Treatment with plasma exchange of a pregnant woman with anti-PP1Pk alloimmunization: A case report.

The histo-blood group antigens P, P1 and Pk are a closely related set of glycosphingolipid structures expressed by red blood cells and other tissues. None of these three characters is expressed on p cells, a null phenotype that arises in the context of homozygous mutation of the A4GALT gene. Subjects with p phenotype spontaneously develop a natural alloantibody named anti-PP1Pk, which is a mixture of IgG and IgM against P1, P and Pk. While anti-P1 is a weak cold antibody with poor clinical significance, anti-P and anti-Pk antibodies are potent haemolysins responsible for severe hemolytic trans

Diagnostic laboratory technologies for the fetus and neonate with isoimmunization.

Maternal-fetal blood group incompatibility is common but less commonly results in hemolytic disease of the fetus and newborn (HDFN). HDFN is associated with greater peak bilirubin, at an earlier age, and for longer duration than other causes of hyperbilirubinemia. It poses a substantial risk for kernicterus and accounts for the majority of exchange transfusions for hyperbilirubinemia. Advances in diagnosis and management are described, from identification of the alloimmunized pregnancy by maternal ABO and Rh typing, antibody screen (indirect Coombs test), identification and titration; laborato

Amniocentesis and chorionic villus sampling.

Amniocentesis and chorionic villus sampling (CVS) remain the most commonly used invasive prenatal diagnostic procedures. Recent reports on early amniocentesis demonstrate its application to the prenatal detection of certain biochemical disorders. However, its role in the evaluation of open fetal defects of the neural tube or ventral wall is still under investigation. The fact that many reports concerning early amniocentesis include a majority of patients beyond 11 to 12 weeks' gestation, thus placing the procedure outside the first trimester, make comparisons with CVS (usually performed betwee

Maternal lymphocytotoxic antibodies and bilirubin levels in the postpartum offspring.

HLA antigens are present on immature red cells (Harris & Zervas 1969), but disappear from their surface when they become mature and enter into the circulation (Silvestre et al. 1970). Nevertheless some remnants of HLA substance may survive this antigenic switch and possibly persist on adult red cells (Morton et al. 1969, 1971, Doughty et al. 1973). A shortened survival of HLA incompatible reticulocytes has been reported when they are transfused into recipients with circulating anti-HLA antibodies (Zervas et al. 1972). As to anti-HLA antibodies possibly reacting with erythrocytes in cases of fe

[External version from breech to vertex presentation (author's transl)].

An analysis of 51 attempts at version was made of which 27 were successful (53%). In 3 cases an emergency cesarean section had to be performed following the attempt at version. In 2 of these cases there was partial premature detachment of the placenta. Attempts at version were begun in the 36th week of pregnancy. Up to the 38th week there was a 67% success rate, while from the 39th week onward it was only 41%. In cases of parity it can be shown clearly that the success rate increases with parity. When the pregnancy is the mother's first, 58% of attempts fail, while in fourth pregnancies this f

Autopsy findings in a series of five cases of fetomaternal haemorrhages.

AIMS: Fetal blood cells enter the maternal circulation in up to 95% of pregnancies, but usually in minute volumes. Haemodynamically significant fetomaternal haemorrhage (FMH) is a much rarer event reported in approximately 1 in 2800 pregnancies. Most of the literature on this phenomenon emphasises the clinical aspects, and there is no comprehensive description of the autopsy findings. We present a series of five fatal FMH. The aim of this series is to highlight some of the autopsy findings that may prompt consideration of a diagnosis of FMH and lead to appropriate confirmatory testing and coun

Transplacental fetal hemorrhage after amniocentesis.

A retrospective survey of Winnipeg Rh laboratory data from January 1, 1981 to December 31, 1984 determined that, despite placental localization, 2.6% of 974 women having amniocenteses performed at 16 to 18 weeks' gestation for genetic reasons and 2.3% of 1215 women having amniocenteses performed between 32 and 38 weeks' gestation had fetal-maternal transplacental hemorrhages greater than or equal to 0.1 mL of fetal red cells due to placental trauma. In 1.6 and 1.8%, respectively, the fetal transplacental hemorrhages were greater than or equal to 1 mL. Four of 99 alloimmunized women undergoing

Trauma in pregnancy.

