Colonic bleeding in the elderly.

In the elderly population bleeding from the colon is not unusual. In evaluating a patient with colonic bleeding, the rate of bleeding should be assessed, and the possibility of upper gastrointestinal and anorectal bleeding should be excluded. The most common cause of massive bleeding from the colon in the elderly patient is diverticulosis, and the next most common cause is angiodysplasia. A number of other less common causes of bleeding should be excluded when making the diagnosis. If angiography is employed and the bleeding site is identified, initial therapy can be started concurrently by ei

[Benign solitary ulcer and acute diverticulitis of the cecum].

The report of diverticulosis of right colon in a patient formerly operated on for perforation of caecum single ulcer appeared to the authors as a confirmation of a common etiology between both lesions. The single ulcer would probably represent a form of manifestation for the acute phlogosis of a diverticulum.

[Perforated cecum diverticulum as cause for acute abdomen].

Perforation of a solitary cecal diverticulum is a rare cause of acute abdomen and an uncommon differential diagnosis for acute appendicitis. Nine hundred cases have been described since Potiers first description in 1912, either localized in the cecum or in the ascending colon. A further case and its course are described.

Colonic Diverticulitis After Endoscopic Band Ligation Performed for Colonic Diverticular Hemorrhage.

A 72-year-old woman presented with massive hematochezia. Colonoscopy revealed active bleeding from a diverticulum in the ascending colon, diagnosed as colonic diverticular hemorrhage. Endoscopic band ligation (EBL) was performed for the hemostasis. The patient developed abdominal pain in the right lower quadrant 1 day after EBL. Computed tomography (CT) demonstrated dirty fat around the ascending colon and thickened peritoneum without perforation or abscess formation, consistent with uncomplicated diverticulitis. The patient was discharged with intravenous antibiotic therapy 5 days after EBL.

Colovesical fistula caused by an ingested chicken bone.

Colovesical fistula involving the sigmoid colon is the most common fistulous communication between the gastrointestinal tract and the urinary bladder. These tracts are most commonly associated with diverticulitis but might arise secondary to a neoplasm, inflammatory bowel disease, iatrogenic injuries, radiation therapy, trauma, or foreign bodies. We describe a patient who developed a fistula between the sigmoid colon and bladder secondary to an ingested chicken bone that lodged in a colonic diverticulum. A portion of the foreign body was removed by direct visualization on colonoscopy; the rema

[Colonic diverticulitis: diverticulitis with complications and its treatment (author's transl)].

Complications (stenosis, intestinal obstruction, abscess or fistula formation, inflammation, etc.) of colonic diverticulitis are seen in 35% of patients affected. Multiple operative procedures involve less risk than surgery performed in a single session. The mortality in emergency surgery is reported to be up to 40%, while subsequent resection of the diseased colon involves the usual mortality of surgery on the colon, i.e. 1.5-9%. The vital factor in this illness is the earliest possible surgical treatment of the diseased diverticulum, so that the extremely problematical complications can be a

Delayed Perforation after Endoscopic Detachable Snare Ligation for Colonic Diverticular Hemorrhage.

A 74-year-old man was admitted to our hospital with severe hematochezia. Abdominal enhanced computed tomography (CT) demonstrated extravasation of contrast material from the descending colon. Colonoscopy revealed recent bleeding in the descending colon diverticulum. Bleeding was stopped using detachable snare ligation. Eight days later, the patient developed abdominalgia, and CT revealed free air caused by delayed perforation. The patient underwent emergency surgery. Perforation at the ligation site was detected using intraoperative colonoscopy. This report is the first to describe a case of d

[A case of vesicosigmoidal fistula secondary to diverticulitis of the colon].

A case of vesicosigmoidal fistula secondary to the diverticulitis of colon is reported. A 63-year-old man was admitted to our clinic with the chief complaint of pneumaturia. Cystoscopy revealed an edematous, papillomatous lesion at the left posterior bladder wall. Although multiple diverticulosis of descending and sigmoid colon were demonstrated by barium enema, the presence of a fistula from the intestine to bladder was not confirmed. The presence of fistula was confirmed by detecting the orally administered charcoal in the urine. Resection of sigmoid colon with partial cystectomy was perform

Radiographic evaluation of spastic colon disease, diverticulosis, and diverticulitis.

