The effects of prolonged weightlessness and reduced gravity environments on human survival.

The manned exploration of the solar system and the surfaces of some of the smaller planets and larger satellites requires that we are able to keep the adverse human physiological response to long term exposure to near zero and greatly reduced gravity environments within acceptable limits consistent with metabolic function. This paper examines the physiological changes associated with microgravity conditions with particular reference to the weightless demineralizatoin of bone (WDB). It is suggested that many of these changes are the result of physical/mechanical processes and are not primarily

Sustaining humans in space.

One goal of NASA's Ames Research Center is to extend the presence of humans in space. Biomedical research, human performance, and life-support systems form the core of the Ames program. Major questions highlight how the effects of microgravity on living systems are modified by exercise, artificial gravity, autogenic feedback training, and nutrition. Bioengineering and life science studies include humans, animals, and plants. Ames investigators collaborate extensively with outside university and commercial scientists, both within the United States and internationally.

Demonstration of postural effects and layering by cranial computed tomography.

The superb contrast resolution of computed tomography permits visualization of layering of fluid components of varying densities in tumors and hematomas which cannot be observed with conventional radiographic techniques. Layering confirms the fluid nature of a cystic tumor and suggests the possibility of draining the fluid contents in an effort to reduce mass effect. Verification of layering is achieved by noting shifting of the dependent material with axial rotation of the patient. While unusual, this finding is significant because solid tumors occasionally show low densities simulating fluid

The influence of volume depletion and central hypovolemia on the plasma concentration of parathyroid hormone in dialysis patients.

Because changes in extracellular volume during dialysis cause reflex neurohonnonal changes that may influence parathyroid hormone (PTH) release independently of calcium, the influence of isotonic volume depletion (by isolated ultrafiltration) and central hypovolemia (70 degrees tilt) on serum PTH1-84 was studied in 16 hemodialysis patients. Tilting was performed in volume depleted state, i.e., immediately after hemodialysis. In the control study, patients underwent sham ultrafiltration (UF = 0) and after dialysis maintained the supine position for the same length of time they remained in the t

First case report of serous otitis media as a complication of severe ovarian hyperstimulation syndrome: Case report and literature review.

Ovarian hyperstimulation syndrome (OHSS) may be a severe complication of controlled ovarian hyperstimulation during assisted reproductive technology. During OHSS, fluid shifts from the intravascular space to the third-space compartments as the result of an increase in capillary permeability. This can cause fluid accumulation in peritoneal as well as thoracic cavities. The patient presented with symptoms of severe OHSS (bilateral hydrothorax and pulmonary effusion), requiring bilateral ultrasound-guided paracentesis and bilateral thoracentesis during her Emergency Room visits and hospitalizatio

Individualized Perioperative Blood Pressure and Fluid Therapy in Esophagectomy: A Prospective, Single-blind Randomized Controlled Trial.

BACKGROUND: Esophagectomy is a key treatment for esophageal cancer but carries a high risk of postoperative complications, some of which are potentially preventable through optimized hemodynamic management. Goal-directed fluid therapy individualizes cardiac output targets but often applies fixed blood pressure thresholds and is discontinued before major postoperative fluid shifts occur. Extending goal-directed fluid therapy into the postoperative period with individualized blood pressure thresholds may address these limitations. METHODS: In this single-center, prospective, blinded, randomized

Cold-induced fluid extravasation during cardiopulmonary bypass in piglets can be counteracted by use of iso-oncotic prime.

OBJECTIVE: Hypothermic cardiopulmonary bypass is associated with increased fluid extravasation. This study aimed to compare whether iso-oncotic priming solutions, in contrast to crystalloids, could reduce the cold-induced fluid extravasation during cardiopulmonary bypass in piglets. METHODS: Three groups were studied: the control group (CT group; n = 10), the albumin group (Alb group; n = 7), and the hydroxyethyl starch group (HES group; n = 7). Prime (1000 mL) and supplemental fluid were acetated Ringer solution, 4% albumin, and 6% hydroxyethyl starch, respectively. After 1 hour of normother

New Approach to Calculation of Renal Cation Clearance in Rats after V1a Receptor Stimulation.

V1a receptor agonist [Phe2, Ile3, Orn8]-vasopressin produces maximum diuretic and saluretic effects in a dose of 0.1 nmol/100 g body weight: renal excretion of Na+ increases by 50 times in comparison with the initial level over 90 min, excretion of K+, Mg2+, and Са2+ increases by 3, 2, and 16 times, respectively. New formulas were proposed for calculation of the clearance considering total cation content in the extracellular fluid and the time of total effect of the injected dose. After injection of V1a receptors agonist, clearance of Na+ was 7% of its total content in the extracellular fluid,

Nature of organic fluid-montmorillonite interactions: an FTIR spectroscopic study.

The changes in the H-O-H stretching vibration in the interlayer water and Si-O stretching vibration of a Na-montmorillonite (MMT) structure in the FTIR (Fourier transform infrared) spectra provide insight into the effect of fluids of different dielectric constants on the clay structure. Mechanisms by which the different fluids of varying polarities enter into the clay interlayer and the rates at which these molecules interact with the clay structure and the interlayer water are studied at the molecular level using six different fluids with dielectric constants ranging from 110 to 2.4. The shif

Effect of human head flexion on the control of peripheral blood flow in microgravity and in 1 g.

