Use of long-acting cabotegravir and rilpivirine in people living with HIV: A prospective observational cohort study.

OBJECTIVES: Cabotegravir and rilpivirine long-acting (CAB+RPV LA) is a novel switch strategy for people living with HIV. Our aim was to identify factors associated with therapeutic failure (TF) and virological failure (VF). METHODS: We conducted a single-centre prospective cohort study. We reviewed all people living with HIV who initiated CAB+RPV LA between 14 February 2023 and 6 May 2025 in the hospital. Reasons for treatment discontinuation (VF, toxicity, poor adherence, death, pregnancy, transfer to another centre or oral bridging) were analysed, as well as virological effectiveness and th

Barriers and Facilitators Related to Providing Long-Acting Injectable HIV Preexposure Prophylaxis in Australian Settings: A Qualitative Study of Clinical and Policy Stakeholders.

Given injectable HIV preexposure prophylaxis (PrEP) has unique service-delivery requirements, there is a need to investigate the feasibility of its implementation and equitable access for people at risk of HIV. Data were collected from 45 informants (nurses, physicians, general practitioners, policy makers, and community representatives) to identify barriers to and facilitators of implementing long-acting injectable cabotegravir (CAB-LA) in Australian settings. Findings were mapped onto the domains of the updated Consolidated Framework for Implementation Research. Informants welcomed new PrEP

External Evaluation of Population Pharmacokinetic Models of Cabotegravir, During Its Oral and Intramuscular Administration in HIV-Infected Patients.

Cabotegravir (CAB), combined with rilpivirine, is the first long-acting injectable therapy approved for HIV-1 maintenance treatment. While adherence and patient satisfaction have been improved, pharmacokinetic (PK) variability remains a concern. Two population PK models have been developed: one based on data from phase I-III trials and the other on routine clinical data. However, neither model has undergone thorough external evaluation. The aim of this study was to evaluate the predictive performance of these models using an independent prospective dataset to evaluate their suitability to supp

Cabotegravir and rilpivirine for treatment of HIV infection in Africa: week 96 results from the phase 3b randomized, open-label, noninferiority CARES trial.

Evaluation of the durable efficacy and safety of long-acting injectable therapy for HIV is needed in African populations. In a multicenter, open-label phase 3b trial, 512 African adults with HIV-1, stable on first-line oral therapy, with screening plasma viral load (VL) <50 copies ml-1 and without past virologic failure were randomized (1:1) to continue oral therapy or switch to cabotegravir (600 mg) and rilpivirine (900 mg) intramuscular injections every 8 weeks (optional 4-week oral lead-in). VL was monitored every 24 weeks. Here the primary outcome for our analysis up to 96 weeks was VL <50

Hydrogel-forming microarray patches with cyclodextrin drug reservoirs for long-acting delivery of poorly soluble cabotegravir sodium for HIV Pre-Exposure Prophylaxis.

Hydrogel-forming microarray patches (HF-MAPs) offer minimally invasive, pain-free and prolonged drug delivery. These devices are designed to be self-administered and self-disabling, avoiding contaminated sharps waste generation. Cabotegravir sodium (CAB-Na) is a poorly soluble anti- human immunodeficiency virus (HIV) drug for the treatment and pre-exposure prophylaxis of HIV infection that lends itself to depot formation following intradermal delivery but presents significant challenges when delivered via HF-MAPs, whose nature is aqueous. Herein, we have investigated, for the first time, the u

Cost-effectiveness of the long-acting regimen cabotegravir plus rilpivirine for the treatment of HIV-1 and its potential impact on adherence and viral transmission: A modelling study.

INTRODUCTION: Combination antiretroviral therapy (cART) improves outcomes for people living with HIV (PLWH) but requires adherence to daily dosing. Suboptimal adherence results in reduced treatment effectiveness, increased costs, and greater risk of resistance and onwards transmission. Treatment with long-acting (LA), injection-based ART administered by healthcare professionals (directly observed therapy (DOT)) eliminates the need for adherence to daily dosing and may improve clinical outcomes. This study reports the cost-effectiveness of the cabotegravir plus rilpivirine LA regimen (CAB+RPV L

Long-acting drugs and formulations for the treatment and prevention of HIV infection.

Long-acting and extended-release formulations represent one of the most important approaches to improving the treatment and prevention of chronic HIV infection. Long-acting small molecules and monoclonal antibodies have demonstrated potent anti-HIV activity in early- and late-stage clinical trials. Strategies to manage toxicity and falling drug concentrations after missed doses, as well as primary and secondary resistance to current drugs and monoclonal antibodies are important considerations. Long-acting injectable nanoformulations of the integrase inhibitor cabotegravir and the non-nucleosid

Clinical Consequences of Delaying Implementation of Long-Acting Antiretroviral Therapy for People With HIV and Persistent Viremia in the United States.

BACKGROUND: People with HIV (PWH) with persistent viremia and adherence challenges to oral antiretroviral therapy (ART) can achieve viral suppression (VS) with long-acting cabotegravir/rilpivirine (LA-CAB/RPV). The US guidelines, however, recommend CAB/RPV only in limited situations. We projected the impact of delaying LA-CAB/RPV implementation while awaiting trial data. METHODS: Using a microsimulation model, we considered 2 approaches for PWH with persistent viremia and intermittent care engagement: daily first-line oral ART or LA-CAB/RPV, both with intensive-support-services (ISS) to maxim

Influence of patient characteristics and oral lead-in on long-acting cabotegravir and rilpivirine pharmacokinetics and outcomes in people with HIV: a real-world study.

Factors influencing suboptimal pharmacokinetics (PK) of long-acting (LA) cabotegravir (CAB) and rilpivirine (RPV) in people with HIV (PWH) remain poorly understood. Lower plasma concentrations in underrepresented populations in clinical trials and direct initiation without an oral lead-in (OLI) require further investigation. This study examined CAB and RPV plasma levels in a real-world cohort, focusing on OLI's role. This prospective cohort study followed PWH transitioning from oral antiretroviral therapy to CAB + RPV injections. Participants were sequentially assigned to start with injections
