Improvement of injury severity prediction (ISP) of AACN during on-site triage using vehicle deformation pattern for car-to-car (C2C) side impacts.

OBJECTIVE: The Advanced Automatic Crash Notification (AACN) system needs to predict injury accurately, to provide appropriate treatment for seriously injured occupants involved in motor vehicle crashes. This study investigates the possibility of improving the accuracy of the AACN system, using vehicle deformation parameters in car-to-car (C2C) side impacts. METHODS: This study was based on car-to-car (C2C) crash data from NASS-CDS, CY 2004-2014. Variables from Kononen's algorithm (published in 2011) were used to build a "base model" for this study. Two additional variables, intrusion magnitud

Evaluation of an Integrated Rescue Task Force Model for Active Threat Response.

OBJECTIVE: An integrated response to active threat events is essential to saving lives. Coordination of law enforcement officer (LEO) and emergency medical services (EMS) roles requires joint training, as maximizing survival is a shared responsibility. We sought to evaluate the performance of an integrated LEO-EMS Rescue Task Force (RTF) response to a simulated active shooter incident utilizing objective performance measures. METHODS: Following prior didactic training, we conducted a series of evaluation scenarios for EMS providers and patrol officers in our urban/suburban advanced life suppo

Evaluating pediatric advanced life support in emergency medical services with a performance and safety scoring tool.

INTRODUCTION: Pediatric out-of-hospital cardiac arrests (P-OHCA) are infrequent, have low survival rates, and often have poor neurologic outcomes. Recent evidence indicates that high-performance emergency medical service (EMS) care can improve outcomes. OBJECTIVES: To evaluate Pediatric Advanced Life Support (PALS) guideline performance in the out of hospital setting and introduce an easy-to-use tool that scores guideline compliance and patient safety. METHODS: We observed EMS teams responding to standardized pediatric resuscitation simulations. Teams were dispatched to a mock assisted livin

Hypoxia and hypotension in patients intubated by physician staffed helicopter emergency medical services - a prospective observational multi-centre study.

BACKGROUND: The effective treatment of airway compromise in trauma and non-trauma patients is important. Hypoxia and hypotension are predictors of negative patient outcomes and increased mortality, and may be important quality indicators of care provided by emergency medical services. Excluding cardiac arrests, critical trauma and non-trauma patients remain the two major groups to which helicopter emergency medical services (HEMS) are dispatched. Several studies describe the impact of pre-hospital hypoxia or hypotension on trauma patients, but few studies compare this in trauma and non-trauma

Gene expression and epigenetic changes in post-traumatic stress disorder, depression, and anxiety in first responders: A systematic review.

OBJECTIVE: Police, firefighters, dispatchers, and emergency medical technicians-collectively known as first responders-are a unique population frequently exposed to chronic, traumatic incidents. This exposure results in a high prevalence of PTSD, depression, and anxiety, posing a substantial public health concern. Genetic predispositions and epigenetic modifications that regulate gene expression are significant contributors to trauma-related pathologies. This systematic review aims to summarize current data on epigenetic and gene expression changes in first responders related to three post-tra

Effects of a sedentary behaviour intervention in emergency dispatch centre phone operators: a study protocol for the SECODIS randomised controlled cross-over trial.

INTRODUCTION: Sedentary behaviour is a public health problem. We mainly have sedentary behaviour at work, transforming them into occupational risk. To our knowledge, there is no intervention study on the reduction of occupational sedentary behaviour in a real work situation and its impact on health and biomarkers of stress. The main objective is to study changes in sedentary behaviour following a behavioural intervention (sit-and-stand desk and cycle ergometer). METHODS AND ANALYSIS: This is a randomised controlled trial in cross-over design conducted in a single centre. The study will be pro

An international perspective of out-of-hospital cardiac arrest and cardiopulmonary resuscitation during the COVID-19 pandemic.

