Between January 1988 and December 1992, six patients presenting with chronic duodenal ulcer disease, long segment duodenal stricture, gross gastric dilatation and Forrest Ia hemorrhage from penetrating posterior duodenal wall ulcer were managed by oblique duodenal transsection, suture ligation of arroded pancreaticoduodenal vessel, classic Billroth II resection, Nissen duodenojejunostomy and a Tomoda valve disconnecting the Nissen anastomosis from the gastroenterostomy, -resulting in a functional "uncut" Roux anastomosis. The procedure was expeditious and safe in this patient population, with

BACKGROUND: Pyloric atresia (PA) is a rare pathology. Calder presented the first pyloric atresia case in 1749 and Touroff, Sussman, Meltz, and their colleagues presented the first successful operation in 1940. PA has 3 types of anatomic variations: (1) type A, pyloric membrane or web; (2) type B, the pyloric channel is a solid cord; and (3) type C, in which there is a gap between the stomach and duodenum. Associated anomalies also have been described. Epidermolysis bullosa (EB) and intestinal anomalies occur most often with this condition.
METHODS: Charts of 16 cases of congenital PA, aged 1

Since the early 50s, the routine use of operative cholangiography in biliary surgery has decreased the rate of negative choledochotomy and retained stones. In the last twenty years no further progress has occurred and negative choledochotomy rate remains at 30% and for retained stones at 1.3%. Modern endoscopic technology has improved the diagnosis and treatment of biliary diseases so we think that the routine use of endoscopic retrograde cholangiopancreatography (ERCP) as a preoperative study for patients to be surgically treated of biliary disease can reduce the incidence of negative choledo

OBJECTIVES: It is unclear which is the best approach for the drainage of malignant distal biliary obstruction (MDBO) after failed endoscopic retrograde cholangiopancreatography (ERCP). We compared endoscopic ultrasound (EUS)-guided gallbladder drainage (GBD) and EUS-guided choledocoduodenostomy (CDS) with lumen-apposing metal stents (LAMS) as rescue treatment in the case of ERCP failure.
METHODS: This was an international multicenter retrospective observational study at 28 tertiary-care centers. Outcomes were compared using propensity score matching (PSM). Clinical success was the primary out

In the present study, we have described two possible approaches in the management of caustic injuries. Diagnostic emergency laparoscopy can be used for exploration in case of stable patients with Zargar's 3a gastric lesions and equivocal peritoneal signs. On the other hand, in case of patients with Zargar's 3b gastric lesions with perforation, diffuse peritonitis and hemodynamic instability, a new possible technique is described as an option to be used in such extensive caustic injuries: duodenal damage control with "4-tubes ostomy" for duodenal and jejunal wash-out of the caustic agent. The a

Pancreatitis, a late complication of an annular pancreas (AP), results from coexisting pancreaticobiliary malformations including pancreas divisum (PD), and pancreaticobiliary maljunction (PBM). The authors report the case of a 3-year-old boy with an unusual type of AP in which the dorsal anlage encircled the duodenum. The patient developed duodenal obstruction as well as duodenopancreatic reflux with resulting hyperamylasemia and hyperlipasemia. This type of AP associated with duodenopancreatic reflux in AP has not been reported previously. The patient was successfully treated by duodenoduode

Multiple intestinal atresia presents a difficult technical problem because of extreme loss of intestinal length, disparity of residual bowel wall size, and discontinuity of multiple short intestinal segments. The authors report on a 3,000-g infant with gastroschisis complicated by intrauterine volvulus and multiple intestinal atresias who was treated successfully with intraluminal stenting and sutureless anastomoses. A total of 25 cm of small bowel was salvaged including 13 segments each measuring 1 to 8 cm in length. Subsequent radiographic studies showed spontaneous anastomosis with a compar

Management of esophageal atresia with tracheoesophageal fistula (TEF) has evolved over the past 5 years at Children's Hospital and Medical Center, Seattle, in the premature patient weighing less than 2.5 kg. Traditionally, these low birth weight infants underwent gastrostomy and fistula ligation followed by repair after a period of weight gain. However, since 1980, of the 31 neonates weighing 2.5 kg or less with type-C TEF, a primary repair was performed in 22 within the first five days of life. There were no mortalities and only one subclinical anastomotic leak. All anastomoses were done extr

