Focused cardiac ultrasound diagnosis of thoracic aortic aneurysm: two cases.

BACKGROUND: Thoracic aortic aneurysms (TAAs) are less prevalent than abdominal aortic aneurysms. Symptomatic TAAs need to be identified quickly by the emergency physician (EP) since mortality rates increase dramatically once complications such as rupture or dissection occur. Compared with validated EP-performed ultrasound of the abdominal aorta, EP-performed focused cardiac ultrasound that includes evaluation of the thoracic aorta is relatively unreported. OBJECTIVE: Two cases illustrate EP-performed focused cardiac ultrasound and evaluation of the thoracic aorta for aneurysmal dilation. CASE

A Case Report of an Unusual Acute Intramural Hematoma Disguise as Pulmonary Embolism.

Aortic intramural hematoma (IMH) is characterized by an aortic wall hematoma without intimal flap and it is a variant of acute aortic syndromes (AAS). This entity may represent 10%-25% of the AAS involving the ascending aorta and aortic arch (Stanford Type A) in 10%-30% of cases and the descending thoracic aorta (Stanford Type B) in 60%-70% of cases. IMH impairs the aortic wall and may progress to either inward disruption of the intima, which finally induces typical dissection or outward rupture of the aorta. The literature describes some clinical reports where Type A aortic dissection mimics

Growth rate of aortic diameter in patients with type B aortic dissection during the chronic phase.

BACKGROUND: The purpose of this study was to evaluate the growth rate of type B double-barrel aortic dissection with computed tomography (CT) and the factors influencing its enlargement. METHODS AND RESULTS: Sixty-two patients were entered into this study, and regular follow-up CT studies (mean; 49.1 months) were performed. The affected aortas and iliac arteries were divided into 5 segments (aortic arch, descending thoracic, suprarenal abdominal, infrarenal abdominal aorta, and iliac artery). Fifty-two of 62 patients (83.9%) had 1 or more segments increased in size during follow-up period. In

Endovascular treatment of descending thoracic aortic aneurysms and dissections.

Various endovascular techniques have become viable therapeutic alternatives in the treatment of patients with many types of descending thoracic aortic pathology and aortic dissections. Descending thoracic aortic aneurysms can be successfully treated using stent grafts. This technique is less invasive and is associated with acceptable morbidity and mortality rates. Patients who are particularly likely to benefit include the very elderly population; those with markedly compromised cardiac, pulmonary, or renal status; and individuals who have previously undergone complex operations on the thoraci

Computational fluid dynamics analysis for enlargement of the descending aorta after frozen elephant trunk implantation.

BACKGROUND: Acute type A aortic dissection (ATAAD) treated with total arch replacement (TAR) plus frozen elephant trunk (FET) can achieve favorable early remodeling; however, late enlargement of the distal descending aorta still occurs in some patients. METHODS: Study 1 included ATAAD patients who underwent TAR with FET between January 2015 and December 2023 and achieved complete false-lumen obliteration at the distal FET level. We identified cases with more than 5 mm enlargement at the distal FET level during follow-up and performed statistical analyses. Among these, patients with contrast-en

Novel technique to fenestrate an aortic dissection flap using electrocautery.

Chronic distal thoracic dissections treated with thoracic endovascular repair are prone to type Ib false lumen perfusion. When the supraceliac aorta is of normal caliber, fenestration of the dissection flap proximal to the visceral vessels creates a seal zone for the thoracic stent graft and eliminates the type Ib false lumen perfusion. We describe a novel way of crossing the septum using electrocautery delivered through a wire tip then fenestrating the septum using electrocautery delivered over a 1-mm area of uninsulated wire to cut the septum. We believe the use of electrocautery creates a c

How to manage the left subclavian artery during endovascular stenting for thoracic aortic dissection? An assessment of the evidence.

