Three-step surgical management algorithm for annular rupture in transcatheter aortic valve replacement.

OBJECTIVE: Whereas transcatheter aortic valve replacement is widely implemented, annular rupture is a devastating complication and could be highly mortal. However, owing to its rare incidence, the optimal treatment algorithm has not been established. Thus, we evaluated the feasibility and effectiveness of a 3-step algorithm to treat annulus rupture. METHODS: From 2009 to 2022, 8 patients of 1083 transcatheter aortic valve implantation (0.8%) developed annulus rupture and were treated with the three-step algorithm. The algorithm was composed of a first step (pericardial drainage and protamine

Effect of an app-guided advanced cardiac life support algorithm on teamwork, non-technical skills, and workload in simulated in-hospital cardiac arrest: a randomized controlled feasibility trial.

BACKGROUND: In-hospital cardiac arrest carries high mortality despite advances in monitoring, rapid response systems, and adherence to advanced life support (ALS) guidelines. Digital decision-support tools, including mobile applications, are increasingly explored to improve guideline adherence, yet their impact on cognitive workload, team performance, and ALS performance remains uncertain. METHODS: We conducted a prospective, randomized, simulation-based controlled feasibility trial with 40 interprofessional teams (80 participants) comparing ALS performance with and without the use of the 'CP

[Acute low back pain--assessment and management].

Acute low back pain is a very common symptom. Up to 90% of all adults suffer at least once in their life from a low back pain episode, in the majority of cases a nonspecific lumbago. They are, with or without sciatica, usually self-limited and have no serious underlying pathology and subside in 80-90% of the concerned patients within six weeks. Beside a sufficient pain medication and physiotherapy, reassurance about the overall benign character and the favourable prognosis of the medical condition should be in the centre of the therapeutic efforts. A more thorough assessment is required for se

Rate Control for Video-based Point Cloud Compression.

Rate control is a necessary tool for video-based point cloud compression (V-PCC). However, there is no solution specified on this topic yet. In this paper, we propose the first rate control algorithm for V-PCC. Generally, a rate control algorithm is divided into two processes: bit allocation and bitrate control. In V-PCC, the total bits are composed of three parts: the header information including the auxiliary information and occupancy map, the geometry video, and the attribute video. The bit allocation aims to assign the total bits to these three parts. Since the auxiliary information and oc

An AI-Guided Mechanotyping Instrument for Fully Automated Oocyte Quality Assessment.

The mechanical properties of oocytes are regarded as important indicators of their developmental potential. During fertilization, deviations from the normal mechanical range can hinder sperm penetration, ultimately reducing fertilization efficiency and compromising embryo quality. However, current methods for measuring oocyte mechanics often suffer from low automation levels and predominantly provide local, rather than whole-cell, mechanical characterization. To address these limitations, we developed an AI-guided µN-scale mechanical measurement instrument for safe and automated oocyte quality

Development of a finite element neck model for head-first compressive impacts: Toward the assessment of motorcycle neck protective equipment.

Head-first compressive impacts occur in motorcycle crashes and may result in serious to fatal neck injuries to riders. Equipment to protect the riders' necks from these injuries are available in the market; however, their effectiveness in reducing injury risk is not clear, either due to the lack of scientific evidences or assessment with any prevalently accepted standard. This paper presents a finite element ligamentous neck model, developed as a computationally efficient tool, for future use in the computational phase of assessment process of neck protective equipment. The 3D cervical spine w

Salvage of cervical motor radiculopathy using peripheral nerve transfer reconstruction.

Purpose: Motor nerve transfer surgery involves re-innervation of important distal muscles using either an expendable motor branch or a fascicle from an adjacent functioning nerve. This technique is established as part of the reconstructive algorithm for traumatic brachial plexus injuries. The reproducible outcomes of motor nerve transfer surgery have resulted in exploration of the application of this technique to other paralysing conditions. The objective of this study is to report feasibility and increase awareness about nerve transfer as a method of improving upper limb function in patients

Hybrid Metalens-Compressive Sensing Imaging System for Ultra-Low-Power Edge Detection.

Low-power edge detection is essential for next-generation vision systems that require real-time on-device processing of optical information. Conventional imagers rely on power-intensive digital postprocessing for edge detection, creating a data bottleneck between the image sensor and signal processor. We propose a system-level hybrid optoelectronic edge imaging framework that combines an analog optical computing metalens with a compressive sensing CMOS image sensor (CS-CIS). The spiral metalens with a topological charge of l = + 2 directly images and encodes second-order edge information in

Ultrasound-Guided Hydrodissection for Peripheral Neuropathy: An Evidence-Based Intervention Whose Time Has Finally Come.

