Percutaneous mitral valve therapy: when? Which patients?

PURPOSE OF REVIEW: Transcatheter management of valvular heart disease is an emerging area of intense interest. Patients with mitral regurgitation present a large clinical unmet need for therapeutic advances. RECENT FINDINGS: A large randomized, pivotal trial of one device, which performs an edge-to-edge repair, has recently completed enrollment and is awaiting all patients to reach the primary clinical endpoint. A host of other devices are still in preclinical or early clinical feasibility trials after having undergone device iteration. Clinical trials with two devices have been stopped becau

Expert consensus document on the treatment of descending thoracic aortic disease using endovascular stent-grafts.

Between 43,000 and 47,000 people die annually in the United States from diseases of the aorta and its branches and continues to increase. For the thoracic aorta, these diseases are increasingly treated by stent-grafting. No prospective randomized study exists comparing stent-grafting and open surgical treatment, including for disease subgroups. Currently, one stent-graft device is approved by the Food and Drug Administration for descending thoracic aortic aneurysms although two new devices are expected to obtain FDA approval in 2008. Stent-graft devices are used "off label" or under physician

Transcatheter closure of atrial septal defect by "buttoned" devices.

During a 21-month period ending in August 1991, 12 patients underwent transcatheter closure of atrial septal defect (ASD) with a "buttoned" device through an 8Fr sheath, using an institutional review board-approved, custom-made device protocol initially, and Food and Drug Administration-approved clinical trials subsequently. Ten children had left-to-right shunts across the ASD and 2 adults had their patent foramen ovale closed to prevent recurrent paradoxical embolism (including cerebrovascular accidents). The device dislodged in 1 of the 4 patients in whom a first-generation device was used,

Comparing treatment outcomes of various intracranial bifurcation aneurysms locations using the Woven EndoBridge (WEB) device.

BACKGROUND: The Woven EndoBridge (WEB) device has Food and Drug Administration approval for treatment of wide-necked intracranial bifurcation aneurysms. The WEB device has been shown to result in adequate occlusion in bifurcation aneurysms overall, but its usefulness in the individual bifurcation locations has been evaluated separately only in few case series, which were limited by small sample sizes. OBJECTIVE: To compare angiographic and clinical outcomes after treatment of bifurcation aneurysms at various locations, including anterior communicating artery (AComA), anterior cerebral artery

[Pediatric and congenital arrhythmia treatment : A complex interplay between expertise, re-imbursement, and industry].

Children, adolescents, and patients with congenital heart defects exhibit a unique and broad spectrum of arrhythmias. Diagnosis and treatment of these arrhythmias requires highly specialized medical care structures. In addition to highly qualified medical expertise, this includes appropriate antiarrhythmic drugs and medical devices specifically tailored to this patient group for catheter ablation and device therapy. The increasing economization of the healthcare system and legal requirements-particularly the EU Medical Device Regulation-are jeopardizing existing and urgently needed new care st

Hysterosalpingography for assessing efficacy of Essure microinsert permanent birth control device.

OBJECTIVE: The Essure microinsert is a new U.S. Food and Drug Administration-approved method of birth control. The objective of this study is to report our initial experience using hysterosalpingography (HSG) to assess its efficacy for permanent tubal occlusion. CONCLUSION: The Essure microinsert produced tubal blockage in all patients. As this device may become more widely used, radiologists should be aware of the device's appearance and be able to assess device position and presence of tubal occlusion on HSG.

A critical review of endoscopic therapy for gastroesophageal reflux disease.

The US Food and Drug Administration has approved 2 endoscopic devices for treating gastroesophageal reflux disease, and several thousand procedures have been performed to date. At least 6 other endoscopic devices designed to treat gastroesophageal reflux are in various stages of testing and may soon obtain approval for clinical use. Short-term follow-up studies uniformly report improvement in heartburn symptoms and quality-of-life scores, as well as decreases in use of antisecretory medications. However, esophageal acid reflux is not normalized after these treatments, nor is esophagitis improv

Advancing Health Equity in the Cardiovascular Device Life Cycle.

Despite advancements in diagnostics and therapeutics for cardiovascular disease, significant health disparities persist among patients from historically marginalized racial and ethnic groups, women, individuals who are socioeconomically under-resourced or underinsured, and those living in rural communities. While transcatheter interventions have revolutionized the treatment landscape in cardiology, populations bearing the greatest burden of disease continue to face inequitable access and poorer outcomes. A notable gap in the literature concerns the role of modern approaches to cardiovascular d

Combining sound with tongue stimulation for the treatment of tinnitus: a multi-site single-arm controlled pivotal trial.

