Effects of remimazolam vs. propofol on plasma neurofilament light chain and postoperative delirium in frail elderly patients undergoing major non-cardiac surgery: a prospective, randomized, assessor-blinded controlled trial.

BACKGROUND: The relationship between remimazolam administration and early neurological complications, particularly postoperative delirium (POD), in elderly patients undergoing major non-cardiac surgery remains unclear. Plasma neurofilament light chain (NfL) is a potential biomarker for central nervous injury. This study aims to compare the effects of remimazolam and propofol on postoperative plasma NfL concentrations and the incidence of POD in frail elderly patients undergoing major non-cardiac surgery. METHODS: This was a single-center randomized controlled trial on frail patients who under

When to pursue acute rib fracture fixation.

PURPOSE OF REVIEW: This review will explore acute rib fracture management focusing on indications and timing for acute surgical stabilization of rib fractures (SSRF). RECENT FINDINGS: SSRF is a well tolerated and effective approach for a variety of rib fracture patterns and is most commonly used to correct either clinical flail chest or multiple displaced fractures. Objective tools that assess for deranged pulmonary dynamics may identify patients with other fracture patterns who will also benefit from SSRF. Multimodal analgesia approaches are essential regardless of whether SSRF is pursued; i

Perioperative neurocognitive disorders: A narrative review focusing on diagnosis, prevention, and treatment.

Perioperative neurocognitive disorders (NCDs) refer to neurocognitive abnormalities detected during the perioperative periods, including preexisting cognitive impairment, preoperative delirium, delirium occurring up to 7 days after surgery, delayed neurocognitive recovery, and postoperative NCD. The Diagnostic and Statistical Manual of Mental Disorders-5th edition (DSM-5) is the golden standard for diagnosing perioperative NCDs. Given the impracticality of using the DSM-5 by non-psychiatric practitioners, many diagnostic tools have been developed and validated for different clinical scenarios.

Simultaneous two-level minimally invasive lumbar laminectomy performed with dual tubular retractor systems in a 93-year-old under spinal anesthesia: illustrative case.

BACKGROUND: Lumbar spinal stenosis is a common degenerative condition in elderly patients that often requires surgical intervention. Minimally invasive techniques combined with spinal anesthesia (SA) can reduce patient complications and improve outcomes. Here the authors present the case of an elderly patient who underwent minimally invasive surgery, using dual tubular retractors and SA to address severe lumbar stenosis. OBSERVATIONS: A 93-year-old female presented with severe back pain radiating bilaterally to her lower extremities. MRI demonstrated severe central canal stenosis at L4-5 and

Investigation of the Medium- and Long-Term Results of a Pioneering Method in the Treatment of Geriatric Intertrochanteric Femur Fractures: Osteosynthesis Using the WALANT Technique.

Background/Objectives: Femoral neck and proximal femur fractures in the elderly can result from low-energy trauma due to osteoporotic changes and contribute significantly to increased morbidity and mortality. Despite various treatment options, closed reduction and internal fixation (CRIF) with intramedullary nails has become the predominant approach. While a minimally invasive approach reduces complications and speeds recovery, this outcome is not always feasible in practice. The primary surgical goal remains achieving a stable and precise fracture reduction, favoring CRIF when possible. Our s

Cerebral oximetry-guided anaesthesia and postoperative delirium in elderly patients undergoing off-pump coronary artery bypass grafting: a randomized controlled trial.

INTRODUCTION: While cerebral oximetry monitoring has been reported to reduce neurological dysfunction and hospital stay after cardiac surgery, its effect on postoperative delirium (POD) during off-pump coronary artery bypass grafting (CABG) remains underexplored. This study examines whether using cerebral oximetry to guide anaesthesia can reduce POD in patients aged ≥ 65 years undergoing off-pump CABG. METHODS : This single-center, randomized trial enrolled 200 patients scheduled for off-pump CABG. Participants were randomly assigned (1:1) to either the intervention or control group using seal

[Analgesia Nociception Index: assessment of acute postoperative pain].

BACKGROUND AND OBJECTIVES: Patient self-rating based scales such as Numerical Rating Scale, Visual Analog Scale that is used for postoperative pain assessment may be problematic in geriatric or critically ill patients with communication problems. A method capable of the assessment of pain in objective manner has been searched for years. Analgesia nociception index, which is based on electrocardiographic data reflecting parasympathetic activity, has been proposed for this. In this study we aimed to investigate the effectiveness of analgesia nociception index as a tool for acute postoperative pa

Xenon for the prevention of postoperative delirium in cardiac surgery: study protocol for a randomized controlled clinical trial.

BACKGROUND: Postoperative delirium (POD) is a manifestation of acute postoperative brain dysfunction that is frequently observed after cardiac surgery. POD is associated with short-term complications such as an increase in mortality, morbidity, costs and length of stay, but can also have long-term sequelae, including persistent cognitive deficits, loss of independence, and increased mortality for up to 2 years. The noble gas xenon has been demonstrated in various models of neuronal injury to exhibit remarkable neuroprotective properties. We therefore hypothesize that xenon anesthesia reduces t

Incidence and significance of cardiac arrhythmia in geriatric oral surgery patients.

