Exploring Barriers and Facilitators to Deprescribing Psychotropics in Child and Adolescent Psychiatry: A Qualitative Study from India.

BACKGROUND: Deprescribing, a structured, planned, and supervised approach to medication discontinuation, is increasingly recognized as an imperative rather than an elective component of rational psychopharmacological practice. Deprescribing is particularly compelling in child and adolescent psychiatry, where, despite accumulating evidence for pharmacotherapy, uncertainties regarding long-term risks and benefits persist. This makes deprescribing an important and often complex clinical consideration. Despite growing international attention, deprescribing remains challenging to implement in routi

Bovine oviductal and uterine extracellular vesicles enhance blastocyst development in confined individual embryo culture.

PURPOSE: In vitro-produced embryos exhibit reduced developmental competence compared to their in vivo-derived counterparts due to the inability to accurately replicate the complex milieu of the maternal reproductive tract. A key aspect in vivo is the confinement of embryos in small volumes of oviductal and uterine fluids, where maternal extracellular vesicles (EVs) deliver their cargos into developing embryos, influencing their competence. The main aim of this study was to investigate the effects of the sequential exposure to oviductal and uterine EVs on the developmental competence of bovine

Effects of luteinizing hormone and follicle stimulating hormone on the developmental competence of porcine preantral follicle oocytes grown in vitro.

BACKGROUND: There has been controversy over the role of FSH in the regulation of preantral follicle development. LH is a survival and differentiation factor that increases oocyte maturation in FSH-supplemented cultures of mouse preantral follicles. However, little information exists on the action of LH and FSH in the developmental competence of porcine preantral follicle oocytes in vitro. MATERIALS AND METHODS: Porcine preantral follicles were cultured for 3 days in the presence or absence of FSH or LH. Oocytes from these follicles were then matured, fertilized in vitro, and embryos were cult

Ammonia Determines the Alternative Pathways of Sexual or Asexual Development in the Cellular Slime Mold Dictyostelium discoideum: (Dictyostelium discoideum/cell fusion/developmental pathways/ammonia effect/sexual development).

The sexual development, macrocyst formation, of Dictyostelium discoideum is initiated by sexual fusion of cells. The sexual fusion is only taken place under the culture conditions of excess water and darkness. Under these conditions, cells acquire the fusion competence, but lose it when cell density is high. The loss of the fusion competence is caused by accumulation of ammonia excreted by cells in a culture. Ammonia suppresses the fusion competence of cells at a certain concentration, and consequently inhibits formation of macrocysts and induces fruiting-body formation. Thus, excess water ind

Are You Qualified, ChatGPT? Examining Clinical Skills and Competencies of ChatGPT in Delivering Systemic Interventions.

Large language model agents such as ChatGPT are increasingly used for relationship advice and mental health support, with many users describing these interactions as therapeutic. This study assessed the clinical and cultural competence of a GPT-based model trained to provide systemic interventions aligned with the AAMFT Code of Ethics and core skills. A volunteer client discussed a relational concern with the model, and the resulting transcript was evaluated by 12 experienced supervisors using standardized measures. Supervisors, unaware of the study's purpose and the therapist being an AI, rat

Patient identity narratives through the cultural formulation interview in a New York City outpatient clinic.

Anthropologists have critiqued cultural competence programs in medical settings while introducing mental health clinicians to social theories on culture for practice. We explore how patients articulated narratives about themselves and how clinicians responded to such narratives through an intervention known as the Cultural Formulation Interview that anthropologists have helped develop. We conducted over 500 hours of fieldwork from 2014 to 2019 at an outpatient clinic in New York, analyzing multiple data (participant observation, medical records, patient-clinician sessions, and individual debri

BLACK-ARTS: a qualitative study exploring the insights of black young people with psychiatric disorders and the uses of creative arts therapies to improve mental well-being.

BACKGROUND: Black people experience persistent inequalities in mental health outcomes, access and care quality compared with their white counterparts. These disparities are compounded by structural racism and a lack of culturally competent care within National Health Service (NHS) services. Creative arts therapies (CATs), including music, art, dance and drama therapy, may offer culturally relevant, non-verbal approaches to mental health support for this population. OBJECTIVE: To explore how black young people with mental health challenges and NHS clinicians perceive the usefulness and cultura

Infertility in the Midwest: perceptions and attitudes of current treatment.

BACKGROUND: Although infertility affects an estimated 6.1 million individuals in the United States, only half of those individuals seek fertility treatment and the majority of those patients are White and of high socioeconomic status. Research has shown that insurance mandates are not enough to ensure equal access. Many workplaces, schools, and medical education programs have made efforts in recent years to improve the cultural humility of providers in efforts to engage more racially and economically underrepresented groups in medical care. However, these efforts have not been assessed on a po

Oro-faecal transmission of SARS-CoV-2: A systematic review of studies employing viral culture from gastrointestinal and other potential oro-faecal sources and evidence for transmission to humans.

The extent to which the oro-faecal route contributes to the transmission of SARS-CoV-2 is not established. We systematically reviewed the evidence on the presence of infectious SARS-CoV-2 in faeces and other gastrointestinal sources by examining studies that used viral culture to investigate the presence of replication-competent virus in these samples. We conducted searches in the WHO COVID-19 Database, LitCovid, medRxiv, and Google Scholar for SARS-CoV-2 using keywords and associated synonyms, with a search date up to 28 November 2023.We included 13 studies involving 229 COVID-19 subjects - pr

The power of representation in health care: Examining the effect of physician's race on interest in seeking medical care.

OBJECTIVE: Most U.S. physicians are White, creating racially/ethnically discordant patient-physician interactions for Latine Americans, the largest U.S. racial/ethnic minority group and the least likely to receive preventive care. Signaling in-group representation can increase use of medical services, so we examined whether Latine Americans perceive a Latine physician (minoritized ingroup) or a Black physician (minoritized outgroup) as more culturally competent and elicit greater interest in seeking care than a White physician (privileged outgroup). METHOD: Across two studies, participants (N

Contextualizing Competence: Language and LGBT-Based Competency in Health Care.

Changes in the language and terminology used to refer to individuals identifying as lesbian, gay, bisexual, and transgender (LGBT), as well as how best to discuss issues of sexual and gender identity, can prove challenging for health care providers due to (1) lack of training; (2) interdisciplinary issues; and (3) prejudices on personal and institutional levels. Given the importance of language in the relationship between health care provider and patient as well as the myriad ways in which language can reflect knowledge, skills, and attitudes, we contend that language is both a facilitator and
