[A method for the surgical treatment of maxillary micro- and retrognathism].

The proposed method of surgical treatment of maxillary micro- and retrognathia radically differs from the known methods: osteotomy of the osseous base of the hard palate is carried out parallel to the vomer to reach the level of the first premolars by a median dissection of the mucosa and periosteum, and vertical osteotomy of the anterior portion of the vomer is carried out. Horizontal osteotomy is performed from the vestibule of the mouth. Subsequent osteotomy of the anterior and lateral surfaces of the upper jaws under mucosal periosteal "tunnels" and detachment of the maxillary tuberosities

Biomechanical evaluation of suture buttons versus cortical screws in the Latarjet-Bristow procedure: a fresh-frozen cadavers study.

INTRODUCTION: A commonly used method of fixation of the transferred coracoid in the traditional Latarjet-Bristow procedure (open or arthroscopic) is by two bicortical screws. Although mechanically effective, screw fixation is also a major source of hardware and neurologic complications. This study aimed to compare the biomechanical performances of traditional metal screws and endobuttons as fixators of the Latarjet-Bristow procedure. MATERIALS AND METHODS: Nine fresh-frozen cadaveric human scapulae with the conjoined tendon attached to the coracoid process were used for the Latarjet-Bristow p

Arthroscopic and Endoscopic Technique for Subcoracoid Synovial Chondromatosis of the Shoulder Through a Medial Transpectoral Portal.

Synovial chondromatosis of the shoulder is a rare disorder characterized by metaplastic synovial proliferation, causing multiple loose bodies usually localized intra-articularly. Surgical treatment with open techniques through a deltopectoral approach has been commonly used. The evolution of arthroscopy has allowed a complete joint assessment and the extraction of intra-articular loose bodies with less morbidity than open techniques. Nevertheless, this pathology occurs less frequently in the subcoracoid bursa. Access to this bursa may be more complicated when extracting loose bodies that cause

Subclavian vein cannulation: an approach with definite landmarks.

An interesting observation, ie, the coracoclavicular line (from the lower border of the coracoid process to the upper border of the medial head of the clavicle) was parallel to the subclavian vein catheter in bedside chest roentgenograms (anteroposterior view), prompted us to use this subclavian approach. After supine positioning of the patient with the arm alongside the body and the left shoulder elevated 10 to 15 degrees, the puncture point was selected 1.5 cm away from the point where the coracoclavicular line crossed inferior border of the clavicle on the skin. The needle was moved horizon

Defining Diagnostic Criteria for Neurogenic Thoracic Outlet Syndrome: An Expert Consensus.

BACKGROUND: Neurogenic thoracic outlet syndrome (nTOS) can present itself in a number of ways, often resulting in diagnostic ambiguity. PURPOSE: To establish a consensus among upper extremity experts using the Delphi method to determine the most clinically relevant criteria in distinguishing nTOS to help create a diagnostic algorithm. STUDY DESIGN: Consensus statement. METHODS: A panel of 20 upper extremity experts rated the importance of 54 different clinical items in reference to distinguishing nTOS. Items were ranked on a scale from 0 (least important) to 10 (most important). The mean an

Anatomical study of the position and orientation of the coracoclavicular ligaments: Differences in bone tunnel position by gender.

BACKGROUND: Reconstructing both coracoclavicular ligaments following acromioclavicular dislocation has recently been reported to restore the function of the acromioclavicular joint better than traditional procedures. Knowing the appropriate position and orientation of the bone tunnels and the potential risks of neurovascular injuries leads to safe reconstruction. We aimed to answer the following questions: what is the difference in the accurate clavicular bone tunnel positions (BTPs) during coracoclavicular ligament reconstruction between sex, and what are the potential risks for neurovascular

[The Latarjet procedure in patients with anterior shoulder instability, with outcome depending on coracoid abutment location on radiography].

Anterior shoulder instability following traumatic dislocation in young adults is a common complication. The Latarjet procedure is the most widely used technique for treating this instability. It is based on coracoid abutment repair of the anterior edge of the glenoid. However, joint exposure during this surgical technique is often limited and makes it difficult to position the abutment. The purpose of this study was to evaluate the result of coracoid abutment procedure on functional outcome in the short and medium term. We assessed the positioning of the coracoid abutment on standard postopera

Innovative use of single-incision internal fixation of distal clavicle fractures augmented with coracoclavicular stabilisation.

INTRODUCTION: The management of displaced fractures of the distal clavicle remains controversial, particularly in younger patients where there is no consensus as to which surgical intervention is best. Each surgical method has unique surgical complications and rates of persistent pain and post-traumatic arthritis. We report an innovative surgical technique using a plate fixation augmented with minimally invasive tension slide coracoclavicular fixation using a cortical tenodesis button (8.5 mm). METHODS: A single-surgeon series, comprising of eleven cases, underwent retrospective review. A low

Concomitant Acromioclavicular and Coracoclavicular Ligament Reconstruction with a Duo-Figure-8 Autogenic Graft Wrapping Technique for Treating Chronic Acromioclavicular Separation.

BACKGROUD: Coracoacromial ligament transfer is the traditional procedure for treating chronic acromioclavicular separation, but it is significantly inferior to ligament reconstruction according to biomechanical and clinical studies. However, ligament reconstruction carries the risk of complications of graft loosening and peri-tunnel fractures. Currently, there is no ligament reconstruction procedure optimal for preventing such complications. The purpose of this study was to describe and retrospectively analyze the clinical and radiological outcomes of a "duo-figure-8" autogenic graft wrapping

Uncommon Localization of Tenosynovial Giant Cell Tumor in the Coracoclavicular Ligament: Diagnostic Challenges.

