Intracellular Ca2+ modulates hERG channel expression involving Ca2+/calmodulin-dependent kinase and MEK-ERK pathways.

The hERG channel (Kv11.1 potassium channel) is the main carrier of the cardiac rapid delayed rectifier potassium current (Iₖᵣ), which plays a crucial role in the repolarization of the cardiac action potential. This study investigates the effect of intracellular Ca2+ signaling on the expression and function of the hERG channel. The results show that Ca2+ significantly upregulates hERG mRNA expression and IhERG or Iₖᵣ current density with pharmacological evidence implicating Ca2+/calmodulin-dependent kinase (CaMK) and mitogen-activated protein kinase kinase-extracellular signal-regulated kinase

Lumacaftor and fexofenadine prevent donepezil-induced LQTS via PHE656-mediated hERG chaperoning.

Donepezil (DPZ), a first-line therapeutic agent for Alzheimer's disease, causes acquired long QT syndrome (LQTS) by inhibiting the hERG potassium channel. However, there are no specific countermeasures to prevent or reverse this side effect. This study aimed to investigate whether fexofenadine (FEX) and lumacaftor (LUM) can prevent DPZ-induced cardiotoxicity and elucidate the underlying molecular mechanisms. We employed molecular docking and molecular dynamics simulations to analyze interactions among FEX, LUM, DPZ, and hERG channel. Site-directed mutagenesis (F656V, Y652A) was utilized to val

Stop codons and the +4 nucleotide may influence the efficiency of G418 in rescuing nonsense mutations of the HERG gene.

The importance of the local sequence context in determining how efficiently aminoglycosides rescue nonsense mutations has been established previously in disease models. Different stop codons appear to facilitate the termination process with differing efficiencies. Furthermore, the efficiency with which termination is suppressed may also be influenced by the local sequence context surrounding the stop codon. The strongest bias has usually been identified with the nucleotide base that immediately follows the stop codon in the majority of experiments. However, how the sequence context influences

Hypoxia reduces mature hERG channels through calpain up-regulation.

Human ether-a-go-go-related gene (hERG) encodes the pore-forming subunit of the rapidly activating delayed rectifier potassium current (IKr) potassium channel, which is important for cardiac repolarization. Impairment of hERG function is the primary cause of acquired long QT syndrome, which predisposes individuals to cardiac arrhythmias and sudden death. Patients with hypoxia due to conditions such as cardiac ischemia or obstructive sleep apnea display increased incidence of cardiac arrhythmias and sudden death. We sought to understand the mechanisms that underlie hypoxia-associated cardiac ar

Generation of three induced pluripotent stem cell lines from a long QT syndrome type 2 family harboring the pathogenic KCNH2 c.209A > G (p.His70Arg) variant.

Long QT Syndrome Type 2 (LQT2) is a heart rhythm disorder caused by a loss-of-function mutation in the KCNH2 gene, characterized by a prolonged QT interval on an electrocardiogram (ECG) and symptoms such as syncope and potentially life-threatening Torsades de Pointes arrhythmia. We derived three induced pluripotent stem cell (iPSC) lines from a LQT2 family: two individuals with LQT2 who carry the pathogenic variant c.209A > G (p.His70Arg), and one healthy individual. These iPSC lines exhibit normal karyotype, differentiation capability, stem cell pluripotency, and stem cell morphology, providi

Fetal and neonatal presentation of long QT syndrome.

This report describes a fetus presenting with intrauterine tachycardia and hydrops fetalis. Soon after birth the neonate was noted to be in torsades de pointes that responded dramatically to medical management. Long QT syndrome (LQTS) was diagnosed on electrocardiogram obtained soon after birth. The prognosis is poor when LQTS presents in utero or during the first week of life. However, our infant did well with medical management and has remained free of arrhythmias at follow-up.

A mitochondrial DNA polymorphism associated with cardiac arrhythmia investigated in sudden infant death syndrome.

AIM: Long QT syndrome (LQTS) has been shown to be the cause of death in some cases originally diagnosed as sudden infant death syndrome (SIDS). Such cardiac arrhythmias have also been noted in families with mitochondrial disease, and studies indicate that mitochondrial disease could be involved in SIDS. This makes the mtDNA polymorphism T3394C interesting, as a previous study has shown it to be associated with electrocardiographic (ECG) changes after exercise in a family with LQTS, where some members harboured a KCNH2 mutation. SUBJECTS: A total of 245 SIDS cases and 176 control cases. METHO

Antimalarial efficacy of hydroxyethylapoquinine (SN-119) and its derivatives.

Quinine and other cinchona-derived alkaloids, although recently supplanted by the artemisinins (ARTs), continue to be important for treatment of severe malaria. Quinine and quinidine have narrow therapeutic indices, and a safer quinine analog is desirable, particularly with the continued threat of antimalarial drug resistance. Hydroxyethylapoquinine (HEAQ), used at 8 g a day for dosing in humans in the 1930s and halving mortality from bacterial pneumonias, was shown to cure bird malaria in the 1940s and was also reported as treatment for human malaria cases. Here we describe synthesis of HEAQ

A common single nucleotide polymorphism can exacerbate long-QT type 2 syndrome leading to sudden infant death.

