Dynamic magnetic resonance imaging of femoral head perfusion in femoral neck fracture.

A prospective followup study of 36 femoral neck fractures according to the assessment of femoral head perfusion using dynamic magnetic resonance imaging is reported. Patients were divided into three groups based on the dynamic magnetic resonance imaging findings (dynamic curve pattern and relative enhancement ratio) that were conducted within 48 hours of the injury. Traction was used to achieve anatomic reduction and to prevent additional damage to vascularity until minimally invasive internal fixation could be done. The fractures of all 17 patients whose femoral head perfusion was normal (Typ

[Prediction of segmental collapse of femoral head after femoral neck fracture by scintimetry].

A follow-up study regarding the occurrence of segmental collapse of femoral head has been done in 53 patients of united femoral neck fracture including a periodic examination by 99mTc-MDP scintimetry. The results showed that the sequential variations in the nuclide uptake ratio of the femoral heads could reflect the histological repairing process and metabolic level in the femoral head. A sustainedly high nuclide uptake in 12 months after operation often denotes that there are defects in the repairing process of the ischemic femoral head, and that the segmental collapse of the femoral head wil

[Femoral neck stress fracture in a young female runner].

Stress fractures of the femoral neck account for 2-5 % of all stress fractures in athletes, and generally affect military recruits and endurance athletes. The initial symptomatology is insidious, with atraumatic coxalgia, not always inguinal, occurring in a context of intensifying sports activity and relieved by rest. After standard X-ray, MRI is the second-line examination of choice for early diagnosis of this type of fracture. Non-surgical management consists of a period of unloading, followed by a gradual resumption of physical activity in the broad sense, and then of the activity in questi

Prosthetic Neck Fracture of a Cementless Titanium Femoral Stem.

This case report presents a unique instance of a prosthetic neck fracture in an 81-year-old male with a CERAVER CERAFIT titanium (Ti6Al4V) femoral implant paired with a ceramic-on-ceramic total hip replacement. No fractures of the neck of a titanium femoral stem coated with hydroxy-apatite have been reported in the literature for this type of implant. The patient experienced a sudden, non-traumatic fracture, many years after his primary surgery. The report discusses the clinical presentation, diagnostic findings, known risk factors for femoral neck implant fractures, surgical management, and t

Femoral Neck Fracture in a Pediatric Patient with Primary Hyperparathyroidism.

CASE: A previously healthy 11-year-old girl underwent expedited surgical fixation of a femoral neck fracture sustained while jump-roping. After further work up, she was diagnosed with primary hyperparathyroidism. Parathyroidectomy of a hypertrophic adenoma proved curative. Now, five months post left hip surgery, the patient is pain-free and walks without a limp. CONCLUSION: We describe the first published case of primary hyperparathyroidism presenting as a pathologic hip fracture in a child. Although presentation with a fracture is exceedingly rare, bone pain is a frequent complaint of pediat

Femoral Neck Fracture in the Setting of Plasmacytoma: A Case Report.

Solitary bone plasmacytoma (SBP) is a proliferation of monoclonal plasma cells found in a solitary osteolytic lesion. These osteolytic lesions can present as either bone pain or pathological fracture. We present this interesting case of a 63-year-old male that was found to have a plasmacytoma confined to the femoral neck following the presentation of a pathological fracture. After surgical resection and biopsy, we placed a distally fixating hemiarthroplasty. To our knowledge, there is only one other reported case of a pathological fracture of the femoral neck due to plasmacytoma.

[Influence of medical outliers in patients with femoral neck fracture].

OBJECTIVES: The objective is to describe and analyze how outlier admission influences hospital stay and the appearance of complications in patients with a femoral neck fracture treated with arthroplasty. MATERIAL AND METHOD: A historical cohort study was carried out in which the group of patients with a displaced fracture of the femoral neck who had an outlier admission was defined as an exposed cohort, that is, they were admitted to a hospitalization area not belonging to the Orthopedic Surgery and Traumatology department, unlike the unexposed cohort, that included patients admitted to a hos

Heterotopic Ossification Following Arthroplasty for Femoral Neck Fracture.

BACKGROUND: Heterotopic ossification (HO) is a frequent complication following hip surgery. Using data from the Hip Fracture Evaluation with Alternatives of Total Hip Arthroplasty versus Hemiarthroplasty (HEALTH) trial, we aimed to (1) determine the prevalence of HO following total hip arthroplasty (THA) for femoral neck fracture in patients ≥50 years of age, (2) identify whether HO is associated with an increased risk of revision surgery within 24 months after the fracture, and (3) determine the impact of HO on functional outcomes. METHODS: We performed a multivariable Cox regression analysi

[Medial fracture of the femoral neck with enclosed Küntscher nail].