BACKGROUND Tenosynovial giant cell tumor (TGCT) can be characterized as localized when it occurs in the fingers and wrist, or diffuse, when it occurs in large weight-bearing joints, such as hips, knees, and ankles. However, the occurrence of TGCT in the coracoclavicular ligament is rare and has not been reported in the literature. In this case, inexperienced doctors are prone to misdiagnosis or underdiagnosis, which in turn leads to patients not being able to get the correct treatment in time. CASE REPORT This is a report of a 16-year-old female patient with symptoms of right shoulder pain and

The long-term efficacy of the GraftRope technique.

BACKGROUND: Acromioclavicular joint separations continue to be a challenge for surgeons, and modern arthroscopically assisted techniques are becoming increasingly widespread. The aim of this study is to evaluate if the use of a biological support in association with a nonresorbable subcoracoid fixation can improve long-term stability in acromioclavicular joint dislocation surgically treated. We assessed clinical and radiographic results, patients' return to daily activities and the risk of complications. MATERIALS AND METHODS: Fourteen patients underwent subcoracoid fixation with the GraftRop

Ultrasound-guided coracohumeral ligament release.

Long-term follow-up of patients with adhesive capsulitis (AC) reveals that approximately half of them suffer from a limited range of shoulder motion, particularly external and/or internal rotation. We report the surgical technique and short-term clinical outcomes of ultrasound-guided release of the thickened coracohumeral (CH) ligament in 8 patients (9 shoulders) with AC. Passive external rotation with the arm by the side significantly increased from an average of 18° preoperatively to 47° immediately after CH ligament release. VAS and ASES scores were improved at 3months follow-up in all 9 sh

The treatment of lateral clavicle fractures.

This study assesses the results of surgical treatment of 15 displaced Neer type II fractures of the lateral clavicle in 15 patients, which occurred between November 1988 and March 1995 and which were followed up for a mean period of 4.6 years (range 2-9 years). The patients fell into two groups, one 'acute group' and one 'non-union' group. Patients treated initially by a non-operative approach had suffered prolonged morbidity and time off work prior to and after surgery. The ultimate result was good. The fixation used was a Dacron arterial graft as a sling around the clavicle and coracoid proc

[Costo-clavicular outlet syndrome].

Syndromes caused by the compression of the neurovascular bundle are not a rare pathology, though their exact frequency cannot be assessed. Neurovascular bundle in the upper limbs may be compressed at the costo-clavicular space, interscalene triangle or at the insertion of the minor pectoralis muscle into the coracoid process. More than 90% of patients present neurological symptoms and 10% also have vascular problems. Diagnosis depends on a careful clinical and instrumental study of the patients. Arteriography, phlebography and Doppler tests were of value in the diagnostic approach, showing the

An historical look at the thoracic outlet compression syndrome.

The story of the thoracic outlet compression syndrome begins with the identification of the anatomical abnormality of the cervical rib, and the symptoms related to it. Resection of the cervical rib, however, was soon followed by the recognition that symptoms could occur in the absence of a cervical rib. The scalenus anticus was then held to be the culprit, although several different mechanisms were advanced to explain the development of symptoms. Subsets of the thoracic outlet compression syndrome were then codified; costoclavicular compression; compression under the coracoid process during hy

Considerations For Surgical Planning Of Humeral Osteotomy In Brachial Plexus Birth Palsy Based On The Elbow Crease And Humeral Retroversion Measurement.

In obstetric brachial plexus, injuries are characterized by contractures, weakness and an association with different bone deformities, such as elongation of the coracoid process and/or acromion, retroversion of the glenoid cavity, flattening of the humeral head and Scapular Hypoplasia with Elevation and Rotation (SHEAR). Another humeral deformity is the retroversion of the humeral head, cases of which are increased in the presence of plexus lesions. The purpose of the current manuscript is to highlight two aspects of this latter deformity. First, the importance of taking into consideration the

Shoulder Acromioclavicular and Coracoclavicular Ligament Injuries: Common Problems and Solutions.

Injuries to the acromioclavicular joint and coracoclavicular ligaments are common. Many of these injuries heal with nonoperative management. However, more severe injuries may lead to continued pain and shoulder dysfunction. In these patients, surgical techniques have been described to reconstruct the function of the coracoclavicular ligaments to provide stable relationship between the clavicle and scapula. These surgeries have been fraught with high complication rates including clavicle and coracoid fractures, infection, loss of reduction and fixation, hardware migration, and osteolysis. This

Automated radiographic shoulder balance assessment in scoliosis via deep learning.

The objective of this study was to develop an automated deep learning-based method for the assessment of shoulder balance in adolescent idiopathic scoliosis (AIS) patients using X-ray images in order to provide a reliable and efficient alternative to manual measurements. A total of 940 AIS radiographs were screened; 937 cases were included in the model-development cohort after quality control and were annotated for precise identification and segmentation of the T1 vertebra, both clavicles, and both coracoids. A deep learning neural network was used to segment these structures. Landmarks were e

Is the two-dimensional computed tomography scan analysis reliable for coracoid graft positioning in Latarjet procedures?

BACKGROUND: The aim of the study was to develop a computed tomography (CT)-based measurement protocol for coracoid graft (CG) placement in both axial and sagittal planes after a Latarjet procedure and to test its intraobserver and interobserver reliability. METHODS: Fifteen postoperative CT scans were included to assess the intraobserver and interobserver reproducibility of a standardized protocol among 3 senior and 3 junior shoulder surgeons. The evaluation sequence included CG positioning, its contact area with the glenoid, and the angle of its screws in the axial plane. The percentage of C
