Indoleamine 2,3-Dioxygenase 1 Deletion-Mediated Kynurenine Insufficiency Inhibits Pathological Cardiac Hypertrophy.

BACKGROUND: Aberrant amino acid metabolism is implicated in cardiac hypertrophy, while the involvement of tryptophan metabolism in pathological cardiac hypertrophy remains elusive. Herein, we aimed to investigate the effect and potential mechanism of IDO1 (indoleamine 2,3-dioxygenase) and its metabolite kynurenine (Kyn) on pathological cardiac hypertrophy. METHODS: Transverse aortic constriction was performed to induce cardiac hypertrophy in IDO1-knockout (KO) mice and AAV9-cTNT-shIDO1 mice. Liquid chromatography-mass spectrometry was used to detect the metabolites of tryptophan-Kyn pathway.

Constrictive pericarditis versus restrictive cardiomyopathy: challenges in diagnosis and management.

This is the case of a patient who presented with severe right-sided heart failure due to diastolic dysfunction that caused a dilemma of differential diagnosis between restrictive cardiomyopathy and constrictive pericarditis. Restrictive cardiomyopathy was diagnosed based on noninvasive and invasive hemodynamic testing. However, the patient did not respond to therapy and succumbed to worsening heart failure and multiple comorbidities. Clinical features of right heart failure with edema, ascites, jugular venous distention, and tender hepatomegaly are commonly seen in clinical practice. When syst

Constrictive pericarditis in chronic ulcerative colitis.

Acute pericarditis has been described as an extraintestinal manifestation of inflammatory bowel disease (IBD), as well as a consequence of IBD treatment, specifically sulfasalazine and mesalamine. Until now, there have been no reported cases of constrictive pericarditis associated with IBD or its treatment. A 37-year-old woman with a 24-year history of chronic ulcerative colitis (CUC) presented with a 3-month history of fevers, palpitations, dyspnea, syncope, and retrosternal chest pain. Two weeks before symptoms, she had initiated oral mesalamine for an ongoing CUC flare. Physical examination

Diagnostic procedure for localized constrictive pericarditis leading to bilateral pleural effusion: two case reports.

Constrictive pericarditis is a rare disease. Localized constrictive pericarditis leading to bilateral pleural effusion is more difficult to recognize, and the diagnostic procedure can be ambiguous. Here, we report two patients diagnosed with localized constrictive pericarditis who presented with bilateral pleural effusion. A thorough work-up showed that the pleural effusion was nonspecific, as was the pathology of the pleura. One patient had a history of pericardial effusion 2 years ago, and the other had undergone surgery for an anterior mediastinum teratoma. Pericardial scarring was found on

[Comprehensive diagnosis and treatment of distal biliary strictures: multidisciplinary collaboration and precision surgical strategies].

Distal biliary strictures (DBS) are a common and complex clinical problem in pancreaticobiliary diseases, with etiologies involving multiple factors such as inflammation, benign fibrous strictures, and malignant tumors. Differentiating between benign and malignant DBS is challenging, and clinical decision-making often requires a collaborative approach involving imaging, endoscopic techniques, histopathology, and molecular diagnostics. In recent years, with the development of endoscopic ultrasound-guided fine-needle aspiration/biopsy,endoscopic retrograde cholangiopancreatography biopsy, molecular bioma

Transformers for rapid detection of airway stenosis and stridor.

Upper airway stenosis is a potentially life-threatening condition involving the narrowing of the airway. In more severe cases, airway stenosis may be accompanied by stridor, a type of disordered breathing caused by turbulent airflow. Patients with airway stenosis have a higher risk of airway failure and additional precautions must be taken before medical interventions like intubation. However, stenosis and stridor are often misdiagnosed as other respiratory conditions like asthma/wheezing, worsening outcomes. This report presents a unified dataset containing recorded breathing tasks from patie

ACG Clinical Guideline: Diagnosis and Management of Biliary Strictures.

A biliary stricture is an abnormal narrowing in the ductal drainage system of the liver that can result in clinically and physiologically relevant obstruction to the flow of bile. The most common and ominous etiology is malignancy, underscoring the importance of a high index of suspicion in the evaluation of this condition. The goals of care in patients with a biliary stricture are confirming or excluding malignancy (diagnosis) and reestablishing flow of bile to the duodenum (drainage); the approach to diagnosis and drainage varies according to anatomic location (extrahepatic vs perihilar). Entropy generation optimization for the electroosmotic MHD fluid flow over the curved stenosis artery in the presence of thrombosis.

