Enthesitis as an initial presentation of vascular Behçet's syndrome: a case-based review.

Enthesitis is a characteristic manifestation of spondyloarthropathy (SpA). Historically, Behçet's syndrome (BS) was classified within SpA. Although they are now classified separately, the association between BS and SpA remains controversial. The concept of MHC-I (major histocompatibility complex class I)-opathy has been proposed based on the overlap in immunopathological mechanisms among diseases associated with human leukocyte antigen (HLA) class I. Enthesitis is a frequent complication in patients with BS who also have acne and arthritis. However, information regarding enthesitis in patients

[CME Sonography 93: Ultrasound of the Enthesis - Not Every "Enthesitis" Signals a Spondyloarthritis].

CME Sonography 93: Ultrasound of the Enthesis - Not Every "Enthesitis" Signals a Spondyloarthritis Abstract. In this CME we will focus on the ultrasound anatomy of the enthesis. In addition, we describe the terms enthesopathy and enthesitis and discuss related differential diagnostic considerations. It is important to know all the diseases that can manifest themselves at the enthesis, so that diagnoses can be made, and patients classified correctly. At this point it is worth mentioning that the findings of enthesitis do not necessarily need to be associated with spondylarthritis.

Can we resolve the conflation of activity-related enthesal bone changes with injury-derived enthesal reaction?

Postreactive morphological alterations of enthesal surfaces, often termed historically and collectively as "enthesitis," were thought to reflect typical daily activities. Actually, an important real cause of altered enthesal surfaces is augmentation of the osseous base, a physiological reaction. Further, altered enthesal attachments often reveal partial-to-complete avulsion and dystrophic calcification (ossification) secondary to injury, a pathological process. Descriptively, the suffixes "sis," "osis," and "asis" (as appropriate grammatically) signal a general "affected with" state. The more

Visualization of Enthesitis by Ultrasound: a Key Diagnostic Tool in Spondyloarthropathy Diagnosis and Management.

PURPOSE OF REVIEW: The purpose of this review is to report on the current status of evaluation of enthesitis with sonography. Sonography can visualize peripheral entheses in great detail since most of them are superficial. Visualization of entheses has provided insights into the pathophysiology of enthesitis and highlighted the interconnectivity of stress dissipation mechanisms about the enthesis. The primary use of entheseal sonography presently revolves around diagnostic use. RECENT FINDINGS: There is increasing awareness of biomechanical confounders that lead to entheseal changes, which ar

State-of-the-art ultrasonographic findings in lower extremity sports injuries.

Athletes sometimes experience overuse injuries. To diagnose these injuries, ultrasonography is often more useful than plain radiography, computed tomography (CT), or magnetic resonance imaging (MRI). Ultrasonography can show both bone and soft tissue from various angles as needed, providing great detail in many cases. In conditions such as osteochondrosis or enthesopathies such as Osgood-Schlatter disease, Sinding-Larsen-Johansson disease, bipartite patella, osteochondritis dissecans of the knee, painful accessory navicular, and jumper's knee, ultrasonography can reveal certain types of bony ir

Evaluation of semi-quantitative bone scintigraphy in canine elbows.

Scintigraphy is an extremely sensitive tool for the detection of early changes in bone metabolism. Sixty-eight lame dogs underwent a scintigraphic examination. For each elbow lateromedial (LM), caudomedial (CdM) flexed and caudomedial (CdM) extended scintigraphic views were obtained. Semi-quantitative analysis was performed to determine radiopharmaceutical uptake at the medial coronoid process (MCP) and at the attachment of the flexor muscles at the medial humeral epicondyle, normalised to activity registered in either the total elbow joint or the radius/ulna. MCP pathology/flexor enthesopathi

Magnetic Resonance Imaging (MRI) of the Knee as an Outcome Measure in Juvenile Idiopathic Arthritis: An OMERACT Reliability Study on MRI Scales.

OBJECTIVE: There is increasing evidence that early therapeutic intervention improves longterm joint outcome in juvenile idiopathic arthritis (JIA). Given the existence of highly effective treatments, there is an urgent need for reliable and accurate measures of disease activity and joint damage in JIA. Our objective was to assess the reliability of 2 magnetic resonance imaging (MRI) scoring methods: the Juvenile Arthritis MRI Scoring (JAMRIS) system and the International Prophylaxis Study Group (IPSG) consensus score, for evaluating disease status of the knee in patients with JIA. METHODS: Fo

Juvenile Spondyloarthropathies.

Juvenile spondyloarthropathies represent a clinical entity separate from the adult disease. Initial clinical signs of juvenile spondyloarthropathies often include lower extremity arthritis and enthesopathy, without axial involvement at the disease onset. Asymmetrical oligoarthritis of lower extremities is typically seen in this type of arthritis. Enthesopathy, which is the hallmark of the disease, is most commonly seen in the Achilles tendon, being manifested by heel pain. Anterior uveitis and HLA-B27 positivity are seen in a proportion of cases. Sacroiliitis is generally asymptomatic in the p
