[Experience in the treatment of interstitial cystitis: review of 17 cases].

BACKGROUND: The Interstitial Cystitis (IC) has been considered in the past an uncommon pathology, however it has received a special attention during the last 20 years, (1678 scientific articles published since 1984 to 2009). There are many therapeutic options not at all satisfactory because there isn't consensus about diagnostic and treatment. OBJECTIVE: To share our experiences in the treatment of interstitial cystitis, additionally, to comment on the therapeutic response of treatment used. MATERIAL AND METHOD: Observational, retrospective an analytic study of 17 treated patients from 22 wi

Bladder pain syndrome/interstitial cystitis: a reappraisal for the clinician.

Bladder pain syndrome--formally known as interstitial cystitis--is a chronic bladder disorder characterized by pelvic pain and urinary irritability symptoms. The physiopathology is still unclear but is thought to involve bladder mucosal injury, inflammation and neurologic dysfunction. It is hard to diagnose this entity due to symptoms that are common to several other pathologies such as chronic pelvic pain, endometriosis, overactive bladder, urinary tract infection, and vulvodynia, and due to the lack of specific findings. A combination of history, physical examination, and diagnostic tools he

The relationship of interstitial cystitis/painful bladder syndrome to vulvodynia.

INTRODUCTION: Many patients have interstitial cystitis/painful bladder syndrome (IC/PBS), a condition of frequency, urgency, and pain affecting more than 1 million women in the United States. The vulva, not the urethra or bladder, may actually be the site of some of the reported pain in women with IC/PBS. PURPOSE: The purpose of this study was to identify the presence of vulvodynia in women diagnosed with IC/PBS. METHOD: A mailed survey was used to identify women with IC/PBS who also reported vulvar pain. The survey also identified related factors, such as menstrual/hormonal status, sexual f

Prevalence of pelvic floor dysfunction in patients with interstitial cystitis.

OBJECTIVES: To evaluate the prevalence of pelvic floor dysfunction in women with interstitial cystitis (IC). METHODS: Women with IC and pelvic pain were referred to the Beaumont Women's Initiative for Pelvic Pain and Sexual Health program. A comprehensive patient history and pelvic examination were completed by a certified women's health nurse practitioner. RESULTS: Seventy women with a mean age of 45 years were evaluated. Of these 70 women 87% had levator pain consistent with pelvic floor dysfunction. The mean levator pain score was 4.48 out of 10. Nearly two thirds of these women (64%) had

The mast cell in interstitial cystitis: role in pathophysiology and pathogenesis.

Current evidence from clinical and laboratory studies confirms that mast cells play a central role in the pathogenesis and pathophysiology of interstitial cystitis (IC). In this article, we focus on the role of the mast cell in IC and examine the ways in which mast cells and other pathophysiologic mechanisms are interrelated in this disease. Identifying the patients with IC who have mast cell proliferation and activation will enable us to address this aspect of disease pathophysiology in these individuals with targeted pharmacotherapy to inhibit mast cell activation and mediator release.

The role of pentosan polysulfate in treatment approaches for interstitial cystitis.

Studies of the mechanisms and causes of interstitial cystitis (IC) and of the properties of pentosan polysulfate have provided a scientific rationale for using pentosan polysulfate to treat IC. In randomized, double-blind studies, patient and investigator evaluations of pentosan polysulfate in the treatment of IC resulted in favorable assessments of the drug. In addition, IC patients in two out of four randomized, prospective trials improved significantly in most variables with treatment by oral pentosan polysulfate; in the two other studies, the IC patients improved in some domains with pento

Observations on the presentation, diagnosis, and treatment of interstitial cystitis in men.

This article presents and evaluates the symptoms, presentation, diagnosis, and treatment of men with interstitial cystitis (IC). A retrospective chart review and an interview of all men in our practice diagnosed with IC since 1990 was performed. The patients' presenting symptoms, physical findings, clinical evaluation, and responses to therapy were reviewed. A total of 52 men were identified during the study who met the National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK) criteria for diagnosis of IC. The most common referral diagnosis was prostatitis with the most common p

Seeking A Rational Approach to the Diagnosis and Treatment of Interstitial Cystitis.