The present study deals with the entropy generation analysis on the flow of an electrically conductive fluid (Blood) with [Formula: see text]-suspended nanoparticles through the irregular stenosed artery with thrombosis on the catheter. The fluid flow can be actuated by the interactions of different physical phenomena like electroosmosis, radiation, Joule heating and a uniform radial magnetic field. The analysis of different shapes and sizes of the nanoparticle is considered by taking the Crocine model. The velocity, temperature, and concentration distributions are computed using the Crank-Nic

Entropy optimization and response surface methodology of blood hybrid nanofluid flow through composite stenosis artery with magnetized nanoparticles (Au-Ta) for drug delivery application.

Entropy creation by a blood-hybrid nanofluid flow with gold-tantalum nanoparticles in a tilted cylindrical artery with composite stenosis under the influence of Joule heating, body acceleration, and thermal radiation is the focus of this research. Using the Sisko fluid model, the non-Newtonian behaviour of blood is investigated. The finite difference (FD) approach is used to solve the equations of motion and entropy for a system subject to certain constraints. The optimal heat transfer rate with respect to radiation, Hartmann number, and nanoparticle volume fraction is calculated using a respo

Analysis and numerical investigation of bile flow dynamics within the strictured biliary duct.

The mechanics of bile flow in the biliary system plays an important role in studying bile stasis and gallstone formation. Bile duct stricture is an abnormal phenomenon that refers to the bile duct getting smaller or narrower. The main objective of this study is to study the influence of stricture on bile flow dynamics using numerical methods. We employed a numerical Computational Fluid Dynamics model of the bile flow within a strictured hepatic duct. We studied and compared the influence of stricture severity, stricture length, eccentricity, and bile flow property on the bile flow dynamics. Th

A novel molecular communication inspired detection method for the evolution of atherosclerosis.

BACKGROUND AND OBJECTIVE: Atherosclerosis is a leading cause of potentially serious cardiovascular diseases such as heart attack, stroke, and peripheral artery disease. Due to the prolonged and non-reversible process of thickening arteries walls, atherosclerosis plaques in the blood vessels are formed that restrict the blood flow. Early detection plays a vital role in minimizing the risk as there is no reliable method to detect the early stage of the disease. This paper proposes a novel atherosclerosis detection method based on the emerging paradigm of molecular communications. The work could

Entropy and stability analysis on blood flow with nanoparticles through a stenosed artery having permeable walls.

In this research, the electro-osmotic effects are highlighted for a blood-based hybrid nanofluid flow across an artery infected with multiple stenosis. The artery has permeable walls together with slip boundary effects. The slip and permeable boundary conditions model the more realistic blood flow problems. The governing equations of the problem are converted into non-dimensional form by introducing adequate dimensionless variables and acquired the exact solutions. The detailed study of heat transfer is given by Joule heating and viscous dissipation effects. The disorder of fluid flow is inves

Phasicity and resistance of the arterial spectral Doppler waveform in a canine femoral focal artery stenosis model.

INTRODUCTION: This study aims to evaluate changes in the arterial spectral Doppler waveform in a canine artery stenosis model. METHODS: Canine femoral artery stenosis models were established in 12 beagle dogs. Doppler waveforms were recorded in the femoral artery preoperatively and postoperatively in the femoral artery and at the ankle after formation of a 50%, 70%, and 90% stenosis or occlusion. Major descriptors for arterial Doppler waveform were used to analyse waveforms. RESULTS: The proportion of multiphasic waveforms proximal to a moderate stenosis decreased compared to normal baseline

Percutaneous Nephrostomy in the Treatment of Hydronephrosis in Renal Transplant Patients - Case Report.

Percutaneous nephrostomy is a first-line minimal invasive treatment option for ureteral obstruction following kidney transplantation, with high effectiveness and a low complication rate. Percutaneous nephrostomy might be used as a temporary salvage therapy, providing acute decompression of the kidney collecting system and preventing graft loss. It can also function as a permanent and sometimes only possible option in transplant patients with frequent recurrences of ureteral stenosis who either fail an open surgical reconstruction or who are not good candidates for these procedures. We present
