Crumley's Classification of Laryngeal Synkinesis: A Comparison of Laryngoscopy and Electromyography.

OBJECTIVES/HYPOTHESIS: Applying the principles of misdirected nerve regeneration to the larynx, Roger Crumley in 1989 coined the term laryngeal synkinesis (LS) which he later (2000) classified into 4 types (type I - good voice, type II - involuntary twitches and poor voice, type III - adduction during inspiration, type IV - abduction during phonation). Neurophysiological data were not available for all LS patients at that time. The current study was undertaken to utilize and test the Crumley classification for a clinical interrater comparison and, secondly, compare predicted with actual laryng

Electromyography as a new means of navigation during endotracheal intubation.

This study tested a method of using rapid analysis of electromyographic response patterns to electrical stimulation to enable real-time navigation during endotracheal intubation. An electromyographic response detection device was constructed and integrated into a standard endotracheal tube. The rebound rates of the response voltages were measured in the trachea and oesophagus after stimulation in an acute study performed in three freshly euthanized male Suffolk sheep. In a blind study, a physician attempted to identify the tissue type solely from the electrical response signals. In the acute s

Value of Laryngeal Electromyography in Spasmodic Dysphonia Diagnosis and Therapy.

OBJECTIVE: To investigate the role of laryngeal electromyography (LEMG) in the diagnosis and treatment of spasmodic dysphonia (SD). METHODS: The clinical manifestations, characteristics of motor unit potentials (MUPs), recruitment potentials, and laryngeal nerve evoked potentials (EPs) in LEMG, as well as the changes after botulinum toxin (BTX) treatment, were analyzed in 39 patients with adductor SD. RESULTS: The main clinical manifestations were a strained voice and phonation interruptions; in addition, the patients displayed hyper-adducted vocal folds during phonation. LEMG revealed signi

[Electromyography (EMG) and neurography in patients with severe neuromuscular diseases].

BACKGROUND: Many neuromuscular diseases are potentially severe, and EMG and neurography are methods used in the assessment of these conditions. METHOD: The article is based on the authors' knowledge and experience, with special emphasis on the use of these methods in the assessment of severe diseases affecting striated muscle and peripheral nerves. A PubMed search was performed with the cut-off fifteen years back in time, and in addition a discretionary selection was made of articles known to the authors. RESULTS: EMG is the most valuable method for assessing myopathy, and neurography provid

Experience of using electromyography of the genioglossus in the investigation of paediatric dysphagia.

AIM: The aim of the study was to assess, retrospectively, the utility of genioglossus electromyography (gEMG) in evaluating children with suspected neurogenic feeding and swallowing difficulties. METHOD: Children who were evaluated using gEMG at a tertiary paediatric neurology dysphagia service were reviewed. Data were analysed by the presence/absence of neurogenic changes on gEMG and the method of feeding at their most recent follow-up. RESULTS: The study group comprised 59 individuals (36 males, 23 females; median age 20 mo; range 1 mo-15 y). The study cohort included individuals with hete

Laryngeal electromyography: a proposal for guidelines of the European Laryngological Society.

Although recognized as a valuable diagnostic tool for more than 60 years, many laryngologists do not routinely use laryngeal electromyography (LEMG). This may be due to a persisting lack of agreement on methodology, interpretation, validity, and clinical application of LEMG. To achieve consensus in these fields, a laryngeal electromyography working group of European neurolaryngologic experts was formed in order to (1) evaluate guidelines for LEMG performance and (2) identify issues requiring further clarification. To obtain an overview of existing knowledge and research, English-language liter

The innervation of the axillary arch determined by surface stimulodetection electromyography.

The axillary arch (AA) is a muscular anatomical variation in the fossa axillaris that has been extensively studied in cadaveric specimens. Within these dissections, different innervations of the AA have been proposed, but this has never been explored in vivo. Knowledge of the innervation of the AA is required in order to better understand its function (e.g. predisposition for certain sports and/or activities, understanding shoulder injuries in overhead sports). Here, we report on the use of surface stimulodetection electromyography (SSEMG) to resolve the innervation of the AA in 20 subjects (1

Use of uterine electromyography to diagnose term and preterm labor.