BACKGROUND: Despite the publication of recent guidelines for management of the left subclavian artery (LSA) during endovascular stenting procedures of the thoracic aorta, specific management for those presenting with dissection remains unclear. This systematic review attempts to address this issue. METHODS: Systematic assessment of the published data on thoracic aorta dissection was performed identifying 46 studies, which incorporated 1,275 patients. Primary outcomes included the prevalence of left arm ischemia, stroke, spinal cord ischemia, endoleak, stent migration, and mortality. Outcomes

Initial Single-Center Experience With the Knickerbocker Technique During Thoracic Endovascular Aortic Repair to Block Retrograde False Lumen Flow in Patients With Type B Aortic Dissection.

OBJECTIVE: Effectiveness of thoracic endovascular aortic repair in type B aortic dissection is impaired by persistent retrograde false lumen flow via distal re-entry tears. Controlled, stentgraft-assisted balloon dilatation of the true lumen at its lower end, or Knickerbocker technique, may block retrograde false lumen flow and consequently improve effectiveness by inducing immediate thrombosis along the entire descending thoracic aorta. MATERIALS AND METHODS: A single-center retrospective analysis was performed for all consecutive patients with aortic dissection treated with the Knickerbocke

Early and mid-term results of endovascular treatment of thoracic aorta diseases: a single-center experience.

We conducted an analysis to assess early and mid-term outcomes of patients after thoracic endovascular aortic repair (TEVAR) for type B thoracic aorta dissection, descending thoracic aneurysm, or traumatic aortic transection. From January 2016 through December 2018, twenty-seven patients (23 male, 4 female, mean age of 57 years) affected by type B dissection (n = 13 [48.2%]), thoracic aneurysm (n = 9 [33.3%]), and post-traumatic aortic isthmus rupture (n = 5 [18.5%]) were treated using TEVAR with and without left subclavian artery revascularization. All procedures were performed in a hybrid op

[Long-term results of patients undergone surgical repair for acute type A dissection].

We evaluated the long-term results of 28 operative survivors who underwent surgical repair for acute type A dissection. Residual dissection of the thoracic aorta was found in 21 patients (75.0%) postoperatively, and 6 of them underwent reoperation with no operative death. There were 2 late death; one died in the third operation for thoracoabdominal aorta and the other died of cancer. Two of 4 patients with Marfan's syndrome required total aortic replacement ultimately. Since the elective reoperation can be performed safely, careful follow-up and management is essential for the improved surviva

Treatment of delayed retrograde dissection after endovascular stenting of thoracoabdominal aortic aneurysm: graft-to-endograft anastomosis.

INTRODUCTION: Endovascular treatment of thoracic aortic aneurysms may be associated with procedure-related early and late complications. REPORT: We describe the case of a 70-year-old man who developed acute retrograde dissection of the ascending aorta 30 days after endovascular stent grafting of a thoracoabdominal aortic aneurysm and prior revascularization of the left subclavian artery. The ascending aorta and aortic arch were successfully replaced with a Dacron graft; the distal part of the graft was partially anastomosed to the previously placed endograft. DISCUSSION: Surgical repair of d

Dissection of the abdominal aorta associated with traumatic aneurysms of the iliac vessels. Case report.

Dissection of the abdominal aorta without an associated thoracic dissection is uncommon. It is usually associated with blunt abdominal trauma and commonly presents early with aortic occlusion. A case is presented where abdominal aortic dissection presented two months after lower abdominal trauma. It is suggested that the initiating event in this case was injury to the iliac vessels.

Severe stenosis of the true lumen in the abdominal aorta after an ascending aorta reconstruction for an acute type a dissection: report of a case.

We encountered a case of severe symptomatic stenosis of the abdominal aorta after a surgical repair of an ascending aortic dissection. A 75-year-old woman underwent a reconstruction of the ascending aorta to treat a Stanford type A acute aortic dissection and cardiac tamponade. Eight hours postoperatively, the patient was anuric. An abdominal computed tomography scan revealed severe stenosis of the true lumen of the suprarenal abdominal aorta due to a dilatation of the false lumen. The patient's ischemic symptoms progressed to include a weakening femoral pulse and, as a result, an emergency ri

Thoracic endovascular repair for retrograde acute type A aortic dissection as an alternative choice.