BACKGROUND: Out-of-hospital cardiac arrest (OHCA) accounts for a substantial proportion of sudden cardiac events globally, with hundreds of thousands of cases reported annually in the United States. The mortality rate of patients who suffer OHCA remains high despite extensive utilization of resources. OBJECTIVES: We aim to describe the current landscape of OHCA during the COVID-19 pandemic and provide an overview of the logistical challenges and resuscitation protocols amongst emergency medical service (EMS) personnel. DISCUSSION: Recent studies in Italy, New York City, and France characteri

Air pollution and humidity as triggering factors for stroke. Results of a 12-year analysis in the West Paris area.

BACKGROUND AND PURPOSE: Previous studies have suggested an association between stroke and meteorological factors, air pollution and acute respiratory infections as triggering factors. Often, these factors have been evaluated separately. We evaluated the association between all these environmental triggering factors and calls for suspected stroke in a suburb in west Paris from 2004 to 2015. METHODS: We used data from the emergency medical dispatching center of all calls for suspected stroke (SAMU 78), climatic parameters (MétéoFrance), pollution (AIRPARIF), and data from influenza epidemic sur

The Gradual Establishment and Historical Implications of Hospital Ship Mobile Medical Services along Korea's Western and Southern Coasts in the 1960s-1970s.

The term muui (doctorless) refers to a state of absolute deprivation of medical care. In modern Korean history, chronic shortages and regional disparities of medical resources resulted in the widespread existence of muuichon (doctorless village). In response, the Ministry of Health and Social Affairs implemented a series of policies and initiatives, including the establishment of a public health center system and the mobilization of public doctors. Yet the very term muuichon also suggested that the problem of public health was inseparable from that of space. Above all, there was a distinct spa

Prehospital Outcomes Associated with an Initial Strategy of Intranasal versus Intramuscular Naloxone Administration by EMS Clinicians: A Nationwide Cohort Study.

OBJECTIVES: Previous studies have suggested that intranasal (IN) naloxone is less effective than intramuscular (IM) naloxone at reversing opioid toxicity but may result in a lower prevalence of precipitated withdrawal. Because limited real-world comparisons have been performed, we aimed to compare initial strategies of IM or IN naloxone administration by emergency medical services (EMS) clinicians using a nationwide dataset. METHODS: We included all adult (>18 years of age) patients in the 2023-2024 ESO Data Collaborative research datasets who received intranasal or intramuscular (IN or IM) n

Physician-based emergency medical service deployment characteristics in severe traumatic brain injury: a Dutch multicenter study.

INTRODUCTION: Prehospital guidelines advise advanced life support in all patients with severe traumatic brain injury (TBI). In the Netherlands, it is recommended that prehospital advanced life support is particularly provided by a physician-based helicopter emergency medical service (P-HEMS) in addition to paramedic care (EMS). Previous studies have however shown that a substantial part of severe TBI patients is exclusively treated by an EMS team. In order to better understand this phenomenon, we evaluated P-HEMS deployment characteristics in severe TBI in a multicenter setting. METHODS: The

Out-of-hospital cardiac arrest following trauma: What does a helicopter emergency medical service offer?

INTRODUCTION: Helicopter emergency medical services (HEMS) are often dispatched to patients in traumatic cardiac arrest (TCA) as they can provide treatments and advanced interventions in the pre-hospital environment that have the potential to contribute to an increased survival. This study, aimed to investigate the added value of HEMS in the treatment of TCA. METHODS: We performed a retrospective cohort study of all patients with a pre-hospital TCA who were attended by a non-urban HEMS (Kent, Surrey and Sussex Air Ambulance trust) between July 1st 2013 and May 1st 2018. We investigated how ma

Real-time CPR feedback in out-of-hospital cardiac arrest: A systematic review of CPR quality and outcomes.

BACKGROUND: Out-of-hospital cardiac arrest (OHCA) remains a major cause of mortality in industrialised countries, with survival highly dependent on high-quality cardiopulmonary resuscitation (CPR). Real-time feedback systems have been introduced to optimise CPR performance by improving compression depth, rate, chest compression fraction (CCF), and recoil. AIM: To evaluate the effectiveness of real-time feedback systems on CPR quality indicators and clinical outcomes in adult OHCA patients treated by prehospital emergency medical services. METHODS: A systematic review was conducted according
