BACKGROUND: Carrying out dialysis against a patient's will poses specific legal and ethical problems. Decisions have to be made under time pressure, there is often an immediate threat to life, the intervention is rather difficult to carry out against a patient's will, and the same dilemma repeats itself continuously within some days.
METHOD: We report on a 45-year-old man with moderate dementia due to hypertensive encephalopathy who was involuntarily admitted to a psychiatric hospital by a court order because of his refusal of dialysis.
RESULTS: Dialysis could be carried out with moderate co

Data from a sample of severely and persistently mentally ill involuntary patients indicated that differences in violence between males and females in the 4 months prior to hospital admission depended on the measure. In the bivariate analysis, males had a greater prevalence of violence on the two indicators which separated more serious violence from lesser and no violence; but there was no gender difference on the more inclusive measure which incorporated threats and fights not involving weapons or injuries. In multivariate analysis when other relevant predictors were controlled, gender was sig

This article begins with the history of the rise and fall of the state hospitals and subsequent criminalization of persons with serious mental illness (SMI). Currently, there is a belief among many that incarceration has not been as successful as hoped in reducing crime and drug use, both for those with and those without SMI. Moreover, overcrowding in correctional facilities has become a serious problem necessitating a solution. Consequently, persons with SMI in the criminal justice system are now being released in large numbers to the community and hopefully treated by public sector mental he

Involuntary detention is a feature of psychiatric care in many countries. We previously reported an involuntary admission rate of 67.7 per 100,000 population per year in inner-city Dublin (January 2008-December 2010), which was higher than Ireland's national rate (38.5). We also found that the proportion of admissions that was involuntary was higher among individuals born outside Ireland (33.9%) compared to those from Ireland (12.0%), apparently owing to increased diagnoses of schizophrenia in the former group. In the present study (January 2011-June 2013) we again found that the proportion of

Research in Montreal's St-Jean-de-Dieu Asylum archives has revealed a number of letters from family members and local physicians pleading for asylum care for married women between 1890 and 1921. When added to other admission documents in patients' medical files, these letters allow an intimate glimpse into private lives of families and highlight the pain and distress of dealing with mentally ill people in the home before the introduction of community mental health services. Far from easily abandoning a spouse or mother, close-knit French Canadian families struggled until they could no longer c

The case records of 55 people from the West Midlands (UK) fulfilling the Mental Health Act 1983 criteria for mental impairment or severe mental impairment, were studied. Most were young men with mild mental retardation. 73 per cent were resident in (or on leave from) mental handicap hospitals, and 27 per cent resident in special hospitals. 29 per cent were subject to a Restriction Order. Most had lived in hospital for more than six years. The commonest problem behaviours were aggression, property offences and inappropriate or offending sexual behaviour. 31 per cent were mentally ill or had a p

BACKGROUND: Few studies have addressed the relationship between dementia and crime. We conducted a study of persons who got a primary or secondary diagnosis of dementia or cognitive disorder in a forensic psychiatric examination.
METHODS: In Sweden, annually about 500 forensic psychiatric examinations are carried out. All cases from 2008 to 2010 with the diagnoses dementia or cognitive disorder were selected from the database of the Swedish National Board of Forensic Medicine. Out of 1471 cases, there were 54 cases of dementia or cognitive disorder. Case files were scrutinized and 17 cases of

Established by the law on the orientation and programming of justice of September 2002, the specially equipped hospital unit (UHSA) provides psychiatric treatment with full medical care to prisoners, in direct cooperation with the prison. In this specific social context, can the organisation of the care and the structure itself represent a lasting solution?

We hypothesized that the integration of forensic psychiatrists and the forensic system into the general stream of mental health should lead to better reintegration of forensic patients into the society. The aims of the study were to explore the link between violence, murder, psychiatric disorders and other variables, and to suggest a mode of prevention of criminal recidivism. This investigation provided a more reflective profile of mentally ill persons convicted of murder, hospitalized in Psychiatric Hospital in Demir Hisar in Macedonia between 2007 and 2009. Study results indicated the offend

BACKGROUND: Perpetrators who act together violently occur frequently in police and media discussions, but are rarely the focus of forensic psychiatric research.
AIMS: We aimed to characterise people who act together when committing a serious crime and to map the frequency of such crimes over 21 years in Finland.
METHODS: Data for the study were retrieved from the national database of forensic psychiatric examinations for the period 2000-2020, with reports on file for nearly all people charged with serious criminal offences in the country. Index cases were defined as those with two or more pe

