Bidirectional Control of Generalized Epilepsy Networks via Rapid Real-Time Switching of Firing Mode.

Thalamic relay neurons have well-characterized dual firing modes: bursting and tonic spiking. Studies in brain slices have led to a model in which rhythmic synchronized spiking (phasic firing) in a population of relay neurons leads to hyper-synchronous oscillatory cortico-thalamo-cortical rhythms that result in absence seizures. This model suggests that blocking thalamocortical phasic firing would treat absence seizures. However, recent in vivo studies in anesthetized animals have questioned this simple model. Here we resolve this issue by developing a real-time, mode-switching approach to dri

TMS combined with EEG in genetic generalized epilepsy: A phase II diagnostic accuracy study.

OBJECTIVES: (A) To develop a TMS-EEG stimulation and data analysis protocol in genetic generalized epilepsy (GGE). (B) To investigate the diagnostic accuracy of TMS-EEG in GGE. METHODS: Pilot experiments resulted in the development and optimization of a paired-pulse TMS-EEG protocol at rest, during hyperventilation (HV), and post-HV combined with multi-level data analysis. This protocol was applied in 11 controls (C) and 25 GGE patients (P), further dichotomized into responders to antiepileptic drugs (R, n=13) and non-responders (n-R, n=12).Features (n=57) extracted from TMS-EEG responses aft

Idiopathic generalized epilepsy and hypokalemic periodic paralysis in a family of South Indian descent.

Inherited channelopathies are a heterogeneous group of disorders resulting from dysfunction of ion channels in cellular membranes. They may manifest as diseases affecting skeletal muscle contraction, the conduction system of the heart, nervous system function, and vision syndromes. We describe a family of South Indian descent with hypokalemic periodic paralysis in which four members also have idiopathic generalized epilepsy. Hypokalemic periodic paralysis is a genetically heterogeneous channelopathy that has been linked to mutations in genes encoding three ion channels CACNIAS, SCN4A, and KCNJ

Diffusional kurtosis imaging reveals a distinctive pattern of microstructural alternations in idiopathic generalized epilepsy.

OBJECTIVES: Idiopathic generalized epilepsy (IGE) arises from paroxysmal dysfunctions of the thalamo-cortical network. One of the hallmarks of IGE is the absence of visible abnormalities on routine magnetic resonance imaging (MRI). However, recent quantitative MRI studies showed cortical-subcortical structural abnormalities in IGE, but the extent of abnormalities has been inconsistent in the literature. The inconsistencies may be associated with complex microstructural abnormalities in IGE that are not completely detectable using conventional diffusion tensor imaging methods. The goal of this

Polyspike and waves do not predict generalized tonic-clonic seizures in childhood absence epilepsy.

About 40% of children with childhood absence epilepsy develop generalized tonic-clonic seizures. It is commonly held that polyspike-wave pattern on the electroencephalogram (EEG) can predict this development of generalized tonic-clonic seizures. However, there is no firm evidence in support of this proposition. To test this assumption, we used survival analysis and compared the incidence of generalized tonic-clonic seizures in 115 patients with childhood absence epilepsy having either isolated 3-Hz spike-wave or coexisting 3 Hz and polyspike-waves and other variables. There was no evidence tha

[A case of inv dup (15) mosaic with mental retardation and symptomatic generalized epilepsy].

We report a case of inv dup (15) mosaic with moderate mental retardation and symptomatic generalized epilepsy whose attacks were successfully controlled by phenytoin. Battaglia et al. (1997) reported four cases of inv dup (15) syndrome, whose symptoms contained intractable Lennox-Gastaut syndrome, moderate or severe mental retardation, diffuse hypotonia, and minor anomaly. The present case was diagnosed as mosaicism of inv dup (15) syndrome. It was suspected that the patient had a treatable epileptic seizure and no minor anomaly because of the mosaicism.

Semiology of functional/dissociative seizures in idiopathic generalized epilepsy: An inpatient video-electroencephalographic study.

OBJECTIVE: This study was undertaken to estimate the prevalence and characterize the semiology of co-occurring functional/dissociative seizures (FDSs) and identify clinical features potentially associated with the development of video-electroencephalography (video-EEG)-diagnosed FDSs in patients with idiopathic generalized epilepsy (IGE). METHODS: In this retrospective single-center inpatient video-EEG study, we characterized clinical features of FDSs recorded in patients with IGE aged ≥16 years admitted into our epilepsy monitoring unit between January 2008 and December 2024. We compared cli

Correlation of polygenic risk score and clinical phenotype in patients with genetic generalized epilepsy.

