Progressive Conduction Disease in a Mitochondrial Disorder.

CASE SUMMARY: We describe the case of a 45-year-old man with progressive external ophthalmoplegia and pigmentary retinopathy who was referred for evaluation of a bradyarrhythmia due to exertional dyspnea and fatigue. A baseline electrocardiogram and ambulatory monitoring revealed clinically significant conduction abnormalities. TAKE-HOME MESSAGES: In Kearns-Sayre syndrome, conduction abnormalities may progress unpredictably; early permanent pacing should be strongly considered. Consider implantable cardioverter-defibrillator rather than pacemaker alone when additional high-risk features are p

Inherited progressive cardiac conduction disorders.

PURPOSE OF REVIEW: Progressive cardiac conduction disorder (PCCD) is an inherited cardiac disease that may present as a primary electrical disease or be associated with structural heart disease. In this brief review, we present recent clinical, genetic, and molecular findings relating to PCCD. RECENT FINDINGS: Inherited PCCD in structurally normal hearts has been found to be linked to genetic variants in the ion channel genes SCN5A, SCN1B, SCN10A, TRPM4, and KCNK17, as well as in genes coding for cardiac connexin proteins. In addition, several SCN5A mutations lead to 'cardiac sodium channelop

[Idiopathyc progressive disorder of atrioventricular conductivity].

Atrioventricular (AV) blockades are classified into congenital and acquired ones. Conductive system fibrosis or sclerosis and coronary heart disease are the most frequent reasons for disturbances in AV conductivity. Congenital AV conductivity disorder may be associated with an isolated lesion of the AV conductive system or with other congenital diseases of the heart. The author describes a case of AV conductivity disorder which has been progressing for many years.

[Mechanisms of conduction disorders in digitalis poisoning].

Ever since the first papers on the mechanism of the arrhythmias of digitalis were first published, during the first two decades of this century, the issue has been a controversial one. Since the automaticity induced by digitalis intoxication has different characteristics when compared to normal automaticity, it has been suggested that probably the mechanisms for these two types of spontaneous activity are different. It has been recently proposed that the automaticity induced by digitalis intoxication could be secondary to the after potential oscillations described in isolated conducting fibers

[Systolic intervals in disorders of intraventricular conduction].

The duration of the systolic intervals in 50 patients was studied. They were divided into the following groups of 10 each: 1) with normal intraventricular conduction; 2) with advanced right bundle branch block; 3) with advanced left bundle branch block; 4) with left anterior fascicular block, and 5) endocardial pacemaker in the right ventricle. The systolic intervals do not suffer modification in the presence of a right bundle branch block or a left anterior fasicular block. The left bundle branch block produces changes fundamentally in the preejection period at the expense of its two componen

Conduction Disorders in the Setting of Acute STEMI.

ST-elevation myocardial (STEMI) is frequently associated with conduction disorders. Regional myocardial ischemia or injury may affect the cardiac conduction system at various locations, and neural reflexes or changes in the balance of the autonomous nervous system may be involved. Sinoatrial and atrioventricular blocks are more frequent in inferior than anterior STEMI, while new left anterior fascicular block and right bundle branch block indicate proximal occlusion of the left anterior descending coronary artery. New left bundle branch block is associated with multi-vessel disease. Most condu

Conduction and refractory disorders in the diabetic atrium.

Diabetes mellitus (DM) is an independent risk of atrial fibrillation. However, its arrhythmogenic substrates remain unclear. This study sought to examine the precise propagation and the spatiotemporal dispersion of the action potential (AP) in the diabetic atrium. DM was induced by streptozotocin (65 mg/kg) in 8-wk-old male Wister rats. Optical mapping and histological analysis were performed in the right atrium (RA) from control (n = 26) and DM (n = 27) rats after 16 wk. Rate-dependent alterations of conduction velocity (CV) and its heterogeneity and the spatial distribution of AP were measur

Congenital disorders of cardiac rhythm and conduction.

Electrical instability of the heart has numerous congenital origins. This review approaches the problems from a morphological standpoint, utilizing selected examples from my own studies of the human cardiac conduction system performed during the last three decades. Subjects discussed include multifocal Purkinje cell tumors, benign congenital polycystic tumors of the AV node, several types of congenital heart block, postnatal morphogenesis of the AV node and His bundle (including considerations of persistent fetal dispersion and crib death), the Wolff-Parkinson-White syndrome, left superior ven

[Conduction disorders in aortic periprosthetic abscesses].