We recorded heart rhythms of 40 older patients (20 medicated for cardiovascular disease and 20 not medicated for cardiovascular disease) during an outpatient oral surgery visit to determine overall arrhythmia incidence and severity, differences in incidence associated with cardiovascular medication status, and the impact of surgical intervention on arrhythmia incidence. We hypothesized that both groups would show similar arrhythmia numbers and types during surgical visits and that a history of medication for cardiovascular disease would not be an indicator of cardiac arrhythmia. Differential impacts of propofol and etomidate on hemodynamic stability and postoperative cognition in elderly surgical patients: A retrospective observational analysis.

As the aging population continues to expand, tailoring anesthetic strategies to improve safety and recovery outcomes in elderly patients has become increasingly important. Propofol and etomidate are 2 widely used agents for intravenous induction in general anesthesia, each with distinct pharmacologic properties. This study explores the comparative safety profiles and perioperative outcomes of these agents in an elderly surgical cohort. A retrospective observational study was conducted on 150 patients aged 65 and above who underwent elective surgery under general anesthesia from January 2023 to

Interventions to prevent and treat delirium: An umbrella review of randomized controlled trials.

Delirium is a common condition across different settings and populations. The interventions for preventing and managing this condition are still poorly known. The aim of this umbrella review is to synthesize and grade all preventative and therapeutic interventions for delirium. We searched five databases from database inception up to March 15th, 2023 and we included meta-analyses of randomized controlled trials (RCTs) to decrease the risk of/the severity of delirium. From 1959 records after deduplication, we included 59 systematic reviews with meta-analyses, providing 110 meta-analytic estimat

Comparison of the recovery time of remimazolam besylate and propofol for gastrointestinal endoscopy sedation in elderly patients.

Background: Recovery time is a crucial factor in ensuring the safety and effectiveness of both patients and endoscopy centers. Propofol is often preferred due to its fast onset and minimal side effects. Remimazolam is a new intravenous sedative agent, characterized by its rapid onset of action, quick recovery and organ-independent metabolism. Importantly, its effect can be specifically antagonized by flumazenil. The primary goal of this study is to compare the recovery time of remimazolam besylate and propofol anesthesia during endoscopic procedures in elderly patients. Methods: 60 patients ag

Low-Volume Superior Trunk and Superficial Cervical Plexus Block With Sedation as the Sole Regional Anesthetic for Proximal Humerus Fracture Fixation in a Patient With Alzheimer Disease.

BACKGROUND Proximal humerus open reduction and internal fixation (ORIF) is usually performed under interscalene brachial plexus block (ISB) or general anesthesia (GA). ISB causes ipsilateral phrenic nerve paresis in nearly all patients, and GA may raise the risk of postoperative neurocognitive disorders in elderly patients with cognitive impairment. We report the feasibility of a low-volume regional technique, used as the sole regional anesthetic with sedation, in a patient in whom GA avoidance was desirable. Clinical Efficacy Evaluation of Percutaneous Endoscopic Interlaminar Decompression Under Local Anesthesia for the Treatment of Degenerative Lumbar Spinal Stenosis in Elderly Patients with Medical Comorbidities: A Multicenter Retrospective Study.

OBJECTIVE: Percutaneous endoscopic interlaminar discectomy (PEID) is recognized as a safe and effective minimally invasive treatment for lumbar spinal stenosis (LSS). This study introduces a novel awake PEID strategy that involves the use of conscious sedation with stepwise local anesthesia (LA) as an alternative to general anesthesia (GA). The objective of this study was to evaluate the feasibility of awake PEID in elderly patients with degenerative lumbar spinal stenosis (DLSS) and multiple comorbidities. METHODS: This retrospective analysis included 123 consecutive patients older than 70 y

The effect of patient sex on the incidence of early adverse effects in a population of elderly patients.

Patient sex is known to influence the response to general and regional anaesthesia and recovery after surgery. However, most studies come from analyses carried out on middle-aged patients. As most of the patients admitted to the post-anaesthesia recovery room in our institution are elderly, we took the opportunity to investigate the association between sex and incidence of early adverse events in this older population of patients after major surgery. Consecutive patients undergoing general, orthopaedic, urological and gynaecological surgery, admitted to the recovery room of our institution ove

Effects of olfactory enrichment on perioperative neurocognitive disorders in elderly patients undergoing orthopedic surgery: a study protocol of a multi-center randomized controlled trial.

BACKGROUND: Neurocognitive disorders are prevalent among older surgical patients, yet their underlying mechanisms and effective interventions remain largely unknown. Studies in rodents and patients suggest a connection between olfactory impairment and perioperative neurocognitive disorder (PND). Previous work in mice demonstrates that anesthesia and surgery can lead to both olfactory impairment and cognitive deficits. Olfactory enrichment, which is also referred to as olfactory training (i.e., daily exposure to multiple odorants), may prove helpful. For example, olfactory enrichment appears to