OBJECTIVE: The polygenic risk score (PRS) for individuals with genetic generalized epilepsy (GGE) quantifies the common risk variants in genes identified in genome-wide association studies. We hypothesized that the phenotype of GGE patients differs based on their GGE PRS. METHODS: We identified participants with highest (n = 59) versus lowest (n = 48) PRS from the GGE patients (n = 2256) recruited through the Epi25 Collaborative for comparison. Detailed clinical data were acquired retrospectively for the 59 high PRS and 48 low PRS individuals with GGE from the Epi25 database and from the cont

Disrupted periodic spatiotemporal pattern and dynamic reorganization of its basic states in generalized epilepsy.

Objective. The anti-correlation between the default mode network (DMN) and task-positive network (TPN) is a stable characteristic of normal brain activity. However, in idiopathic generalized epilepsy (IGE), this anti-correlation is often disrupted and strongly associated with epileptic seizures. This study aims to use periodic spatiotemporal patterns (PSTP) analysis to elucidate the relationship between the DMN-TPN anti-correlation and epileptic activity, providing new insights into the neural mechanisms underlying IGE.Approach.Resting-state functional magnetic resonance imaging was used to ana

Trends in antiseizure medication prescription in Idiopathic generalized epilepsy over the last 10 years.

BACKGROUND: Idiopathic Generalized Epilepsies (IGE) are a subset of syndromes defined by the International League against Epilepsy (ILAE) with the particularity to respond to a narrow number of ASMs and particularly to valproic acid (VPA). Recommendations have changed in the last decade. We aimed to describe changes in antiseizure medication (ASM) in adult IGE over the last 10 years. METHODS: Cross-sectional study comparing two cohorts of patients ≥ 16 years receiving ASM for IGE in a tertiary center (year 2013 and 2023). We collected clinical-demographic variables and ASM, analysing diagnosi

Sleep quality and architecture in Idiopathic generalized epilepsy: A systematic review and meta-analysis.

Idiopathic generalized epilepsies are a group of sleep related epilepsy syndromes with sleep deprivation as a strong trigger for seizures and increased spike-wave activity during sleep and transition to sleep. Neuropsychological deficits are common in Idiopathic generalized epilepsy patients. Learning and memory processes are closely linked to sleep. Therefore, this systematic review and meta-analysis investigates the evidence of sleep disturbances in Idiopathic generalized epilepsy patients. A search of the databases EMBASE, Medline and Scopus identified 22 studies comparing polysomnographic

Heart rate variability in patients with refractory epilepsy: The influence of generalized convulsive seizures.

OBJECTIVE: Patients with epilepsy, mainly drug-resistant, have reduced heart rate variability (HRV), linked to an increased risk of sudden death in various other diseases. In this context, it could play a role in SUDEP. Generalized convulsive seizures (GCS) are one of the most consensual risk factors for SUDEP. Our objective was to assess the influence of GCS in HRV parameters in patients with drug-resistant epilepsy. METHODS: We prospectively evaluated 121 patients with refractory epilepsy admitted to our Epilepsy Monitoring Unit. All patients underwent a 48-hour Holter recording. Only patie

Efficacy of Lacosamide in a Patient with Refractory Generalized Epilepsy Based on Video Electroencephalography.

A 20-year-old man with drug-resistant generalized epilepsy (GE) was admitted for video electroencephalography (vEEG) monitoring under treatment with multiple antiepileptic drugs, including levetiracetam (3,000 mg/day), valproic acid (800 mg/day), and lacosamide (LCM) (100 mg/day). No seizures were noted after the withdrawal of levetiracetam. However, after the withdrawal of LCM, atypical absence seizures with a 2- to 2.5-Hz generalized spike and wave complex frequently appeared, followed by subsequent generalized-onset tonic-clonic seizures. After re-administration of LCM, the seizures and epi

Differences in visual information processing style between Idiopathic Generalized Epilepsy with and without photosensitivity.

PURPOSE: Recently, altered visual cortical processes i.e., lack of habituation to visual evoked potentials (VEP), has been highlighted in both photosensitive epilepsy and in a specific i.e., analytic mode of processing visual inputs. In this study we aimed at evaluating the relationship between photosensitivity (PS) and analytic style of processing visual information, in a sample of 30 patients with Idiopathic Generalized Epilepsy (IGE) and matched healthy controls. METHODS: At our Epilepsy unit of the Sapienza University of Rome, we consecutively enrolled 15 patients with IGE with PSand matc

Increased Functional MEG Connectivity as a Hallmark of MRI-Negative Focal and Generalized Epilepsy.

Epilepsy is one of the most prevalent neurological diseases with a high morbidity. Accumulating evidence has shown that epilepsy is an archetypical neural network disorder. Here we developed a non-invasive cortical functional connectivity analysis based on magnetoencephalography (MEG) to assess commonalities and differences in the network phenotype in different epilepsy syndromes (non-lesional/cryptogenic focal and idiopathic/genetic generalized epilepsy). Thirty-seven epilepsy patients with normal structural brain anatomy underwent a 30-min resting state MEG measurement with eyes closed. We o
