Venous thromboembolism (VTE) is caused by the interaction between genetic, individually acquired, and environmental factors. The aim of this narrative review is to summarize advances in the research on genetic susceptibility to VTE and its recurrence, focusing on genome‑wide association study (GWAS)-derived polygenic risk scores (PRSs) and sequencing‑based approaches, and to discuss current barriers to their clinical implementation. Testing for the classical inherited thrombophilias, such as the deficiencies of natural anticoagulants (antithrombin, proteins C and S, the factor V Leiden variant

Formalin-fixed paraffin-embedded (FFPE) tissues stored in biobanks and pathology archives are a vast but underutilized source for molecular studies on different diseases. Beyond being the "gold standard" for preservation of diagnostic human tissues, FFPE samples retain similar genetic information as matching blood samples, which could make FFPE samples an ideal resource for genomic analysis. However, research on this resource has been hindered by the perception that DNA extracted from FFPE samples is of poor quality. Here, we show that germline disease-predisposing variants and polygenic risk

BACKGROUND: Genomic cardiology is an emerging field integrating genetic, molecular, imaging, and digital health data to improve cardiovascular disease (CVD) prevention, diagnosis, and treatment. The genetic architecture of CVD encompasses rare, high-impact variants as well as polygenic susceptibility, both of which influence disease risk, severity, and clinical outcomes.
HYPOTHESIS: Integration of genomic information, including rare variants and polygenic risk scores (PRS), with clinical, imaging, biomarker, and artificial intelligence (AI)-based approaches may enhance personalized cardiovasc

Genetic disposition accounts for about half of the risk to develop coronary artery disease (CAD). Yet, Mendel's Laws fail to explain its high prevalence. The conceptual dilemma was resolved by the common-disease-common-variant hypothesis, which proposes that common diseases like CAD are caused by multiple common genetic variants. Mapping of the human genome and consecutive genomewide association studies verified this hypothesis showing that hundreds of risk alleles contribute to the development of atherosclerosis. Many of these risk alleles are found with a high frequency in our population. Th

Colon cancer is one of the deadliest types of cancer in the United States, with close to 50,000 projected deaths in 2024. The disease requires early diagnosis to optimize chances of survival by enabling timely administration of treatment. To investigate the key non-genetic (NG) factors influencing the onset of colon cancer and evaluate how genetic factors enhance the performance of machine learning (ML) models in predicting incidence, we incorporated polygenic risk scores (PRSs) alongside NG data in ML models to predict 10-year incident risk prediction of colon cancer using data from the UK Bi

BACKGROUND: Genetic, lifestyle, reproductive, and anthropometric factors are associated with the risk of developing breast cancer. However, it is not yet known whether polygenic risk score (PRS) and absolute risk based on a combination of risk factors are associated with the risk of progression of breast cancer. This study aims to estimate the distribution of sojourn time (pre-clinical screen-detectable period) and mammographic sensitivity by absolute breast cancer risk derived from polygenic profile and the other risk factors.
METHODS: The authors used data from a population-based case-contro

Sarcopenia is a common age-associated condition characterized by the progressive loss of skeletal muscle mass, strength, and physical functionality. While large-scale genome-wide association studies (GWAS) have previously addressed isolated traits of sarcopenia, the multifactorial genetic architecture underlying this condition remains largely undefined. To characterize the common genetic basis of sarcopenia-related traits, genomic structural equation modeling (Genomic-SEM) was implemented. Multiple post-GWAS analytic approaches were integrated to pinpoint susceptibility loci. These analyses en

Limited data are available on genomic risk factors for breast cancer in diverse populations. We evaluated breast cancer risk conferred by high and moderate penetrance genes, polygenic risk scores (PRS), and family history (FH) in an ancestrally diverse biobank. We identified expected pathogenic variants (EPVs) in 14 genes among 30,223 sequenced individuals, including 15,919 unrelated women. Using electronic health records (EHRs), we evaluated associations of EPVs and PRS with breast cancer, stratified by FH and relative to individuals without EPVs and without FH. EPVs in high penetrance genes

BACKGROUND: Several genomic risk stratification tests are available to predict the risk of metastasis and mortality for prostate cancer patients at the time of diagnosis. However, the evidence supporting the clinical utility of genomic risk stratification tools is fragmented, posing challenges in assessing their real-world clinical impact and cost-effectiveness.
OBJECTIVE: To review and summarise the clinical impact and health economic evidence of the four types of genomic risk stratification tests and to validate their clinical impact and health economic evaluation outcomes.

