Temporal pattern of gene expression and histology of stress fracture healing.

Loading of the rat ulna is an ideal model to examine stress fracture healing. The aim of this study was to undertake a detailed examination of the histology, histomorphometry and gene expression of the healing and remodelling process initiated by fatigue loading of the rat ulna. Ulnae were harvested 1, 2, 4, 6, 8, and 10 weeks following creation of a stress fracture. Stress fracture healing involved direct remodelling that progressed along the fracture line as well as woven bone proliferation at the site of the fracture. Histomorphometry demonstrated rapid progression of basic multicellular un

A new method for evaluation of fracture healing by echo tracking.

Assessment of bone healing on radiographs depends on the volume and radio-opacity of callus at the healing site, but is not necessarily objective, and there are differences of judgment among observers. To overcome this disadvantage, a clinical system was developed to quantify the stiffness of healing fractures of the tibia in patients by the echo tracking (ET) method in a manner similar to a three-point bending test. The purpose of this study was to ensure that the ET system could clinically assess the progress, delay or arrest of healing. The fibular head and the lateral malleolus were suppor

Effects of human amniotic fluid on fracture healing in rat tibia.

BACKGROUND: Human amniotic fluid (HAF), including hyaluronic acid (HA) and several growth factors, has been used experimentally in tendon, nerve, and cartilage regeneration and in bone defects because of its positive stimulating effects on regeneration potential. This study was performed to investigate whether HAF was effective on fracture healing. MATERIALS AND METHODS: We created 36 tibial fractures in 20-week-old Wistar rats that were divided into three groups. In group 1, fracture lines were instilled with HAF collected at 18th week of the gestation and, in group 2, fracture lines were in

Osteopenia and impaired fracture healing in aged EP4 receptor knockout mice.

The EP4 receptor, one of the subtypes of the prostaglandin E2 (PGE2) receptor, plays a critical role in the anabolic effects of PGE2 on bone. However, its role in the maintenance of bone mass in aged animals and its role in fracture healing is not well known. Our studies addressed these issues by characterizing the skeletal phenotype of aged, EP4 receptor knockout (KO) mice, and by comparing fracture healing in aged KO mice versus wild type (WT) mice. There was no significant difference in body weight and femoral length between KO and WT mice at 15 to 16 months of age. Lower bone mass was seen

Fracture healing and callus innervation after peripheral nerve resection in rats.

The effects of femoral and sciatic nerve resection on fracture healing and innervation of the fracture callus were studied using a stable fracture model. In 34 rats the right tibia was subjected to a standardized closed fracture and stabilized with a modular intramedullary nail. In half of the animals, resection of 1 cm of the femoral and sciatic nerves was performed (nerve resection group), whereas the other animals had sham operations (sham group). To avoid unequal load-bearing between the two groups, all fractured hindlimbs were immobilized in a plaster of Paris cast. The trial was terminat

The effects of extraperiosteal and subperiosteal dissection. II. On fracture healing.

The effects of subperiosteal and extraperiosteal dissection on the healing of tibial osteotomies were studied in fourteen rabbits killed three weeks after the osteotomy. Of the tibiae exposed subperiosteally, 71 per cent healed; of those exposed extraperiosteally, only 7 per cent had solid union. The first group had a significantly lower level of hydroxyproline in the callus and higher values for breaking load, breaking strength, and absorption of energy to failure. In ten animals that also had muscle transection at the osteotomy site, the rate and type of healing was similar whether no dissec

A radiographic and histologic study of fracture healing in osteopetrotic rats.

Repair of tibial fractures in osteopetrotic rats was delayed in comparison to that of normal littermates, due to reduced remodeling. Reduced bone resorption, known to be the cause of the disease in this mutation, is expressed in both skeletal development and fracture repair. The possible implications for human juvenile osteopetrosis are discussed.

Effect of belt electrode-skeletal muscle electrical stimulation on fracture healing in a rat femoral fracture model.

BACKGROUND: Belt electrode–skeletal muscle electrical stimulation (B-SES) has been reported to influence bone remodeling and muscle function. However, its potential role in fracture healing remains incompletely understood. This study aimed to evaluate the effects of B-SES on fracture repair in a rat femoral fracture model. METHODS: Twelve male Sprague–Dawley rats with surgically induced femoral fractures were randomly assigned to either a B-SES group or a control group (n = 6 per group). B-SES was initiated immediately after fracture induction and applied for 20 min per day, 5 days per week, f

The effect of teriparatide on fracture healing after atypical femoral fracture: A systematic review and meta-analysis.

UNLABELLED: This meta-analysis demonstrated that a greater prevalence of delayed union and nonunion and a longer time to fracture healing in the group that did not receive TPTD treatment after AFFs than in the group that received TPTD treatment. PURPOSE: To date, there is no hard evidence for medical management after atypical femoral fracture (AFF), even though weak data indicate faster healing with teriparatide (TPTD). Herein, we aimed to investigate the effect of postfracture TPTD treatment on AFF healing using a pairwise meta-analysis focusing on delayed union, nonunion, and fracture heali

Combination therapy with low-dose teriparatide and zoledronate contributes to fracture healing on rat femoral fracture model.

