Analgesia and Fascia Iliaca Compartment Block Utilisation for Neck-of-Femur Fracture Patients: A Closed-Loop Audit.

Introduction Neck-of-femur (NOF) fractures are a major source of morbidity and mortality among older adults. Timely and effective pain management, including fascia iliaca compartment block (FICB), is recommended to optimise outcomes and minimise opioid-related complications. This study compares two audit cycles assessing improvements in analgesic practice, FICB utilisation, and adherence to national guidelines following targeted educational and procedural interventions. Methods Two retrospective audit cycles were conducted in the emergency department at a district general hospital. Cycle 1 (Se

Compliance With Antibiotic Prophylaxis in Hip Arthroplasty Following Neck of Femur Fractures: A Retrospective Clinical Audit Study.

BACKGROUND: Periprosthetic joint infection (PJI) and superficial wound or skin infection are serious complications following hip arthroplasty for neck of femur (NOF) fractures. To reduce these risks, perioperative antibiotic prophylaxis is recommended in national guidance, and our trust's local guideline specifies three postoperative intravenous doses of flucloxacillin or teicoplanin in patients with a penicillin allergy. This audit aimed to assess compliance with this protocol and its association with postoperative wound and skin infection rates. METHODOLOGY: A retrospective audit study was

Femur geometry and body composition influence femoral neck stresses: A combined fall simulation and beam modelling approach.

Metrics of femur geometry and body composition have been linked to clinical hip fracture risk. Mechanistic explanations for these relationships have generally focused on femur strength; however, impact loading also modulates fracture risk. We evaluated the potential effects of femur geometry and body composition on femoral neck stresses during lateral impacts. Fifteen female volunteers completed low-energy sideways falls on to the hip. Additionally, participants completed ultrasound and dual-energy x-ray absorptiometry imaging to characterize trochanteric soft tissue thickness (TSTT) over the

Does Screw Location Affect the Risk of Subtrochanteric Femur Fracture After Femoral Neck Fixation? A Biomechanical Study.

BACKGROUND: Case reports suggest that there is an increased risk of subtrochanteric femur fracture after femoral neck fixation with cannulated screws when the distal-most screw is placed distal to the lesser trochanter. However, to our knowledge, there are no biomechanical data supporting this observation. QUESTIONS/PURPOSES: (1) Is there an increased risk of subtrochanteric femur fracture after femoral neck fixation with cannulated screws in normal density and osteoporotic Sawbones when the distal-most screw is started distal to the lesser trochanter? (2) Does the screw starting point positi

Femoral nerve block Intervention in Neck of Femur fracture (FINOF): study protocol for a randomized controlled trial.

BACKGROUND: Hip fractures are very painful leading to lengthy hospital stays. Conventional methods of treating pain are limited. Non-steroidal anti-inflammatories are relatively contraindicated and opioids have significant side effects.Regional anaesthesia holds promise but results from these techniques are inconsistent. Trials to date have been inconclusive with regard to which blocks to use and for how long. Interpatient variability remains a problem. METHODS/DESIGN: This is a single centre study conducted at Queen's Medical Centre, Nottingham; a large regional trauma centre in England. It

Analgesic efficacy of continuous femoral nerve block commenced prior to operative fixation of fractured neck of femur.

BACKGROUND: Peripheral nerve blocks are effective in treating acute pain, thereby minimizing the requirement for opiate analgesics. Fractured neck of femur (FNF) is a common, painful injury. The provision of effective analgesia to this cohort is challenging but an important determinant of their functional outcome. We investigated the analgesic efficacy of continuous femoral nerve block (CFNB) in patients with FNF. METHODS: Following institutional ethical approval and with informed consent, patients awaiting FNF surgery were randomly allocated to receive either standard opiate-based analgesia

Fractured neck of femur in the mobile independent elderly patient: should we treat with total hip replacement?

PURPOSE: To report the outcome of displaced sub-capital neck of femur fractures in the independent elderly (>70 years) managed with total hip arthroplasty through a modified Hardinge approach. METHODS: Between 1998 and 2000, a surgeon performed a cemented hip replacement using a modified lateral approach in 36 consecutive patients in the Hawke's Bay Regional Hospital, Hastings. Medical charts and out-patient follow-up clinic records were reviewed with respect to outcomes, with particular reference to complications. Independent review of functional outcome was completed at one year postsurgery

A Rare Case of Metabolic Disorder of Bilateral Fracture Neck Femur Treated with Bilateral Total Hip Arthroplasty.

