Customized anesthetic preservation of ictal threshold in electroconvulsive therapy: role of adjunctive remifentanil with etomidate.

We present a case of anesthesia for electroconvulsive (ECT) therapy that was complicated by emetic sensitivity to etomidate, fragile ictal threshold, and mild pseudocholinesterase deficiency. The anesthetic was designed in this patient taking all his issues in consideration. The mild pseudocholinesterase deficiency necessitated a (50-75%) reduction in succinylcholine dosage, careful monitoring of the train of four, and postictal amnestic coverage to prevent paralysis upon waking. The significant emetic response to etomidate prompted substitution to propofol and preemptive ondansetron. Propofol

Propofol - not thiopental or etomidate - with remifentanil provides adequate intubating conditions in the absence of neuromuscular blockade.

PURPOSE: Administration of remifentanil followed by propofol provides adequate conditions for tracheal intubation without muscle relaxants. Other hypnotic drugs have not been thoroughly investigated in this regard. Intubating conditions with remifentanil followed by propofol, thiopentone or etomidate are compared in this study. METHODS: In a randomized, double-blind study 45 healthy males were assigned to one of three groups (n = 15). After iv atropine, remifentanil 3 microg x kg(-1) were injected over 90 sec followed by propofol 2 mg x kg(-1) (Group I), thiopentone 6 mg x kg(-1) (Group II) o

Influence of etomidate, a new short acting anesthetic agent, on cerebral blood flow in man.

Cerebral blood flow was measured by means of a ten channel cerebrograph. This investigation was done after a corotid angiography; 2 to 3 miCi of Xenon133 was injected in the internal carotid artery. Beside CBF we measured arterial bloodpressures, heart rate, stroke volume, cardiac output, central venous pressures, blood gases and glucose in arterial blood, blood gases and glucose in the bulbus of the internal jugular vein and blood gases in mixed venous blood. In most regions CBF decreased significantly after Etomidate. The other parameters did not change. Only the heartrate slowed significant

Effect of oliceridine pretreatment on etomidate-induced myoclonus: a prospective, randomized, double-blind, controlled study.

PURPOSE: Etomidate often induces adverse effects such as myoclonus during anesthesia induction, thereby increasing perioperative risks for patients to some extent. Oliceridine is a novel opioid with fewer opioid-related adverse reactions. This study aims to investigate the effect of oliceridine pretreatment on etomidate-induced myoclonus. PATIENTS AND METHODS: This study is a prospective, randomized, double-blind, controlled study. Patients scheduled for elective surgery under general anesthesia were selected and randomly divided into the oliceridine group (Group O) and the normal saline grou

Etomidate protects retinal ganglion cells from hydrogen peroxide-induced injury via Nrf2/HO-1 pathway.

AIM: To determine whether etomidate (ET) has a protective effect on retinal ganglion cells (RGCs) injured with hydrogen peroxide (H2O2) and to explore the potential mechanism underlying the antioxidative stress effect of ET. METHODS: Cultured RGCs were identified by double immunofluorescent labeling of microtubule-associated protein 2 and Thy1.1. An injury model of H2O2-induced RGCs oxidative stress was established in vitro. Cells were pretreated with different concentrations of ET (1, 5, and 10 µmol/L) for 4h, followed by further exposure to H2O2 at 1000 µmol/L. Cell counting kit 8 and Annex

Etomidate versus propofol for electroconvulsive therapy: An intraindividual comparative study during the COVID-19 pandemic.

OBJECTIVES: The Coronavirus disease 2019 (COVID-19) pandemic markedly influenced mental health care practices, notably regarding electroconvulsive therapy (ECT). This was due to the redistribution of anesthetic agents used during ECT such as propofol, for intensive care units. Because in our center propofol was switched to etomidate to avoid ECT activity discontinuation, we undertook this study in order to compare Propofol and Etomidate regarding electroencephalographic seizure duration and stimulus intensity. METHODS: We performed a retrospective and comparative study, each patient being its

Comparison of priming versus slow injection for reducing etomidate-induced myoclonus: a randomized controlled study.

BACKGROUND: Etomidate injection is often associated with myoclonus. Etomidate injection technique influences the incidence of myoclonus. This study was designed to clarify which of the two injection techniques-slow injection or priming with etomidate-is more effective in reducing myoclonus. METHODS: This prospective randomized controlled study was conducted on 189 surgical patients allocated to three study groups. Control group (Group C, n = 63) received 0.3 mg/kg etomidate (induction dose) over 20 s. Priming group (Group P, n = 63) received pretreatment with 0.03 mg/kg etomidate, followed af

Etomidate in severe hypercortisolism: early cortisol kinetics and clinical outcomes in a real-world cohort.

PURPOSE: Severe hypercortisolism is a life-threatening endocrine emergency requiring rapid cortisol reduction. Oral steroidogenesis inhibitors may be limited in critically ill patients due to delayed onset, impaired absorption, and toxicity. Intravenous etomidate enables rapid, titratable suppression of cortisol synthesis, but data on early treatment response and real-world safety remain limited. METHODS: We conducted a retrospective study of consecutive patients with severe endogenous hypercortisolism treated with intravenous etomidate at a tertiary care centre over a 3-year period.