BACKGROUND: Delay in fracture healing or non-union can be devastating complication. Recent studies have reported that teriparatide (TP) demonstrated effectively on callus formation and mechanical strength and zoledronate (ZA) increased the callus size and resistance at the fracture site in rat fracture model. In this study, the effects of combination therapy with low dose TP and ZA on fracture healing was evaluated. METHODS: From 1 week post-operation, TP (5 times a week administration) and ZA (0.1 mg/kg single administration) were administered by dividing the rats into the following five gro

Proximal Humerus Fracture: Fix, Replace, or Let Heal.

Proximal humerus fractures occur frequently, especially in the aging population, and require a tailored approach considering patient-specific factors like age, activity level, and comorbidities. While nonoperative treatment is common, surgical options such as open reduction internal fixation, reverse total shoulder arthroplasty (rTSA), and hemiarthroplasty are available based on fracture complexity and patient needs. Recent studies highlight the potential benefits of acute rTSA over delayed procedures, emphasizing improved outcomes in range of motion and reduced complications. Treatment choice

Effect of different composite plates on the healing of femoral fractures.

In this paper, the effects of different composite plates on the healing of femoral fractures were studied by numerical simulation. The healing model of femoral fracture was established by ABAQUS display solver. Based on the fuzzy logic theory, the process of callus differentiation at femoral fracture was considered under the joint action of biological variables and mechanical stimulation, and the healing process of femur was simulated. Compare the stress on the screw, concentration of callus bone and cartilage, and callus healing performance of the carbon/epoxy composite (WSN3k) plate, glass/p

Nanocomposites drug delivery systems for the healing of bone fractures.

The skeletal system is fundamental for the structure and support of the body consisting of bones, cartilage, and connective tissues. Poor fracture healing is a chief clinical problem leading to disability, extended hospital stays and huge financial liability. Even though most fractures are cured using standard clinical methods, about 10% of fractures are delayed or non-union. Despite decades of progress, the bone-targeted delivery system is still restricted due to the distinctive anatomical bone features. Recently, various novel nanocomposite systems have been designed for the cell-specific ta

The Influence of Ibuprofen on the Healing of Nonsurgically Treated Colles' Fractures.

Nonsteroidal anti-inflammatory drugs can delay bone healing. This knowledge is mainly derived from retrospective and animal studies. The authors therefore conducted a human study to investigate whether ibuprofen affects radiological, functional, densitometrical, and biochemical outcomes following a Colles' fracture, as well as the analgesic effect of ibuprofen. This was a single-center, triple-blinded, randomized, placebo-controlled clinical trial with a total of 96 patients. All of the patients received basic treatment with 1000 mg of acetaminophen 4 times daily. The placebo group received a

Stem Cell Interventions for Bone Healing: Fractures and Osteoporosis.

With the ageing population, musculoskeletal conditions are becoming more inherent. Delayed union is defined as a slower than normal fracture healing response, with no healing after 4 to 6 months; however, the union is anticipated given sufficient time. In the context of delayed/non-union, fragility fractures in osteoporotic populations carry significant patient morbidity and socioeconomic costs. Multiple mechanisms hinder fracture healing in osteoporotic patients, imbalanced bone remodelling leads to impaired bone microarchitecture due to reduced osteoblast number and activity and as such, cal

The Effect of Osteoporosis on Healing of Distal Radius Fragility Fractures.

Although the decision for operative versus nonoperative treatment of distal radius fractures remains subjective and is performed on a case-by-case basis, evaluation and treatment of patients with concomitant osteoporosis requires understanding of the behavior of this injury as a distinct subset of distal radius fractures. Age, infirmity, and osteoporosis affect every aspect of the fracture. Understanding what makes these fractures unique assists surgeons in more effective and efficient treatment. The authors present the current understanding of osteoporotic fragility fractures of the distal ra

The temporal and spatial development of vascularity in a healing displaced fracture.

Underlying vascular disease is an important pathophysiologic factor shared among many co-morbid conditions associated with poor fracture healing, such as diabetes, obesity, and age. Determining the temporal and spatial patterns of revascularization following a fracture is essential for devising therapeutic strategies to augment this critical reparative process. Seminal studies conducted in the last century have investigated the pattern of vascularity in bone following a fracture. The consensus model culminating from these classical studies depicts a combination of angiogenesis emanating from b

Histology of 8 atypical femoral fractures: remodeling but no healing.

BACKGROUND AND PURPOSE: The pathophysiology behind bisphosphonate-associated atypical femoral fractures remains unclear. Histological findings at the fracture site itself may provide clues. PATIENTS AND METHODS: Between 2008 and 2013, we collected bone biopsies including the fracture line from 4 complete and 4 incomplete atypical femoral fractures. 7 female patients reported continuous bisphosphonate use for 10 years on average. 1 patient was a man who was not using bisphosphonates. Dual-energy X-ray absorptiometry of the hip and spine showed no osteoporosis in 6 cases. The bone biopsies were