This study evaluated, in six healthy subjects, whether head flexion, which stimulates the vestibular system and the tonic neck receptors, interferes with cardiovascular regulation. Arterial parameters were measured continuously using a pulsed Doppler ultrasound probe during parabolic flights with subjects either in the supine craned-head position (control) or in the supine anterior neck flexion bent-neck position. Exposure to 0 g induced a fluid shift towards the head (stroke volume +8%, P<0.05). Compared to the control situation the mean (SD) blood flow in the femoral artery decreased [ -10 (

Phase angle as a reproducible bedside tool to assess reduced muscle mass in patients with decompensated cirrhosis and ascites: An observational cross-sectional study.

BACKGROUND: Reduced muscle mass in patients with cirrhosis is linked to poor clinical outcomes. Bioimpedance analysis estimates fat mass (FM), fat-free mass (FFM), and phase angle (PhA), a marker of cell membrane integrity correlating with computed tomography-based muscle mass assessments. We hypothesized that PhA would remain stable after paracentesis. METHODS: Bioelectrical impedance was measured in patients with cirrhosis immediately before and after paracentesis using a 256-frequency bioimpedance spectroscopy device. We assessed median paired differences (with 95% confidence intervals) fo

Determinants of hypokalemia following hypertonic sodium bicarbonate infusion.

The hypokalemic response to alkali infusion has been attributed to the resulting extracellular fluid (ECF) expansion, urinary potassium excretion, and internal potassium shifts, but the dominant mechanism remains uncertain. Hypertonic NaHCO3 infusion (1 N, 5 mmol/kg) to unanesthetized dogs with normal acid-base status or one of the four chronic acid-base disorders decreased plasma potassium concentration ([K+]p) at 30 min in all study groups (Δ[K+]p, - 0.16 to - 0.73 mmol/L), which remained essentially unaltered up to 90-min postinfusion. ECF expansion accounted for only a small fraction of th

Intradialytic hypotension and splanchnic shifting: Integrating an overlooked mechanism with the detection of ischemia-related signals during hemodialysis.

In the most simple analysis, a patient's hematocrit during hemodialysis will rise when the rate of ultrafiltration exceeds the rate at which the fluid is mobilized from extravascular spaces; the greater the rise in hematocrit, the lower blood volume is and the more likely intradialytic hypotension (IDH) is to occur. A secondary mechanism of IDH may be due to sudden shift of blood volume away from the heart under conditions of borderline cardiac filling. A substantial portion of blood volume resides in the splanchnic venous system. During the early part of dialysis, a centripetal shift of red c

Pancreatic Fluid Enzymes as a Functional Chronic Pancreatitis Biomarker: A Pilot Endoscopic Study.

OBJECTIVE: We previously validated a short endoscopic test with pancreas fluid enzyme activity in pediatrics and young adults. We assessed the role of pancreatic fluid enzyme activity as a candidate to address the lack of a diagnostic biomarker for chronic pancreatitis (CP). METHODS: In this pilot study, we assessed pancreatic fluid samples collected endoscopically 30-minute post-secretin injection from adult controls (n=10) and CP subjects (n=20). Protein-normalized enzymatic activity for amylase, lipase, trypsin, chymotrypsin, and elastase was compared between CP and control groups using lo

Acute bacterial and viral meningitis.

PURPOSE OF REVIEW: Most cases of acute meningitis are infectious and result from a potentially wide range of bacterial and viral pathogens. The organized approach to the patient with suspected meningitis enables the prompt administration of antibiotics, possibly corticosteroids, and diagnostic testing with neuroimaging and spinal fluid analysis. RECENT FINDINGS: Acute meningitis is infectious in most cases and caused by a potentially wide range of bacterial and viral pathogens. Shifts in the epidemiology of bacterial pathogens have been influenced by changes in vaccines and their implementati

[Orthostatic hypotension after space flight].

Cardiovascular changes occur during spaceflight. Because of the loss of hydrostatic pressure there is, during weightlessness, a fluid shift of extra- and intra-vascular volumes to the upper part of the body. This provokes objective and subjective symptoms, especially in the first days of spaceflight. The fluid shift is perceived by baroreceptors as a relative central hypervolaemia and induces neuro-hormonal mechanisms which provoke a real hypovolaemia. Cardiovascular deconditioning exists during spaceflight with the following symptoms when astronauts return to earth: decreased orthostatic tole

The pathophysiology of paraquat nephrotoxicity in the sheep.

The effect of intravenous paraquat on ovine renal function was examined at four dose schedules. Paraquat caused dose dependent reduction in ovine glomerular and tubular function, but glomerular filtration rate increased at a low dose. The earliest reduction in function involved the major energy dependent processes of the kidney to inhibit sodium reabsorption. There were marked shifts in fluid balance to which the kidney initially responded. As nephrotoxicity progressed the homeostatic response failed. Paraquat which is a quaternary amine may mimic the pharmacological behaviour of similar compo

Stressor states and the cation crossroads.

Neurohormonal activation involving the hypothalamic-pituitary-adrenal axis and adrenergic nervous and renin-angiotensin-aldosterone systems is integral to stressor state-mediated homeostatic responses. The levels of effector hormones, depending upon the degree of stress, orchestrate the concordant appearance of hypokalemia, ionized hypocalcemia and hypomagnesemia, hypozincemia, and hyposelenemia. Seemingly contradictory to homeostatic responses wherein the constancy of extracellular fluid would be preserved, upregulation of cognate-binding proteins promotes coordinated translocation of cations

[Uveal effusion syndrome and scleral thickening].

The uveal effusion syndrome associates bullous no rhegmatognous retinal detachment with shifting subretinal fluid, choroidal detachment and elevated spinal fluid protein. This syndrome could result of nonophthalmos, scleral thickening and/or vortex vein attenuation. The author describes a case of uveal effusion syndrome with isolated scleral thickening. Theoretical explanations of this syndrome are emphasized on the basis of literature and clinical and surgical guidelines are given.