The treatment of pregnant patients with traumatic injuries requires knowledge of the fundamentals of general trauma management as well as the specific anatomic and physiologic changes brought about by pregnancy. This article provides a review of the spectrum of trauma prevention and treatment in pregnant women, from counseling strategies that can be used during any emergency department visit to a step-by-step evaluation protocol for patients with trauma during pregnancy and the severe injuries that might be encountered by providers during the treatment of these women, including maternal cardio

[Differential diagnosis of thrombocytopenia in pregnancy].

Thrombocytopenia (< 150,000/microliter) is a common finding, occurring in 7-8% of pregnancies. Some conditions, such as gestational thrombocytopenia pose no maternal or fetal risks. Idiopathic thrombocytopenic purpura (ITP) is an acquired haematologic disorder, common among children and adults, with unknown etiology and autoimmune pathogenesis. The incidence of severe fetal and neonatal thrombocytopenia is very rare, and neonatal intracranial hemorrhage is unlikely to be related to the mode of delivery. Alloimmune thrombocytopenia occurs with an incidence of 1/1,000 livebirths and is induced b

The Kleihauer-Betke test. Clinical utility, indication, and correlation in patients with placental abruption and cocaine use.

This study examines the indications for performing the Kleihauer-Betke (KB) test and makes recommendations for its use. Results of 523 KB tests performed during 1993 at our hospital (Cleveland, Ohio) are reviewed in conjunction with surgical pathology reports of placental findings, obstetric records, and toxicology results. We conclude that the KB test should be performed following a positive screening test on all Rh negative mothers of Rh positive infants. Additional indications include cases of maternal trauma, unexplained increased maternal alpha-fetoprotein levels, fetal distress with abno

Decreased fetal movements: a practical approach in a primary care setting.

BACKGROUND: The association between perceived decreased fetal movement (DFM) and adverse outcomes in pregnancy is widely acknowledged. However, in the general practice setting, a common first point-of-call for pregnant women, guidelines for appropriate management of DFM are lacking. OBJECTIVE: This article reviews the current evidence surrounding women presenting with DFM and suggests appropriate management in the community setting and the indications for hospital referral. DISCUSSION: Maternal perception of DFM is a common reason for women to make contact with their healthcare provider. Wom

Significant fetal-maternal hemorrhage after termination of pregnancy: implications for development of fetal cell microchimerism.

OBJECTIVE: Recent reports that an association exists between fetal cell microchimerism and autoimmune disease has increased interest in the postpartum persistence of fetal cells. The purpose of this study was to determine, by means of quantitative polymerase chain reaction amplification, whether a significant fetalmaternal hemorrhage occurs after elective termination of pregnancy. STUDY DESIGN: Blood samples were obtained from 23 women who underwent termination of pregnancy immediately before venipuncture; these samples were subjected to analysis by quantitative polymerase chain reaction ampl

The scientific basis of antenatal prophylaxis.

BACKGROUND: Standard post-delivery administration of anti-D, together with further anti-D for events known to result in fetomaternal haemorrhage (FMH) during pregnancy, has reduced the incidence of alloimmunisation in RhD women to 0.83-1.5% in the UK. Residual alloimmunisation occurs mainly for two reasons: i) failure to administer sufficient anti-D at the correct time after known at-risk events, either during pregnancy or at delivery; and ii) alloimmunisation during pregnancy as a result of 'silent' FMH. The RhD antigen is well developed by 6 weeks' gestation and the fetoplacental blood volum

Chorionic villus sampling and materno-fetal transfusions: an immunological pathogenesis of vascular disruptive syndromes?

Experimental materno-embryonic transfusions with serum that is immunologically active against blood group antigens cause congenital malformations in the rat embryo. In view of the possible increased incidence of vascular disruptive syndromes after chorionic villus sampling (CVS), we investigated the occurrence of materno-fetal transfusions (MFTs) in this procedure. In 18 pregnant women experiencing two needle introductions at CVS, we looked immunohistochemically at the presence of haemoglobin A1-containing maternal erythrocytes in the fetal circulation of the separately collected first and sec

A case of severe neonatal thrombocytopenia with schizencephaly associated with anti-HPA-1b and anti-HPA-2a.

We report a family with a neonate who was severely damaged by intracranial haemorrhages. These probably occurred before the 20th week of gestation. The neonate had a moderate thrombocytopenia. In the maternal serum anti-HPA-1b and anti-HPA-2a alloantibodies were detected. Third-generation assays were applied to identify the alloantibodies. No other cause for the bleeding was found. Probably the combination of anti-HPA-1b and anti-HPA-2a alloantibodies, directed against the platelet fibrinogen receptor and the von Willebrand receptor, respectively, induced a thrombocytopenia and a thrombocytopa
