We recommend implementing a cognition assessment tool to monitor cognition during ECT. This may improve patient information and provide

While nigrostriatal dopaminergic cell loss is the pathological hallmark of Parkinson's disease, it is well-known that dopamine deficiency cannot fully explain the clinical severity or disease progression. Emerging data suggest that resilience, i.e. the brain's capacity to maintain function despite pathology, plays a key role in shaping clinical variability of both motor and non-motor symptoms. However, different contributing terms have been proposed, including brain compensation and reserve, and it remains unclear how these terms can be delineated both conceptually and mechanistically. Therefo

The concept of brain maintenance refers to the preservation of brain integrity in older age, while cognitive reserve refers to the capacity to maintain cognition in the presence of neurodegeneration or aging-related brain changes. While both mechanisms are thought to contribute to individual differences in cognitive function among older adults, there is currently no "gold standard" for measuring these constructs. Using machine-learning methods, we estimated brain and cognitive age based on deviations from normative aging patterns in the Whitehall II MRI substudy cohort (N = 537, age range = 60

OBJECTIVE: Reserve refers to the brain's ability to maintain function after an injury and strongly relates to traumatic brain injury (TBI) outcomes. This study examined (1) whether associations between pre-injury reserve proxies and outcomes differed across injury severity categories, and (2) whether the impact of injury severity varied across functional, symptomatic, and cognitive outcomes.
METHODS: This observational cohort study used data from CENTER-TBI. Of 4509 enrolled patients, 3377 were between 18 and 75 years and alive 6 months post-injury. Outcomes included functional outcome (GOSE,

Resilience mechanisms to counteract aging and the onset of neurological and neurodegenerative diseases are becoming an increasingly central issue due to the aging of the population. Several mechanisms have been hypothesized, and in the context of cognitive resilience, the construct of cognitive reserve has attracted considerable interest. However, it is becoming increasingly clear that there are several possible mechanisms of resilience (e.g., motor, affective, behavioral, etc.) which interact with each other, influencing inter-individual variability in healthy and pathological aging trajector

The mechanism of neurocognitive failure in Alzheimer's disease remains obscure. While the mainstream hypothesis in the field posits that brain tau pathology is the only process that drives cognitive decline in AD, other complementary mechanisms link vascular brain lesions with beta-amyloid pathology as an important factor leading to neurodegeneration. Recently, it was also proposed that the brain's network's functional imbalance could primarily drive cognitive decline in neurodegenerative diseases. Here, we investigated whether the anticorrelation between the default mode (DMN) and dorsal atte

Bilingualism has been argued to have an impact on cognition and brain structure. Effects have been reported across the lifespan: from healthy children to ageing adults, including clinical (ageing) populations. It has been argued that active bilingualism may significantly contribute to the delaying of the expression of Alzheimer's disease symptoms. If bilingualism plays an ameliorative role against the expression of neurodegeneration in dementia, it is possible that it could have similar effects for other neurodegenerative disorders, including Multiple Sclerosis, Parkinson's and Huntington's Di

Default-mode network (DMN) may be the earliest affected network and is associated with cognitive decline in Alzheimer's disease (AD). Repetitive transcranial magnetic stimulation (rTMS) may help to modulate DMN plasticity. Still, stimulation effects substantially vary across studies and individuals. Global left frontal cortex (gLFC) connectivity, a substitute for reserve capacity, may contribute to the heterogeneous physiological effects of neuro-navigated rTMS. This study investigated the effects of left angular gyrus-navigated rTMS on DMN connectivity in different reserve capacity participan

Covert brain infarcts (CBIs) are five times more prevalent than symptomatic brain infarcts. CBIs are associated with cognitive impairment and therefore may be a target for preventing cognitive decline and dementia. This review focuses on strategies for preventing CBI-related cognitive impairment, either by preventing incident or recurrent CBI or by enhancing cognitive reserve. CBIs begin to become prevalent during midlife and are highly prevalent in later life. The distribution of vascular pathologies of CBI differs from those that cause symptomatic stroke; therefore, preventive treatments may

Education may reduce risk of dementia through passive reserve, by increasing neural substrate. We tested the hypotheses that education is associated with thicker cortex and reduced rates of atrophy in brain regions related to literacy and intellectual ability. Healthy older adults and those with mild cognitive impairment were categorized into high (≥18 years) and low (≤13 years) education groups. Higher education was associated with thinner cortices in several areas, but one-year atrophy rates in these areas did not differ by education group. These results do not support a passive reserve mode

Brain aging is characterized by the progressive and gradual accumulation of detrimental changes in structure and function, which increase risk of age-related cognitive decline and dementia. This devastating chronic condition generates a huge social and economic burden and accounts for 11.2% of years of disability. The increase in lifespan has contributed to the increase in dementia prevalence; however, there is currently no curative treatment for most causes of dementias. This paper reviews evidence-based strategies to build, enhance, and preserve cognition over the lifespan by examining appro

OBJECTIVE: Formal education and other cognitive challenges influence brain structure and improve function. It is believed that cognitive activities create a cognitive reserve (CR) that can slow the decline due to aging and neurodegenerative diseases. This study investigated alterations of regional cerebral blood flow (rCBF) associated with high and low CR in different stages of Alzheimer's disease (AD) and examined whether rCBF alteration mediates the relationship between education and cognitive performance.
METHODS: Patients with AD or amnestic mild cognitive impairment (aMCI) and healthy co
