Infective endocarditis: the European viewpoint.

Infective endocarditis (IE) is a difficult and complex disease. In recent years epidemiology and microbiology have changed. In developed countries IE is now affecting older patients and patients with no previously known valve disease. Prosthetic IE (prosthetic valve endocarditis [PVE]) and endocarditis in patients with pacemakers and other devices (cardiac device related infective endocarditis [CDRIE]) are becoming more frequent. The number of Staphylococcus aureus IE is increasing related to the number of endocarditis that occurs because of health care associated procedures, especially in dia

[Infective endocarditis in octogenarian patients].

Our aims were to investigate the clinical features and prognosis of endocarditis in octogenarian patients (aged > 79 years) and in comparison with those in younger elderly patients (aged 65-79 years) and young patients (aged < 65 years). Octogenarian subjects more frequently were male and had a community-acquired infection, mitral valve disease, and chronic anemia. Their clinical course was more insidious and benign: they presented less often with fever or new heart murmurs. When heart failure was present, it tended to be less severe. The most frequently isolated microorganisms were streptococ

Treatment strategies for infective endocarditis.

IMPORTANCE OF THE FIELD: Despite significant advances in medical, surgical, and critical care interventions, infective endocarditis (IE) remains a disease associated with considerable morbidity and mortality. Estimates from the American Heart Association place the incidence of IE in the US at 10,000 - 15,000 new cases each year. This may be due to the changing epidemiology of IE, including increasing antimicrobial resistance, increasing heart surgeries, prosthetic valve implantation, and widespread use of intravenous drugs. Furthermore, a new form of the disease, healthcare-associated IE, whic

Endocarditis presenting as acute abdomen.

Infective endocarditis remains an interesting and challenging disease in which the presenting features have been modified by medical progress. We report a case of a 63-year-old woman who presented with pyrexia, weight loss, night sweats and fatigue over a period of 7 weeks, at the end of which she developed severe right hypochondrial pain, mimicking acute abdomen. She was provisionally diagnosed with and treated for acute cholecystitis to no avail. An abdominal computed tomography scan was performed and revealed the presence of right kidney infarction, the source of which was later proven to o

Candidatus Bartonella mayotimonensis and endocarditis.

We describe a new Bartonella species for which we propose the name Candidatus Bartonella mayotimonensis. It was isolated from native aortic valve tissue of a person with infective endocarditis. The new species was identified by using PCR amplification and sequencing of 5 genes (16S rRNA gene, ftsZ, rpoB, gltA, and internal transcribed spacer region).

[Infective endocarditis as cardiovascular emergency].

Infective endocarditis is an infection of cardiovascular structures which is typically caused by bacteria. Despite recent medical advances mortality reaches up to 26% which is even higher with mortality rates of up to 84% in complex cases leading to admission to intensive care units. The diagnosis is based on positive blood culture results with identical microorganisms and the demonstration of endocardial involvement. A rapid initiation of an adequate therapeutic regimen is important to prevent the patients from severe complications such as heart failure, uncontrolled infection or septic embol

[Surgical treatment for infective endocarditis].

OBJECTIVE: Surgery for infective endocarditis (IE) is technically demanding, especially the one for active IE. METHODS: Operations were performed in 21 patients with a mean age of 52.2 +/- 18.8 years. Fifteen patients were male, and 6 were female. There were 15 patients with active IE and 6 patients with healed IE. Isolated pathogens were Streptococcus in 8 cases, Staphylococcus in 3, and Enterococcus in 2. Two patients had prosthetic valve endocarditis. When the lesions affected the aortic valve, aortic valve replacement (AVR) was performed. When the lesions affected the mitral or tricuspid

Gemella morbillorum prosthetic valve endocarditis.

The authors describe a case of prosthetic valve infective endocarditis (IE) caused by Gemella morbillorum (GM), a gram-positive coccus that grows slowly in culture and occasionally causes a subacute to chronic form of IE. To the author's knowledge, this is the third case of GM prosthetic valve IE to be reported. Subacute to chronic IE caused by slow-growing organisms can be subtle in presentation and should be suspected in patients who have prosthetic valves presenting with worsening heart failure.

[Pseudoinfectious endocarditis in antiphospholipid syndrome].

The antiphospholipid syndrome (APhS) is autoimmune non-inflammatory trombotic vasculopathy, associated with damage of vessels of any caliber and localization that determines the diversity of clinical manifestations of this syndrome. One of possible localizations of thrombosis in AphS is pseudoinfective endocarditis (PsIE). We have performed analysis of clinical and laboratory features of APhS, associated with PsIE. 28 APhS patients with PsIE, average age--44+/-13 years, were included in the study. Among them 21 patients had primary form of APhS, 7 cases had secondary form of APhS. For statisti

[Staphylococcus aureus bacteremia and endocarditis].

