Modern treatment of acute mesenteric ischaemia.

BACKGROUND: Diagnosis of acute mesenteric ischaemia in the early stages is now possible with modern computed tomography (CT), using intravenous contrast enhancement and imaging in the arterial and/or portal venous phase. The availability of CT around the clock means that more patients with acute mesenteric ischaemia may be treated with urgent intestinal revascularization. METHODS: This was a review of modern treatment strategies for acute mesenteric ischaemia. RESULTS: Endovascular therapy has become an important alternative, especially in patients with acute thrombotic superior mesenteric a

Acute and Chronic Pulmonary Emboli.

Under most circumstances, the goal of treatment of pulmonary embolism is the prevention of recurrent embolic events, achieved through conventional anticoagulant therapy with unfractionated heparin or a low molecular weight heparin, followed by warfarin therapy for a minimum of 6 months. When acute pulmonary embolism is associated with significant right ventricular dysfunction or systemic hypotension, more aggressive intervention may be warranted. Under these circumstances, potential interventions include thrombolytic therapy (either systemic or catheter-directed), placement of an inferior vena

Complications of Pulmonary Embolism in a Pediatric Patient.

A 16-year-old boy presents with a massive acute pulmonary embolism requiring emergent surgical embolectomy and extracorporeal membrane oxygenation for right ventricular failure. Subsequently he was diagnosed with catastrophic antiphospholipid syndrome requiring immunosuppression, and then pneumatoceles causing tension pneumothoraxes. The rarity of presentation in a child required collaboration across pediatric and adult disciplines.

Peripheral atheromatous embolization following blunt abdominal trauma.

A 65-year-old man sustained blunt abdominal trauma in an automobile accident which resulted in severe acute ischemia of the right lower leg and foot. Complete arteriography showed extensive aortoiliac arteriosclerosis and embolic occlusion of the distal right popliteal artery. Prompt embolectomy accomplished limb salvage and confirmed the diagnosis of traumatic artheromatous embolization.

The removal of floating right heart thrombi and pulmonary embolus using AngioJet device and venoarterial extracorporeal membrane oxygenation: a case report.

BACKGROUND: Floating right heart thrombi (FRHTS), known as thrombi in transit, are usually located in the atrium or ventricle. Generally, it occurs in patients with pulmonary embolism (PE) and dyspnea, chest pain, syncope and palpitations are the most common symptoms on presentation. The mortality of patients with FRHTS is higher than that of those without FRHTS. Current treatment includes anticoagulation, systemic thrombolysis, catheter directed interventions, and surgical embolectomy. However, there is no consensus on the optimal management options. CASE DESCRIPTION: Herein, we report the c

Modern treatment of pulmonary embolism.

Modern treatment of acute pulmonary embolism requires rapid and accurate diagnosis followed by risk stratification to devise an optimal management strategy. Patients at low risk have good outcomes simply with intensive anticoagulation treatment. Higher-risk patients may require more aggressive intervention with thrombolysis or embolectomy. Clinical risk factors for an adverse outcome include increasing age, cancer, congestive heart failure, systemic arterial hypotension, chronic obstructive pulmonary disease and right ventricular dysfunction. A promising approach is the Geneva Prognostic Score

Thrombolytic toxicity: blood brain barrier disruption in human ischemic stroke.

BACKGROUND: In experimental models of cerebral ischemia, thrombolytic drugs have been demonstrated to have a number of neurovascular toxic effects including blood brain barrier disruption. Early barrier opening caused by focal cerebral ischemia in human stroke can be assessed by the presence of gadolinium enhancement of cerebrospinal fluid, termed 'Hyperintense Acute Injury Marker' (HARM). METHODS: In a retrospective analysis, the frequency of HARM was studied in 140 patients, 38 receiving intra-arterial thrombolytics, 18 undergoing mechanical embolectomy, 24 receiving intravenous tissue plas

An Unusual Case of Arterial Embolism in an Adolescent With a Mitral Valve Repair With a Ring.

We report a case of an adolescent young man who presented with embolism of both lower legs. The patient had undergone mitral valve repair with a Kalangos biodegradable ring (Bioring SA, Lonay, Switzerland) 9 months earlier. Bilateral embolectomy was performed. Histopathologic examination revealed minute fragments of synthetic material within the embolus, which otherwise consisted of recent thrombus.

Acute ischemic stroke secondary to cardiac embolus of a 'foreign body' material after a redo sternotomy for mitral valve replacement: A case report.