BACKGROUND: Patients with psychoses have an increased risk of becoming victims of violence. Previous studies have suggested that higher symptom levels are associated with a raised risk of becoming a victim of physical violence. There has been, however, no evidence on the type of symptoms that are linked with an increased risk of recent victimization.
METHODS: Data was taken from two studies on involuntarily admitted patients, one national study in England and an international one in six other European countries. In the week following admission, trained interviewers asked patients whether they

The author have first studied the outcome of 68 patients, committed on the Social Defense law, out of the institution for five years. He observes that 27 patients have been well integrated, both professionally and socially (in their family). Fifteen patients had only a passive reinsertion and ten others are failures. Eight patients deceased, one through fire immolation, an other one through medicinal overdose during a holiday at home. Eight other had to be transferred: 3 in a specialized therapeutic milieu, 3 in a more closed setting and 2 in another region for linguistic reasons. In the secon

In New York State, patients who have been found not criminally responsible by reason of mental disease or defect, eventually become outpatients while still under the supervision of the courts. The treatment of these patients on outpatient orders of condition poses special problems. Treatment needs to center on the issues related to patient's potential for harmful/violent behavior. Therefore, outpatient clinicians need to conduct periodic risk assessments and must continuously monitor identified risk indicators. The author outlines steps for information gathering and evaluation necessary for ri

This article discusses the nature of psychiatric evidence in Icelandic criminal proceedings and presents findings from an investigation of the role of amnesia, malingering and overcontrolled hostility among different types of offenders. Amnesia was most common in homicide cases, and was almost invariably associated with alcohol intoxication. An unexpectedly high rate of overcontrolled hostility was found among sex offenders, which may have important theoretical and clinical implications. The results indicate that deliberate faking of an intellectual deficit on psychometric tests occurs rarely

Differences in child abuse perpetration between individuals with and without mental disorders remain obscure. This study compared the risk difference and further investigated the association between the category of mental disorders and child abuse perpetration. A total of 681,970 adults from the 2002 to 2013 Taiwan National Health Insurance Research Database were analyzed, including 340,985 patients with psychiatric disorders (International Classification of Diseases, Ninth Revision, Clinical Modification [ICD-9-CM] codes 290.x-319.x) and 340,985 sex- and age-matched individuals without psychi

At a maximum-security forensic hospital over a three-year period, 79 patients (35 percent of all patients) were involved in 157 staff injuries. Staff members' average postinjury absence from work was 85 days, at a cost equivalent to about 2 percent of the hospital's budget. Nursing staff, particularly psychiatric technicians, had the highest injury rate. Male staff were about twice as likely to be injured as female staff. More injuries occurred on the second shift. Patients who injured staff were younger than those who did not. Florid psychotic behavior, nonpsychotic agitation, and the recent

BACKGROUND: Mental disorder can increase the likelihood of taking violent acts of some individuals, but only a small percentage of violence in societies could be attributed to patients with mental health problems. For the past several years numerous studies related to forensic psychiatry has confirmed a close causal relationship between violent offenders and psychiatric comorbidity. Several studies have provided strong evidence that antisocial personality disorders (APD) represent a significant clinical risk for violence.
AIM: This study aims to show the relationship between antisocial person

This study aimed to determine the suicide mortality within 1 year after discharge from psychiatric inpatient care and identify the risk factors for suicide completion during this period. A total of 8403 patients were admitted to general hospitals in Seoul, Korea, for psychiatric disorders from January 1989 to December 2006. The suicide mortality risk of these patients within 1 year of discharge was compared with that of gender- and age-matched subjects from the general population of Korea. The standardized mortality ratios (SMR) for suicide in the year following discharge were 49.7 for males a

OBJECTIVE: The purpose of the study was to identify risk factors and correlates of violence committed by patients in an acute adult psychiatric inpatient unit in a district general hospital of the United Kingdom's National Health Service.
METHODS: Incidents of violence committed by inpatients over a one-year period in 1997-1998 were retrospectively analyzed. The clinical characteristics of 49 violent patients were compared with those of all patients admitted to the unit during the study period (N=474) and with a random sample of nonviolent patients (N=140).
