Hand-Assisted Laparoscopic Surgery: A Versatile Tool for Colorectal Surgeons.

PURPOSE: Hand-assisted laparoscopic surgery (HALS) is a minimally invasive surgical technique with the combined benefits of laparoscopic surgery while allowing the use of the surgeon's hand for better tactile control. Obesity has been associated with higher conversion rates with multiport laparoscopic surgery, but not with HALS. This study aimed to examine the versatility of HALS in various clinical contexts. METHODS: All HALSs performed at 2 major tertiary centers in Sydney were prospectively collected for retrospective analysis. Variables including age, sex, body mass index (BMI), previous

Nausea, vomiting and return of bowel function after colorectal surgery.

BACKGROUND: Although patterns of return of bowel function (ROBF) following colorectal surgery with enhanced recovery after surgery (ERAS) programmes have been well delineated, regular morphine use is uncommon. This study describes the patterns of post-operative nausea and vomiting (PONV) and ROBF in this context. METHOD: Patients undergoing elective major colorectal surgery on an ERAS programme over 1 year were included. Patient details, intra-operative course, post-operative management, outcomes and complications were collected retrospectively from clinical records. Statistical analysis was

Multi-layered intra-abdominal adhesion prophylaxis following laparoscopic colorectal surgery.

BACKGROUND: Small bowel obstruction secondary to intra-abdominal adhesions is a frequent postoperative complication. Less invasive surgery carries a lower risk of postoperative adhesions, but adhesions may still occur after laparoscopic colorectal surgery. We present here some of our methods of adhesion prophylaxis for laparoscopic colorectal surgery. METHODS: The 167 patients who underwent laparoscopic colorectal surgery at our center from 2007 to 2012 were retrospectively reviewed. To prevent postoperative intra-abdominal adhesions, anti-adhesion barriers were placed using the half-overlap

Transanal colorectal resection using natural orifice translumenal endoscopic surgery (NOTES).

The surgical management of rectal cancer has evolved over the past century, with total mesorectal excision (TME) emerging as standard of care. As a result of the morbidity associated with open TME, minimally invasive techniques have become popular. Natural orifice translumenal endoscopic surgery (NOTES) has been held as the next revolution in surgical techniques, offering the possibility of 'incisionless' TME. Early clinical series of transanal TME with laparoscopic assistance (n = 72) are promising, with overall intraoperative and postoperative complication rates of 8.3% and 27.8%, respective

Results of laparoscopic colorectal surgery from a national training center.

BACKGROUND/PURPOSE: Trials have shown laparoscopic colorectal surgery to be safe. We aim to analyze the long-term results from a single national training center for laparoscopic surgery, especially in patients with high predicted mortality scores as well as in octogenarians. We also aim to explore the trend in the length of the learning curve among consultants and colorectal trainees, and determine whether or not laparoscopic colorectal surgery is amenable to surgical training. METHODS: All patients between July 2003 and July 2011 having laparoscopic colorectal surgery were included in a pros

Intra-operative tumour detection and staging in colorectal cancer surgery.

AIM: Surgical resection for colorectal cancer involves segmental resection and regional lymphadenectomy. The appropriateness of this 'one-size-fits-all' strategy is questioned as bowel cancer screening programmes result in a shift to earlier stage disease. Currently, the nodal status of a colorectal cancer can only be reliably determined by histopathological examination of the resected specimen. New methods of intra-operative staging are required to allow surgical resection to be tailored to the stage of the disease. METHOD: A literature search was performed of PubMed and Embase databases usi

Prospective evaluation of intraoperative peripheral nerve injury in colorectal surgery.

AIM: Intraoperative peripheral nerve injury can have permanent neurological consequences. Its incidence is not known and varies according to the location and the surgical specialty. This study was a prospective analysis of intraoperative peripheral nerve injury as a complication of abdominal colorectal surgery. METHOD: All patients who underwent major colorectal abdominal surgery in our Colorectal Unit between 1996 and 2009 were analyzed. Data on nerve injury were prospectively collected. RESULTS: There were 2304 patients, of whom eight (0.3%) experienced intraoperative peripheral nerve inju

Perceived information after surgery for colorectal cancer--an explorative study.

AIM: With fast track surgery and shorter hospital stay, discharge from hospital after cancer surgery is becoming more challenging for the individual patient. The aim of this study was to explore how patients perceive information after surgery for colorectal cancer, what their information needs are and to determine factors affecting received information. METHOD: A hundred patients filled in the questionnaires QLQ-C30, CR38 and INFO25 created by the European Organization for Research and Treatment of Cancer, the Eastern Cooperative Oncology Group (ECOG) instrument and the SOC (sense of coherenc

Transrectal natural orifice translumenal endoscopic surgery (NOTES) for colorectal resection.