INTRODUCTION: Bilateral intracapsular neck of femur fractures are rare and are usually associated with underlying metabolic bone diseases such as osteomalacia or osteoporosis. Direct anterior approach (DAA) total hip arthroplasty (THA) is considered the treatment of choice for displaced intracapsular fractures of the femoral neck in both elderly and selected young patients with poor bone quality. Performing THA on both sides in a single sitting can facilitate early mobilization and reduce hospital stay. CASE REPORT: We report a case of a 40-year-old male who presented with bilateral displaced

Multiple screw fixation versus cementless bipolar hemiarthroplasty for femur neck fracture using a nationwide hip fracture registry.

Cementless bipolar hemiarthroplasty (BHA) recently gained popularity as a treatment for femur neck fracture (FNF), but there have been few studies comparing this with multiple screw fixation (MSF) in the elderly population. The purpose of this study is to compare (1) surgery-related parameters, (2) reoperation rate as a local complication, (3) in-hospital systemic complication rate, and (4) mortality rate at 1 year after MSF and cementless BHA in patients with FNF using nationwide data. Six-hundred sixty-six hips (aged ≥ 50 years) extracted from nationwide Hip Fracture Registry were included i

Inferior and superior gluteal nerve paresis and femur neck fracture after spondylolisthesis and lysis: a case report.

A 33-year-old woman is presented with spondylolisthesis of L4 on L5 with osteolysis of the L4 vertebra, left-sided superior and inferior gluteal nerve paresis and a fracture of the neck of the left femur. The gluteal muscle paresis was thought to be caused by an entrapment of the gluteal nerves through the piriformis muscle, as a consequence of the lumbar lordosis and inadequate stabilization of the back. The femoral fracture was explained by lack of the compensating action of the paretic gluteal muscles, through which an abnormal strain on the femoral neck occurred while climbing a staircase.

Audit of Group and Save Sample Rejection in Fractured Neck of Femur Patients at a United Kingdom District Hospital.

BACKGROUND: Accurate preoperative blood grouping and antibody screening are critical for trauma and orthopaedic (T&O) surgery. Group and Save (G&S) samples are frequently rejected due to labelling discrepancies, incomplete documentation, or sample quality issues. Such rejections delay surgery, increase patient discomfort from repeated venepuncture, and generate avoidable financial and operational burdens. This audit assessed the frequency, underlying causes, and financial implications of rejected GS samples for T&O patients in the Accident and Emergency (A&E) department and developed targeted

Time to surgery and its impact on length of stay and mortality for neck of femur fractures at a major trauma centre.

PURPOSE: Neck of femur (NOF) fractures are a major public health concern. Timely surgical intervention (current guidelines is within 36 h) is associated with improved survival, shorter hospital stays, and better functional outcomes. However, surgical delays remain common in hospitals, especially in major trauma centres, and their impact on length of stay (LOS) and mortality is not well defined across different NOF fracture subtypes. METHODS: We conducted a retrospective cohort study of 4415 patients with NOF fractures, classified into intracapsular (n = 2507) and extracapsular (n = 1908). The

Proximal Femoral Erosion: A Sequelae of Psuedoaneursym Formation Following Operative Fixation of an Intertrochanteric Neck of Femur Fracture.

A 72-year-old male sustained a left intertrochanteric neck of femur fracture following a fall. He underwent operative fixation with a dynamic hip screw and was discharged home. Fifteen months later, the patient presented again with ongoing left thigh pain and swelling. A pelvic radiograph showed scalloping of the medial proximal femoral cortex. Further investigation revealed a left profunda femoris artery pseudoaneurysm. Vascular injury during operative fixation of intratrochanteric fractures is a rare complication, which may be missed due to a delayed presentation. Treating physicians should

The effect of time to surgery on outcomes and complication rates following total hip arthroplasty for fractured neck of femur.

INTRODUCTION: Total hip arthroplasty is recommended for elderly patients with fractured neck of femur who are independently mobile, have few co-morbidities and are not cognitively impaired. Providing a daily total hip arthroplasty service is challenging for some units in the UK and considering that these patients may be physiologically distinct from the average hip fracture patient, loss of the best practice tariff as a result of surgical delay may be unjustified. The aim of this study was to determine whether time to surgical intervention for patients eligible for total hip arthroplasty had a

Is Head and Neck Resection of the Femur (Girdlestone's Procedure) Still Relevant? Indications and Results About 24 Cases.

INTRODUCTION: Head and neck resection of the femur was described by Girdlestone in 1928 in the treatment of coxalgia. Very invasive at the beginning, this intervention is much less so today, but the term of "Girdlestone's operation" or "Girdlestone's procedure" has remained in use. The reported results are controversial. In resource-limited countries, Girdlestone's procedure is often indicated for lack of a better one. In this context, we report the results of a series of 24 patients operated in a regional hospital (Ouahigouya, Burkina Faso) with the aim of showing that this technique remains
