We conducted a retrospective study to measure laryngeal descent in human infants and to determine if there is any correlation between the associated anatomic changes and the timing of the peak incidence of sudden infant death syndrome (SIDS), which is 2 to 4 months of age. We performed a computerized search of hospital records at our institution to identify magnetic resonance imaging (MRI) scans of the head and neck and plain radiographs of the lateral neck in patients younger than 1 year of age (range: 1 to 357 days). After unusable images were excluded, 79 head and neck MRI scans and 111 lat

OBJECTIVE: To determine the effect of bilateral hypoglossal and and glossopharyngeal nerve block on epiglottic and soft palate position and tracheal and pharyngeal pressures in exercising horses.
ANIMALS: 5 Standardbreds.
PROCEDURE: Tracheal and pharyngeal pressures were measured in 5 Standardbreds exercising at the speed at which the horses achieved 50, 75, and 100% of maximal heart rate after bilateral hypoglossal and glossopharyngeal nerve block and without nerve block. Nerve block was achieved by injection of 1 to 2 ml of 2% mepivicaine hydrochloride between the glossopharyngeal and hypo

BACKGROUND: In pediatric patients, obstruction of the upper airway is a common problem during general anesthesia. Chin lift is a commonly used technique to improve upper airway patency. However, little is known about the mechanism underlying this technique.
METHODS: The authors studied the effect of the chin lift maneuver on airway dimensions in 10 spontaneously breathing children (aged 2-11 yr) sedated with propofol during routine magnetic resonance imaging. The minimal anteroposterior and corresponding transverse diameters of the pharynx were determined at the levels of the soft palate, dor

INTRODUCTION: Kimura disease (KD) is a chronic lymphoproliferative disorder of unknown etiology that affects the skin and lymph nodes, mostly observed in males of Asian descent. The natural history of asymptomatic epiglottal KD remains unknown. This rare site of KD is often only diagnosed when tumor growth starts to obstruct the upper airways.
OBSERVATION: A 34-year-old North African male presented with fatigue and multiple, slowly progressive, fluctuating skin nodules in the right mandibular and retroauricular regions. Computed tomography of the head and neck revealed a large soft tissue tum

PURPOSE: To report a case of unusual difficult intubation secondary to oral submucosal fibrosis (SMF) associated with malignancy of the hypopharynx.
CLINICAL FEATURES: A 57-yr-old male, ASA-I physical status presented for percutaneous nephrolithotripsy. The patient was a known alcoholic and smoker with a habit of chewing quids for the past 36 years. Airway assessment revealed oral SMF and slight difficulty in opening the mouth fully. Mentohyoid and mentothyroid distances were normal. However, during intubation, unanticipated difficulty was encountered. On direct laryngoscopy, the tip of the e

Laryngomalacia is the most common cause of stridor in newborns and infants. Up until now, different surgical techniques for the treatment of this disease have been described. We report on a modified technique, the so called epiglottopexy, which is a laser-surgical treatment strategy in severe laryngomalacia. A total number of six children (ages: 6 weeks-10.4 years) were treated for life-threatening stridor, which was due to an isolated posterior displacement of the epiglottis during inspiration. In two patients, shortened aryepiglottic folds contributed to the stridor. In all of the children,

The hyoepiglottic ligament (HL) is a connective tissue structure that serves as the roof of both the paraglottic and the preepiglottic spaces and thereby anatomically separates the supraglottic larynx from the tongue base. Whole mount serially sectioned larynges with supraglottic cancer were reviewed to help clarify cephalad spread of cancer in this region. The whole mount slides were analyzed from 70 laryngectomy specimens that were resected for supraglottic cancer. The HL was breached by cancer in 13 specimens, and all of these displayed clinical and histopathologic invasion of the preepiglo

Cineradiographic study of the movement patterns of oropharyngeal and laryngeal structures during breathing and panting in dogs, correlated with recordings of expiratory and inspiratory airflow patterns (via thermocouples) at the nose and mouth show that the soft palate is the principal structural component regulating the path of respiratory in these animals. Cyclical movements of the soft palate during panting are accompanied by complementary movements of the posterior dorsum of the tongue (and epiglottis) to open and to close alternately the oropharynx and nasopharynx. The epiglottis appears

The motility of the pharynx, upper esophageal sphincter (UES), and proximal esophagus in patients with oropharyngeal dysphagia is still not entirely understood. High-resolution manometry (HRM) was recently added to the armamentarium for the study of this area. This study aims to describe HRM findings in patients with vagal paralysis. Sixteen patients (mean age 54 years, 69% females) with oropharyngeal dysphagia due to unilateral vagal paralysis were prospectively studied. All patients underwent HRM. Motility of the UES and at the topography of the velopharynx and epiglottis were recorded. (1)

We report a 68-year-old woman with tracheobronchitis and laryngitis associated with Crohn's disease (CD), which was discovered during the evaluation of suspected lung cancer. She had no symptoms induced by these upper airway diseases (UADs). Bronchoscopy revealed swelling of the epiglottis with edematous change and a mass like epiglottis fold. There were nodular and edematous changes in the trachea and bilateral main bronchus. Histological findings demonstrated infiltration by numerous lymphocytes and plasma cells. Dexamethasone as the premedication for chemotherapy against lung cancer was eff