The prevalence of Stapylococcus bacteriaemia is increasing worldwide, because of the increasing use of invasive procedures leading to nosocomial infections, but also of a changing way of life (increasing fashion for tattoos or piercing, use of intravenous drugs). Infective endocarditis develops in 10-30% of the cases of staphylococcus bacteriaemia. Staphylococcus aureus endocarditis must be suspected when it develops in the year following heart surgery or implantation of permanent devices. In drug users, it usually involves the tricuspid valve. According to the resistance of the germ to metici

Streptococcus endocarditis after tongue piercing.

While body piercing has been practiced for many centuries, the popularity of this body art has increased vastly in recent years. It is estimated that up to 51% of college-age individuals in the United States have ear piercing or other forms of piercing or tattoo. Although perceived as a relatively safe practice, medical complications, mostly resulting from bleeding and infections, occur in 17% of all cases after piercing. The case is reported of a patient with native mitral valve endocarditis after tongue piercing, resulting in acute mitral valve insufficiency that required valve replacement.

Surgical management of infective endocarditis.

BACKGROUND AND AIM OF THE STUDY: Although retrospective reviews evaluating the surgical management of infective endocarditis (IE) have been conducted in Europe and in the USA, few data exist regarding management of the condition in Canada. The study aim was to evaluate the surgical management of individuals with culture-positive active IE at a Canadian tertiary care university hospital. METHODS: A retrospective analysis was performed of 74 patients (53 males, 21 females; mean age 56 +/- 14 years) with a preoperative diagnosis of acute IE between 1995 and 2003 at the Queen Elizabeth II Health

Changing epidemiology of infective endocarditis.

Infective endocarditis (IE) continues to be a serious infection with a stable incidence rate over time and a persistently high mortality. Population-based studies from France, Netherlands, Sweden, and the United States reported on the epidemiology of this infection. These studies suggest a changing distribution of underlying valvular heart disease in patients with IE, namely a decreasing proportion of patients with rheumatic heart disease and an increasing proportion with underlying prosthetic valves or mitral valve prolapse. There is controversy regarding the change of spectrum of IE-causativ

Brucella endocarditis in prosthetic valves.

Human brucellosis is a multiple organ disease that presents with fever and is most often transmitted via contaminated, unpasteurized goat milk and cheese. In chronic cases, focal complications (eg, spondylitis, neurobrucellosis and endocarditis) are frequently seen. Although the disease may be severely debilitating, the mortality rate is low. Fatal cases are often due to endocarditis. Because Brucella endocarditis is a rare complication (2% to 5%), therapeutic considerations are based on single-case experiences only. Therapy includes long-term antibiotic treatment using combinations of various

Medical dental prophylaxis of endocarditis.

Antibiotics have long been the main reason for the increase in man's longevity. Since their discovery, man has tried to reduce the level of infection by treating with antibiotics. At the same time, prophylactic use has been suggested, although this is controversial. Their routine use is not recommended, and empirical treatments at non-therapeutic doses, and indiscriminately, should be avoided, because they may become dangerous and harmful, causing among other things, the prevalence of resistant microorganisms and the eventual potentiation of an increase in morbid states. Infectious endocarditi

Fungal endocarditis: an autopsy study.

Between 1990 and 2002, 237 hearts were examined at autopsy, including 16 with infective endocarditis; 6 showed fungal endocarditis. The preceding pathology was chronic rheumatic heart disease in 2 patients, one of whom had undergone double valve replacement, 2 patients had been treated for acute lymphoblastic leukemia, and one had protein-energy malnutrition. The underlying cause was unknown in one case. The organisms isolated were Aspergillus in 3 patients, Zygomycota in 1, Candida in 1, and both Candida tropicalis and Aspergillus in 1 patient. New aspects of infective endocarditis.

The current incidence of infective endocarditis (IE) is estimated as 7 cases per 100,000 population per year and continues to increase. The prognosis is significantly influenced by proper diagnosis and adequate therapy. In cases with unconfirmed IE, transesophageal echocardiography is the imaging technique of choice. Culture-negative endocarditis requires either termination of antimicrobial treatment initiated without mircobiological test results and reevaluation of blood samples or serological/molecular biological techniques to identify the causative organism. Antimicrobial therapy should be