At least 90% of the half-million Americans with interstitial cystitis (IC) are women. But correctly diagnosing and treating the problem are not as clear as the statistics. The clinical hallmarks--bladder pain, urinary urgency and frequency, nocturia, and dysuria--are not confined to IC. A key feature of diagnosing the problem is to exclude the vast array of other possibilities, including carcinoma and detrusor hyperreflexia. To confirm the diagnosis, cystoscopic exam with hydrodistention is needed to visualize diffuse glomerulations or classic Hunner's ulcer. Treatment is empiric and can invol

Living With Interstitial Cystitis: A Qualitative Study of Women's Experiences.

BACKGROUND: Interstitial cystitis is a complex, highly challenging chronic inflammatory condition that is most predominant in women. It is characterised by persistent pain in the bladder and pelvis area and an urgent and frequent need to urinate during the day and at night. Despite the chronic and burdensome nature of interstitial cystitis, the lived experiences of women suffering from it have been inadequately explored. OBJECTIVE: We sought to acquire in-depth knowledge and insights into the experiences of Norwegian women living with interstitial cystitis. METHODS: A qualitative approach wa

Lifestyle and behavioral modifications made by patients with interstitial cystitis.

Interstitial cystitis/bladder pain syndrome (IC/BPS) negatively affects the quality of life. In this study, we investigated the lifestyle behavioral changes patients with IC/BPS make to cope with their symptoms. This prospective study was conducted between August 2018 and June 2019. All patients had a primary symptom of suprapubic pain with a full bladder and other lower urinary tract symptoms for more than 6 weeks as well as cystoscopic findings. All participants completed our self-developed questionnaire, which included informations about their living and work environment, occupational garme

Urinary microbiome in uncomplicated and interstitial cystitis: is there any similarity?

PURPOSE: Acute/uncomplicated cystitis is the most common bacterial infection causing inflammation in the bladder tissues and predominantly diagnosed in women. Interstitial cystitis may too, cause inflammation in the bladder but its etiology has been elusive. Even though the site and symptoms of both diseases are largely shared, state of the urinary microbiome in these disorders have not been comparatively evaluated before. The purpose of this review is to assess and qualitatively compare structure and composition of the urinary microbiome in acute/uncomplicated cystitis and interstitial cystit

Development of an interstitial cystitis risk score for bladder permeability.

BACKGROUND: Interstitial cystitis/bladder pain syndrome (IC) is a multifactorial syndrome of severe pelvic and genitalia pain and compromised urinary function; a subset of IC patients present with Hunner's lesions or ulcers on their bladder walls (UIC). UIC is diagnosed by cystoscopy, which may be quite painful. The objective of this study was to determine if a calculated Bladder Permeability Defect Risk Score (BP-RS) based on non-invasive urinary cytokines could discriminate UIC patients from controls and IC patients without Hunner's ulcers. METHODS: A national crowdsourcing effort targeted

The Challenges of Interstitial Cystitis: Current Status and Future Prospects.

Interstitial cystitis/bladder pain syndrome (IC/BPS) is a syndrome of unpleasant bladder sensations and lower urinary tract symptoms. The three main proposed etiologies are bladder urothelial dysfunction, bladder inflammation (possible neurogenic), and neuropathic pain. Despite decades of basic and clinical research, IC/BPS remains difficult to treat. A variety of treatments are used, each aimed towards one etiology. For example, glycosaminoglycans are thought to improve the urothelial permeability barrier, anti-inflammatory agents are used to decrease general inflammation, and mast cell stabi

Alterations of microbiota in urine from women with interstitial cystitis.

BACKGROUND: Interstitial Cystitis (IC) is a chronic inflammatory condition of the bladder with unknown etiology. The aim of this study was to characterize the microbial community present in the urine from IC female patients by 454 high throughput sequencing of the 16S variable regions V1V2 and V6. The taxonomical composition, richness and diversity of the IC microbiota were determined and compared to the microbial profile of asymptomatic healthy female (HF) urine. RESULTS: The composition and distribution of bacterial sequences differed between the urine microbiota of IC patients and HFs. Red