High myopia (HM) is a complex condition influenced by both genetic and environmental factors, yet its early prediction and clinical intervention remain challenging due to heterogeneous progression patterns. To support early identification of individuals at risk for HM, we developed MIRAGE, a deep learning framework combining exome-wide genotypes with fundus images for personalized prediction. The model combines DeepExGRS for genetic risk modeling and a convolutional network for imaging, fused via a gating attention mechanism. Applied to a cohort of 1991 individuals, MIRAGE achieved high predic

IMPORTANCE: Polygenic risk scores (PRSs) have proven to be as strong as or stronger than established clinical risk factors for many cardiovascular phenotypes. Whether this is true for aortic stenosis remains unknown.
OBJECTIVE: To develop a novel aortic stenosis PRS and compare its aortic stenosis risk estimation to established clinical risk factors.
DESIGN, SETTING, AND PARTICIPANTS: This was a longitudinal cohort study using data from the Million Veteran Program (MVP; 2011-2020), UK Biobank (2006-2010), and 6 Thrombolysis in Myocardial Infarction (TIMI) trials, including DECLARE-TIMI 58 (2

Genetic predisposition is a risk factor for office hypertension. We sought to determine whether genetic predisposition identifies individuals with ambulatory daytime hypertension. 1444 participants from the GAPP study (ages 25-41) were analyzed. We evaluated two measures of predisposition to hypertension: family history and polygenic risk scores (PRS). We evaluated correlation of predisposition with blood pressure traits and compared incremental value of each predisposition measure to a validated ambulatory BP prediction model. 12% of participants had office hypertension, while 37% had out-of-

Complex diseases are shaped by heritable factors and non-genetic environmental, behavioral, and social determinants of health, but these are rarely modeled together. The growing availability of large-scale, multimodal biobanks creates new opportunities to integrate diverse data types into more accurate disease risk models. Here, we apply multiple correspondence analysis (MCA) to over 100 environmental, behavioral, and social variables from the All of Us biobank (n = 413,457 individuals) to generate low-dimensional embeddings that quantify non-genetic risk for six common chronic conditions: ast

Thyroid cancer is the most common endocrine malignancy, yet its biological underpinnings remain incompletely understood. Here we show that common risk alleles for thyroid cancer point to distinct biological pathways underlying disease susceptibility. We perform a multi-ancestry genome-wide association meta-analysis of thyroid cancer (16,167 cases and 2,430,374 controls), identifying 51 independent loci, including 21 not previously reported. By integrating these loci with genetic associations for 151 thyroid-cancer-related traits, we identify pleiotropic mechanistic clusters linked to thyroid f

PURPOSE: Dupuytren disease (DD) is a benign fibrosing disorder of the hand and fingers. Recently, we identified 9 single nucleotide polymorphisms (SNPs) associated with DD in a genome-wide association study. These SNPs can be used to calculate a genetic risk score for DD. The aim of this study was to test whether certain clinical characteristics (including the DD diathesis features) of patients with DD are associated with a high genetic risk score.
METHODS: Between 2007 and 2010, we prospectively invited all DD patients (1,120 in total) to participate. Clinical characteristics were noted usin

BACKGROUND & AIMS: Single nucleotide polymorphisms in the interleukin23 receptor gene are associated with Crohn's disease, suggesting a role in pathogenesis, and several biologic agents targeting this pathway are now established therapies. Interleukin23 has been suggested to be involved in regulation of intestinal barrier function and may impact gut microbial composition. We investigated whether interleukin23 receptor genetic variants predict Crohn's disease risk and influence gut barrier function and microbiome composition in healthy first-degree relatives.
