Neurosurgical care of spinal epidural, subdural, and intramedullary abscesses and arachnoiditis.

Epidural abscess of the spinal column is a rare but potentially devastating disease. When recognized early and treated appropriately, the outcome can be excellent. Abscesses in the spinal subdural space or in the spinal cord proper are even more unusual but can also lead to complete and irreversible loss of neurologic function if not diagnosed and treated rapidly. Although infection frequently can result in arachnoiditis, there are other more common causes, such as surgery and chemical irritants. It remains a difficult condition to treat, and long-term therapeutic results are mediocre. Newer t

Spinal epidural abscess as a complication of duodenolumbar fistula. A case report.

The characteristic delay in diagnosis and treatment of spinal epidural abscess of unusual presentation in a patient with significant comorbidities often results in permanent neurologic deficits. Fortunately, in the case reported here, the patient made a complete recovery. Rapid diagnosis and intervention are essential, because degree of recovery is proportional to the extent of the neurologic deficit present before surgery.

[An epidural spinal abscess with caudal symptoms as a complication of Crohn's disease].

Progressive symptoms of caudal compression (flaccid paraparesis, sensory disorders), accompanied by severe pain and fever, developed over a few days in a 26-year-old man with Crohn's disease for 11 years. Spinal computed tomography, performed under the diagnosis of herniated disc, revealed intraspinal soft tissue, as well as gas in the spinal canal (L2-S3) and the paravertebral muscles. This led to the diagnosis of acute epidural abscess and a laminectomy was performed (at L4-S2). Intraspinally there was thickened, bluish fatty tissue; thick pus exuded between dura and the sacral roots. Suctio

De novo subgaleal abscess complicated by spontaneous osteomyelitis and epidural abscess: a case report and review of literature.

INTRODUCTION AND IMPORTANCE: A subgaleal abscess is a collection of pus in a potential space between the galea aponeurotica and pericranium. De novo subgaleal abscesses are a subset of subgaleal abscesses that develop in the absence of identifiable risk factors such as head trauma or procedures. However, these have rarely been reported in the literature. CASE PRESENTATION: We present the case of a 65-year-old woman who presented with a headache for two and a half months, followed by swelling of the right parieto-occipital scalp. She denied any history of trauma, procedures, or anticoagulant u

A case report of frontal spontaneous epidural hematoma associated with cranial osteomyelitis and epidural abscess due to paranasal sinusitis.

BACKGROUND: Intracranial epidural hematoma (EDH) is frequently secondary to trauma, but in some rare cases, spontaneous EDH (SEDH) could develop without trauma. Cranial osteomyelitis is an uncommon osseous infection that most frequently presents as a postoperative complication but also rarely originates from paranasal sinusitis and can develop extracranially to form a subperiosteal abscess or intracranially to form an epidural, subdural, or cerebral abscess. Intracranial epidural abscess (EDA) is an uncommon infection that forms in the space between the cranial bone and dura mater. It is rare

Holospinal epidural abscess: a review of clinical characteristics, outcomes and prognostic factors.

BACKGROUND: Holospinal epidural abscess (HEA) is a rare, serious infection associated with substantial morbidity. Evidence is limited to case reports and small series, with heterogeneous presentation, management and outcomes. Prognostic factors remain poorly defined. This study aimed to characterize the features of HEA and describe associations with outcomes. METHODS: A systematic review of MEDLINE, PubMed, Cochrane and Google Scholar identified cases of HEA involving three or more contiguous spinal regions. Eligible studies included cases and case series reporting outcomes. Because of hetero

Extensive spinal epidural abscess treated utilizing a single limited incision: illustrative case.

BACKGROUND: Extensive spinal epidural abscess is an uncommon and potentially life-threatening condition that mandates appropriate early diagnosis and treatment to avoid potentially disastrous complications. Limited literature is available guiding the management of such an entity. OBSERVATIONS: The authors report the case of a 59-year-old female patient with a history of 5 days of severe back pain, fever, and associated lower limb neurological deficit. MRI revealed an epidural abscess extending from C2 to the sacrum. One-level segmental decompression was performed at the level of L2-3 in combi

Spinal Epidural Abscess After Radiofrequency Denervation of the Lumbar Facets: Case Report.

