Dysbaric osteonecrosis: a reassessment and hypothesis.

Dysbaric osteonecrosis is associated with exposure to large ambient pressure changes, and comprises necrotic lesions in the fatty marrow-containing shafts of the long bones, and the ball and socket joints (hips and shoulders). The fundamental causes are still in question and the illness remains a significant health hazard. Radiological and pathological features of both dysbaric and non-dysbaric osteonecrosis are indistinguishable and both are characterized by intramedullary venous stasis, ischemia and necrosis of bone. It has been generally accepted that gas bubbles (probably by initiating int

Dysbaric osteonecrosis in divers and caisson workers. An animal model.

Dysbaric osteonecrosis was induced successfully in adult sheep after 12 to 13, 24-hour exposures to compressed air (2.6-2.9 atmospheres absolute) during a 2-month period. All exposed sheep had decompression sickness and extensive bone and marrow necrosis in their long bones. Radiographic analysis of these progressive lesions showed mottled to distinct medullary opacities and endosteal thickening characteristic of dysbaric osteonecrosis. Six months after the last hyperbaric exposure, neovascularization of once ischemic fatty marrow was centripetal from the diaphyseal cortex. Proliferating endos

Acute ischemic colitis during scuba diving: report of a unique case.

The presentation of clinical symptoms due to decompression during diving, varies significantly, as mainly minor disturbances for the gastrointestinal tract in particular have been reported. The following case debates whether diving can cause severe symptoms from the gastrointestinal system. We describe a clinical case of ischemic colitis presented in a 27-year-old male, who manifested abdominal pain while in the process of scuba diving 20 meters undersea, followed by bloody diarrhoea as soon as he ascended to sea level. Taking into account his past medical history, the thorough, impeccable cli

Decompression illness secondary to occupational diving: recommended management based current legistation and practice in Malaysia.

Occupational divers are exposed to hazards which contribute to the risk of developing decompression illnesses (DCI). DCI consists of Type I decompression sickness (DCS), Type II DCS and arterial gas embolism (AGE), developed from formation of bubbles in the tissues or circulation as a result of inadequate elimination of inert gas (nitrogen) after a dive. In Malaysia, DCI is one of the significant contributions to mortality and permanent residual morbidity in diving accidents. This is a case of a diver who suffered from Type II DCS with neurological complications due to an occupational diving a

Decompression from saturation using oxygen: its effect on DCS and RNA in large swine.

INTRODUCTION: The use of hyperbaric oxygen (HBO) to expedite decompression from saturation has not been proven and may increase risk of toxicity to the pulmonary system. To evaluate any benefit of HBO during decompression, we used a 70-kg swine model of saturation and examined lung tissue by microarray analysis for evidence of RNA regulation. METHODS: Unrestrained, non-sedated swine were compressed to 132 fsw (5 ATA) for 22 h to achieve saturation. Animals then underwent decompression on air (AirD) or HBO (HBOD) starting at 45 fsw (2.36 ATA). Animals were evaluated for Type I and Type II deco

Characteristics and Prognostic Analysis in Diving-Induced Ear Trauma and Sudden Hearing Loss.

Background/Objectives: Diving exposure can cause auditory injury involving both the middle and inner ear structures. Inner ear barotrauma (IEB) and inner ear decompression sickness (IEDCS) are the major inner ear disorders and frequently present with auditory and vestibular symptoms. This study examined how diving characteristics relate to patterns of auditory trauma. Methods: A retrospective chart review of 30 patients, with 36 affected ears, was performed. Diving depth, clinical manifestations, and treatment responses were analyzed to identify factors influencing related prognosis. Results:

Measuring whole body inert gas wash-out.

INTRODUCTION: Quantifying inert gas wash-out is crucial to understanding the pathophysiology of decompression sickness. In this study, we developed a portable closed-circuit device for measuring inert gas wash-out and validated its precision and accuracy both with and without human subjects. METHODS: We developed an exhalate monitor with sensors for volume, temperature, water vapor and oxygen. Inert gas volume was extrapolated from these inputs using the ideal gas law. The device's ability to detect volume differences while connected to a breathing machine was analysed by injecting a given ga

Measuring whole-body inert gas uptake and washout during submersion.

INTRODUCTION: Quantifying inert gas uptake and washout is critical for understanding decompression sickness (DCS). However, the limited amount of data has made it difficult to integrate inert gas kinetics into risk models for DCS. Measuring whole-body inert gas kinetics during submersion is technically challenging. This study presents a novel method for quantifying inert gas uptake and washout in human divers using a rebreather-based system. METHODS: During constant-depth diving with a closed-circuit system that maintains a constant oxygen partial pressure, changes in buoyancy will reflect th

Spinal cord syndromes.

Spinal cord infarction is much rarer than cerebral stroke, but its early recognition is important as it may signify serious aortic conditions. The most frequent type is anterior spinal artery syndrome, presenting with bilateral weakness (usually paraparesis), impairment of spinothalamic sensation and preservation of deep sensation. Depending on its level, it may present with respiratory dysfunction. More rarely, posterior infarcts sparing spinothalamic sensation but involving lemniscal sensation may be encountered. Unilateral, central or transverse infarction may also be seen probably on accou

[Patent foramen ovale: current state of the art].