A newborn delivered by emergency lower segment caesarean section at 35 weeks of gestation suspected duodenal atresia antenatally with maternal ultrasonogram showing double bubble sign. After birth, the baby was stabilised and taken up for laparotomy and was found to have duodenal atresia along with intestinal malrotation, annular pancreas and preduodenal portal vein (PDPV). Ladd's procedure with Kimura's duodenoduodenostomy was performed. In a similar previously reported case study, a gastrojejunostomy was conducted;5 here, we were able to do a duodenoduodenostomy which is a better physiologic

Single Anastomosis Duodeno-ileostomy with Sleeve Gastrectomy (SADI-S) is an emerging bariatric procedure that can sometimes lead to severe malnutrition. This report describes two cases of conversion surgery for post-SADI-S malnutrition and reviews management strategies. Both patients, who had undergone sleeve gastrectomy followed by SADI-S for weight recurrence, developed severe protein-energy malnutrition with hypoalbuminemia (1.6-1.8 g/dL) requiring parenteral nutrition, which normalized following conversion surgery. They underwent laparoscopic conversion to Roux-en-Y duodenojejunal bypass w

BackgroundSuperior Mesenteric Artery Syndrome (SMAS) is a very rare disease that causes starvation and malnutrition secondary to a mechanical obstruction of the 3rd portion of the duodenum between the superior mesenteric artery and aorta. Long-term outcomes following surgery for SMAS by current methods have a high failure rate of 21%-67%. We report the 3-year outcomes of a novel operation for SMAS described as the duodenoduodenostomy (DD).MethodsThis is a longitudinal case series of 12 patients who underwent the DD operation for SMAS, comparing pre-surgery to post-surgery 3-year outcomes. BACKGROUND: Superior mesentery artery syndrome (SMAS) is a rare vasculo-anatomic occlusive pathologic entity for which a period of conservative medical management is advocated with surgery reserved for nonresponsive cases. We present our management plan that entails a single admission approach and complete rendering of medical and surgical treatment to the patient on a background of the socioeconomic and cultural trends prevalent in this geographic region.
METHODS: A retrospective analysis of 22 cases of SMAS admitted in our health care system who underwent a period of preoperative conditioni

We report the case of a child who had a duodenal atresia and also a gastric-antral web. The two conditions are not known to be associated. The child had a duodenoduodenostomy for the duodenal atresia that had been diagnosed on a "double bubble" appearance at antenatal ultrasound scan. She eventually had a second laparotomy due to persistent gastric outlet obstruction, at which a pyloroplasty was performed and an antral web excised. The child remains well four months after discharge. This case has taught us several salutary lessons, which we consider relevant to share with colleagues.

We have performed 9 pylorus preserving pancreaticoduodenectomies (PPPD) for periampullary malignancies. In these 9 patients, six had been diagnosed as having carcinoma of the ampulla of Vater, two pancreatic carcinoma and one duodenal carcinoma. We have no operative mortality and a 44 per cent rate of morbidity. Complications include wound infections (3), pleural effusion (1), intraabdominal abscess (1), delayed gastric emptying (1). There was neither biliary nor pancreatic leakage in these patients. None of the patients had gastrointestinal haemorrhage and marginal ulceration. We have complet

Congenital cystic dilatation of the common bile duct, also known as choledochal cyst, is an uncommon malformation, observed mainly in asiatic children, with a 3:1 female--male ratio. The entity is classified into 4 different types according to the anatomy of the malformation, which effects or not the intrahepatic biliary tree. The authors review theirs experience with 16 consecutive patients with choledochal cyst operated between 1971 and 1985. In 12 children (mean age: 5,8 years, 11 girls) without previous surgery, preoperative diagnosis was made with abdominal ultrasonography and, in some ca

INTRODUCTION: Endoscopic ultrasound (EUS)-guided pancreatic cysto-gastrostomy/duodenostomy is the current accepted practice for management of symptomatic pancreatic pseudocysts with insertion of two or more double-pigtail (DP) stents. There is no much work on the efficacy of using a single wide-caliber DP stent, aiming to decrease the time, complications, and accessories used in the procedure.
AIM OF THE WORK: The aim of this study was to assess technical and clinical outcomes of using a single wide-caliber DP stent in EUSguided pancreatic pseudocyst drainage.
METHODOLOGY: This multicenter p