Natural orifice translumenal endoscopic surgery (NOTES) is a new surgical paradigm involving performance of intra-abdominal surgery via a natural orifice and thereafter peritoneal access through an intentionally created hole in a hollow viscus. The vast majority of research in this rapidly evolving field had involved access via an oral or vaginal route. Access via a transanal route, other than the obvious concern over contamination, has many appealing attributes. In addition, transanal surgery has long been a common procedure lending a valuable clinical experience to the foundation of this fie

Mechanical bowel preparation before elective colorectal surgery: is it necessary?

AIM: To present the worldwide accepted guidelines concerning the use of mechanical bowel preparation (MBP) before elective colorectal surgery (ECS). PATIENTS AND METHODS: We conducted a retrospective review of the Pubmed Databases for randomized controlled trials (RCTs) and meta-analyses, which included adult patients who underwent elective colorectal surgery. We compared the patients who had a preoperative MBP with those who did not. Significant factors that were taken into account were postoperative septic complications and anastomotic dehiscence. RESULTS: Our search revealed 5 RCTs and 2

[Perineal cutaneous metastases from adenocarcinoma after surgery for colorectal cancer].

The development of cutaneous metastases in the context of colorectal cancer is exceptional, especially in the absence of visceral lesions. We present the case of a 50-year-old woman who underwent surgery for a T3N0M0 tumor in the sigmoid colon, with resection of ovarian metastases at 12 months. Reoperation was performed 14 months later for local anastomotic recurrence. Four months after surgery, a nodular ulcerated lesion was observed in the perineum due to metastases from adenocarcinoma. Aggressive local surgery was performed and the patient has presented no recurrences after a 5-year follow-

Percutaneous postoperative intra-abdominal abscess drainage after elective colorectal surgery.

Intra-abdominal abscesses are a frequent source of morbidity and mortality following both elective and emergent surgery of the alimentary tract. CT-guided percutaneous drainage of intra-abdominal abscess is an alternative to immediate surgical intervention. We studied the clinical characteristics and outcomes of patients undergoing percutaneous drainage of intra-abdominal abscesses arising after elective colorectal procedures. We retrospectively identified 40 patients with postoperative intraabdominal abscess following elective colorectal surgery who underwent CT-guided percutaneous drainage w

Emergency one-stage surgery for obstructing left-sided colorectal carcinomas.

A one-stage operation for obstructing left-sided colorectal carcinoma remains controversial. This study was performed to compare our surgical management of patients with left-sided colorectal cancer (at or distal to splenic flexure) obstruction presenting to the Kaohsiung Medical University Hospital from January 1995 to December 2000. Ninety-six patients underwent immediate operation within 24 hours of admission. Of these, 73 patients (76%) who underwent immediate tumor resection and anastomosis in one stage after appropriate resuscitation were enrolled into our study. In one-stage operation s

Suprapubic versus transurethral catheterisation of males undergoing pelvic colorectal surgery.

A prospective, randomised double-blind trial of suprapubic (SPC) versus transurethral (TUC) catheterisation was undertaken in fifty consecutive male patients of median age 66 (range 32-81) years undergoing pelvic colorectal surgery. Twenty-four patients were randomised to SPC. Catheter removal times were comparable between the two groups: SPC = mean 7.2 (3-14) days; TUC = man 7.5 (2-13) days; P > 0.5. Acute urinary retention was recorded in 5 patients with SPC and 6 in the TUC group. Chronic retention with overflow was recorded in one TUC patient. Frequent voiding after catheter removal occurr

Laparoscopic colorectal surgery--are we being honest with our patients?

PURPOSE: A survey was undertaken to assess the impact of laparoscopy on the practice of colorectal surgery. METHODS: A total of 1,520 questionnaires were mailed to all members of the American Society of Colon and Rectal Surgeons; 635 (42 percent) surgeons responded, 50 percent, and indicated that one questionnaire represented their entire group practice. RESULTS: Two hundred seventy-eight (47 percent) respondents currently perform laparoscopic colorectal surgery; 62 percent (171) use the laparoscope for < or = 20 percent of their bowel resections. Conversely, only 6 percent (16) use the lapa

A prospective study of sexual function after major colorectal surgery.

Excision of the recto-sigmoid and rectum for cancer or inflammatory disease frequently damages the autonomic nerve supply. The anatomy and function of the autonomic nervous system in the pelvis is reviewed. Thirty-two patients who underwent excision of the rectum have been studied: 28 operations were for carcinoma, 4 for ulcerative colitis. A detailed history of sexual function was obtained preoperatively together with a neurological examination of the genitalia and perineum. Biopsies were taken from the surgical specimens and examined for the presence of nerve tissue. A further sexual history

Surgery for colorectal liver metastases: Predictive factors of biliary fistula.

BACKGROUND: Colorectal liver metastases (CRLM) are the most common secondary site of colorectal cancer. Hepatic resection remains the potentially curative standard treatment, but postoperative morbidity remains substantial, with biliary fistula representing the most frequent and clinically significant specific complication. This study aimed to identify predictive factors of biliary fistula following CRLM surgery. METHODS: A retrospective single-center study was conducted including 129 patients who underwent surgery for CRLM at Mongi Slim Hospital, La Marsa, between January 2020 and December 2
