Cardiovascular disease in patients with dysvascular amputation.

Cardiovascular diseases are the most common causes of morbidity and mortality in individuals with peripheral vascular disease (PVD). Among patients who have undergone lower extremity amputation as a result of PVD, the prevalence of concomitant cardiovascular disease may be as high as 75%. Comorbid heart disease may complicate the postamputation course of recovery, delay initiation of rehabilitation training, and inhibit the achievement of maximal functional independence. A variety of methods have been used to assess cardiac status and risk in amputation patients undergoing physical training; t

The importance of the subfascial lymphatics in the diagnosis of lower limb edema: investigations with semiquantitative lymphoscintigraphy.

A new method of semiquantitative lymphoscintigraphy for the evaluation of lower limb edema is characterized by (1) the evaluation of both the epifascial and subfascial system in order to assess type and stage of the edema, (2) the use of high-resolution digital whole-body imaging to facilitate the calculation of functional parameters, and (3) the use of active, standardized ergometry for reproducibility. The appearance time of 99mTc-labeled human albumin nanocolloid in inguinal lymph nodes after injection and the percent uptake of colloid into lymph nodes at 40 and 120 min after injection serv

[In-stent-stenosis of a bare-metal stent 11 years after its implantation].

HISTORY AND ADMISSION FINDINGS: A 52-year-old man was admitted to our hospital with unstable angina pectoris. A balloon angioplasty of his left anterior descending artery had been performed 12 years ago and two bare-metal stents had been implanted two months later for an early restenosis. DIAGNOSTIC PROCEDURES: Bicycle ergometry was stopped because the patient experienced angina pectoris and the electrocardiogram showed ST-segment depression in leads V4-V6. TREATMENT AND OUTCOME: A coronary angiography was performed and a single-vessel disease with a significant in-stent stenosis of the prox

[Pulmonary arteriovenous fistula in an adolescent].

We report the case of a patient, a 19-year-old young man, with a rare malformation of the pulmonary blood vessels--a complex arteriovenous (A-V) fistula. The disease was characterized by typical signs of the right-to-left shunt: cyanosis, clubbed fingers and exertional dyspnea. Hypoxemia and polyglobulia were present in the blood examination findings and functional tests showed significantly reduced diffusion lung capacity and progressive dyspnea, while ergometry revealed cyanosis. The physical examination, ECG and ultrasound of the heart were normal as well as aortography. The final diagnosis

[Physical work capacity of ischemic heart disease patients with various types of hyperlipoproteinemias].

The results of ergometric examination of 316 patients with chronic coronary insufficiency and various types of hyperlipoproteinemia have shown a clear diminution of physical working capacity in patients with hyperlipidemia as compared to patients with hyperlipidemia as compared to patients in whom the lipid content is within normal values. Working capacity was most diminished in IIa and particularly IIb types of hyperlipoproteinemia; in type IV physical working capacity was not reduced. The effect of hyperlipoproteinemia is most significant in the mildest degree of coronary insufficiency, but

[Physical performance in patients with focal infection ergospirometric stress-testing before and after tonsillectomy (author's transl)].

The diagnosis of a focal infection in 13 patients (6 men and 7 women, aged 18 to 36) was based on the findings of chronic tonsillitis and general complaints interpreted as focal symptoms. The patients underwent symptom-limited maximal stress-testing before and after tonsillectomy to rule out any impairment of physical performance on the basis of computer-assisted processing of ergospirometric data. It can be concluded that the actual data of maximal oxygen uptake (aerobic power), the phenomenon of adaptation to increasing work loads (derived from rating of the increase in oxygen uptake during

Inpatient rehabilitation for adult patients with Marfan syndrome: an observational pilot study.

BACKGROUND: Advances in medical, interventional and surgical treatment have increased average life expectancy of patients with congenital heart defects. As a result a new group of adult patients with congenital cardiac defects requires medical rehabilitation. Patients with Marfan syndrome (MFS) are a relevant group among these patients. So far, no reports on the effectiveness of specialized rehabilitation programmes for MFS patients exist. We implemented an inpatient 3-week rehabilitation program for MFS patients at the Muehlenberg-Clinic for rehabilitation and assessed the medical safety as w

Receptor binding of propranolol is the missing link between plasma concentration kinetics and the effect-time course in man.

In a double-blind, placebo-controlled study in 6 healthy volunteers, the correlation between beta-adrenoceptor binding, the time course of the effect and plasma concentration kinetics was investigated from 0 to 48 h after a single oral dose of propranolol 240 mg. First, the in vitro beta-adrenoceptor interaction of propranolol was investigated. Propranolol inhibited beta-adrenoceptor binding to rat parotid (beta 1) and reticulocyte (beta 2) membranes in the presence of pooled human plasma with a Ki of about 8 ng/ml plasma. After oral administration of 240 mg propranolol, concentration kinetics

Cerebellar neuromodulation synchronized to ankle-assist cycling yields prolonged post-assist dorsiflexion.