Patent foramen ovale is a congenital cardiac lesion that persists into adulthood and which is present in over 25% of the adult population. Its diagnosis, evaluation and treatment have attracted increasing interest as it has been suggested that it may be associated with various pathologic conditions, such as cryptogenic stroke, platypnea-orthodeoxia syndrome, decompression sickness and migraine. However, data on these associations are contradictory. Similarly, the optimum treatment of patients with patent foramen ovale is still debated. This article contains a review of the anatomy, embryology

Cochleo-vestibular disturbances in diving.

Insidious development of high-tone sensorineural hearing loss may be associated with diving, but the evidence is not certain and further research is needed. 'Internal ear barotrauma' can cause an acute or relatively acute onset of hearing loss and/or vertigo, and it may be that 'alternobaric vertigo' provides a link between the insidious and acute forms of labyrinthine injury in diving. With deep diving, decompression sickness and other syndromes can also affect the cochleo-vestibular system. These aetiologies and effects will be discussed, together with evidence from an audiometric survey of

A Rare Complication of Hyperbaric Oxygen Therapy: Air Bubbles in Systemic Veins.

Venous air embolism is a rare but clinically significant complication of hyperbaric oxygen therapy (HBOT), which is widely used in the treatment of conditions including carbon monoxide poisoning, decompression sickness, and wound healing. We report a case of systemic venous air embolism following HBOT in an 85-year-old woman treated for carbon monoxide poisoning. The patient had syncope, dizziness and nausea. Abdominal CT revealed multiple venous air bubbles. This case highlights a scarcely reported complication of HBOT and the importance of recognizing imaging findings and potential clinical

Rhabdomyolysis in McArdle disease caused by scuba diving.

McArdle disease is a glycogen storage disease that results in rhabdomyolysis during intense exercise. A number of different triggers have been described. We evaluated a patient with McArdle disease who presented with rhabdomyolysis after recreational scuba diving. There was no concern for barotrauma or decompression sickness. His symptoms resolved with standard-of-care management for non-diving-related rhabdomyolysis. Features of his experience provoked questions about the diving-related factors contributing to his presentation. We present the case and explore possible mechanisms of diving-rel

Headache and facial pain in scuba divers.

Headache occasionally occurs during or after scuba diving. Although its significance often is benign, headache may signal a serious neurological disorder in some circumstances. In addition to the usual causes of headache, the diagnostic evaluation should consider otic and paranasal sinus barotrauma, arterial gas embolism, decompression sickness, carbon dioxide retention, carbon monoxide toxicity, hyperbaric-triggered migraine, cervical and temporomandibular joint strain, supraorbital neuralgia, carotid artery dissection, and exertional and cold stimulus headache syndromes. Focal neurologic sym

The role of hyperbaric oxygen in current emergency medical care.

HBO is the definitive treatment for carbon monoxide poisoning, air or gas embolism, and decompression sickness. It is an important adjunct to other treatment modalities in the management of patients with cyanide poisoning, exceptional blood loss (anemia), crush injury, and gas gangrene. In addition, HBO has many investigational uses such as reduction of edema in acute bowel obstruction before surgical intervention. It should be remembered that the best hope for tissue recovery occurs when HBO is initiated within 4 to 6 hours of injury. After this time irreversible muscle and peripheral nerve d

Patent Foramen Ovale and Decompression Illness: The Present and Future.

Presence of patent foramen ovale (PFO), particularly if high-grade, increases the risk of decompression illness (DCI) and its severe forms. In unprovoked or recurrent DCI, neurologic, cutaneous, or cardiopulmonary DCI, testing for PFO is indicated with bubble contrast echocardiography or transcranial Doppler using provocative maneuvers. In patients with PFO and history of DCI, evaluation by a cardiologist with expertise in diving medicine is recommended. Consideration should be given to PFO closure if cessation of diving or conservative diving cannot be achieved. Prospective studies evaluating

Dysbaric osteonecrosis in diving fisherman: a case report.

Dysbaric osteonecrosis (DON) is defined as avascular bone necrosis, usually involving specific parts of the long bones, which is seen in divers or compressed-air workers due to exposure to pressure. We describe a case of DON in an artisanal diving fisherman working underwater for many years. Methods: A 48-year-old male case was admitted to the occupational disease outpatient with left shoulder and arm pain for 1-2 years. Since the age of 20, he has been artisanal diving fishing with a hookah at a depth of 20-25 meters in the sea. In 2011, he received hyperbaric oxygen therapy for widespread pa

Clinical problem solving: Mental confusion and hypoxaemia after scuba diving.

INTRODUCTION: We report a case of a diving accident associating both cerebral symptoms and signs of respiratory impairment after two dives. The objective is to describe the process for obtaining the diagnosis. CASE REPORT: A 52-year-old man experienced mental confusion associated with hypoxaemia after surfacing. All decompression procedures were fully respected. The diver had a spatio-temporal disorientation accompanied by a marked tendency to fall asleep spontaneously. He had no dyspnoea and no cough, but crepitations at both lung bases were found with oxygen saturation at 80%. CONCLUSIONS:

Do skin rash and cutis marmorata stem from lamellar bodies within the skin?

Cutis marmorata (CM) manifests as bluish-red spots on the skin following decompression. These are often itchy or painful to touch, and appear half to one hour after surfacing. The pathogenesis of skin lesions in decompression illness (DCI) remains unresolved. The common belief has been that bubbles that shunted to the arterial circulation reached the skin and clogged blood vessels. An alternative explanation from studies in which air was injected into the internal carotid artery of swine is that arterial bubbles at the brain stem disturb the control of skin blood flow, causing CM. Other brain
