OBJECTIVES: The aim of this study was to examine the frequency and causes of surgical explantation of patent foramen ovale (PFO) closure devices.
BACKGROUND: PFO has been linked with cryptogenic strokes, recurrent transient neurologic deficits, sleep apnea, decompression illness, and migraines. Several randomized trials are in progress to determine whether PFO closure is preferable to medical management in the treatment of patients with cryptogenic strokes or migraine. The majority of PFO closures are performed off-label, because there is no U.S. Food and Drug Administration approval for use

Left ventricle (LV) lead placement in a coronary sinus branch for cardiac resynchronization therapy may fail because of anatomical variants, phrenic nerve stimulation, and/or lead instability. We report a case of successful resynchronization from a lead inserted from the left subclavian vein and positioned through a patent foramen ovale (PFO). In conclusion, endocardial LV lead insertion through a PFO enables effective resynchronization delivery without the risks associated with a thoracotomy or atrial transseptal puncture.

Right-to-left intracardiac shunt through a patent foramen ovale sometimes induces a significant arterial oxygen desaturation. We describe two cases of severe hypoxia due to a patent foramen ovale, treated successfully by transcatheter closure of the intracardiac shunt. One case occurred after implantation of a cardiac assist device, the other patient presented with a platypnoeaorthodeoxia syndrome. Clinical considerations are outlined.

UNLABELLED: The goal of this study was to assess the feasibility, safety and success of a system which uses radiofrequency energy (RFE) rather than a device for percutaneous closure of patent foramen ovale (PFO).
METHODS: Sixteen patients (10 men, 6 women, mean age 50 years) were included in the study. All of them had a proven PFO with documented right-to-left shunt (RLS) after Valsalva manoeuvre (VM) during transoesophageal echocardiography (TEE). The patients had an average PFO diameter of 6 +/- 2 mm at TEE and an average of 23 +/- 4 microembolic signals (MES) in power M-mode transcranial D

A 39-year-old man was hospitalized with symptoms of acute deep vein thrombosis, and computed tomography showed that he had pulmonary thromboembolism. Transthoracic echocardiography showed a large, right atrial thrombus and transesophageal echocardiography showed migrating thrombus trapped in a patent foramen ovale and extending all the way up to the ascending aorta. He underwent surgical embolectomy under cardiopulmonary bypass. At the conclusion of the operation, repeat transesophageal echocardiography examination revealed fresh but small thrombus in the right atrium. This continued entry of

Patent foramen ovale (PFO) has been implicated in the pathogenesis of cryptogenic stroke, arterial desaturation, decompression illness, and migraine headache (MH). This study evaluated the safety of percutaneous transcatheter PFO closure in patients with cryptogenic stroke, transient ischemic attack, or arterial desaturation. Additionally, symptomatic reduction in MH was determined after interatrial shunt closure. Of the 252 patients referred to the University of California, Los Angeles, with PFO, 131 underwent closure of the interatrial communication with a CardioSEAL (n = 30) or Amplatzer (n

OBJECTIVE: To review the procedures, results and follow-up of patients who underwent percutaneous device closure of a patent foramen ovale (PFO) during pregnancy. We report modifications made to the established technique that allowed absolute minimization of radiation exposure. Relevant management issues of this unusual presentation of cardiac disease in pregnancy are discussed.
BACKGROUND: Stroke during or immediately prior to pregnancy is a catastrophic event. Paradoxical embolism may be more frequent due to the hypercoagulable state and increased incidence of venous thromboembolism. Treatm

We describe a case of impending paradoxical embolism in a 43-year-old male patient with pulmonary embolism. Transesophageal echocardiography revealed a thromboembolus straddling a patent foramen ovale. The patient underwent emergency removal of the intracardiac clot with closure of the patent foramen ovale. A postoperative work-up for a hypercoagulable state revealed a protein-S deficiency and bilateral lower extremity deep vein thromboses. A Greenfield inferior vena cava filter was inserted, anticoagulation was performed, and warfarin treatment was continued after the patient was discharged h