Rhythmic lower-limb movements require precise coordination across multiple joints, a process in which the cerebellum is thought to play a critical role. This study investigated whether cerebellar neuromodulation synchronized with mechanical ankle assistance modulates use-dependent motor adaptation during cyclic movement in healthy young adults. A portable ankle-assist ergometer (AAE) delivered phase-specific dorsiflexion torque during cycling, and participants completed three within-subject conditions: Sham stimulation, synchronized transcranial alternating current stimulation (tACS), and unsy

Force-velocity and power-velocity relationships in on-water rowing.

Force-velocity (FV) relationships are used to evaluate athletes and individualise training in several tasks. In rowing, only one previous study has determined FV relationships using ergometers. This study aimed to develop a method to obtain FV relationships during on-water rowing, to assess their quality and inter-day reliability and to compare them with ergometer profiles. Fourteen participants performed 8 all-out starts in instrumented double scull boats with 3 scull lengths under two conditions: two active rowers or one active and one passive rower. The FV relationships, reconstructed with

Evaluation of the strength of elbow flexors in patients with neuromuscular diseases.

In planning the optimum treatment for patients with neuromuscular diseases (NMD), it is essential to know as much as possible about their functional state. Assessment of the strength of certain muscles is the most direct measure of motor deficiency. In the development of normative data needed for patients with NMD, the use of torque measurements is required. Forty-nine patients (31 men and 18 women),f rom 18 to 54 years (mean age 33 +/- 8.9 years), were included in the study. Five groups of patients, each having one of five different NMDs, were formed. We tested unilaterally the biceps brachii

Effects of heart-rate feedback on estimated cardiovascular fitness in patients with panic disorder.

Psychological parameters that are believed to affect estimations of cardiovascular fitness were examined in patients with panic disorder and nonclinical controls. Fifty-four participants [panic disorder patients (n = 27) and age- and sex-matched nonclinical controls (n = 27)] completed a cycle ergometer test and were compared on the basis of estimated VO2 max. Participants were randomly assigned to experimental conditions in which they received heart-rate feedback or no feedback during the test. Patients with panic disorder exhibited lower VO2 max and decreased exercise tolerance (i.e., were m

[Ergospirometry in the diagnosis of disorders of the oxygen transport system in patients with chronic bronchitis].

Ergospirometry was used to carry out a study with the increasing exercise in 56 patients with chronic bronchitis and 46 healthy subjects. The limit physical potentialities, qualitative and quantitative characteristics of ventilation, hemodynamics, gas exchange at the height of the exercise were compared. It has been established that in the early disease stages, there was a tendency to hyperventilation during exercise in response to the deterioration of bronchial patency, which was accompanied by significant deterioration of the efficacy of ventilation with respect to the level of gas exchange

Early cardiac rehabilitation in pediatric fenestrated Fontan completion: A case report despite postoperative complication.

Early rehabilitation after Fontan surgery in children is poorly described, especially in complicated postoperative courses. We report the clinical evolution and rehabilitation of an 8-year-old girl with double-inlet left ventricle and patent ductus arteriosus who underwent fenestrated Fontan completion. The postoperative period was complicated by bilateral pleural effusions, pulmonary hypertension, hematemesis, psychomotor alteration, and mechanical ventilation with inhaled nitric oxide. A structured bedside rehabilitation program was started early and adjusted according to clinical stability.

Effects of passive pedaling exercise on the intracortical inhibition in subjects with spinal cord injury.

Cortical reorganization can be induced by exercise below the level of the lesion after spinal cord injury (SCI). The aim of the present study was to investigate the effect of passive and active pedaling exercise on leg motor cortical area excitability of subjects with traumatic SCI. Ten subjects with chronic cervical or thoracic SCI were enrolled in the study. We found a significant effect of pedaling on short-interval intracortical inhibition (SICI), which did not interact with the experimental condition (active vs. passive). This corresponded to a significant reduction of SICI in the subject

Thermoregulation during intermittent exercise in athletes with a spinal-cord injury.

PURPOSE: Individuals with a spinal-cord injury have impaired thermoregulatory control due to a loss of sudomotor and vasomotor effectors below the lesion level. Thus, individuals with high-level lesions (tetraplegia) possess greater thermoregulatory impairment than individuals with lower-level lesions (paraplegia). Previous research has not reflected the intermittent nature and modality of wheelchair court sports or replicated typical environmental temperatures. Hence, the purpose of this study was to investigate the thermoregulatory responses of athletes with tetraplegia and paraplegia during