Peripheral neuropathy remains a prevalent and challenging clinical condition, with management strategies traditionally centered on pharmacologic and neuromodulatory approaches. While these interventions play essential roles, an additional minimally invasive, evidence-based procedure warrants greater recognition: ultrasound-guided perineural hydrodissection. This editorial presents a contemporary perspective on hydrodissection as a key tool within the peripheral neuropathy treatment algorithm. High-resolution ultrasound enables precise injection of solutions, such as 5% dextrose, to release ner

A radiomics nomogram based on MRI for differentiating vertebral osteomyelitis from vertebral compression fractures.

OBJECTIVES: This study aims to investigate the value of a radiomics nomogram based on magnetic resonance imaging (MRI) in distinguishing vertebral compression fractures (VCFs) from vertebral osteomyelitis (VOs). MATERIALS AND METHODS: We conducted a retrospective analysis of the clinical data from 100 patients with VCFs and VOs, respectively at our hospital. The cases were randomly divided into training (n = 140) and testing sets (n = 60) in a 7:3 ratio. Two experienced radiologists outlined the regions of interest (ROI) on the MRI images using T2-weighted fat suppression (T2WI-FS) images and

Biologically derived companding algorithm for high dynamic range mammography images.

The screening mammography is currently the best procedure available for early detection of the breast cancer. The acquired mammograms are high dynamic range (HDR) images having a 12 bit grayscale resolution. When viewed by a radiologist, a single image must be examined several times, each time focusing on a different intensity range. We have developed a biologically derived mammography companding (BDMC) algorithm for compression, expansion, and enhancement of mammograms, in a fully automatic way. The BDMC is comprised of two main processing stages: 1) preliminary processing operations which in

[Role of multispiral CT angiography in diagnosis and treatment of patients with chronic cerebral circulatory insufficiency].

Implementing in our clinical practice multispiral CT angiography (MSCTA) of the brachiocephalic arteries into the diagnostic algorithm has made it possible to somewhat deviate from the "golden standard", i.e., the conventional contrast-enhanced angiography, in determining the indications for surgical interventions in a total of 328 patients presenting with chronic cerebral circulatory insufficiency (CCCI). All the patients diagnosed as having multifocal atherosclerosis are subjected to duplex scanning of the brachiocephalic arteries Detecting haemodynamically significant stenoses of the caroti

Evaluation of the thyroid nodule.

BACKGROUND: Thyroid nodules are common, yet treatment modalities range from observation to surgical resection. Because thyroid nodules are frequently found incidentally during routine physical examination or imaging performed for another reason, physicians from a diverse range of specialties encounter thyroid nodules. Clinical decision making depends on proper evaluation of the thyroid nodule. METHODS: The current literature was reviewed and synthesized. RESULTS: Current evidence allows the formulation of recommendations and a general algorithm for evaluating the incidental thyroid nodule.

On the Effects of Constitutive Properties and Roughness of a Hard Inclusion in Soft Tissue on B-mode Images.

We perform finite element modeling of pulse-echo ultrasound of a hard inclusion in a soft tissue to gain a better understanding of B-mode image brightness characteristics. We simulate a pressure wave emitted by an ultrasound transducer through the inclusion-tissue medium by prescribing suitable boundary conditions, and collect the scattered wave response to simulate the behavior of the transducer array used for pulse-echo ultrasound. We form B-mode images from simulated channel data using standard delay and sum beamforming. We establish the accuracy of the finite element model by comparing the

Automatic Seizure Detection Based on Stockwell Transform and Transformer.

Epilepsy is a chronic neurological disease associated with abnormal neuronal activity in the brain. Seizure detection algorithms are essential in reducing the workload of medical staff reviewing electroencephalogram (EEG) records. In this work, we propose a novel automatic epileptic EEG detection method based on Stockwell transform and Transformer. First, the S-transform is applied to the original EEG segments, acquiring accurate time-frequency representations. Subsequently, the obtained time-frequency matrices are grouped into different EEG rhythm blocks and compressed as vectors in these EEG

Real-time Robust Semantic Segmentation via Low-Resolution Ensemble.

Semantic segmentation algorithms constitute a critical component for vision-based perception in robotics and autonomous driving. Real-world deployment in uncertain environments requires such algorithms to be both low-latency and robust against common corruptions. However, the limited representation ability of real-time models and the high computational overhead of existing robust models render real-time robust semantic segmentation particularly challenging. In this paper, we begin by systematically evaluating the robustness of existing real-time models and observe that inference on down-sample
