Liminality: a major category of the experience of cancer illness.

Narrative analysis is well established as a means of examining the subjective experience of those who suffer chronic illness and cancer. In a study of perceptions of the outcomes of treatment of cancer of the colon, we have been struck by the consistency with which patients record three particular observations of their subjective experience: (1) the immediate impact of the cancer diagnosis and a persisting identification as a cancer patient, regardless of the time since treatment and of the presence or absence of persistent or recurrent disease; (2) a state of variable alienation from social f

A Review of Palliative Sedation.

Palliative sedation has become a standard practice to treat refractory symptoms at end-of-life. Dyspnea and delirium are the two most commonly treated symptoms. The medications used in palliative sedation are usually benzodiazepines, barbiturates, antipsychotics, and/or anesthetics. Some ethical considerations remain, especially surrounding the use of palliative sedation in psychological distress and existential suffering.

The midlife transition: crisis or chimera.

The construct of the midlife crisis is examined on definitional, developmental, psychoanalytic, and existential dimensions, and the literature reviewed. The DSM-III-R criteria for Adjustment Disorder appear to subsume the midlife crisis paradigm, and only a small percentage of people high in neuroticism are prone to developmental crises. It is argued that the crisis model is a contemporary social construct that has outlived its usefulness.

What have you learned from your homeless experience? A phenomenological approach for counseling unsheltered homeless men.

Field notes were obtained from 145 unsheltered homeless men regarding what they had learned from an experience of homelessness. Responses represented an aspect of each person's internal frame of reference. Statements about trust in self and an efficacy to survive were common themes. Findings were placed in a humanistic-existential model useful for counseling.

Navigating a fragile capability: meanings of encountering patients in a suicidal process from the perspective of ambulance clinicians.

PURPOSE: To illuminate the meanings of encountering patients in a suicidal process from the perspective of ambulance clinicians. METHODS: Narrative interviews were conducted with eighteen ambulance clinicians in Sweden. Data were analyzed using a phenomenological hermeneutical approach inspired by the philosophy of Ricœur. RESULTS: The main theme, "Navigating a fragile capability," reveals a profound tension within professional identity. Traditionally anchored in technical agency, this identity is challenged by a movement toward vulnerability and shared humanity. Capability in these encounte

Integrating Holistic Communication into Psychedelic-Assisted Therapies in Hospice and Palliative Care: An Approach Based on Peplau's Theory.

Psychedelic-assisted therapy (PAT) has shown promising results in alleviating psychological and existential suffering among individuals with serious illnesses. In parallel, nursing offers a robust theoretical framework to guide therapeutic communication in this context. This article explores the application of Peplau's Theory of Interpersonal Relations (PTIR) as a foundation for holistic communication in PAT, particularly in hospice and palliative care. We examine how PTIR's core concepts (person, health, environment, and nursing) along with its articulation of therapeutic roles, phases of the

Shared Death Anxiety: Obsessive-Compulsive Disorder and Advanced Paternal Age.

Obsessive-compulsive disorder (OCD) often emerges during childhood, presenting as distressing obsessions and compulsions that interfere with daily functioning. Advanced paternal age (APA) introduces unique psychosocial dynamics within families, potentially exacerbating stress, anxiety, and intergenerational emotional challenges. This case report examines the presentation of OCD in a 10-year-old boy, characterized by pervasive death anxiety shared with his 83-year-old father, whose advanced age intensifies existential concerns. The report delves into the complex interplay between APA, the child

Communication at the End of Life.

End-of-life patients experience physical, mental, social, and existential distress. While medical personnel provide medication and care to alleviate patients' distress, listening to and interacting with patients remains essential for understanding their psychological condition. The most important tool, though difficult to implement in practice, is end-of-life discussion (EOLD). EOLD has been shown to have positive effects on end-of-life treatment choices, achievement of patients' life goals, improvements in the quality of life of patients and their families, and the prevention of depression an

Learning to live with Parkinson's disease in the family unit: an interpretative phenomenological analysis of well-being.

We investigated family members' lived experience of Parkinson's disease (PD) aiming to investigate opportunities for well-being. A lifeworld-led approach to healthcare was adopted. Interpretative phenomenological analysis was used to explore in-depth interviews with people living with PD and their partners. The analysis generated four themes: It's more than just an illness revealed the existential challenge of diagnosis; Like a bird with a broken wing emphasizing the need to adapt to increasing immobility through embodied agency; Being together with PD exploring the kinship within couples and

'Stay Curious': guest experiences of philosophical counselling in palliative care - a qualitative study.

BACKGROUND: Philosophical counselling creates a dialogical space for exploring existential questions arising from serious illness, confrontation with mortality, bereavement, and caregiving. Although philosophy has long engaged with mortality and existential concerns, philosophy in form of philosophical counselling is not established in palliative care and has seldom been empirically studied. This study examines the views and experiences of individuals who engaged in philosophical counselling in palliative care-related contexts. METHODS: Nine counselling processes were completed; post-counsell

Psychedelic-Assisted Interventions in Palliative Care: A Narrative Overview and Critical Evaluation.

Background/Objectives: Patients in palliative care frequently experience multidimensional suffering that extends beyond physical symptoms to include existential distress, demoralization, and loss of meaning. Psychedelic-assisted therapies (PAT), including ketamine-assisted psychotherapy (KAP), have re-emerged as promising interventions for these domains. This study aimed to provide a narrative overview and critical evaluation of the existing secondary literature on PAT in palliative care and serious illness and to examine the extent to which emerging best-practice recommendations are reflected

Presence in Absence: A Daoist Critique of Interventionist Care and the Reconstruction of Nursing Presence.

Presence is widely affirmed as a core moral value of nursing, yet dominant conceptualizations in both nursing philosophy and clinical practice consistently collapse it into observable intervention and active engagement, underpinned by an implicit subject-object dualist ontology. Derivative concepts such as silent presence and holding space have sought to recover non-interventional modes of care, yet most remain trapped within an action-oriented logic and stop short of fundamental ontological reorientation. Drawing on classical Daoist concepts of wu-wei (non-coercive action) and ziran (naturaln

Major stressors facing patients with amyotrophic lateral sclerosis (ALS): a survey to identify their concerns and to compare with those of their caregivers.

OBJECTIVE: To identify and compare the primary existential, physical, and psychosocial stressors affecting patients with ALS and their caregivers. Health care providers, together with patients and their caregivers, are challenged to identify both physical and psychosocial concerns that have the greatest impact on quality of life over the course of a serious illness. It is also helpful to understand the priorities of these concerns from the patients' and caregivers' perspectives so that we can render optimal care and help patients and their families with the myriad problems that accompany a pro

[Mixed connective tissue disease and its management].

Mixed connective tissue disease (MCTD) is a rare autoimmune condition. Since its first description 50 years ago, its mere existence has been debated, given that it shares features of other autoimmune diseases, such as systemic lupus erythematosus (SLE), systemic sclerosis, inflammatory myopathy, rheumatoid arthritis and Sjogren's syndrome. Also, while antibodies to U1-RNP are essential for the diagnosis of MCTD, these antibodies may be expressed in other circumstances, such as in case of SLE. Nevertheless, the patient fulfilling criteria for MCTD needs specific management. In this review, we d

Sedation for comfort at end of life.

Treatment for patients who are dying from cancer and are suffering with physiologic and existential symptoms is an important and valuable skill for health care providers. However, the treatment for suffering at the end of life and the use of sedation for comfort often are misunderstood. The following is a discussion of the clinical skills and ethical considerations that health care providers should have when treating terminal patients with cancer.