Cardiac surgery has been shown to be associated with increased risk of acute ischemic stroke. This report presents a case of a successful mechanical embolectomy procedure to treat a patient for an acute ischemic stroke, which was caused by the cardiac embolization of a 'foreign body' containing debris following a redo sternotomy procedure for mitral valve replacement and tricuspid valve annuloplasty.

Salvage pulmonary thromboembolectomy for massive pulmonary embolism in a child presented with syncope and subsequent cardiac arrest.

Pulmonary embolism is frequently under-recognised in children and, therefore, a high index of suspicion should be exerted on patients with exertional dyspnoea, presyncope/syncope and unexplained cardiopulmonary arrest. We discuss a 10-year-old previously healthy girl who presented with syncope and subsequent cardiac arrest related to massive pulmonary embolism and was salvaged successfully by emergent pulmonary embolectomy.

Lost and found: an unusual late complication of the Angio-Seal closure device.

We report an unusual case of breakage and embolisation of a femoral access closure device (Angio-Seal), which caused acute leg ischemia after percutaneous coronary intervention in a diabetic patient with documented peripheral vascular disease. Embolectomy from the superficial femoral artery revealed the polymer anchor split in two parts and embedded within a large (25x5 mm) thrombus.

Endovascular Therapy is Effective for Large Vessel Occlusion Despite Concurrent Cancer.

OBJECTIVES: Ischemic stroke and concurrent cancer is increasingly recognized. However, optimal management is uncertain. As mechanical thrombectomy has become the standard of care for large vessel occlusion, more patients with cancer are presenting for embolectomy. However, it is unknown whether this subgroup has the same benefit profile described in multiple randomized trials for thrombectomy for large vessel occlusion. Our objective was to retrospectively evaluate a North American embolectomy database for safety and outcomes in patients with active cancer. MATERIALS AND METHODS: A case serie

Pulmonary embolism from right ventricular myxoma in a child with undiagnosed Carney complex.

Pediatric pulmonary embolism occurs in 8.6-57 per 100,000 hospitalised children. We report a novel case of bilateral pulmonary emboli in a child presenting with dyspnoea who was found to have large right ventricular myxoma and subsequent diagnosis of Carney complex. After resection of the right ventricular myxoma and bilateral pulmonary embolectomy, she had a full recovery and an excellent outcome.

Entrapped thrombus in a foramen ovale causing pulmonary emboli treated with emergent surgery. Literature review.

A 60-year-old woman was admitted due to massive bilateral pulmonary thromboembolism and entrapped thrombus in the patent foramen ovale and severe right ventricular dysfunction. The patient underwent on-pump/beating heart removal of the intracardiac thrombus and bilateral pulmonary embolectomy. The postoperative course was uneventful. The chosen surgical strategy seems to offer excellent outcome in patients with severe right ventricular dysfunction.

Massive thrombus in the aortic arch: a 59-year-old lady with an unknown familial predisposition to vascular thrombosis.

The following is an account of a 58-year-old lady who presented to our institution with two episodes of acute limb ischemia: one upper, which required embolectomy; and one lower, which required a below knee amputation. We subsequently performed transesophageal echocardiography (TOE), which revealed very large mobile echogenic masses adherent to the wall of her aortic arch, described below.

Clot-in-transit and pulmonary embolism: an urgent call for awareness and action.

Patients with acute pulmonary embolism (PE) have a wide spectrum of clinical presentations, from incidental findings to sudden cardiac death. Management and treatment recommendations in currently available guidelines vary according to patient risk and haemodynamic profile. A clot-in-transit (CiT) in the right heart chambers may be occasionally identified and is, therefore, an under-recognised but challenging condition, often preceding an abrupt clinical deterioration, and associated with increased mortality. Data on the detection of a CiT are sparse but consistent in attributing negative progn

Acute pulmonary embolism with coexisting right heart thrombi in transit-surgical treatment of 20 consecutive patients.

OBJECTIVES: The presence of right heart thrombi in transit (RHTiT) in the setting of acute pulmonary embolism (PE) is associated with high mortality. The optimal management in such cases is inconclusive. We present the results of surgical treatment of 20 consecutive patients diagnosed with high- or intermediate-high-risk PE with coexisting RHTiT. METHODS: A retrospective analysis was performed of all consecutive patients undergoing surgical treatment in the Medicover Hospital between 2013 and 2021 for acute PE with coexisting thrombi in-transit in right heart cavities. The diagnosis was based