We sought to evaluate the effects of atrial septal and patent foramen ovale (PFO) morphology on the efficacy of transcatheter closure. We performed a retrospective analysis of all patients who underwent PFO device closure from January 1997 to January 2002. Forty-seven patients underwent percutaneous closure of a PFO with a median age of 45 years (range, 8-75) and weight of 76 kg (range 28-115). The septal morphology was flat in 33 and aneurysmal in 14 patients. The PFO morphology was a simple flap in 20 and complex in 27 patients. Complex morphologies included long-tunnel PFO (n = 15), coexist

Patency of the foramen ovale (FO) is a very rare complication after lobectomy. Completion pneumonectomy after FO reopening has never been described before. In the reported case, a patent FO was diagnosed in a 52-year old man 9 months after a left upper sleeve lobectomy for a squamous cell carcinoma pT2N1. At the same time, bronchoscopy showed an endobronchial recurrence in the left main bronchus. The foramen was closed percutaneously by a 35 mm Amplatzer prosthesis. Dyspnea improved dramatically in the following 4 weeks and no more oxygen therapy was required. After complete restaging, a compl

This study investigated anatomic and pathophysiologic variables that may determine which patent foramen ovale (PFO) are associated with cerebrovascular accidents (CVAs). Anatomic features of a PFO have been identified as risk factors that predispose certain people to cryptogenic strokes (strokes of unknown cause). However, potential pathophysiologic variables that can determine the pressure gradient between left and right atria, which could influence the right-to-left shunt through a PFO, have not been examined. A retrospective study included 78 consecutive patients in whom PFOs were detected

The role of patent foramen ovale (PFO) in cryptogenic stroke is still debated, but from recent follow-up studies it seems that the amount of right-to-left shunt (RLS) and the association with atrial septal aneurysm (ASA) are major determinants of stroke recurrence. PFO and RLS through the atrial chambers have been recently studied in a number of conditions not or marginally related to cerebrovascular disease. Historically the first studies addressed the presence of RLS in scuba divers as a possible abnormality related to decompression sickness (DS) of unknown aetiology. Despite initial debate

Transcatheter closure of a patent foramen ovale (PFO) offers a valuable alternative to surgery in selected patients with presumed paradoxical embolism. Like for all other devices, several complications have been described. Although the majority of these complications are caused by technical problems of the device itself, thrombosis is a relatively infrequent but feared finding, as it may result in recurrent embolic events and persistent neurological deficits. The majority of the complications after implantation are dealt with by surgical removal of the device and subsequent surgical closure of

Patent foramen ovale (PFO) closure is a common intervention aimed at reducing recurrent stroke in patients with cryptogenic embolism. Complex PFO anatomies, characterized by features such as long tunnels, large defect sizes, atrial septal aneurysms, multifenestrated septa, prominent Eustachian valves or Chiari networks, and rigid or thickened septal tissue, pose significant challenges for successful device deployment and closure. Advances in imaging technologies, including transesophageal echocardiography, intracardiac echocardiography, and 3-dimensional imaging, have significantly enhanced th

INTRODUCTION: A patent foramen ovale (PFO) is present in 20%-30% of the adult population. It is usually found in patients incidentally, but clinical presentations could include stroke, seizures, and migraines. The aim of this study is to review the experience of PFO closure at our institution.
METHODS: In this IRB approved clinical research study, we completed a retrospective chart review of 289 patients between the ages of 18-100 y old who came to our institute between January 2012 and December 2022. Our cohort had 173 female patients and 116 male patients. The patients' average age was 50.3

The foramen ovale is a flap in the atrial septum that is open in the fetus to allow oxygenated blood returning from the placenta to pass to the systemic circulation. In early life, the primum septal flap should close, but in 25% of people the flap does not seal fully. Most patent foramen ovale (PFO) are small and open only following Valsalva. Resting right-to-left shunts are less common, occurring in 5% of individuals. This British Cardiovascular Intervention Society position statement describes the diagnosis, indications and procedure for PFO closure.
