Synergistic activity of fosfomycin and chloramphenicol against vancomycin-resistant Enterococcus faecium (VREfm) isolates from bloodstream infections.

Vancomycin-resistant Enterococcus faecium (VREfm) infections are increasing. Current anti-VREfm options (linezolid and daptomycin) are suboptimal. Fosfomycin maintains good efficacy against VREfm and chloramphenicol is active against ≥ 90% of VREfm. We tested chloramphenicol + fosfomycin (CAF+FOS) against 10 VREfm isolated from blood. MICs were 64 to 512 µg/mL for fosfomycin and 8 to 16 µg/mL for chloramphenicol. The combination decreased both MICs, with a synergic effect in 50% of the isolates and an additive effect in the remaining 50%. Time-kill assays performed on fractional inhibitory con

fosZ, a novel plasmid-borne fosfomycin resistance gene in Pseudomonas species, especially carbapenem-resistant Pseudomonas aeruginosa isolates.

A novel fosfomycin-resistant glutathione S-transferase (FR-GST) gene, fosZ, was investigated, and its structural characteristics were characterized in Pseudomonas species. The fosZ gene was cloned and expressed in P. aeruginosa PAO1 and HS355, where it displayed reduced susceptibility to fosfomycin (8- to 64-fold) and its inhibitor sodium phosphonoformate (PPF). FosZ shares less than 70% amino acid identity with known FosA proteins. Bioinformatics analyzes revealed that fosZ was a transposable passenger gene within ISPa75, likely captured from Pseudomonas species. A total of 159 fosZ-bearing P

Development of loop-mediated isothermal amplification (LAMP)-based assay for rapid detection of fosfomycin resistance gene fosA.

Fosfomycin is an old treatment agent that has been reintroduced as a first-line antibiotic for the treatment of acute uncomplicated urinary tract infections caused by multidrug-resistant bacteria. However, resistance to fosfomycin is rising, and limited options for its detection are available. In this study, we developed a molecular technique based on loop-mediated isothermal amplification (LAMP) as a rapid and easy-to-perform method for the detection of the fosA resistance gene. LAMP primers were designed using the Primer Explorer V5 software. A total of 63 bacterial strains (34 Klebsiella pn

In Vitro Activity of Fosfomycin and Nitrofurantoin Against Contemporary Enterobacterales Pathogens Isolated from Indian Tertiary Care Hospitals.

Background: In India, multidrug resistance in community and hospital associated Gram-negative pathogens has increased sharply over the past few years. In the absence of novel oral multidrug resistant-pathogen active therapies, the therapeutic situation with regard to community infections is even more challenging. Hence, the focus is now shifting toward potentially expanding the utility of older antibiotics such as fosfomycin and nitrofurantoin beyond their approved pathogen coverage. The current study was undertaken to assess the activity of fosfomycin and nitrofurantoin against Enterobacteral

Case Commentary: Imipenem/Cilastatin and Fosfomycin for Refractory Methicillin-Resistant Staphylococcus aureus Infection: a Novel Combination Therapy.

Given that it is unlikely that randomized clinical trials will yield answers for treating the most challenging bacteremic infections caused by methicillin-resistant Staphylococcus aureus, clinicians, microbiologists, and pharmacists will have to cooperate to discover novel ways to select successful individualized antimicrobial therapy for these patients. An example of such a strategy was demonstrated in the identification and utilization of imipenem/cilastatin plus fosfomycin to treat a particularly recalcitrant MRSA bacteremia and spinal abscess.

Intravenous fosfomycin for pulmonary exacerbation of cystic fibrosis: Real life experience of a large adult CF centre.

BACKGROUND: The increased prevalence of multi-drug resistant strains of P.aeruginosa and allergic reactions among adult patients with cystic fibrosis (CF) limits the number of antibiotics available to treat pulmonary exacerbations. Fosfomycin, a unique broad spectrum bactericidal antibiotic, might offer an alternative therapeutic option in such cases. AIM: To describe the clinical efficacy, safety and tolerability of intravenous fosfomycin in combination with a second anti-pseudomonal antibiotic to treat pulmonary exacerbations in adult patients with CF. METHOD: A retrospective analysis of d

Characterisation of fosfomycin resistance mechanisms and molecular epidemiology in extended-spectrum β-lactamase-producing Klebsiella pneumoniae isolates.

Although fosfomycin is a treatment option for infections caused by extended-spectrum β-lactamase (ESBL)-producing Enterobacteriaceae, fosfomycin resistance has been documented. To our knowledge, fosfomycin resistance mechanisms in Klebsiella pneumoniae have not been systematically investigated. A total of 108 ESBL-producing K. pneumoniae isolates collected from Kaohsiung Medical University Hospital, Taiwan, from August 2012 to May 2013 were analysed in this study. Pulsed-field gel electrophoresis (PFGE) revealed 64 pulsotypes and six non-typeable isolates, indicating high genetic diversity. Mo

Daptomycin plus fosfomycin versus daptomycin monotherapy in treating MRSA: protocol of a multicentre, randomised, phase III trial.

INTRODUCTION: Despite the availability of new antibiotics such as daptomycin, methicillin-resistant Staphylococcus aureus (MRSA) bacteraemia continues to be associated with high clinical failure rates. Combination therapy has been proposed as an alternative to improve outcomes but there is a lack of clinical studies. The study aims to demonstrate that combination of daptomycin plus fosfomycin achieves higher clinical success rates in the treatment of MRSA bacteraemia than daptomycin alone. METHODS AND ANALYSIS: A multicentre open-label, randomised phase III study. Adult patients hospitalised
