OBJECTIVE: Traumatic brain injury (TBI) in pediatric patients is associated with significant mortality. Management ranges from conservative treatment to decompressive craniectomy (DC). Current guidelines weakly recommend DC in pediatric patients experiencing neurologic deterioration, herniation, or refractory intracranial hypertension. There are significant deficits in the literature regarding the benefits and optimal timing of DC for pediatric TBI. The objective of this review is to address these gaps via the largest meta-analysis on this subject to date.
METHODS: Several electronic database

PURPOSE: Traumatic brain injury (TBI) is a leading cause of death and disability in the pediatric population. Decompressive craniectomy (DC) is an important surgical intervention for severe TBI with refractory intracranial hypertension (IH). However, it is classically performed early for primary injuries such as acute subdural hematoma. In rare cases, delayed IH develops, requiring rescue DC. Here, we analyze a cohort of pediatric patients with severe TBI who required rescue DC (> 24 h) for predictive characteristics.
METHODS: A retrospective case series through the Children's Hospital of Pit

PURPOSE: Acute ischemic stroke induces rapid neuronal death and time is a key factor in its treatment. Despite timely recanalization, malignant cerebral infarction can ensue, requiring decompressive surgery (DC). The ideal timing of surgery is still a matter of debate; in this study, we attempt to establish the ideal time to perform surgery in this population.
METHODS: We conducted a retrospective study of patients undergoing DC for stroke at our department. The indication for DC was based on drop in level of consciousness and standard imaging parameters. Patients were stratified according to

OBJECTIVE: Surgeons commonly perform Decompressive craniectomy (DC) to manage patients with cerebral ischemic infarction. However, there are conflicting data on the long-term functional outcomes following DC. Therefore, this study aims to determine the functional outcome of patients with cerebral ischemic infarction after DC.
METHODS: This prospective and retrospective cross-sectional study included 148 patients with cerebral ischemic infarction who underwent DC at Ghaem Hospital, Mashhad, Iran, from March 2011 to March 2021. The Modified Rankin Scale (mRS) assesses disability in these patien

OBJECTIVES: Craniectomy is widely performed to lower the intracranial pressure in various conditions, such as traumatic brain injury, stroke, or brain swelling. Several complications can occur after craniectomy and cranioplasty, which significantly affect the prognosis of the patients after surgery. We studied the complications of craniectomy and cranioplasty and the factors affecting prognosis after the operation.
METHODS: Patients who underwent cranioplasty after craniectomy at Daejeon St. Mary's Hospital from 2015 to 2021 were included. We retrospectively reviewed their medical records and

Euglycemic diabetic ketoacidosis (DKA) is a well-recognized adverse effect associated with the use of sodium-glucose co-transporter-2 (SGLT-2) inhibitors. Early recognition of this medical emergency and timely intervention can prevent the notorious consequences of this serious complication. However, this form of DKA can easily be masqueraded by normal serum glucose levels. This article describes a 49-year-old man diagnosed with type 2 diabetes mellitus (DM) on dapagliflozin who presented with a large right-sided middle cerebral artery (MCA) stroke complicated by euglycemic DKA, developed 72 ho

OBJECTIVE: Decompressive craniectomy (DC) is used to treat intractable intracranial hypertension after severe traumatic brain injury (TBI). Cranioplasty (CP) is typically performed weeks or months later. However, the optimal timing for CP is unknown. We aimed to determine the earliest possible time point for CP.
METHODS: We retrospectively reviewed brain computed tomography images from 159 patients who underwent CP after DC for TBI at 3 hospitals. We determined the earliest possible day for CP by reviewing the resolution of intracranial pressure in serial brain computed tomography images betw

INTRODUCTION: Literature remains sparse regarding decompressive craniectomy (DC) in traumatic brain injury (TBI) in very young children. This study analysed the indications, complications and outcome of young children undergoing DC for TBI at our institute.
STUDY DESIGN: Retrospective.
OBSERVATIONS: The total number of patients was 71. Mean age was 1.6 years. Mean duration from injury to surgery was 11.9 hours (range = 3-80 hours). Around 50% had severe head injury. Intracranial pressure (ICP) monitoring was done in 33 patients. Mean ICP was 22.2 mm Hg (range = 9-50 mm Hg). The threshold ICP

