Implementation of Electronic Medical Record Interventions to Maximize Opioid Disposal after Surgery in Routine Clinical Practice.

BACKGROUND: Despite guideline-directed opioid prescribing after operation, many patients retain excess opioids. Leftover pills increase the risk for misuse, diversion, and dependency. It is unclear whether interventions applied in routine clinical practice can increase excess opioid disposal rate. STUDY DESIGN: All adult patients at our institution with postoperative opioid prescriptions were identified in general surgery, otolaryngology, plastic surgery, neurosurgery, urology, and transplant surgery. Disposal rates for 453 preintervention patients (operation between March 1, 2023, and June 3

Gestational diabetes mellitus prediction using image-encoded electronic medical records and Transformer-based fusion.

Accurate prediction of gestational diabetes mellitus (GDM) is critical for improving maternal and fetal outcomes. This study develops a Transformer-based multimodal fusion model that integrates tabular clinical features and image-encoded electronic health records (EHRs), aiming for accurate end-to-end classification of GDM. Preprocessed EHRs were transformed into grayscale, RGB, and heatmap, with visual features were extracted by a Vision Transformer and tabular features by an MLP. A modality-aware attention mechanism enhances cross-modal fusion. Evaluated on two public datasets, performance g

Towards a prescribing monitoring system for medication safety evaluation within electronic health records: a scoping review.

BACKGROUND: Medical care can fail for various reasons: diseases can remain undetected and their severity misjudged, therapies can be incorrectly dosed or ineffective, and therapies can trigger new conditions or adverse drug reactions (ADR). To manage the complexity of changing patient circumstances, data-driven techniques play an increasingly important role in monitoring patient safety and treatment success. Therefore, clinical prediction models need to consider longitudinal factors ("Prescribing Monitoring") to ensure clinically meaningful results and avoid misclassification in the dynamic he

Longitudinal Analysis of Electronic Health Records Reveals Medical Conditions Associated with Subsequent Alzheimer's Disease Development.

BACKGROUND: Several health conditions are known to increase the risk of Alzheimer's disease (AD). We aim to systematically identify medical conditions that are associated with subsequent development of AD by leveraging the growing resources of electronic health records (EHRs). METHODS: This retrospective cohort study used de-identified EHRs from two independent databases (MarketScan and VUMC) with 153 million individuals to identify AD cases and age- and gender-matched controls. By tracking their EHRs over a 10-year window before AD diagnosis and comparing the EHRs between AD cases and contro

Risk for hypertension and heart failure linked to high normal serum sodium and tonicity in electronic medical records.

BACKGROUND AND AIMS: Population aging is fueling an epidemic of age-related chronic diseases. Managing risk factors and lifestyle interventions have proven effective in disease prevention. Epidemiological studies have linked markers of poor hydration with higher risk of chronic diseases and premature mortality. Many individuals do not adhere to recommended hydration levels and could benefit from improved hydration habits. Our study evaluates the use of electronic medical records to confirm the relationship between inadequate hydration and the risk of chronic diseases, which may inform hydratio

Analyzing electronic medical records to extract prepregnancy morbidities and pregnancy complications: Toward a learning health system.

INTRODUCTION: Preexisting and pregnancy-related medical conditions frequently co-occur, leading to multimorbidity (≥2 morbidities) in pregnant women, and much of this information is in semi-structured format in electronic medical records (EMRs). The aim was to advance the learning health system as a platform for automating information extraction from EMRs and to uncover the prevalence of common morbidities during pregnancy and their association with pregnancy-related complications. METHODS: This study included 48 502 pregnant women attending Monash Health maternity hospitals from 2016 to 2021

Investigating documentation of alcohol and non-medical substance use in oncology treatment: an electronic health record review.

PURPOSE: Little is known about non-tobacco substance use (SU) and its treatment in cancer patients. National guidelines address tobacco only, and assessment of SU in cancer patients is not standardized. It is not clear how oncology clinicians assess, document, and follow-up on SU. METHODS: We conducted an electronic health record review of patients enrolled in a smoking cessation trial at one large hospital site (N = 176). Chart review of oncology treatment notes assessed whether SU assessment was documented, the content of the documentation/assessment (e.g., frequency of use), and details ab