BACKGROUND: Identification of infants at risk for sudden arrhythmic death remains one of the leading challenges of modern medicine. We present a family in which a common polymorphism (single nucleotide polymorphism) inherited from the father, combined with a stop codon mutation inherited from the mother (both asymptomatic), led to 2 cases of sudden infant death. METHODS AND RESULTS: KCNQ1, KCNH2, SCN5A, KCNE1, KCNE2, CACNA1c, CACNB2b, and KCNJ2 genes were amplified and analyzed by direct sequencing. Functional electrophysiological studies were performed with the single nucleotide polymorphism

Probing the outer mouth structure of the HERG channel with peptide toxin footprinting and molecular modeling.

Previous studies have shown that the unusually long S5-P linker lining human ether a-go-go related gene's (hERG's) outer vestibule is critical for its channel function: point mutations at high-impact positions here can interfere with the inactivation process and, in many cases, also reduce the pore's K+ selectivity. Because no data are available on the equivalent region in the available K channel crystal structures to allow for homology modeling, we used alternative approaches to model its three-dimensional structure. The first part of this article describes mutant cycle analysis used to ident

The enhancement of cardiac toxicity by concomitant administration of Berberine and macrolides.

As is well-known, hERG plays an essential role in phase III repolarization of cardiac action potentials. Blocking of hERG channels can lead to LQTS. Inhibition of the metabolism of CYPs activities may elevate plasma levels, to further increase accumulation of drug on cardiac. The elevated serum levels may however elicit unexpected toxicities. Therefore, the inhibition tests of hERG and CYP are central to the preclinical studies because they may lead to severe cardiac toxicity. Berberine is widely used as an antibacterial agent and often combined with macrolides to treat gastropathy. Our object

QTc prolongation and family history of sudden death in a patient with desmin cardiomyopathy.

This case report describes a pregnant female patient who presented with new-onset congestive heart failure symptoms and prolonged QTc, with strong family history of sudden death. Endomyocardial biopsy and genetic testing revealed myocardial desmin accumulation and a previously described mutation in the DES (desmin) gene, as well as variants in two LQT genes, SCN5A and KCNH2. The case highlights the phenotypic variability for a particular desmin genotype, and the possible interaction of desminopathy with LQT variants not independently associated with large differences in current properties or Q

New binding site on common molecular scaffold provides HERG channel specificity of scorpion toxin BeKm-1.

The scorpion toxin BeKm-1 is unique among a variety of known short scorpion toxins affecting potassium channels in its selective action on ether-a-go-go-related gene (ERG)-type channels. BeKm-1 shares the common molecular scaffold with other short scorpion toxins. The toxin spatial structure resolved by NMR consists of a short alpha-helix and a triple-stranded antiparallel beta-sheet. By toxin mutagenesis study we identified the residues that are important for the binding of BeKm-1 to the human ERG K+ (HERG) channel. The most critical residues (Tyr-11, Lys-18, Arg-20, Lys-23) are located in th

BeKm-1 is a HERG-specific toxin that shares the structure with ChTx but the mechanism of action with ErgTx1.

Peptide toxins with disulfide-stabilized structures have been used as molecular calipers to probe the outer vestibule structure of K channels. We want to apply this approach to the human ether-a-go-go-related gene (HERG) channel, whose outer vestibule is unique in structure and function among voltage-gated K channels. Our focus here is BeKm-1, a HERG-specific peptide toxin that can suppress HERG in the low nM concentration range. Although BeKm-1 shares the three-dimensional scaffold with the well-studied charybdotoxin, the two use different mechanisms in suppressing currents through their targ

Voltage gated ion channels blockade is the underlying mechanism of BIMU8 induced cardiotoxicity.

BIMU8 is a 5-HT4a receptor agonist and used as an experimental drug to counteract opioid induced respiratory depression. In preliminary experiments serious disturbances in ECG were observed in anesthetized rabbits which prompted us to explore the underlying cause of BIMU8 induced abnormal changes in ECG recordings. Electrophysiological experiments were performed on HEK-293 cells expressing hERG, CaV1.2 and NaV1.5 ion channels. In whole-cell recordings BIMU8 effectively blocked these three channels, with IC50 values of 0.06±0.05, 1.46±0.26 and 4.66±0.58μM for hERG, NaV1.5 and CaV1.2, respective

Beneficial action potential duration-shortening effects, but deleterious negative inotropism of IKs-activator docosahexaenoyl glycine in long QT syndrome type 2.

AIMS: Loss-of-function mutations in KCNQ1 and KCNH2 (α-subunits of the slow delayed IKs and rapid delayed IKr-conducting repolarising K+ channels) lead to long QT syndrome type 1 (LQT1) and 2 (LQT2), respectively. These channelopathies present with longer action potential duration (APD) and prolonged QT interval on electrocardiogram, which can ultimately lead to deadly arrhythmias. Here, we investigated the therapeutic potential of the polyunsaturated fatty acid docosahexaenoyl glycine (DHA-gly) in normalizing APD and QT interval in LQT2 by increasing IKs. METHODS AND RESULTS: The effects of

Two four-marker haplotypes on 7q36.1 region indicate that the potassium channel gene HERG1 (KCNH2, Kv11.1) is related to schizophrenia: a case control study.

BACKGROUND: The pathobiology of schizophrenia is still unclear. Its current treatment mainly depends on antipsychotic drugs. A leading adverse effect of these medications is the acquired long QT syndrome, which results from the blockade of cardiac HERG1 channels (human ether-a-go-go-related gene potassium channels 1) by antipsychotic agents. The HERG1 channel is encoded by HERG1 (KCNH2, Kv11.1) gene and is most highly expressed in heart and brain. Genetic variations in HERG1 predispose to acquired long QT syndrome. We hypothesized that the blockade of HERG1 channels by antipsychotics might als