BACKGROUND: Renewed interest has developed in decompressive craniectomy, and improved survival is shown when this treatment is used after malignant middle cerebral artery infarction. The aim of this study was to investigate the frequency and possible risk factors for developing surgical site infection (SSI) after delayed cranioplasty using autologous, cryopreserved bone.
METHODS: This retrospective study included 74 consecutive patients treated with decompressive craniectomy during the time period May 1998 to October 2010 for various non-traumatic conditions causing increased intracranial pre

INTRODUCTION: Giant frontal mucocele (GFM) is an extremely rare cause of frontal lobe syndrome. Subdural empyema (SDE) is an uncommon complication of paranasal sinisutis, for which craniotomy and decompressive craniotomy are the most effective surgical procedures.
CASE REPORT: A 54-year-old man was brought unconscious to the Emergency Room where recurrent generalized seizures occurred. Heroine abuse, HCV related hepatitis, prolonged antibiotic therapy for treatment of purulent rhinorrhea, along with recent personality changes, was reported. High white blood cell count, pansinusitis, GFM, SDE

BACKGROUND: It was recently suggested that communicating hydrocephalus is an almost universal finding after hemicraniectomy and that early cranioplasty may prevent the need for permanent cerebrospinal fluid diversion in these patients.
OBJECTIVE: To conduct a study in an attempt to verify these findings.
METHODS: The medical records of all patients who underwent decompressive craniectomy for medically refractory elevated intracranial pressure between 2001 and 2009 were retrospectively reviewed. Patients with subarachnoid hemorrhage, intraventricular hemorrhage, or head trauma were excluded.

OBJECTIVE: Optimal timing of ventriculoperitoneal shunting (VPS) relative to cranioplasty (CP) after decompressive craniectomy remains controversial. VPS may be performed before, during, or after CP, but no consensus exists on which sequence minimizes complications. We performed a systematic review and network meta-analysis to compare the safety profiles of pre-, simultaneous, and post-CP shunting.
METHODS: We searched PubMed/MEDLINE, Embase, and CENTRAL from inception to October 2025. Eligible studies directly compared at least two timing strategies in patients undergoing both CP and VPS. Pr

OBJECTIVE: To describe the long-term functional outcome of traumatic brain injury (TBI) patients treated by decompressive craniectomy (DC).
METHODS: Data was collected on decompressive craniectomy performed on TBI patients admitted between May 1, 2018, and May 1, 2021, using a multi-center, cross-sectional study design. The long-term outcomes of survivors were assessed using a structured extended Glasgow Outcome Scale (GOSE) questionnaire. Descriptive statistics, including frequency, mean, median, and range, were analyzed. Predictors of functional outcomes were determined using multivariate r

BACKGROUND AND OBJECTIVES: Traumatic brain injury (TBI) and stroke constitute 60% of the global neurosurgical case volume. Although decompressive craniectomy (DC) has been historically used for treating elevated intracranial pressures (ICP), it remains a controversial technique and is also associated with cranioplasty-related complications and additional costs. Hinge craniotomy (HC) may offer a potentially safer and, importantly, cheaper alternative to DC in low- and middle-income countries (LMICs), which bear the greatest burden of TBI cases. In this article, we aimed to provide a comprehensi

INTRODUCTION: In cases of space-occupying cerebellar ischemic strokes, guidelines recommend suboccipital decompressive surgery (SDC). While in supratentorial hemispheric stroke, the size of the bone flap has been the subject of many studies and ample debate, no studies have been conducted to determine the optimal size of the bone flap to be removed in SDC.
RESEARCH QUESTION: To determine the optimal size of SDC in ischemic cerebellar stroke.
METHODS: This is a multicentric retrospective study of patients undergoing SDC for ischemic cerebellar stroke. SDC size was determined in two perpendicu

Giant ruptured distal anterior cerebral artery aneurysms are rare, challenging pathologies that may require a combination of microsurgical and endovascular techniques for optimal treatment [1-9]. We describe the case of a female in her 40 s who presented with a Hunt-Hess 4, Fisher 4 subarachnoid hemorrhage from a multiply ruptured, giant distal anterior cerebral artery aneurysm. The patient underwent coil and n-BCA glue embolization of the aneurysm and its feeding A2 anterior cerebral artery. She subsequently underwent decompressive craniectomy, intracerebral hematoma evacuation, and microsurg