Current methodologies to assess the process of labor, such as tocodynamometry or intrauterine pressure catheters, fetal fibronectin, cervical length measurement and digital cervical examination, have several major drawbacks. They only measure the onset of labor indirectly and do not detect cellular changes characteristic of true labor. Consequently, their predictive values for term or preterm delivery are poor. Uterine contractions are a result of the electrical activity within the myometrium. Measurement of uterine electromyography (EMG) has been shown to detect contractions as accurately as

Surface electromyography as a screening method for evaluation of dysphagia and odynophagia.

OBJECTIVE: Patients suspected of having swallowing disorders, could highly benefit from simple diagnostic screening before being referred to specialist evaluations. The article analyzes various instrumental methods of dysphagia assessment, introduces surface electromyography (sEMG) to carry out rapid assessment of such patients, and debates proposed suggestions for sEMG screening protocol in order to identify abnormal deglutition. DATA SOURCES: Subject related books and articles from 1813 to 2007 were obtained through library search, MEDLINE (1949-2007) and EMBASE (1975-2007). METHODS: Speci

Artifact on electromyography produced by an implanted sacral nerve stimulator.

A patient with muscle cramps was referred for nerve conduction studies and electromyography. Her study demonstrated bursts of spontaneous electromyography activity, which waxed and waned in amplitude with a "dive bomber"-like sound. The abnormal finding was found to be an artifact from an implanted sacral nerve stimulator for bladder incontinence.

Sphincter electromyography and the penilo-cavernosus reflex: are both necessary?

AIMS: For electrodiagnostic assessment of patients with suspected peripheral sacral nerve lesions, sphincter muscle electromyography (EMG) and sacral reflex testing are mainly used. However, it has not been established whether a single test or both tests are necessary in individual patients. The present study aimed to clarify this dilemma. METHODS: Fifty-two men with clinical, electrodiagnostic and radiological signs supportive of chronic cauda equina or conus medullaris lesions were included. Clinical examination, including perianal sensation and penilo-cavernosus reflex testing, and neuroph

The contribution of electromyography to the diagnostics of some rare palatal anomalies.

The paper presents the electromyographic (EMG) findings of the soft palate in three patients: a patient with Mohr syndrome and cleft palate, a patient with palatal asymmetry and rhinolalia and a patient with vertical oro-ocular facial cleft with marked asymmetry of the cleft palate. In the first patient, electrical silence was registered in one half of the palate. In the second patient, moderate loss of active motor units was registered in the hypoplastic part of the palate. In the third patient, in spite of asymmetry, the EMG finding was normal on both sides of the palate.

The role of manometry, electromyography and radiology in the assessment of intractable constipation.

An analysis is made of functional studies performed in 96 constipated patients to see how these studies influenced the choice of surgical treatment. All patients underwent anal manometry, and other investigations included colonic transit studies (56), anal sphincter electromyography (42) and defaecatory proctography (34). Additionally nine patients underwent full thickness rectal biopsy. The resting anal canal pressures of the patients studied were lower than controls, and fibre density studies on electromyography were abnormal in half the patients studied suggesting a degree of denervation of

The role of manometry, electromyography and radiology in the assessment of faecal incontinence.

The results of laboratory investigations in 156 patients presenting with faecal incontinence are reviewed to see if and how these investigations supplement a careful clinical evaluation, and in particular to see if they help in the practical management of the problem. All patients underwent anal manometry, and in addition 52 underwent anal sphincter electromyography and 27 defaecatory proctography. Anal manometry quantified sphincteric weakness, and proved superior to digital assessment in this regard. Resting and squeeze pressures were less in those with complete than those with partial incon
