Protozoa of the genus Sarcocystis are obligatory heterogenous parasites with both definitive and intermediate hosts. BACKGROUND: Obstructed hemivagina and ipsilateral renal anomaly (OHVIRA), or Herlyn-Werner-Wunderlich syndrome, is a rare Mullerian duct anomaly with uterus didelphys, unilateral obstructed hemivagina, and ipsilateral renal agenesis. Patients with this anomaly usually present after menarche with pelvic pain and/or a mass and rarely, in later years, with primary infertility. Strong suspicion and knowledge of this anomaly are essential for a precise diagnosis.
CASE: A 14-year-old female presented with acute retention of urine and was diagnosed as a case of OHVIRA syndrome with uterus didelphys.

A 16-year-old adolescent girl presented with chronic pelvic pain. Pelvic ultrasound and MRI showing a uterus didelphys, normal left uterus and cervix, right pelvic fluid collection and right unilateral renal agenesis. After two unsuccessful vaginal surgeries for drainage of hematotrachelos and creation of an outflow tract, patient underwent unilateral total abdominal hysterectomy with final pathology confirming hematotrachelos and non-communicating cervical atresia on the right. BACKGROUND: Bladder exstrophy is an uncommon anomaly rarely associated with uterus didelphys. Fertility is limited by associated vaginal malformations, which have traditionally required surgical reconstruction.
CASE: A woman with a history of bladder exstrophy and hypoplastic vagina presented complaining of painful intercourse. The patient underwent vaginal dilator treatment after corrective surgery for the bladder defect. Without any further fertility therapy she subsequently conceived twins, with one implantation in each horn of a didelphic uterus.

Gynecologic and urinary congenital malformations are often associated and mostly diagnosed in teenagers. To update a 116-case international case registry, three cases of uterus didelphys with obstructed hemivagina and ipsilateral renal agenesis, detected in young girls, are reported. This retrospective study evaluated 29 free-living white-eared opossums (Didelphis albiventris) necropsied between 2022 and 2025. Among 19 adults, 13 (68.4%) presented intestinal remodeling associated with acanthocephalan (Oligacanthorhynchus microcephalus) attachment. Leptospirosis has been investigated in several species of wild animals.