OBJECTIVE: The purpose of this study was to determine the clinical outcomes of thoracic endovascular aortic repair in 8 patients with acute type A aortic dissection with an entry tear in the descending aorta. SUBJECTS AND METHODS: From January 2016 to December 2018, eight patients (mean age 76 years; range 54-92 years) were treated by thoracic endovascular aortic repair due to high operative risk for conventional open repair. All patients had significant comorbidities, and two had critical organ malperfusion due to aortic dissection. Surgical outcomes were retrospectively reviewed. RESULTS:

Repair of lesions of the descending thoracic aorta with the TDMAC-heparin shunt.

Utilizing a heparinized tridodecylmethylammonium chloride (TDMAC) shunt makes it possible to treat various surgical diseases of the descending thoracic aorta without cardiopulmonary bypass. Since the initial report by Gott and associates on the use of the heparinized shunt, few subsequent clinical trials have appeared in the literature. Six patients with Type III dissecting thoracic aneurysm, acquired and congenital coarctation of the aorta, saccular arteriosclerotic aneurysm, and transection of the descending thoracic aorta were operated upon by means of this technique. Only one patient had m

Inner Branched Endovascular Repair of a Post-Type B Dissection Aneurysm Previously Treated With a Nellix in Both Lumens: Making the Most of a Complex Situation.

INTRODUCTION: Treatment of chronic type B aortic dissection associated aneurysms where key vessels originate from both the true and false lumens is technically challenging. Further aneurysm formation may require new interventions after initial treatment. Innovative endovascular techniques can lead to successful treatment. REPORT: A 74 year old man with a type B dissection was treated with an urgent femorofemoral crossover bypass (in 2001) and surgical graft in the thoracic aorta (in 2003). Due to the combination of a chronic type B dissection and a right sided common iliac artery aneurysm, he

A Novel Reverse Branch Technique for Reconstruction of a Renal Artery Perfused by the False Lumen After Thoracic Endovascular Aortic Repair.

PURPOSE: To describe an innovative endovascular technique that successfully reconstructs a renal artery completely perfused by the false lumen after thoracic endovascular aortic repair (TEVAR). CASE REPORT: A 65-year-old patient diagnosed with acute Stanford type B aortic dissection underwent successful TEVAR 4 years ago. Regular follow-up found that the thoracic aorta was well repaired, but the false lumen in the abdominal aorta had enlarged year by year. The left renal artery was supplied entirely by the false lumen, which caused kidney hypoperfusion. The abdominal aorta was successfully re

Treatment of a persistent false lumen with aneurysm formation following surgical repair of type a dissection.

We describe the case of a 68-year-old man who developed aneurysmal dilatation of the proximal descending thoracic aorta 8 years after repair of a type A dissection. The aneurysm was due to an anastomotic leak at the distal end of the previous repair in the ascending aorta with antegrade perfusion of the false lumen. Surgical repair of the anastomotic leak partially obliterated the false lumen and computed tomography scan demonstrated thrombosis in a large proportion of the false lumen aneurysm. Follow-up with surveillance scans showed persistent filling of this aneurysm due to retrograde flow

Multidetector CT of Aortic Dissection: A Pictorial Review.

Aortic dissection is the most common acute emergency condition of the aorta and often has a fatal outcome. Outcome is determined by the type and extent of dissection and the presence of associated complications (eg, cerebral sequelae, aortic branch involvement, pericardial involvement, and visceral involvement), with early diagnosis and treatment being essential for improved prognosis. Aortic dissections are classified on the basis of the site of the intimal tear according to the Stanford classification system. Type A aortic dissection involves the ascending thoracic aorta and may extend into