Bimodal neuromodulation is emerging as a nonsurgical treatment for tinnitus. Bimodal treatment combining sound therapy with electrical tongue stimulation using the Lenire device is evaluated in a controlled pivotal trial (TENT-A3, NCT05227365) consisting of 6-weeks of sound-only stimulation (Stage 1) followed by 6-weeks of bimodal treatment (Stage 2) with 112 participants serving as their own control. The primary endpoint compares the responder rate observed in Stage 2 versus Stage 1, where a responder exceeds 7 points in the Tinnitus Handicap Inventory. In participants with moderate or more s

Electronically assisted nasolacrimal duct silicone intubation.

PURPOSE: Nasolacrimal duct intubation (NLDI) with silicone stents is a common procedure used by general ophthalmologists, oculoplastic surgeons, and otolaryngologists, among others. The procedure can be difficult to teach and to master. The authors investigated the usefulness and safety of an electronic device to aid endonasal retrieval of the lacrimal stent used in NLDI. METHODS: A prospective interventional surgical technique case series was designed and executed with institutional review board approval. Twelve adult patients who were scheduled to undergo NLDI were enrolled. The audible ala

Clinical Trial Perspective: Cost-effectiveness of Transcatheter Mitral Valve Repair Versus Medical Therapy in Patients with Heart Failure and Secondary Mitral Regurgitation. Results From the COAPT Trial.

Treatment of secondary (or functional) mitral regurgitation had traditionally been limited to optimal medical therapy because studies have failed to show a survival benefit with mitral valve surgery for this condition. However, recently the Cardiovascular Outcomes Assessment of the MitraClip Percutaneous Therapy for Heart Failure Patients with Functional Mitral Regurgitation (COAPT) trial demonstrated a significant decrease in heart failure hospitalizations and mortality in patients with severe secondary mitral regurgitation treated with percutaneous edge-to-edge mitral valve repair (TMVr) usi

First-in-human report of MitraClip G4 implantation for severe degenerative mitral regurgitation.

The new MitraClip G4 device (Abbott Vascular) has been recently approved by Food and Drug Administration and is currently in limited release. A patient with a large mitral regurgitation (MR) jet but a relatively small mitral valve area (MVA) was not a surgical repair candidate nor an optimal MitraClip third-generation device candidate. Therefore, we implanted the new G4 NTW device that resulted in significant MR reduction with a 57% reduction in MVA. To our knowledge, this is the first reported clinical use of the MitraClip G4 NTW device. We find that it may provide better results than a singl

CORE-MD clinical risk score for regulatory evaluation of artificial intelligence-based medical device software.

The European CORE-MD consortium (Coordinating Research and Evidence for Medical Devices) proposes a score for medical devices incorporating artificial intelligence or machine learning algorithms. Its domains are summarised as valid clinical association, technical performance, and clinical performance. High scores indicate that extensive clinical investigations should be undertaken before regulatory approval, whereas lower scores indicate devices for which less pre-market clinical evaluation may be balanced by more post-market evidence.

Analysis of reported adverse events of pipeline stents for intracranial aneurysms using the FDA MAUDE database.

OBJECTIVE: Flow diverting stents (FDS) are a validated device in the treatment of intracranial aneurysms, allowing for minimally invasive intervention. However, after its approval for use in the United States in 2011, post-market surveillance of adverse events is limited. This study aims to address this critical knowledge gap by analyzing the FDA Manufacturer and User Facility Device Experience (MAUDE) database for patient and device related (PR and DR) reports of adverse events and malfunctions. METHODS: Using post-market surveillance data from the MAUDE database, PR and DR reports from Janu

Open-Source Hardware May Address the Shortage in Medical Devices for Patients with Low-Income and Chronic Respiratory Diseases in Low-Resource Countries.

Respiratory diseases pose an increasing socio-economic burden worldwide given their high prevalence and their elevated morbidity and mortality. Medical devices play an important role in managing acute and chronic respiratory failure, including diagnosis, monitoring, and providing artificial ventilation. Current commercially available respiratory devices are very effective but, given their cost, are unaffordable for most patients in low- and middle-income countries (LMICs). Herein, we focus on a relatively new design option-the open-source hardware approach-that, if implemented, will contribute

From clinic to home: At-home neuromodulation for depression and the clinical significance of recent FDA approvals.

BACKGROUND: Electroconvulsive therapy achieves response rates of 70-80%, but it requires anesthesia, recovery time, and infrastructure that most patients cannot easily access. Transcranial magnetic stimulation avoids anesthesia yet still demands daily clinic visits, often for six weeks. The gap between what neuromodulation can accomplish and how few patients actually receive it has been a persistent frustration in clinical psychiatry. RECENT DEVELOPMENTS: The FDA granted premarket approval to two at-home devices for major depression within five weeks of each other: Flow FL-100 (a transcranial