Within the last several months we have seen two patients with pharyngolaryngeal webs. The webs extend from the lateral borders of the epiglottis to the lateral and posterior pharyngeal walls framing the glottis with a "keyhole" type of effect. No web of this kind has ever been documented in the literature to our knowledge. These children suffer from intermittent airway obstruction, probably secondary to tethering of the epiglottis. These webs are very difficult to identify without the panoramic view of the hypopharynx which is afforded by the fiber-illuminated endoscopic telescopes. We recomme

This paper describes the presence of lymphoid tissue in the mucosa of the bovine larynx. A total of 15 bovine larynges were examined both macroscopically after tissue fixation in acetic acid and microscopically using histology. It was found that no paraepiglottic tonsil was present in cattle, although a few lymphoid follicles were present in the mucosa at the base of the epiglottis. This result is in accordance with previous reports. In contrast, numerous lymphoid follicles were seen in the mucosa of the epiglottis and the corniculate processes of the arytenoid cartilages. This suggests that l

The term 'Globus Hystericus' is considered to be a very bad term for such a common condition and it is recommended that it be replaced by the 'Hypopharyngeal Syndrome'. The normal modified swallow initiated from the lingual tonsil--epiglottis area, called the Inferior Constrictor Swallow, can cause all the symptoms. The epiglottis does not bend. There are two groups of symptoms: the first due to sensation from local irritation of the lingual tonsil; and the second due to vicious circles causing increased muscle tension and strain swallowing. If there is pain or dysphagia or impairment in swall

OBJECTIVE: This study aims to explore the factors that contribute to epiglottic collapse (EC) in patients with obstructive sleep apnea (OSA).
METHODS: This study enrolled 35 patients (34 males; median age, 39 years; median apnea-hypopnea index (AHI), 55.4 events/h; median body mass index (BMI), 26.9 kg/m2). EC (epiglottis attaching onto the posterior pharyngeal wall) was diagnosed by drug-induced sleep computed tomography (DI-SCT). Three dimensions were assessed for comparison between the EC and non-EC (NEC) groups that included anatomical measurement: epiglottic length and angle, endoscopic

Difficult laryngoscopy in syndromic children is usually attributed to mandibular or cervical abnormalities; however, supraglottic malformations may also impair laryngeal exposure. We describe three syndromic pediatric cases in which difficult laryngoscopy was associated with abnormal epiglottic morphology. In the index neonate with suspected left-sided branchial arch hypoplasia, bronchoscopy demonstrated ipsilateral hypoplasia of the epiglottis and arytenoid region, with the epiglottis arising unusually deep from the tongue base. These findings suggest that supraglottic malformation should be

Pathologic characteristic of supraglottic carcinoma spreading anterosuperiorly in 100 cases was investigated by whole-organ sections of the larynx. The results showed that 75% of supraglottic carcinomas were found to spread anterosuperiorly. In these cases, carcinoma in the epiglottis-ventricular fold accounted for 90%; epiglottic carcinoma 81.8%; and carcinoma in the ventricular fold 33.3%. Carcinoma in the aryepiglottic fold and arytenoid region was not noted to spread anterosuperiorly. In the cases with anterosuperior spreading of supraglottic carcinoma, the pre-epiglottic space was most co

The predominantly herbivorous diet of the howler and baboon is responsible for modification in the configuration of the hyolaryngeal apparatus. In a herbivorous primate the air sac located in the concavity of the body of the hyoid bone takes on a resonating function. There is definite correlation between : (1) the presence of a large epiglottis with a convex superior surface, (2) close apposition of the soft palate with the epiglottis, (3) presence of high aryepiglottic and lateral epiglottic folds, (4) presence of air sacs and (5) concavity of the posterior surface of the body of the hyoid bo

BACKGROUND: This study constructed three-dimensional finite element (3D-FE) models of the upper airway and surrounding structures from spiral computed tomography (CT) data. An optimized fluid-structure interaction (FSI) analysis was applied to investigate the biomechanical effects of the mandibular advancement device (MAD) on peri-airway tissues, refining its therapeutic mechanism in obstructive sleep apnea (OSA).
METHODS: One adult patient diagnosed with moderate OSA underwent CT scans in natural sleep before and after 2 months of MAD therapy. 3D-FE models of the upper airway and surrounding

Aim Snoring, which falls within the spectrum of sleep-related breathing disorders, is considered to be one of the common symptoms of airway obstruction. Lateral cephalometric analysis is an effective way of diagnosing airway obstruction by evaluating skeletal and soft tissue abnormalities in patients with sleep-disordered breathing (SDB). The present study was planned to analyze the pharyngeal space among snorers and nonsnorers in the retroglossal region and in the region of hypopharynx at the level of epiglottis. Materials and Methods The present cross-sectional study included a total of 60

From 1987 to 1989, 14 adults with acute epiglottitis were seen and treated at the ENT clinic of Geneva. All patients presented with a severe sore throat as primary symptom. An indirect laryngoscopy, which bears no risk in adults, was performed and revealed a swollen, cherry-red epiglottis. The course of the disease is unpredictable, and rapid development of airway obstruction may occur. In most cases, these patients can be treated medically with antibiotics and corticoids, and there is no need for systematic orotracheal intubation. However, patients presenting with respiratory distress syndrom