BACKGROUND: Spinal epidural abscesses (SEAs) are rare with an estimated annual incidence of 0.2-2.8/10,000. Risk factors include immunosuppression, intravenous drug use, diabetes mellitus, HIV, trauma, tattooing, and bacteremia. Presenting symptoms are typically nonspecific with malaise, fever, back pain, and possible neurological deficits. CASE REPORT: We present a case of SEA following radiofrequency ablation (RFA) in a patient with no known risk factors, in a 59-year-old man with axial lower back pain. The patient underwent an L4/L5 laminectomy and was started on antibiotics. His blood cul

Beyond Enteric Infections: A Case of Salmonella Spinal Epidural Abscess-Case Report.

Spinal epidural abscess (SEA) is a serious but rare condition that is often challenging to diagnose early due to atypical presentations, especially in patients without common risk factors such as immunosuppression or sickle cell disease. This case report describes a 49-year-old woman with a history of alcohol use disorder who presented with flank pain and other nonspecific symptoms, initially misdiagnosed as musculoskeletal pain. MRI eventually revealed a SEA caused by nontyphoidal Salmonella (NTS), requiring surgical intervention and antibiotics. The discussion highlights the difficulty of di

[Intracranial epidural abscess associated to sinusitis with positive response to medical treatment].

Epidural abscesses are rare intracranial infections. They can occur from neurosurgical procedures, or head and neck infections. Successful treatment usually requires a combination of drainage procedure and antibiotic therapy. We present the case of a young patient with an intracranial epidural abscess and meningitis secondary to sinusitis who received only corticosteroids and antibiotic therapy, with good evolution.

Neisseria gonorrhoeae cervical spine epidural abscess requiring spinal decompression and instrumented fusion.

Disseminated gonococcal infection (DGI) is an uncommon complication of Neisseria gonorrhoeae infection, and typically presents with either a triad of tenosynovitis, dermatitis and polyarthralgia, or with extra-axial large joint septic arthritis. Spinal epidural abscess is a rare manifestation of DGI, with only a few previously reported cases, none of which required placement of metalware into the infected space. Here we report a severe case of isolated N. gonorrhoeae cervical spine epidural abscess necessitating surgical source control (C7/T1 laminectomy and debridement) and metalware placemen

Methicillin-Resistant Staphylococcus aureus Spinal Epidural Abscess: Local and Systemic Case Management.

Spinal epidural abscess (SEA) is a rare condition with complex pathophysiology and highly variable clinical presentation. While it is known to cause focal peripheral nerve symptoms such as muscle weakness, paresthesia, or pain, these are typically accompanied by complaints of back or spine pain and systemic symptoms indicative of infection. In our case, a 53-year-old male initially presented with unilateral pain and swelling in his right hand, with no fever at presentation and no complaints of back pain. Blood culture confirmed methicillin-resistant Staphylococcus aureus (MRSA)for which he was

Spinal osteomyelitis and epidural abscess caused by ureterovertebral fistula: A case report.

BACKGROUND: Ureteral fistulas are abnormal connections between the ureters and other organs. Maintaining a high index of suspicion is important because they can precipitate dangerous complications such as sepsis and renal failure. Connections to a vertebral body have only been documented in the setting of trauma. Here, we present a 67-year-old female with a ureterovertebral fistula extending into the L3 vertebral body. CASE DESCRIPTION: A 67-year-old female with a history of endometrial adenocarcinoma underwent surgery and radiation therapy complicated by a right ureteral obstruction requirin

A Predictive Model Facilitates Early Recognition of Spinal Epidural Abscess in Adults.

INTRODUCTION: Spinal epidural abscess (SEA), a highly morbid and potentially lethal deep tissue infection of the central nervous system has more than tripled in incidence over the past decade. Early recognition at the point of initial clinical presentation may prevent irreversible neurologic injury or other serious, adverse outcomes. To facilitate early recognition of SEA, we developed a predictive scoring model. METHODS: Using data from a 10-year, retrospective, case-control study of adults presenting for care at a tertiary-care, regional, academic medical center, we used the Integrated Disc