Active infective endocarditis (AIE) involving native and especially prosthetic aortic valve is often complicated by conduction abnormalities. These conduction disturbances are considered to represent extension of infection from the valve to the annulus and surrounding myocardium. The authors report their experience of conduction disorders in 6/8 patients in whom the aortic prosthetic valve infection was complicated by periprosthetic abscess. They underline the importance of conduction abnormalities as early markers of severe complication in patients with AIE. In fact, their detection is a usef

Heart rate and skin conductance associations with physical aggression, psychopathy, antisocial personality disorder and conduct disorder: An updated meta-analysis.

The associations between physiological measures (i.e., heart rate and skin conductance) of autonomic nervous system (ANS) activity and severe antisocial spectrum behavior (AB) were meta-analyzed. We used an exhaustive partitioning of variables relevant to the ANS-AB association and investigated four highly relevant questions (on declining effect sizes, psychopathy subscales, moderators, and ANS measures) that are thought to be transformative for future research on AB. We investigated a broad spectrum of physiological measures (e.g., heart rate (variability), pre-ejection period) in relation to

Default mode network activity and neuropsychological profile in male children and adolescents with attention deficit hyperactivity disorder and conduct disorder.

It is known that patients with Attention Deficit and Hyperactivity disorder (ADHD) and Conduct disorder (CD) commonly shows greater symptom severity than those with ADHD alone and worse outcomes. This study researches whether Default mode network (DMN) is altered in adolescents with ADHD + CD, relative to ADHD alone and controls or not. Ten medication-naïve boys with ADHD + CD, ten medication-naïve boys with ADHD and 10-age-matched typically developing (TD) controls underwent functional magnetic resonance imaging (fMRI) scans in the resting state and neuropsychological tasks such as the Wiscon

Neurobiological stress responses predict aggression in boys with oppositional defiant disorder/conduct disorder: a 1-year follow-up intervention study.

To improve outcome for children with antisocial and aggressive behavior, it is important to know which individual characteristics contribute to reductions in problem behavior. The predictive value of a parent training (Parent Management Training Oregon; PMTO), parenting practices (monitoring, discipline, and punishment), and child neurobiological function (heart rate, cortisol) on the course of aggression was investigated. 64 boys with oppositional defiant disorder or conduct disorder (8-12 years) participated; parents of 22 boys took part in PMTO. All data were collected before the start of t

Common Versus Specific Correlates of Fifth-Grade Conduct Disorder and Oppositional Defiant Disorder Symptoms: Comparison of Three Racial/Ethnic Groups.

The extent to which risk profiles or correlates of conduct disorder (CD) and oppositional defiant disorder (ODD) symptoms overlap among youth continues to be debated. Cross-sectional data from a large, representative community sample (N = 4,705) of African-American, Latino, and White fifth graders were used to examine overlap in correlates of CD and ODD symptoms. About 49 % of the children were boys. Analyses were conducted using negative binomial regression models, accounting for several confounding factors (e.g., attention deficit/hyperactivity disorder symptoms), sampling weights, stratific

Building an evidence base for DSM-5 conceptualizations of oppositional defiant disorder and conduct disorder: introduction to the special section.

The DSM-5 ADHD and Disruptive Behavior Disorders Work Group recently outlined a research agenda designed to support possible revisions to the diagnostic criteria for oppositional defiant disorder (ODD) and conduct disorder (CD). Some of the areas in need of further investigation include (a) examining the clinical utility of the current diagnostic system in girls, (b) further clarifying the developmental progression from ODD to CD, (c) determining whether facets of ODD symptoms can help explain heterotypic continuity and enhance predictive validity, (d) evaluating the clinical utility of a new

Prevalence, comorbidity, and demographic patterns of oppositional defiant disorder and conduct disorder in Chinese children and adolescents: a systematic review.

INTRODUCTION: The prevalence of major disruptive behavior disorders such as oppositional defiant disorder (ODD) and conduct disorder (CD) has not been fully described in Chinese youth, and their respective patterns are also understudied. This systematic review was conducted to explore prevalence rates, comorbidity, and demographic characteristics of ODD and CD in Chinese children and adolescents. METHODS: A thorough search of international databases revealed 19 peer-reviewed studies published in English between 2016 and 2025 that were pertinent and met the eligibility standards. The findings