Spastic colon disease is a syndrome consisting of abdominal pain and abnormal bowel function associated with thickening of the circular muscle layer of the colon. The condition may be related to changes in intraluminal pressure associated with a lifelong low residue diet. Radiologic features include thickening, crowding, irregularity, and distortion of the interhaustral folds of the sigmoid colon, which in the past have been incorrectly considered to be manifestations of acute diverticulitis. Spastic colon disease leads to diverticulosis which develops because of mucosal herniations through th

Coil Embolization of Diverticulum of Kommerell: A Targeted Hybrid Endovascular Technique.

A 57-year-old man was incidentally found to have a dissected diverticulum of Kommerell originating from aberrant origin of right subclavian artery during follow-up for treated colon cancer. A right carotid-axillary bypass was followed by embolization of aberrant artery and the diverticulum by deployment of multiple detachable coils using the Penumbra Ruby System (Penumbra, Alameda, CA). Angiography after embolization showed exclusion of flow to the aberrant artery and a patent right carotid-axillary bypass. The patient recovered uneventfully and remained well for 12 weeks after the operation,

Contrast Enhanced Computed Tomographic Study on the Prevalence of Duodenal Diverticulum in Indian Population.

INTRODUCTION: Duodenal diverticulum (DD) is the second most common diverticulum, yet its incidence varies widely from 1-22% based on the mode of investigation. Computed Tomography (CT) of abdomen is the preferred modality to diagnose acute abdomen including those of complications of DD. Moreover, the prevalence of DD in Indian population is not yet been studied using CT. AIM: The current study aim to look for the prevalence of DD in Indian population using Contrast Enhanced Computed Tomography (CECT) abdomen. MATERIALS AND METHODS: A retrospective study was done to assess the presence of DD

Sepsis caused by endoscopic clipping for colonic diverticular bleeding: a rare complication.

We herein report the rare complication of sepsis caused by endoscopic clipping for colonic diverticular bleeding. A 78-year-old man with a 12-h history of near syncope and painless hematochezia was admitted to our hospital. Following the transfusion of 4 U of blood and continued hematochezia, a colonoscopy was performed. Active bleeding was seen as continuous arterial spurting from a single diverticulum located in the middle ascending colon. This diverticulum was seamed by four endoclips. The next day, the patient became febrile with a temperature of 39.2 degrees C. Laboratory data included a

[Diverticular disease of the colon. Physiopathology and medical treatment].

Almost unknown in 1900, diverticular disease has become the commonest disease of the colon and appears to be a deficiency disease caused by the removal of vegetable fibre from the diet. It occurs most commonly in the sigmoid colon and is frequently asymptomatic, but it may be symptomatic without evidence of inflammation. It may be complicated by inflammation or bleeding. A high residue diet forms the rational basis for successful therapy.

Giant sigmoid diverticula: a review.

Two patients with giant sigmoid diverticula are added to 13 cases reported in the literature and the clinical features of this rare complication of diverticulosis are reviewed. These lesions probably arise as pseudodiverticula of the sigmoid colon with herniation of the mucosa through the muscle wall. They become progressively inflated by colonic gas via a ball-valve type mechanism. They are best treated by resection of the diverticulum, in continuity with the involved sigmoid, and primary anastomosis.

[Clinikomorphological features of complicated colon diverticulosis].

79 surgical and postmortem cases of colon diverticulosis with complications were studied clinicomorphologically. Sequence in the development of inflammatory complications was revealed: diverticulum perforation-peridiverticulitis-pericolitic infiltrate, sometimes with the abscess-fistula. Segmental colitis may join the process. The complications onset depends mainly on discoordination of the intestinal peristalsis and intramural vascular deficiency.

Pediatric colonic diverticulitis: clinical presentation, management, and review of the literature.

Colonic diverticulitis is rare in the pediatric population and often presents with nonspecific symptoms that may be misdiagnosed as other causes of acute abdominal pain, such as appendicitis, leading to diagnostic uncertainty and variability in management. We report a case of pediatric colonic diverticulitis and present a systematic review to characterize its clinical presentation, diagnostic challenges and management. Clinical data for the index patient were retrospectively reviewed, and parental consent was obtained. A structured literature search was performed using PubMed, Scopus, and Emba

Significant bleeding from Meckel's diverticulum after blunt abdominal trauma: a case report.

BACKGROUND: Meckel's diverticulum, with an incidence of 2%, is the most common congenital anomaly in the gastrointestinal tract. Its main complications are perforation, obstruction, and bleeding. A few studies have reported that blunt abdominal trauma may result in perforation or obstruction to Meckel's diverticulum. However, reports of significant major intestinal bleeding from Meckel's diverticulum as a complication of blunt abdominal trauma is rare. This paper present what we believe to be the first reported case of significant intestinal bleeding from a Meckel's diverticulum following blun
