182 patients assessed their condition after automated percutaneous lumbar discectomy (APLD). The analysis of descriptive data parameters++ provides new guidelines for the indication of this minimally invasive procedure: persistent leg pain despite conservative treatment, lumbar disc prolapse in one segment, minimal monological pathology, This ideal constellation implies a restriction of the common guidelines. Under these premises 50% of the patients will be satisfied. So the indication for APLD seems to be justified under more unfavorable patient profiles if the natural course of sciatica was

Percutaneous nucleotomy is designed to treat small and medium-sized contained disk hernias. It is offered to patients who have not responded to conservative treatment for three months or more. The nucleotomy procedure is well tolerated by a majority of patients and the rate of complications is low. The patients can be treated as out-patients. Among a total material of 130 patients, a primary good response to the treatment was observed in 81 (62%). However, owing to recurrencies the success rate had fallen to 52% at 12-39 months (mean 22 months) after nucleotomy. In selected patients with sympt

The results of various surgical treatments for lumbar disc herniation in laborers and athletes were compared on the basis of the relative success of return to work and athletic activity. The rate of successful return to manual labor was the highest at 89% after spinal fusion, but no difference was observed between simple disc excision and percutaneous discectomy. Return to competitive sports was achieved at rate of 81% after percutaneous discectomy, and the time until return to sports activity was significantly shorter than after other procedures. In individuals whose lumbar region is expected

BACKGROUND: For cases of multilevel lumbar disc herniation (LDH), selecting the surgical approach for Percutaneous Transforaminal Endoscopic Discectomy (PTED) presents significant challenges and heavily relies on the physician's judgment. This study aims to develop a deep learning (DL)-based multimodal model that provides objective and referenceable support by comprehensively analyzing imaging and clinical data to assist physicians.
METHODS: This retrospective study collected imaging and clinical data from patients with multilevel LDH. Each segmental MR scan was concurrently fed into a multi-

BACKGROUND: Calcified lumbar disc herniation (CLDH) is one specific type of lumbar disc herniation (LDH). Currently, there are no uniform classification standards for different calcification degrees of herniated lumbar disc tissue. Percutaneous endoscopic interlaminar discectomy (PEID) has been proved to exhibit great efficacy in treating LDH, but whether it can achieve satisfactory results in treating CLDH with different calcification remains unclear.
METHODS: A total of 271 patients who suffered from single CLDH (97 patients) or NCLDH (non-calcified lumbar disc herniation) who received PEID

OBJECTIVE: Percutaneous full-endoscopic anterior cervical discectomy (PEACD) and posterior cervical foraminotomy (PCF) have been reported as effective treatments for the cervical spondylosis radiculopathy (CSR), but the biomechanical effects on the discs and facet joints of PEACD and PCF remain largely unclear. The purpose of this paper is to investigate and compare the biomechanical changes on cervical spine after PECAD and PCF procedures, thus providing evidences for surgeons to select a more appropriate approach.
METHODS: An intact cervical C5-C6 digital model was constructed and then modi

BACKGROUND: Discogenic pain or herniation causing neural impingement of the thoracic vertebrae is less common than that in the cervical or lumbar regions. Treatment of thoracic discogenic pain usually involves conservative measures. If this fails, conventional fusion or discectomy can be considered, but these procedures carry significant risk.
OBJECTIVES: To assess the efficacy and safety of percutaneous laser disc decompression (PLDD) for the treatment of thoracic disc disease.
METHODS: Ten patients with thoracic discogenic pain who were unresponsive to conservative intervention underwent t

Minimally invasive techniques for the treatment of degenerative pathology of the spine have come to be preferred by surgeons since the destructive effect on bony structures is eliminated and scar formation is dramatically reduced. A critical review of the pathogenetic mechanisms for low back pain and sciatalgia has recently yielded that mechanical compression is one but non essential component of the matter. The importance of chemical irritative processes is stressed. Coblation nucleoplasty is one of these minimally invasive techniques. It provokes ablation of the nucleus of the disk by a cont

BACKGROUND: Minimally invasive spine surgery is gaining popularity. Results of currently used percutaneous posterior techniques fall short of standard open microdiscectomy. Using a posterior percutaneous technology with an anterior laparoscopic approach may improve results and still maintain the advantages of a minimally invasive procedure.
METHODS: Patients with symptomatic lumbar protruded discs confirmed by computed tomography or magnetic resonance imaging were offered the procedure. Transperitoneal visualization of the retroperitoneum was supplemented with fluoroscopic guidance. A small w

BACKGROUND: Percutaneous transforaminal endoscopic discectomy (PTED) is efficacious for lumbar disc herniation but harbours a steep learning curve and considerable reliance on intraoperative fluoroscopy. This often results in prolonged procedural times and elevated radiation exposure, particularly for novice surgeons.
OBJECTIVES: This study evaluates whether preoperative three-dimensional virtual planning using Mimics software improves procedural accuracy and reduces radiation exposure.
METHODS: Sixty-five patients with single-level lumbar disc herniation (L1/2 to L5/S1) were allocated to tw

RATIONALE: Minimally invasive surgeries for thoracic disc herniation (TDH) evolved rapidly in recent years, and multiple approaches have been put forward. Thoracic discectomy via percutaneous spine endoscopy (PSE) is inadequately documented because of the low prevalence of TDH and the high difficulty of thoracic spine endoscopy techniques. Herein, we present a TDH case who underwent percutaneous endoscopic thoracic discectomy.
PATIENT CONCERNS: A 28-year-old male suffered backpain and partial paralysis in lower extremities.
DIAGNOSES: Magnet resonance imaging demonstrated T11-12 TDH, with cr

BACKGROUND: Percutaneous endoscopic lumbar discectomy (PELD) is regarded as an alternative treatment for lumbar disc herniation. Although the indication for PELD has expanded with remarkable evolution of the technique, sometimes unexpected complications have occurred during PELD. We report 3 cases of de novo disc prolapse during PELD.
CASE DESCRIPTION: In 3 patients who underwent PELD for lumbar disc herniation with local anesthesia, postoperative magnetic resonance imaging demonstrated newly developed up-migrated disc herniation. Compared with their preoperative states, these patients experi

BACKGROUND: Percutaneous transforaminal endoscopic discectomy (PTED) under local anesthesia (LA) is growing popular in recent years because of its safety, effectiveness and increased patient demands for minimally invasive procedures. To avoid neural injuries, local anesthesia that can keep the patient conscious is recommended. However, many patients complain about the severe pain during surgery. Epidural anesthesia (EA) is an alternative choice. We put forward an anesthetic method that combined preemptive analgesia (PA) and local anesthesia.
OBJECTIVES: The study aimed to assess the effective

BACKGROUND CONTEXT: Percutaneous intradiscal therapies are gaining popularity as a regenerative treatment option for spinal disc degeneration. The risks, benefits, and possible complications associated with such procedures have been poorly defined. As these procedures are performed with increasing frequency, the likelihood that clinicians will be faced with significant complications also increases.
PURPOSE: The purpose of this study is to describe a significant complication of a percutaneous intradiscal bone marrow and adipose tissue transplantation for symptomatic lumbar disc degeneration.
