Rehabilitation Following Fracture in Dystrophinopathy, A Case Series.

BACKGROUND: Boys with dystrophinopathies (DMD) are at increased risk of low bone mineral density and fracture. Femoral fracture is the most common extremity fracture and is accompanied by significant risk of functional loss. Care considerations for DMD have stressed that aggressive management may be needed to maintain ambulation and that surgical fixation allows early mobilization. OBJECTIVES: Describe 5 cases of femoral fracture in ambulatory boys with DMD and the course of care undertaken to optimize function. PATIENTS: Five boys with DMD median age 15y (12-16) who were independently ambul

Predicting ambulatory function at skeletal maturity in children with moderate to severe osteogenesis imperfecta.

Maximizing ambulation is a key treatment aim in moderate to severe osteogenesis imperfecta (OI). Here we investigated which early clinical characteristics predicted ambulation function at skeletal maturity. We assessed Bleck ambulation scores in 88 individuals with OI at 5 to 6 years of age and again at final height (at 15 to 24 years of age). At 5 to 6 years of age, 33 (38%) children were non-ambulators, 32 (36%) were fully independent ambulators, and 23 (26%) had intermediate ambulation skills. At skeletal maturity, 58% of the study participants had the same mobility level as at first assess

"Walking the dog" to enhance postoperative recovery.

BACKGROUND: Nurses on an adult surgical unit perceived a decrease in patients' mobility and an increase in postoperative complications, which led to an initiative to motivate patients to ambulate. RESEARCH QUESTIONS: The questions addressed in this study refer to adult patients having surgery: Is there a relationship between ambulation and length of stay (LOS)? Did "walking the dog" affect how early and how many times they ambulated? Did "walking the dog" affect the incidence of postoperative complications? METHODS: Photos of dogs were made available and patients were encouraged to "walk the

Ambulation Distance Within 72 Hours after Surgical Management Is a Predictor of 90-Day Ambulatory Capacity in Elderly Patients with Hip Fracture.

INTRODUCTION: The inability to mobilize after surgical intervention for hip fractures in the elderly is established as a risk factor for greater morbidity and mortality. Previous studies have evaluated the association between the timing and distance of ambulation in the postoperative acute care phase with postoperative complications. The purpose of this study was to evaluate the association between ambulatory distance in the acute postoperative setting and ambulatory capacity at 3 months. METHODS: Patients aged 65 and older who were ambulatory at baseline and underwent surgical intervention f

Orthosis use and ambulation in adults with myelomeningocele after orthotic management from childhood.

BACKGROUND: Orthotic devices are required for walking in many individuals with myelomeningocele. Evidence concerning orthosis use is sparse, partly because of heterogeneity among groups and different definitions of the neurological level. OBJECTIVES: The objective of this study was to investigate ambulation regarding orthosis use and satisfaction with orthoses after intense orthotic management during childhood. STUDY DESIGN: The study design is a retrospective follow-up with a cross-sectional study at adult age. METHODS: Participants comprised 59 persons born in 1985 or later. Ambulation wa

Treatment of presumed acute cardiogenic pulmonary oedema in an ambulance system by nurses using Boussignac continuous positive airway pressure.

BACKGROUND: Early initiation of continuous positive airway pressure (CPAP) applied by face mask benefits patients with acute cardiogenic pulmonary oedema (ACPE). The simple disposable Boussignac CPAP (BCPAP) has been used in ambulances by physicians. In the Netherlands, ambulances are manned by nurses and not physicians. It was hypothesised that ambulance nurses are able to identify patients with ACPE and can successfully apply BCPAP. A prospective case series of patients with presumed ACPE treated with BCPAP by ambulance nurses is described. METHODS: After training of ambulance nurses, all 3

Mapping of Early Exercise-Based Interventions for Patients Recovering From Acute Heart Failure: A Scoping Review.

Studies have established the safety and benefits of exercise training in patients admitted with acute heart failure, yet heterogeneity in exercise delivery patterns exists. Hence, a scoping review was undertaken to map the evidence on early exercise-based interventions (early mobilization with/without exercise training) in patients recovering from acute heart failure (admission to up to 2 weeks post-hospitalization), for geographic distribution, exercise prescription, and exercise initiation time. A systematic search was conducted across 5 databases until September 2024. Studies, including pro

Combined Effects of Early Mobilization and Nutrition on ICU-Acquired Weakness.

Intensive Care Unit-Acquired Weakness (ICUAW) is a very common condition in patients admitted to intensive care units (ICUs), even after relatively short stays. This weakness can develop with a pre-existing background of sarcopenia or cachexia, although these conditions are not always the direct cause. Over the years, much of the literature has focused on the nutritional aspect of the issue, leading to the development of widely accepted guidelines recommending the initiation of early nutrition, with the goal of achieving caloric and protein targets within the first five days of ICU admission.

Perceived dyspnea and experience of hospitalized patients with acute decompensated heart failure undergoing an early MObilization protocol with immersive Virtual rEality: MOVE study protocol for a parallel superiority randomized clinical trial.