We report about an 81-year-old male patient who suffered a medial femoral neck fracture after a fall on the left leg. A total hip arthroplasty was planned based on the appropriate indications. The planning was complicated by the presence of an intramedullary Küntscher femoral nail inserted after a motorcycle incident approximately 60 years ago. After fenestration of the proximal lateral femur, the nail was sawn off and a cemented total hip arthroplasty and plate osteosynthesis were performed.

Femoral Neck Fracture After Arthroscopic Femoroplasty of the Hip.

Hip arthroscopy is an increasingly common procedure, particularly for the treatment of femoroacetabular impingement. Various complications have been previously reported, and the authors sought to further evaluate the safety of this procedure. This study was conducted to identify the incidence of femoral neck fracture as well as treatment and outcomes after arthroscopic femoroplasty. In April 2013, a survey was administered to 28 established hip arthroscopists regarding the breadth of their experience, including the total number of hip arthroscopies and proximal femoroplasties performed and the

A case report of missed femoral neck stress fracture.

Femoral neck stress fracture (FNSF) is an uncommon but potentially serious orthopaedic problem. This is a case report on missed femoral neck stress fracture in a 62-year-old female who was initially treated as early-onset coxarthrosis. She later presented to us with a displaced intra-capsular neck of left femur fracture and underwent total hip replacement. This case illustrates that causes other than osteoarthritis should be taken into consideration in patients presenting with anterior hip pain where symptoms are disproportionate to clinical and radiological findings. Cemented or uncemented hemiarthroplasty for the treatment of femoral neck fractures?

BACKGROUND AND PURPOSE: Cemented hemiarthroplasty is preferred in treating displaced fractures of the femoral neck in the elderly. The cementing process may cause a fat embolism, leading to serious complications or death. In this study, we wanted to determine whether use of uncemented hemiarthroplasty (HA) would lead to reduced mortality and whether there are differences in the complications associated with these different types of arthroplasty. PATIENTS AND METHODS: From the PERFECT database, which combines information from various treatment registries, we identified 25,174 patients who were

An unusual presentation of femoral neck fracture in a young patient.

Femoral neck fractures in young patients can have unusual presentation making diagnosis and subsequent management more difficult. Early detection is essential to avoid complications; however, it is only achievable with a high index of suspicion. We present an unusual case of a 39-year-old office worker who presented with a 5-month history of spontaneous onset of hip pain. She had multiple clinic visits prior to the diagnosis of neck of femur fracture. Fracture displacement and delayed diagnosis had resulted in a non-union by the time of presentation. She was subsequently managed with a fixatio

Gaps and opportunities in the management of the young femoral neck fracture.

Femoral neck fractures in young adults are difficult to treat. There are substantial gaps in our knowledge regarding treatment and prevention of young adult femoral neck fractures. Avoiding malunion or nonunion of these fractures after fixation remains a challenge. Currently available fixation techniques may allow for healing to occur, but in a shortened position, with negative consequences on gait mechanics and physical function. Osteonecrosis remains a problem, affecting up to half of patients who sustain femoral neck fractures. Achieving reliable healing may require a reconsideration of fra

Logistic regression in estimates of femoral neck fracture by fall.

UNLABELLED: The latest methods in estimating the probability (absolute risk) of osteoporotic fractures include several logistic regression models, based on qualitative risk factors plus bone mineral density (BMD), and the probability estimate of fracture in the future. The Slovak logistic regression model, in contrast to other models, is created from quantitative variables of the proximal femur (in International System of Units) and estimates the probability of fracture by fall. OBJECTIVES: The first objective of this study was to order selected independent variables according to the intensit

Femoral neck fracture as an atypical presentation of child abuse.

Femoral neck fractures account for 46% of the proximal femur fractures in children; however, the overall incidence of proximal femur fractures in children is <1%. Pathologic causes for proximal femur fractures include local or global causes of bone weakness, such as metabolic bone conditions or neoplastic causes. Nonpathologic causes of femur fractures are caused almost exclusively by high-energy trauma and are readily identified. Nonaccidental injury leading to femoral neck fracture is unusual and can be challenging to diagnose when the medical history is not forthcoming. Femoral neck fractur
