Effect of changing multiple choice questions from "all of the above" to "select all that apply".

INTRODUCTION: The purpose of this study was to describe the effect of converting multiple choice questions (MCQs) that include an "all of the above" (AOTA) answer option to a "select all that apply" (SATA) question type on question performance. METHODS: A summative assessment at the end of the first professional pharmacy year was comprised of approximately 50 multiple choice questions covering material from all courses taught. Eight questions contained AOTA answer options and were converted to SATA items in the subsequent year by eliminating the AOTA option and including the words "select all

Towards a more nuanced conceptualisation of differential examiner stringency in OSCEs.

Quantitative measures of systematic differences in OSCE scoring across examiners (often termed examiner stringency) can threaten the validity of examination outcomes. Such effects are usually conceptualised and operationalised based solely on checklist/domain scores in a station, and global grades are not often used in this type of analysis. In this work, a large candidate-level exam dataset is analysed to develop a more sophisticated understanding of examiner stringency. Station scores are modelled based on global grades-with each candidate, station and examiner allowed to vary in their abili

Reading difficulty of MMPI-2 subscales.

Average reading difficulty based upon the Lexile value was computed for the 567 items and 61 sub/special scales of the MMPI-2. The mean for all items was 600.02 (SD = 304.57), which roughly corresponds to a fifth-grade reading level. Approximately 90% of the items require less than a ninth-grade reading level; 3 items require a high-school education or more. The Amorality (Mal) subscale had an average reading level greater than eighth grade. Nine subscales had 25% or more items above an eighth-grade reading level. The Hypomania (MA) subscales were the most demanding. For persons with low readi

Canadian certification exam pass rates are impacted by genetic counseling program location: A review of Canadian Board of Genetic Counselling data from 2007 to 2025.

Despite significant growth of the genetic counseling profession worldwide, there are differences between genetic counseling training programs (GCPs), professional standards, and professional regulation. Certification of genetic counselors (GCs) in Canada was previously managed by the Certification Board of the Canadian Association of Genetic Counsellors and is now the responsibility of the Canadian Board of Genetic Counselling - Conseil Canadien de Conseil Génétique (CBGC). Since 2007, individuals who attended a Master's level GCP, irrespective of location, have been eligible to sit for the Ca

Performance on the adult rheumatology in-training examination and relationship to outcomes on the rheumatology certification examination.

OBJECTIVE: The American College of Rheumatology (ACR) Adult Rheumatology In-Training Examination (ITE) is a feedback tool designed to identify strengths and weaknesses in the content knowledge of individual fellows-in-training and the training program curricula. We determined whether scores on the ACR ITE, as well as scores on other major standardized medical examinations and competency-based ratings, could be used to predict performance on the American Board of Internal Medicine (ABIM) Rheumatology Certification Examination. METHODS: Between 2008 and 2012, 629 second-year fellows took the AC

Confirming the validity of Part II of the National Board Dental Examinations: a practice analysis.

Successful completion of Part II of the National Board Dental Examinations is a part of the licensure process for dentists. Good testing practice requires that the content of a high stakes examination like Part II be based on a strong relationship between the content and the judgments of practicing dentists on what is important to their practice of dentistry. In an effort to demonstrate this relationship for Part II, the Joint Commission conducted a practice analysis, which involved a two-dimensional model. The sixty-three Competencies of the New Dentist, developed and promulgated by the Ameri

The Path to Independence: Progression in Entrustment Reflected in Entrustable Professional Activity Ratings.

BACKGROUND: In 2016 the American Board of Surgery introduced Entrustable Professional Activities (EPAs) as an assessment framework to support competency-based education within general surgery. EPAs operationalize milestones and competencies into a practical assessment framework, enabling timely, targeted feedback and progressive entrustment. OBJECTIVE: We hypothesize that variability in trainee entrustment may identify specific EPAs that function as performance targets, allowing earlier identification of trainees who may need additional support. METHODS: National ABS EPA data for PGY-1 throu

Development and evaluation of a massive open online course (MOOC) to teach medical students the prudent use of antibiotics.

BACKGROUND: If antibiotics are used appropriately, the development of antimicrobial resistance (AMR) can be curbed. Many medical students feel that they do not receive sufficient training in this respect during their undergraduate medical education. In recent years, digital learning formats are being successfully employed in student teaching. Our aim was to develop and evaluate a massive open online course (MOOC) on appropriate antibiotic therapy and the development of AMR. The intention was to provide the MOOC as an effective learning format in medical schools and to encourage others to devel

A Stepping Stone Toward Necessary Change: How the New USMLE Step 1 Scoring System Could Affect the Residency Application Process.

Score reporting for the United States Medical Licensing Examination Step 1 will change from a 3-digit number to pass/fail as soon as January 1, 2022. The shift is meant to prevent residency program directors from using Step 1 scores to select applicants for interviews, a purpose for which the exam was not designed. Using Step 1 scores in this way also has put undue stress on medical students applying to residency. However, the score reporting change represents only one stepping stone toward an improved transition from undergraduate to graduate medical education. To enable a more reliable and h

Fetal physiology cardiotocography training, a regional evaluation.

INTRODUCTION: Cardiotocography (CTG) has its limits in detecting fetal acidosis and intrapartum asphyxia. Our aim was to evaluate a CTG training programme based on fetal physiology in the Mediterranean perinatal network. METHODS: Professionals from 41 maternity units of the Mediterranean network were invited to participate in a CTG masterclass based on fetal physiology in March 2019 and October 2019. They were asked to react to three practical cases by a physiological approach before the training course (T0), one month after (T1) and six to seven months after (T2). The mean scores were compar

What Regulatory Requirements and Existing Structures Must Change If Competency-Based, Time-Variable Training Is Introduced Into the Continuum of Medical Education in the United States?

As competency-based medical education is adopted across the training continuum, discussions regarding time-variable medical education have gained momentum, raising important issues that challenge the current regulatory environment and infrastructure of both undergraduate and graduate medical education in the United States. Implementing time-variable medical training will require recognizing, revising, and potentially reworking the multiple existing structures and regulations both internal and external to medical education that are not currently aligned with this type of system. In this article
