Navigating Medication Risk in the ED: Communication Preferences of Older Adults Regarding Deprescribing.

OBJECTIVES: Patients and experts agree that potentially inappropriate medications should be reconsidered after adverse drug events (ADEs), yet emergency providers are often hesitant to discuss deprescribing in deference to outpatient prescribers. We sought to explore patient communication preferences for deprescribing in the emergency department (ED) after an ADE. METHODS: We conducted a cross-sectional survey study of older adults aged 65 years and older presenting to a southeastern, academic ED from June 2024 to October 2024. While awaiting results, eligible participants completed a best-wo

Development of a multidisciplinary medication management program in nursing homes: protocol for a randomized controlled trial : Multidisciplinary medication management in nursing homes.

BACKGROUND: Polypharmacy and the use of potentially inappropriate medications are common among nursing home residents and are associated with negative outcomes. Although deprescribing has been proposed as a way to curtail these problems, the best way to implement multidisciplinary comprehensive medication review and deprescribing and its real impact in specific high-risk populations, such as nursing home residents, is still unclear. This multicenter randomized controlled clinical trial aims to assess the effects of a multidisciplinary mediation management program on medication use and health p

Optimising the prescribing of drugs that may cause dependency: An evidence and gap map of systematic reviews.

OBJECTIVES: We set out to map the quantitative and qualitative systematic review evidence available to inform the optimal prescribing of drugs that can cause dependency (benzodiazepines, opioids, non-benzodiazepine hypnotics, gabapentinoids and antidepressants). We also consider how this evidence can be used to inform decision-making in the patient care pathway for each type of medication. METHODS: Eight bibliographic databases were searched for the period 2010 to 2020. All included reviews were initially appraised using four items from the Collaboration for Environmental Evidence Synthesis A

Inhaled corticosteroid overuse and deprescribing eligibility in COPD (iCODEC Study): a nationwide cross-sectional analysis in China.

Inhaled corticosteroid (ICS) overuse is common in chronic obstructive pulmonary disease (COPD) and associated with substantial adverse effects. Deprescribing has emerged as a strategy to optimize therapy, yet large-scale evidence on the prevalence and associated factors of eligibility for ICS deprescribing is limited. This nationwide cross-sectional study analyzed data from the Cough and Wheeze Pharmaceutical Care Clinics database between January 2021 and September 2024. Patients aged ≥40 years with physician-diagnosed COPD receiving long-acting inhaled therapies were included. We examined tre

Impact of an Anticoagulation Management Service-Led Annual Review on Antiplatelet Use and Gastrointestinal Bleeding Prophylaxis in Ambulatory Patients on Background Direct Oral Anticoagulants.

Patients on long-term anticoagulation face increased bleeding risk when antiplatelets are co-prescribed, especially upper gastrointestinal (GI) bleeding. Guidelines recommend limiting antiplatelet use to highly select patients on background anticoagulants and promoting GI prophylaxis with proton pump inhibitors (PPIs) for those requiring dual antithrombotic therapy. To assess the impact of an anticoagulation management service (AMS)-led intervention on reducing excess antiplatelet use and increasing PPI prescribing in patients who require continued antiplatelet therapy. Consumer and multidisciplinary clinician experiences after implementation of the Drug Burden Index intervention bundle to facilitate deprescribing in older inpatients: A mixed method study.

RATIONALE: The Drug Burden Index (DBI) measures exposure to anticholinergic and sedative drugs, which are associated with harm in older adults. To facilitate deprescribing in older Australian inpatients, we piloted an intervention bundle integrating the DBI in Electronic Medical Records, clinician deprescribing guides, consumer information leaflets and a stewardship pharmacist. OBJECTIVES: To understand (i) hospital clinician experiences of using the bundle and (ii) consumer (patient and carer) and General Practitioner (GP) experiences of in-hospital deprescribing, following bundle implementa

Design, implementation, and pilot testing of electronic health record deprescribing tools for persons living with dementia in U.S. primary care.

BACKGROUND AND OBJECTIVES: Electronic health record (EHR) tools are widely used to influence prescribing behaviors. However, the application of EHR tools to deprescribing in older adults, particularly persons living with dementia, is understudied, despite the potential to ameliorate cognitive decline by targeting medications like benzodiazepines and anticholinergics. We explored the design and implementation of EHR-based tools for deprescribing using a multi-phase mixed-methods approach. RESEARCH DESIGN AND METHODS: Within a large healthcare system, we first conducted semi-structured qualitat

Targets for deprescribing in patients with hypertension and reflex syncope.

BACKGROUND: We aimed to identify the target of deprescribing, i.e. the 24-hour SBP increase needed to achieve the greatest reduction of SBP drops. METHOD: Forty hypertensive patients (mean age 73.6 ± 9.3 years, 26 females) with reflex syncope and SBP drops on a screening ABPM were advised to withdraw or to reduce their therapy. The study objective was the reduction of SBP drops <90 mmHg and <100 mmHg on a second ABPM performed within 3 months. RESULTS: Out of a total of 98 drugs taken during ABPM 1, 44 were withdrawn, 16 had a dose reduction and 38 remained unchanged at the time of ABPM 2. 2

Minimising harms of tight glycaemic control in older patients with type 2 diabetes.

In older adults with type 2 diabetes (T2DM), tight glycaemic control (HbA1c 7%) can result in more harm than benefit, especially when using insulin or sulfonylureas. Older adults are at higher risk for adverse drug events, especially hypoglycaemia, which may cause falls, confusion and hospitalisations. This Therapeutic Letter evaluates the risks of tight glycaemic control in older adults with T2DM, focusing on deprescribing diabetes medications in those over 65, especially those with multimorbidity and polypharmacy. It assesses the evidence from clinical trials and guidelines, with a focus on

Reducing the Inappropriate Use of Antipsychotics in Long-Term Care: Strategies for Success.

The inappropriate use of antipsychotic medications in older adults can reduce the quality of life and increase the risk of adverse drug interactions. To address this concern, a quality improvement study was implemented across 34 long-term care (LTC) homes in Ontario, Canada. The study aimed to reduce inappropriate antipsychotic use by implementing evidence-based deprescribing practices. The study resulted in a 40.6% reduction in inappropriate antipsychotic use. The results of this study can inform quality improvement approaches for reducing the inappropriate use of antipsychotics in LTC and be

Potentially inappropriate medications with risk of cardiovascular adverse events in the elderly: A systematic review of tools addressing inappropriate prescribing.

WHAT IS KNOWN AND OBJECTIVE: In the last decades, many lists have been developed to screen for inappropriate prescribing. However, information on which potentially inappropriate medications (PIMs) could increase the cardiovascular risk in the elderly is not objectively presented. This review aimed to identify and quantify those PIMs by extracting information from published PIM-lists. METHODS: In accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses Statement (PRISMA), a systematic review of PIM-lists was conducted. The search strategy was run in PubMed, MEDLIN