BACKGROUND: Immersive virtual reality (VR) is an innovative strategy for inpatient rehabilitation programs. Using immersive VR in early mobilization protocols has not yet been investigated in the setting of hospitalized patients with acute decompensated heart failure (ADHF), especially to improve perceived dyspnea, a common symptom of heart failure (HF). METHODS: This is a single-center parallel superiority randomized clinical trial. The study will be conducted at a public teaching hospital in Brazil from January 2023 to January 2024. The sample will include adult patients with ADHF hospitali

Live Birth Rate Following Bed Rest Versus Early Mobilization After Embryo Transfer: A Systematic Review And Meta-Analysis.

Embryo transfer (ET) is the final step of in vitro fertilization (IVF). Different strategies have been proposed to increase the likelihood of implantation, such as post-transfer bed rest. The objective of this manuscript was to compare the clinical outcomes of embryo transfers after IVF of patients offered rest vs. early ambulation. The patient, intervention, comparison, and outcome(s) (PICO) model was used to select the study population, which included women/couples submitted to IVF and prescribed bed rest or early ambulation. Only studies including live birth (LB) as an outcome were included

Hemodynamic Changes During Neuromuscular Electrical Stimulation and Mobility Therapy for an Advanced Heart Failure Patient with Impella 5.0 Device.

The Impella 5.0 is a catheter-mounted left ventricular assist device that is inserted through the patient's subclavian artery. This device allows patient mobilization. Early mobility improves outcomes, including physical function and exercise tolerance, in critically ill patients and those with heart failure (HF). However, there have been no studies regarding the safety of early mobilization during the period of Impella 5.0 insertion based on hemodynamic assessment.A 39-year-old man with idiopathic dilated cardiomyopathy and cardiogenic shock was transferred to our hospital for Impella 5.0 ins

Early mobilization in neurocritical care patients.

PURPOSE OF REVIEW: To examine the potential benefits of early mobilization in neurocritically ill patients and to summarize the recent evidence for and against early mobilization. RECENT FINDINGS: Early ICU mobilization in medically critically ill patients may decrease ICU and hospital length of stay, increase discharge-to-home, and reduce medical costs. Whether these benefits apply to neurologically critically ill patients remains unclear, as neuro ICU patients are often excluded from trials of early mobility. Neurocritically ill patients may present with hemodynamic instability, acute hemip

A Multidisciplinary Approach to Equipment Use in Pediatric Patient Mobilization.

Research has shown that patients who are mechanically ventilated or immobile for greater than 7 days are at increased risk for deconditioning and muscle atrophy. Immobility impacts length of stay as well as patients' ability to return to their prior level of function. As part of the safe patient-handling initiative created at Michigan Medicine, a special team of nurses and therapists was assembled to adapt an adult mobility framework for the pediatric population. The pediatric mobility model determines each patient's specific mobility "phase" based on detailed criteria. Clinical staff can then

A new model for repair of the Achilles tendon in the rat.

The Sprague-Dawley rat is an excellent model for studies of Achilles tendon repair. Most researchers use a modification of the Kessler technique for suture repair of the Achilles tendon in rats. While this technique provides adequate strength, early mobilization is not recommended. Prior to healing, the load will be borne completely by the suture repair, subjecting it to rupture. To prevent this complication, investigators employing the Kessler repair often immobilize the operative extremity with a cast or splint. This has also been shown to be detrimental to the peak load borne by the tendons

[Postoperative care following hip fracture in a hospital or a psychogeriatric nursing home? A comparative study of short or long-stay demented hospitalized elderly].

In this article the question under discussion is, whether a short stay in hospital, for an inmate of a psychogeriatric nursing home with a hip-fracture, shows better results for mortality, morbidity and mobility than a long stay. Two matched groups, of 27 inmates each, have been compared retrospectively. The first remained in hospital for a week or more (long stay), the second for a period of less than a week (short stay). There was no significant difference in mortality risk between both groups. There were less urinary infections in the short-stay group and one month after the operation mobil

[Surgical treatment of the flail chest (author's transl)].

Six patients with traumatic flail chest underwent surgical stabilization using a procedure introduced by Brunner, Hoffmeister and Koncz (2). Compared with internal stabilization by intermittend positive pressure respiration (IPPR), there are some advantages: Time of artificial respiration is shortened, early mobilization is possible, nursing is easier. The procedure is simple to do and is indicated in those patients, in whom longterm artificial respiration is not necessary for other reasons.

Increasing mobility while in hospital after hip fracture: even the best evidence may not translate to all national health services.

Hip fracture rehabilitation is strongly influenced by the healthcare system in which postoperative care is delivered. The WHiTE 15: INITIATE trial represents an important pragmatic attempt to determine whether increasing the frequency and duration of in-hospital physiotherapy after hip fracture improves days at home within 30 days of admission. However, the transferability of such evidence to the Italian National Health System is limited by structural, regulatory and sociocultural factors. Italy has one of the oldest populations in Europe, a high annual burden of proximal femoral fractures, a
